We are excited to let you know that our school has partnered with CodmanSquare Health Center to organize COVID Vaccine Clinic for all Codman students

ages 12 years and older.

Starting this week, these vaccination clinics will be held at the health center whichis right next door to the school on the following days and time:

Wednesdays, Thursdays and Fridays between 9 a.m.-11 a.m. starting this Wed,May 26th.

Codman school nurse (Candace) and classroom teachers will help set up theappointment and will accompany the students during the time of vaccination.

A signed consent form from a parent/guardian is required, so click on this

link to sign the consent form. It will take about 3-4 minutes to complete this form.

The school does not require parents to be present onsite at the time ofvaccination. We will send information for the second COVID vaccine dose home.

Vaccines are completely free.

CDC recommends everyone 12 years and older should get a COVID-19 vaccinationto help protect themselves and others from COVID related illness and

hospitalizations. Over 600,000 children between ages 12-17 years have alreadyreceived their first dose of vaccine.

The latest science and data shows us that getting vaccinated among people ages12 years and older, is the best way to protect yourself and your loved ones. It isthe only way we can end the pandemic and start getting back to so many things

we cherished.
