What is MyPathway®?

What MyPathway means to the Patient

MyPathway® is an internet-based tool which can be used through a web browser on a computer, or as an app on a phone or tablet. As a NHS system, it is secure and has been co-designed and tested by patients to make it user-friendly.Information entered by patients and clinicians into MyPathway will be used to improve the experience for patients as well as their care and health.

Engagement between the patient and their clinician via the platform leads to:

> Increased accessibility to useful information and resources (such as videos and leaflets)

> Enhanced ability to self-manage> Patient making healthy decisions> Feeling of empowerment> A clear understanding of the next

stage in their treatment journey
