WHY THIS GUIDE?
Those of us who lead “normal” lives, despite the disabilities that laryngectomy imposes,must wonder how far from our homes we wish to travel without taking specialprecautions. Our early brief sorties become more extended with rising confidence. Eachcase is individual and the single person has to be more circumspect than someone with acompanion who can act as a voice when needed.
Getting away from the usual environment and familiar medical staff is a step further. Eventravelling around one’s own country is not straightforward:
“Which hospital can I go to if...... ”
The shadow lurking behind most of us is the possibility of the voice prosthesis comingout and the fistula closing up before anyone can get a dilator or catheter in.
Admission policy varies very widely round the different hospital trusts and it must notbe assumed that the next convenient hospital can change your valve, even if they havethe skills and the staff. It is believed that none of the hospitals listed here would turn alaryngectomee away for budgetary or insurance reasons.
At the end of this booklet you will find emergency flash cards in the main Europeanlanguages in case you find yourself in a country where you have to explain your situationin a language you may not speak, or cannot speak because you have lost your prosthesis.
This booklet has been put together with these factors in mind, providing locations andlists of all hospitals believed to handle emergencies of the type that laryngectomees mayencounter. A small map shows the spread of relevant towns.
The “Emergency” flash card in English is on the back cover giving immediate access forthe reception staff or A&E and the pages immediately inside at the back gives a list ofthe usual conditions to help a voiceless laryngectomee indicate what the problem is. Theinside of the front cover carries a form for you to fill in with all of the relevant personalmedical details you are likely to be asked for. You are strongly advised to fill it in assoon as you get this Guide, so you are not searching around for information ata time which will be stressful for you in any event. If you present yourself atan unfamiliar hospital, they will want all of this information as a minimum.
Should you need a valve change and accidentally not have a spare one with you, makesure you enter valve details inside the front cover. A simple valve change is one thing,sizing a fistula may be quite another.
Before travelling...
There are obvious precautions anyone fitted with a voice valve should take beforetravelling away from familiar surroundings. If your destination is known, use the maps tofind the nearest hospital to it. The NHS is in a constant state of flux so it is essential tomake contact in advance of your arrival, to confirm a suitable service is still available.Speak to your speech therapist in case they have any particular advice for you.
Make sure that your “spare parts” are in order and sufficient for the length of your stay.If your valve is near the end of its usual life, get it changed before you leave and alwaystake a spare one with you. You may get a tracheal infection whilst away, so pack enoughspares to take account of this.

Those who normally use electro-larynxes will only have to consider flat batteries andmany hospitals offering after-care can help. Like oesophageal speakers, they may alsosuffer from local infection or inflammation and should know how to deal with this. Ifnot, consult your specialist nurse.
THE CANCER LARYNGECTOMEE TRUST
The Charity was set up in 1984 by Sydney Norgate who had a laryngectomy in 1979.
Here are some of the activities of the Charity:-
Making grants to Laryngectomees for purposes as varied as help with items of
clothing or bedding, medical equipment for items unavailable on the NHS, holidays
for Laryngectomees, outings for Laryngectomee groups and assisting with training
grants
Supplying copies of the book “Laryngectomy is Not a Tragedy” free of charge to
Laryngectomees and members of the medical profession involved in their care and
arranging for it to be reprinted
Talking to carers who need support as they come to terms with the diagnosis
Acting as a signpost to organizations who could also offer help such as Macmillan
and NALC
Fundraising to ensure the continuation of the Charity with the main event being
the annual race night
Helping with information about benefit claims and providing information
about medical aids and prescriptions
Providing information to enable talks to be given to others such as student nurses
Organizing and funding rehabilitative functions for Laryngectomees , their
carers and medical professionals
Sending a Newsletter twice a year to members at their home or email addresses
Maintaining a website for the Charity at http://www.cancerlt.org

S’il besoin d'oxygène, celui-ci doit être administréau niveau du cou; le donner par le nez ou la bouchene servira à rien.
Si le patient ne peut pas parler il tenterad’indiquer au personnel médical les details duproblème – voir la page suivante.
Si le médecin ou l'orthophoniste ne parle pasanglais, un interprète serait utile.
Tout délai risque d’entrainer de gravesconséquences pour le patient, voire une nouvelleintervention chirurgicale.

Esta persona habla inglés y necesita los serviciosde urgencia de un departamento deotorrinolaringologia en un hospital.
El ha sufrida un LARINGETOMIA. El enfermo nopuede hablar si la prótesis no está bien colocada,o está ausente.
Si necesita oxígeno, esto se debe aplicar por elcuello. No sirve para nada aplicarlo por la boca opor la nariz.
El enfermo tiene más información sobre laproblema, vease al otro lado de este papel.
Si el médico o el ortofonista no habla inglés, seranecesario un interprete.
Cualquiera demora resulterá en gravesconsequencias para el enfermo.

Emergency
The bearer requires the urgent attention of theEAR NOSE & THROAT DEPARTMENT
Having had a LARYNGECTOMY the patient maynot be able to speak if the prosthesis is absent ordefective
If oxygen is required it should be applied TO THENECK ONLY. It serves no purpose in the nose ormouth.
If voiceless, the patient will try to show medicalstaff more details of the problem on the previouspage.
PROLONGED DELAY MAY HAVE LONG-TERMCONSEQUENCES AND INVOLVE THE PATIENT IN
FURTHER SURGERY.
