HOW CAN YOU HELP WITH
MISSING PERSONAL
ITEMS?
In an a)empt to be)er
manage con nued concerns with missing items including
clothing, dentures, hearing aids, and glasses—we have re-
vised our policy and procedures and want to make you
aware of a few changes:
Please be sure that you are labeling any new glasses,
dentures, and hearing aids as you bring them in—
these can o;en be etched by the provider you pur-
chase them from.
Please be sure you are taking all clothing to the laun-
dry room to be properly labeled, even if you are
doing the laundry because things do s ll o;en end
up being placed in our laundry for various reasons.
There is a new form that laundry staff will require
you to turn in WITH all clothing to be labeled—
they cannot accept the clothes without the com-
pleted form. You can obtain this form at the nurs-
ing sta on or from laundry staff.
The facility is NOT responsible for replacing things that
have not been properly labeled when brought in—the new
forms will help us to track these items so that we know
what we have labeled for reimbursement if necessary. We
also have a new form to assist in hopefully loca ng missing
items and to determine the outcome if unable to locate—
you can obtain these forms as needed from the nursing
sta on. Please be aware that all items without proper la-
beling are only kept in our lost and found for 14 days due
to limited storage space.
Thank you in advance for your assistance in working with
us to make sure all items belonging to residents are
properly labeled prior to being used.
