CCC-SLP Chief Operating Officer Clinical Fieldwork and Intern Supervisor Recognition Program Coming in 2016 Rehab Synergies values our Clinical Fieldwork Supervisors; we understand that there is a time and energy commitment involved when our Team Members agree to give back to their profession through Student or Internship supervision. We also recognize that through daily on-site supervision of a student, a Clinical Fieldwork or Intern Supervisor will develop their clinical teaching skills while gaining basic supervisory responsibilities, including establishing and communicating expectations, monitoring performance, providing feedback, and performance evaluations. Through our commitments to Colleges and Universities in and surrounding Texas, Rehab Synergies now enjoys relationships with over 35 educational institutions. In order to acknowledge this mutual opportunity, Rehab Synergies would like to honor each Clinical Fieldwork Supervisor, at the conclusion of a student’s fieldwork in 2016, by paying for their state license fees and provide their department with access to instructional tools and resources as indicated. Each Clinical Fieldwork Supervisor will also receive a gift-card for use at our Rehab Synergies Uniform Store, and certificate of recognition and professional development units toward their designated professional association. Some colleges and universities also offer free or discounted education as an incentive for Clinical Fieldwork Supervisors. Together, we can create a model program for supporting students in Skilled Nursing Facilities. We would like to thank Advanced Healthcare Solutions, our Care Partners and especially our Team Members for making our first 5 years a great success. We have expanded to providing therapy services to 51 Skilled Nursing Facilities, 28 Outpatient Therapy locations as well as providing therapy services in the Home Health Setting. Wow! What a ride!

SNF-REHAB PROVIDERS

UTILIZING ACP’S INTEGRATED CLINICAL PROGRAMS

DEMONSTRATE SIGNIFICANT IMPACT ON QUALITY INDICTORS USED IN

CMS 5 STAR RATINGS

A recent analysis of CMS’s quality indicator data used in their 5-Star Rating System showed that rehab providers partnering with Accelerated Care Plus (ACP) performed 11% better in the Quality Measures directly used to influence a SNF’s 5-Star rating.

Findings included significant improvement in the areas of Pain, Wounds, Falls, and Activities of Daily Living. The analysis indicated that facilities partnering with ACP had 30% fewer short stay patients reporting moderate to severe pain and 20% fewer long stay patients reporting moderate to severe pain. ACP facilities had 24% fewer patients with increased need for assistance with ADLs during their stay, and 22% fewer high-risk ,long stay patients with pressure ulcers. Further, ACP facilities had 8% fewer falls involving injuries.

Texas Implements the Medicare-Medicaid Program

(MMP) in Partnership with Centers for Medicare and

Medicaid Services (CMS) The Centers for Medicare & Medicaid Services (CMS) and the State of Texas Health and Human Services Commission (HHSC) established a federal-state partnership to implement the Texas Dual Eligibles. The Texas Dual Eligibles program promotes an integrated care demonstration model to better serve individuals eligible for both Medicare and Medicaid (Medicare-Medicaid enrollees). The objective of the MMP program is to improve the beneficiary experience in accessing services through:

William attended North Central Texas College in Denton/Corinth

and obtained a degree in Applied Science/Occupational Therapy.

He has had the opportunity to work in various populations,

including pediatrics, adults, and geriatrics. William has provided

services in a vast array of practice settings, including: In-Patient

Acute Care, Home Health, Out-Patient, and Schools and has

achieved certification in various specialty areas of Rehabilitation

including: Job Description Development, Ergonomics, Work

Hardening, Work Conditioning, FCE/Work Capacity Evaluation,

Work Simulation, and Strain/Counterstrain Techniques (Jones

Institute). He enjoys spending time with his two sons, ages twelve

and six. William enjoys taking joy rides in his hot rod, weekend get-

aways, flea markets, theater, and dancing.

Q&A With William

What do you like best about Rehab Synergies? I can honestly say

that Rehab Synergies has some of the strongest Clinical Support I’ve

seen, and continuously strives to be innovative in their approach to

Rehabilitation.

What’s one tip you have for therapists in our company to help

them be more successful? Everyone needs to be Mentors of their

profession/discipline: we can ALL learn from one another!

What’s your favorite quote? “The foundation stones for success are

honesty, character, integrity, faith, love, and loyalty.” – Zig Ziglar

What’s one thing that people don’t know about you? I’ve been

paralyzed from the neck down, two different times in my life, with

recovery each time.

DELIVERY TO HOME HEALTH SETTING Rehab Synergies announces the addition of our

Director of Home Health, Shelli Martinez, LVN.

Rehab Synergies Home Health division officially

kicked off on October 1, 2015. Rehab Synergies is

now able to provide continuity of care to the

community level so our patients can rest assured their

therapy needs are met upon their discharge home until

they are able to return to us for Outpatient.

Integrating this next step not only helps the patient but

the family as well by taking one less worry away from

decisions to make regarding therapy needs. We

currently serve Bowie, Greenville, Vernon and the

DFW markets. Team members interested in being a

part of the patient’s continuum of care should contact

Shelli with their Home Health availability. Home

Health allows flexible scheduling and increases the

therapist’s awareness of the environmental factors a

patient has to master in order to live in the home

independently. The Home Health Setting helps provide

new insight to

patient therapy

goals and is a

great opportunity

for therapists to

expand their

experience.

A Very Common Problem
