green your supply chainIf you’ve never seen The Story of Stuff, a short movie on how we make, use, and throw away the stuff

we buy, I highly recommend it. The 20-minute cartoon narrated by Annie Leonard gives a snapshot of the adverse effects that purchasing has on our communities as well as on the environment. We

need to keep in mind that when we put stuff in trash cans and say we are throwing it away, well in reality, there is no away. It all goes somewhere.

The health care industry spends $200 billion a year on purchasing medical and nonmedical supplies. Producing, shipping, and tossing away that much stuff has considerable detrimental effects on our planet—deforestation, dangerous carbon emission levels, toxic chemicals, hazardous materials, and, of course, the never-ending sea of cardboard boxes, packaging and plastic to name just a few.

Practice Greenhealth’s Business Leadership Coalition members want to replace the current practice of merely responding to the needs of health care with a more proactive and interactive process in which forward-thinking businesses are involved with environmentally preferable purchasing (EPP) specifications and requirements. This collaborative action includes supporting efforts of suppliers to obtain information from their own raw material supply chain to better understand the components of their products, to know what is important to ask about products, and to support the need for greater transparency from their suppliers for more sustainable and less toxic products. Currently our Business Leadership Coalition members are working together to create a total cost-of-ownership (TCO) tool to assist hospitals in evaluating total product costs across the life span, instead of just looking at purchase price. They are also focusing on educational materials for hospital purchasing managers and supply chain staff.

Practice Greenhealth also offers a vast array of resources to help get your facility started on its EPP path. The Standardized Environmental Questions for Medical Products are a set of supplier questions that can be used in the procurement process and cover key areas of concern, such as packaging, manufacturing, use, and the end-of-life of medical products, based on the industry’s environmental priorities. We continue to work with five of the largest GPOs to develop our next step of EPP disclosure questions for medical electronics.

I would also encourage all members to review the EPP Specifications and Resource Guide on the Practice Greenhealth website for a detailed list of specifications and environmental considerations for numerous product and service categories.

We would like to congratulate Kaiser Permanente for their tremendous efforts of building a more sustainable, targeted, and balanced EPP program. The system has implemented a three-plan approach—presourcing, sourcing, and implementation—which has become part of the culture at Kaiser Permanente and not an additional step in the EPP process (see page 6). Also in this issue, we’ve included an extensive sustainable products and services directory to help you green your supply chain.

As we move into 2014, please take a moment to look back at the positive work that has been achieved at your facility. We look forward to working with each member to help you set your goals for next year so we can create healthier hospitals and more sustainable communities!

Kaiser Permanente’s success in rethinking

its environmentally preferable purchasing

EPP as Easy as 1, 2, 3AChIEvInG An Environmentally Preferable Purchasing (EPP) program is simpler than many people realize. From the start, it’s important to determine the results you want to achieve. At Kaiser Permanente, four environmental attributes guide the process: cleaner energy, safer chemicals, less waste and healthier food. Kaiser Permanente has achieved success, year after year, through strategy, organization, and, more recently, by identifying through its procurement process (also known as sourcing) the three key phases: pre-sourcing, sourcing, and implementation. While the concept isn’t new, in the EPP world, it holds a wealth of knowledge.

Building a sustainable, targeted and balanced portfolio of EPP initiatives that organically feeds your program is groundbreaking. To start the journey, you first need to understand each of the three procurement phases, so that you can build it into your planning efforts.

pre-sourcingPre-sourcing is a goal-oriented, metrics-based activity that prepares you and your procurement team for the sourcing effort. Knowing upfront what you want to achieve makes an enormous difference. Perhaps your organization wants to source a product category in the next year or so, due to an expiring contract or changed requirements for good and services, and you know you want meaningful results in one or more of your EPP attribute areas. This is your starting point.

For example, let’s assume skincare is a product category on your organization’s radar for a sourcing effort in 2015; in that case, beginning pre-sourcing activities in 2014 is ideal. In this example, safer chemicals may be the environmental attribute used to achieve results. The next step would be authoring, co-authoring, or adopting a best practice: a chemical disclosure questionnaire. Whether it’s preparing the document for the procurement/sourcing team to use later, prior to the sourcing effort, or preparing the document for immediate use with the incumbent supplier, doing the work upfront will save valuable time. In addition, by setting expectations upfront for which specific EPP attribute you would like to positively impact, you are helping to set the stage for success.

Another example of a pre-sourcing activity might be reviewing a product category against your EPP attributes, such as a review of your organization’s custom packs and/or kits. Reviewing this product category for packaging reduction opportunities, and understanding the potential prior to commencing a sourcing effort can be vital, both as a valuable time-saver and as an educational tool for suppliers.

In 2013, Kaiser Permanente has a pre-sourcing EPP goal to obtain six chemical disclosure forms (for separate product categories) from its incumbent suppliers. Another Kaiser Permanente goal for 2013 is to review 30 customs packs and trays for packaging reduction opportunities. Pre-sourcing is no different from other activities you want to accomplish: Unless you set a goal, then track and measure the progress, your project may fall victim to other priorities.

SourcingSourcing (also known as procurement) is a better-known process. For Kaiser Permanente, it is the alignment of stakeholder requirements to engage suppliers in the marketplace to bid on goods and services. We have found it imperative to involve our stakeholders, especially our clinicians, upfront and throughout the sourcing process. The same premise goes for the sourcing process as it relates to the EPP portfolio mix of initiatives. Understanding when your organization wants or needs to source a product category is necessary to determine any required pre-sourcing activities (and relative timing), as well as the implementation year.

From the goal-oriented and metrics-based viewpoint, achieving success depends largely on working with your procurement/sourcing teams to develop weighted milestones toward completion. Accurately wording goal language is also a must. In Kaiser Permanente’s EPP program, we have developed general milestones for each gate within the sourcing process. For example, measurable milestones include: Issue RFI, Evaluation, Issue RFP, RFP Analysis and negotiation, Standards and Sourcing Team/Core Group Approval, and Governing Body Deal Structure Presentation. It helps to ensure that you, your procurement/sourcing team,

and stakeholders are clear on what the year-end result will be, such as, “complete sourcing for chemically-safest product category (name) available on the market that meet all regulatory, and that best meet all (your organization’s name) clinical requirements by year end.”

implementationIs the product conversion process going to be boots-to-the-ground, facility-by-facility implementation? The oxymoronic answer is, yes and no. Looking through the EPP lens, implementation is the execution phase of a project or initiative that produces a positive, metrics-based, environmental outcome. Again, think goal-oriented; for example, product conversions that produce less waste in tons, or project implementations that produce a kilowatt hour (kWh) increase in renewable energy or perhaps kWh decrease on energy demand. It is the actual metric that can be measured as “the fruits of our labor.”

Everyone plays a part from a strategy/EPP planning perspective. If your effort is a collaborative process involving your procurement/sourcing team, a solid idea of the implementation metric is likely to unfold during the sourcing phase. Your suppliers also are great partners in the implementation process – they will be the ones who report on the metric that you and your procurement team developed as a goal.

So, where is this all going? The benefits of managing your EPP portfolio of initiatives in a conscious process are profound: enhanced strategy planning for a three-year rolling EPP plan with a self-feeding pipeline; better integration with your procurement/sourcing teams, their work, and goal

setting; and ensuring credibility by measuring EPP implementations for targeted projects through a dynamic executive level dashboard.

Your success will hinge on a few key factors: 1. Engage your group purchasing organization

(GPO). Ensure they understand the concept and how to use the technique in the goal setting and tracking/measurement process. This will extend your capacity to help manage the EPP program.

2. Develop simple education and training tools to communicate the technique – perhaps a presentation deck and live discussions through webinars or in-person sessions. highlighting the benefits of using this concept to manage the program will lead to recognition for its ease in use (it has worked for Kaiser Permanente).

3. Any time is a good time to start. however, just before goal setting is optimal.

4. Don’t set hard lines when using this technique. It’s possible, for example, to have sourcing efforts without pre-sourcing. It is important to know your resource capacity – how many initiatives you and your procurement/sourcing team can work on each year – that yield the highest impact in the desired environmental attribute.

Deploying this concept will provide your organization with an EPP mix of pre-sourcing, sourcing, and implementation projects each year, feeding the pipeline in two- to three-year cycles. Just remember, as with any planning process, it is important to conduct a refresh and ensure your EPP plan is a rolling plan that can be adjusted according to changing priorities or circumstances.

ThERE ARE MAnY COMPOnEnTS to Boston Scientific’s environmental sustainability program. Significant, targeted efforts during the past few years have produced improvements to our internal operations, resulting in the reduction of environmental impact, product costs, and operating risks. By sharing our vision of sustainability and working with our supplier community, many of our suppliers have become active participants in our program. Together we have been able to achieve a “triple win:” a win for the environment, a win for the people that work for us and with us, and a win for our business. With annual revenues of more than $7 billion, our global infrastructure includes a complex supply chain, which supports significant manufacturing and distribution facilities on four continents. We have developed an approach simple enough for a range of suppliers to use consistently to achieve improvements in areas that make a meaningful difference.

In 2012, working collaboratively with our supply base, we launched a Green Supply Chain program to build sustainability into the procurement of goods and services used in our business. Our goal was to expand our business processes with suppliers to continuously increase environmental sustainability within our supply chain. The framework for the program includes four principal components. The first involved engaging key suppliers in an annual environmental sustainability assessment. The assessment involved measuring and base-lining strategic suppliers in greenhouse gas emissions, waste generation, reuse/recycle and environmental management practices in order to drive future reductions. The assessment is aligned with Boston Scientific’s internal practices and programs, and is intended to enhance the opportunities to collaborate and leverage knowledge. This collaboration results in continuous improvement as the desired objective.

To enable and increase communication within the supply chain to meet our goals, the second component involves the inclusion of sustainability practices in our supplier business review process and incorporates sustainability language in Boston Scientific’s Supplier Guidebook.

Further building on these elements, the third component involves a sustainability assessment in the supplier selection process. This assessment is a component of the overall selection process for new opportunities and provides information on the sustainability profile of bidders. This process allows Boston Scientific to compare and evaluate suppliers based on their sustainable practices.

The fourth tenet of our program, our Master Supply Agreement, designed to formalize engagement and a commitment to environmental sustainable practices between our suppliers, has been expanded to include language confirming a supplier’s participation in the program.

We invested significant time developing the metrics for measuring our success, as well as effectively communicating expectations to ensure our valued suppliers would be able to participate. We chose to measure our strategic suppliers based on the same metrics that we measure ourselves, with examples such as energy use, waste, and recycling. Most importantly, our metrics are based on quantitative measurement. We developed a method for measuring the performance of our supplier chain through reductions in waste, electrical consumption, oil consumption, etc. Our program philosophy is “make a difference.” We will continue to monitor results by measuring year-to-year improvements in our supply chain, and if our metrics improve, we know we are doing our part in reducing our impact on the environment.

We believe that we have made good progress in this area and consider ourselves in the early stages of program development compared to some of the companies with whom we benchmarked. As we learn through ongoing peer discussions and experiences, we fully expect our program will mature. We are now focused on increased education about sustainability and the “triple win” among internal resources, business units, and employees. We plan to further promote the program across the supply chain to increase supplier participation. By using a simple and robust sustainability platform, educating users, and engaging strategic suppliers, we are on track to make a positive impact on the planet, for people as well as our business.

ARSEnIC IS a naturally occurring element in the Earth’s crust and can be found throughout our environment in the air, land, and water. however, when combined with other elements besides carbon, inorganic arsenic can be harmful to human health. Chronic exposure to inorganic arsenic can lead to adverse health effects, including several types of cancer, body systems’ failures, developmental defects, diabetes, cardiovascular disease, and many other types of illnesses.1 While sources of drinking water, soil, and food staples such as rice and shellfish can become contaminated naturally with inorganic arsenic, mining and industrial operations, as well as tobacco smoke, pollute the environment with the element.

For decades, communities in the U.S. were also being exposed to inorganic arsenic through another medium: poultry production. The U.S. Food and Drug Administration (FDA) historically allowed the routine use of inorganic arsenic in poultry feed, for the purposes of growth promotion and feed efficiency, as an antimicrobial agent, and to give the bird meat an artificial pink hue for aesthetics. According to estimates, nearly 70 percent of the broiler chickens raised annually (over nine billion birds in 2008) were typically fed arsenic.2 This is a practice that has never been approved as safe or necessary in several countries of the European Union.

Arsenic residues have been found in broiler meat, and as arsenic is passed through the birds, it has ended up back on agricultural fields in the form of fertilizer. Run-off from poultry production has also led to the contamination of water supplies.

The FDa Withdraws approvals of arsenicals after pressured by Health Care Groups For years, health professionals decried the unnecessary increase of inorganic arsenic levels in the environment and the food supply as a result of agricultural practices. Four years ago, a group of organizations, including health Care Without harm and the Oregon and San Francisco chapters of Physicians for Social Responsibility, brought forth a citizen petition to the FDA to remove arsenic from animal agriculture. When

the FDA did not respond, they filed a lawsuit in the spring of 2013.

In September 2013, the FDA finally reacted, and announced that it will withdraw 98 of 101 approvals given to arsenic-based animal drugs, which covers three of the four arsenic-containing compounds approved for animal feed. The FDA’s momentous decision around these chemical additives would have been unlikely to occur without the advocacy efforts by the health care sector.

The small state of Maryland produces a large number of broiler chickens, over 300 million on an annual basis, making it the eighth largest poultry producer in the U.S.3 Many of its poultry farms are located on the Eastern shore of the state, along the Chesapeake Bay, the largest estuary in the country.

Unfortunately, the environmental health of the Chesapeake Bay has been deteriorating for years. One significant source of pollution has been waste run-off from poultry farms, full of nutrients like nitrogen and phosphorous, leading to algae blooms within the body of water. Another pollutant has historically been arsenic. Researchers found that arsenic routinely fed to chickens was being excreted in their waste, which was running off from farms and contaminating and accumulating in the Chesapeake Bay watershed.

Given the environmental footprint of poultry production, as well as emerging evidence that there were higher levels of arsenic in chicken meat that had been raised with the chemical element,4 the government of Maryland adopted a ban on arsenic additives in chicken feed in the spring of 2012.

The collective voice of the health care and public health communities helped lead to this

ban, through a long-running legislative battle. 2012 had been the third consecutive year that the state legislature considered prohibiting arsenic use in poultry production Maryland. The American Academy of Pediatrics-Maryland Chapter, Baltimore Medical System, Carroll hospital Center, the Maryland Dietetic Association, the Maryland nurses Association, MedChi-the Maryland State Medical Society, and Union hospital of Cecil County all publicly supported the arsenic ban.

Carroll hospital Center and Union hospital of Cecil County had also been procuring local and arsenic-free poultry for Thanksgiving and national nutrition Month in March for a few years by then, in support of sustainable meat production within the state. According to Louise Mitchell, the health Care Without harm healthy Food in health Care state coordinator in Maryland, “these hospitals were already doing work on sustainable purchasing, and when they learned about arsenic, they reached out to their suppliers to make changes in the poultry they were serving in their facilities.” Procurement efforts by these hospitals

helped lead to increasing awareness about inorganic arsenic, and the eventual legislative change within the state.

While many hospitals start their sustainability journey with a focus on managing waste, it’s only a matter of time before they realize that to truly impact the waste stream and the environment for the long term, they have to learn from the waste generated by the products and services they purchase and use in the delivery of care.

health care is a place of healing—and toxic chemicals, excessive packaging, and sheer volumes of waste are not conducive to a healing environment. If we think of purchasing as the tollbooth to an exclusive club, we can control the flow of materials into health care and the environment. now, visualize the purchasing value analysis team as the gatekeeper. A cast of characters outside are jockeying for position—trying to do everything they can to get in the door. It’s up to the gatekeeper to keep the bad ones from entering. So how do we tell the good guys from the bad guys?

We can start with Practice Greenhealth’s Standardized Environmental Questions for Medical Products. The standardized disclosure language was a team effort developed by Practice Greenhealth, Amerinet, healthTrust, MedAssets, novation LLC, and Premier healthcare alliance—with input and review by membership suppliers, hospitals, and health systems. Using a standardized list of product disclosure criteria helps the supply chain respond to the sector’s growing interest in healthier products and services. The disclosure covers chemicals of concern, recyclability, packaging details, waste prevention opportunities, and more. hospitals can pledge to use the disclosure questions on the Practice Greenhealth website.

Goals of the Standardized questionsThe development of the Standardized Questions for Medical Products seeks to achieve several goals:�� Provide a supply chain tool to identify key environmental attributes of concern to the health care sector in order to inform purchasing decisions.�� Facilitating collaboration among the largest purchasers of medical products to accelerate the demand for and increase the purchase of products and services that—based on evidence—reduce environmental and human health impact on patients, staff, and the community.

�� Inform and educate the health care commu-nity—including suppliers, purchasers, health care professionals, and others—about environ-mental and public health concerns important to health care and the communities served.�� Create a consistent platform to reduce the RFI/RFP burden on suppliers. Each purchasing entity remains free to decide what to do with the information obtained in response to the questions.�� Improve the availability of cost-effective and environmentally preferable products and services and reduce the environmental/public health footprint of the health care sector.�� Engage suppliers in the process to inform them of the product attributes that are of the greatest concern and provide them the opportunity for input into refining the questions for additional clarity.

Using the Standardized Environmental Questions for Medical Products is a requirement of the healthier hospitals Initiative’s Smarter Purchasing Challenge. Any hospital enrolled in the Smarter Purchasing Challenge must pledge to use the disclosure language when contracting for medical products.
