Definitions

NEAR MISS: Any undesired event that

under slightly different circumstances, could have resulted in personal harm, property damage, and / or other losses.

FIRST AID INJURY: An injury requiring only first-aid

treatment, normally given by someone other than a physician.

Definitions A MINOR INJURY:

Requiring first aid treatment at“ Health Centre” and resume duty on same or next day and which is non-reportable.

LOST TIME ACCIDENT:An accident that causes to someone disablement of temporary nature or his or her work impaired for more than 48 hours from the time of the accident.

Note: Inform to factory inspector with in 48 hrs of accident.

Management Responsibility

Ensure that all incidents are reported. Create trust and respect. Establish system and procedures. Communicate lessons to all who may benefit. Provide resources and priority. Implement follow-up system to prevent recurrence.

ACTION BY INJURED PERSON

Immediately report about injury/ accident to the concerned supervisor.

If injury is not serious, get the preliminary accident reporting “form B”, duly filled in by the supervisor and reach the health center for treatment.

Action by Immediate Supervisor

On receipt of information of accident rush to site, ascertain the injury and, if required, provide first aid with available facilities at site.

Prepare Preliminary Accident Report in - “Form-B” and send it to Health Centre through a special messenger or through injured person (Hard copy). The same form also to be e-mailed to Medical Officer for further routing.

Action by Immediate Supervisor In case of serious injury immediately send the

patient to health centre and inform the following:>SM / Mgr. Concerned Plant>Medical Officer / Health centre>Fire & Safety Deptt.

Carry out the detailed investigation and prepare report in “Form-C” and send it to Mgr.-F&S through plant in-charge within 24 hours of accident.

Action by Head of Department:

Head of department shall ensure that:

> Injured person has been moved to Health centre and is under proper medical care.

> Accident is reported well in time in Form-B.> Carryout the detailed investigation and prepare necessary corrective action plan, fill in

Form-C and send it to Mgr-F&S.

Action By Medical Officer On receipt of information of accident, send

ambulance to bring the injured to “Health Centre”.

As Soon as injured person reaches health center provide him the medical aid ascertaining the extent of injury. If injured is serious, arrange to send him to Kota for specialized treatment and inform to concerned officials:

From the initial response and report, management can identify incident that require a comprehensive investigation. The report should be

Brief. Limited to known facts (Date & Time, Place of

injury/accident, Age, Sex, what happened and immediate action taken by dept to avoid reoccurrence.

Investigation Strategy Visit Site of Accident / Near Miss site etc. Gather Information From Immediate

Supervisor / Fellow Worker / Eye Witnesses. Search For & Establish Facts. Isolate Essential Contributing Factors. Find Root Causes. Determine Corrective Actions. Implement Proposed Corrective Actions.

Who Should Investigate Accident ?

Immediate Supervisor of Injured Person Because :He Know the Plant / Equipment/ Operating Conditions & Their Method of Work, Risk Involved in Area and Precautions to Be Taken to Avoid Accident / Near Miss Etc.

When Accident Should Be Investigated ?

Accident Should Be Investigated Immediately After the Occurrence or at the Earliest. Delays in Investigations Usually Result in a Failure to Obtain All the Facts.

Report All Accident Whether It Is Minor or Major or Near Miss.

Falling From: Height / Same Level. Caught : in /on/ Between. Contact With: Chemicals / Electricity /

Heat / Cold / Radiation. Struck Against: Stationary / Moving /

Pressure / Vibration. Leakage of toxic/flammable gases. Fire/explosion /dangerous occurrence. Environmental damage etc.

Interview From victims & Witnesses

Interview As Soon As Possible After the Incident. Get Brief Overview of the Situation From Witness

and Victims Interview Each Person Separately. Do Not Allow Witness to Confer Prior to Interview. Put The Person in a comfortable mode and ask open

ended questions like -What did you see? -What happened ?

Reassure them that this is a fact-finding process to prevent reoccurrence in future only.

Gather Evidence

Examine the Accident Site. Look for Things That Will Help You Understanding What Happened:-

- Dents, Cracks, Scrapes ,Spill Etc.-Scattered or Broken Parts Etc.

Plot Side Sketch With Direction, Location of Equipment, Spill, Nos.. Of Person Present at Site, Temperature / Pressure Etc.

Take Side Photograph. Check Past Maintenance & Incident / Accident

Closure & Cost Analysis of Accident

Accident closure shall be initiated by Medical Officer through Form-E

At the time of issuing fitness certificate, Medical Officer shall also fill cost incurred on the treatment including cost of providing Ambulance and additional nursing assistance. if any.

Closure & Cost Analysis of Accident

Medical Officer shall forward the form to concerned SM / Mgr who shall complete the balance entries on the form and send it to Mgr. (F&S) through HOD.

Based on the information provided on Form-E, Mgr. (F&S) shall complete the accident closure and statistical reports.
