TO: ALL PLANHOLDERS OF RECORD Addendum No. 1 to the Construction Bid Package, Drawings and Specifications, dated November 4, 2015, for CLASS Lounge at Main Classroom Building LF-1526, Cleveland State University, 1802 East 25th Street, Cleveland, Ohio; as prepared by Moody Nolan, Inc., 4315 Euclid Ave. Suite 100, Cleveland, OH 44103. This Addendum shall hereby be done and become part of the Contract Documents the same as if originally bound thereto. The following clarifications, amendments, additions, revisions, changes, and modifications change the original Contract Documents only in the amount and to the extent hereinafter specified in this Addendum. Each Bidder shall acknowledge receipt of this Addendum in his proposal or bid. NOTE: Bidders are responsible for becoming familiar with every item of this Addendum. PART 1 - DRAWINGS:

Item 1-1 Sheet G-002: At CODE SUMMARY, GOVERNING BUILDING CODES; “National Electric Code: 2011” should read “National Electric Code: 2014”. This was the only comment raised during the plan review. Plan approval has been granted and the permit will be ready for the Contractor to secure.

Item 1-2 Sheet G-002: At LIST OF ALTERNATES; delete alternates G-9, G-10, and G-11 as this scope of work is now by Owner.

Item 1-3 Sheet A-101: At EQUIPMENT / MILLWORK SCHEDULE ITEM 1, insert “Pronto Top Track, Quantity 1; Bottom Track, Quantity 1; and Dual Profile Track, Quantity 12. Tracks to be installed inside enclosed bulletin board, full width of interior space, per manufacturer’s instructions to display 4” high Pronto Changeable Letters. PART 2 - MISCELLANEOUS: PRE-BID MEETING CLARIFICATIONS

Item 1-11 Telecommunications wiring to be ran in Electrical Metal Tubing (EMT) vertically in the walls and flexible conduit (i.e. Greenfield). Telecommunication wiring above accessible ceilings can be run in open systems (i.e. cable tray or j-hooks).

Item 1-12 Electrical wiring is to run in EMT conduit vertically in walls and above accessible ceilings and flexible conduit above gypsum board ceilings.

Item 1-13 The Electrical Closet 1-261 where panel RPK1 is shown has no ceiling, open to structure above.

Item 1-14 Surface mount conduit/raceways (i.e. Wiremold) is not acceptable. Wiring and cable to be run in walls in conduit.

Item 1-15 Graphic Vinyl Wrap Wallcovering to be cut tight around the existing drinking fountain. No requirement to remove the drinking fountain for wallcovering installation.

Item 1-16 Patching and Cutting of existing wall/ceiling construction for installation of project scope of work is by the Contractor. Any patching and cutting is to be replaced/repaired and finished to match existing adjacencies.

Item 1-17 Vinyl wallcovering materials specified are generally in stock items and lead times for additional material if required is generally no more than 5 business days.

Item 1-18 VW-5, 1/8” thick acrylic to be standard white base material and shop painted with compatible commercial coating, satin finish. Colors are indicated on the drawings.

Item 1-19 VW-5, acrylic striping to be secured to walls with double sided foam adhesive tape as used by sign manufacturers. 3/4" wide tape continuous at stripe ends and 4” strips at 16” centers, staggered top and bottom.

Item 1-20 VW-5, acrylic striping material procurement and fabrication is estimated at 2 weeks. Item 1-21 Existing carpet, present in the space where new carpet is to be installed, is to removed and

disposed of by Contractor. Asbestos containing materials are not present in the area of project scope per Cleveland State University.

Item 1-22 There are 28 different icons and 57 total icons proposed to be installed on the existing glazing. Therefore 27 icons will be used twice and 1 icon will be used for three decals totaling 57 decals. Decals used for the mock-ups can become part of the final installation.

Item 1-23 VW-4 Graphic Icon Decals are to be of clear vinyl, 0.90 pounds per square foot, 0.2 mm thick, semi-permanent, transparent with White weather, fade, and abrasion resistant UV ink.

Item 1-24 It was discussed that the campus will be closed from Christmas Day through New Year’s Day and that it only leaves three weeks for actual on-site construction. There are actually 19 total on-site working days from start date until substantial completion, nearly 4 weeks of on-site construction. PRE-BID MEETING AGENDA Thursday, November 12, 2015 at 1:30 PM

1. Introductions – University Staff and Consultants

2. Sign-In Sheet A copy of this pre-bid agenda and sign-in sheet will be included with the first addendum.

3. Accounting This project uses Local Funds – your check will be used by CSU. We anticipate proceeding with a portion, if not all, of the alternates which would put the cost of construction over

$75,000, the prevailing wage threshold. Prevailing Wage: Bid this project as a prevailing wage job. (IB – Section 6.5 Wage Rates) Requirement also applies

to sub-contractors. Use http://wagehour.bes.state.oh.us/ for prevailing wage procedures, forms, and current rates. Direct prevailing wage questions to Tawanna Norton [email protected] 216-523-7573.

4. Purpose of Pre-Bid Meeting To assist with a complete understanding of the project. To identify and discuss potential problems. If you find any perceived conflicts, error, omission, or discrepancy - submit an RFI for an interpretation or clarification.

These requests should be faxed to CSU’s Purchasing Department 216-687-9361. RFI’s are to be submitted prior to Monday, November 16, 2015 @ 2:00 pm. Proposed substitutions and/or equals are to be submitted no later than Friday, November 13, 2015 (10 days prior to

bid opening). Addenda to the project will be issued Wednesday, November 18, 2015.

5. Bid Requirements

Contractors must note on outside of their bid envelope the project name and project number. Fill in all relevant blank spaces in the Bid Form in ink or typewriter. Bid amounts to be in both words and figures. If information is not applicable, leave it blank or N/A. All Bids must have:

o Attachment A – Required submittals and signature page o Attachment B – Certification form o Attachment C – Contact information o Attachment D – Scope of services o Attachment E – Bid form/Pricing sheet and Acknowledgement of addenda

Bids due: Monday, November 23, 2015 @ 2:00 pm at CSU Purchasing Services, Parker Hannifin Hall, 2258 Euclid Avenue, Room 118. Immediately following the 2pm deadline, there will be a public bid opening in PH 102.

6. EDGE – State or Ohio Encourages Diversity, Growth and Equity Business Development Program EDGE participation goal is a 5% for each project. Bidders on construction contracts must demonstrate actual participation in the EDGE program, or provide a

demonstration of their good faith efforts (with supporting evidence) to participate in the EDGE program. Use Ohio Department of Administrative Services (ODAS) http://eodreporting.oit.ohio.gov/searchEDGE.aspx to search

for certified companies by county, keyword, or procurement category.

7. Safety ANYONE working on campus is required to wear a CSU-issued contractor ID badge. Contractors will receive a

“Contractor Badge Request Form” from their CSU Project Manager with the information at the top pre-filled. The contactor will sign this form and make copies to distribute to employees and subcontractors as needed. This completed form must be presented along with a valid State or Federal photo ID in order to obtain a CSU contractor badge. Badges are valid for the duration of the project. Contractors with ongoing POs can obtain an annual badge.

ANYONE working on campus is required to watch the EH&S Contractor Safety Video. Both the Contractor Video and ID badging take place Monday through Friday from 8am to 10am on the second floor of

Plant Services. Contractors are to schedule appointments with EH&S at (216) 687-9306 or [email protected] Anyone that shows up without an appointment will be turned away.

Contractors are required to abide by the “CSU Contractors Handbook” provided at the video screening. Anyone working on campus without a CSU badge displayed will be asked to leave immediately, and no time

extensions will be granted to the contractor. Contractors will return all badges to their CSU Project Manager upon project completion. Final retention will not be

released, nor final invoices paid, until this is done.

8. Drug Free Work Place This is a State requirement for all companies interested in doing construction contracting with the State of Ohio.

Requirement also applies to sub-contractors. All bidders shall confirm their enrollment on the space provided on the Bid Form. (IB – Paragraph 2.10.3.15)

9. General Information

CSU has a tobacco free campus policy. Parking: Onsite parking, staging, dumpster and storage of materials is not available. Refer to Contractors Handbook

and http://www.csuohio.edu/services/parking/ for additional parking information. Work hours: This is a first shift project. The project is located within and adjacent to occupied buildings on campus.

Work hours will be set by CSU and the Contractor, Monday through Friday. Installation and delivery of materials will need to be coordinated with the Project Manager to work around class/office schedules. It is anticipated that some evening and/or weekend work hours may be required to complete the project.

Building/ Room/ Elevator Access: Contractors shall submit a Keybox Access Request Form and comply with the Keybox Access Rules & Regulations. This information can be found on the AC&SS website: http://www.csuohio.edu/access-security/ access-security

10. Project Scope (by Consultants) Outline of the project/scope of work:

Changes since the drawings were issued that will be picked up in the first addendum: Carpet will be purchased by CSU prior to contract award, GC to provide labor. Monitors and associated IT/AV components beyond rough-in will be removed from GC’s scope of work.

Architect has applied for permit, and CSU has paid application fee. If required, contractor is responsible for any FA/sprinkler work. Shutdowns to be coordinated with CSU Fire Inspector. Project Construction Length: 30 consecutive days after Notice to Proceed. Anticipated construction start date: Monday, December 14, 2015. Anticipated date of Substantial Completion: Friday, January 15, 2016. It is possible that the CSU campus will be closed from December 25, 2015 through January 1, 2016.

These requirements apply to remodeling projects, in which the building contains existing Cat5e structured cabling

The Data/Telecommunications contractor shall be fully capable and experienced in the

Information transport system specified. To insure the system has continued support, CSU will contract only with Contractors having a successful history of sales, installation, and support.

NFPA COMPLIANCE Contractor must comply with NFPA 70, national, state and local codes as applicable to wiring methods, construction and installation of data and communication cabling systems, and as specified in the BICSI TDM. Any deviations from the products contained in this specification shall be noted and shall be submitted for approval prior to installation. Payment for work will not be approved without this authorization. All four cable pairs shall terminate in a single 8-position modular jack at the work area, telecommunications outlet. The maximum horizontal distance to the farthest workstation shall be limited to 90 m (295 ft), as specified in the ANSI/EIA/TIA 568A standard document. The installation of the horizontal voice and data cables shall follow the appropriate recommendations covered in the Manufacturer's Design Guide and the appropriate standards documents. This shall be done in order to ensure adequate protection against Electro-Magnetic Interference (EMI) sources and ensure that all components and cables are in good condition after installation.

The cable shall be Category-5e, Plenum Rated (unless otherwise noted) and comply with the EIA/TIA 568 Specifications for Category-5e cables. HORIZONTAL USER CABLES Horizontal Data cables for all new installations shall be four pair, plenum-rated, Category-5e as specified: Bertek 10032223/5EP4P24-WH-P-BER-AP White Plenum unless otherwise specified by Cleveland State University. Yellow horizontal data cable shall be used for to serve energy management data jacks. Horizontal Voice Cable Same as horizontal data cables (Terminate in voice patch panel separate from data where applicable.) All cable must be new and free from defects. Unless specified, data communication outlet hardware shall be as specified herein. The work area telecommunications outlet/connector is the interface between the horizontal wiring and the work area telecommunications device. Each work area shall be supplied with a telecommunications outlet/connector (called a modular jack) for connection to the horizontal media. All telecommunications outlet/connectors shall be installed with an appropriate faceplate. All telecommunications outlet/connectors shall be complete with faceplate and attached permanently to a fixed structure, such as building walls, utility poles or modular furniture partitions. Surface raceway or boxes must be fastened to the building surface using mechanical means. Adhesive tapes shall not be used as the sole means of attachment of boxes or raceways or wire supports. Faceplates shall be clean in appearance. Mounting hardware shall not be visible on the faceplate. No jacks shall be left open, hanging or in a state of disrepair. Any discrepancies shall be brought to the immediate attention of CSU contact person before proceeding with the installation. Backbone Pathways Where increased fill in new or existing pathways takes place the appropriately sized inner ducts with installed pull strings shall be installed and identified as to the use. Example: An empty 4” trade size conduit will require a quantity of 2-1 1/2” and a 1” inner ducts, depending on the cable to be pulled, installed before any of the duct may be used. A permanent identifying placard must be placed at the ends and entry points to the duct. Choose the appropriate combination of inner duct for the application and pathway. Digitally photograph and document each of pathway entrance and submit with “as-built documentation”. Horizontal Pathways:

A permanent identifying placard must be placed at the ends and entry points to the duct. Digitally photograph and document each of pathway entrance and submit with “as-built documentation”. SURFACE MOUNT RACEWAY Surface raceway or boxes must fasten to the building surface using mechanical means. Adhesive tapes shall not be used as the sole means of attachment of boxes or raceways. Unless otherwise specified and/or with prior approval, in areas requiring the data/telecommunications faceplate assembly to be surface mounted on a non-fishable wall, a single or double gang deep (1-7/8” minimum) surface mounted box, office white in color, shall be utilized. Coordinate the raceway, associated couplings and fittings and surface mounted box. No exposed openings or rough edges shall be present. Utilize matching Office White flush-mounted faceplates as specified above with the appropriate fittings. Modular Furniture Cabling Contractor shall coordinate the location of all modular partition workstation outlets with the architectural furniture layouts and CSU contact person. Any discrepancies shall be brought to the immediate attention of CSU contact person before proceeding with the installation. Labeling Contractor to install all faceplate and equipment labels in accordance with the specifications. All labels shall be neatly installed and shall be level with the floor and properly aligned on the faceplate. Labels shall be located in the manufacturer designated label location on the faceplate. A minimum of a 10 point Sanserif (or equal) type font shall be used. Submit sample before installing labeling. All pieces of voice and data equipment, including wires, cables and their respective terminations shall be labeled and identified according to CSU cabling specifications. Wire and cable identification shall consist of High-Performance materials such as Brady self laminating cable labels. These labels must withstand the requirements of UL 969 as outlined in the TIA Standard. Self-adhesive self laminating wire labels other than Brady Brand shall be pre-approved. All horizontal copper cables shall be labeled at each end with the matching face plate label number. At the faceplate end of the cable, the labels shall be neatly installed

within 3-6 inches of the associated voice/data module; shall be neatly aligned and clearly marked; and shall not be visible from the outside of the faceplate or surface mounted box assembly. Cables incorrectly labeled or not matching its respective faceplate termination will not be accepted. Additional labeling may be required at intermediate locations such as conduit ends, and pull boxes. Do not install the label closer than 3” to the termination point. Patch panel labels shall be made on green paper or card stock. Patch panel labels shall be computer generated using the appropriate template for the style of patch panel using the manufacturer’s supplied label sleeve. No hand written labels will be accepted. Cross-Connections and Closet Wiring. Cross-connections at the data patch panels to the CSU provided network electronics shall be supplied and installed by the contractor as follows: Gray category 5e patch cable for data unless otherwise approved. Patch cables containing power such as used for Power Over Ethernet shall be Red. Green cross connect cables shall be used for voice cross connects at patch panels. “Velcro” type cable ties shall be used in the closets. Cable supports or ties shall not deform wire or contain any sharp edges. Contractor shall supply and install all cross connect wiring at the patch panels using appropriate length cables for each cross connect. Work area Cables – Supply a gray 3 meter category 5e cable for each work area data jack as specified by the connectivity manufacturer and warranty requirements. Unused work area jacks shall the manufacturer’s dust cover installed. Upon completion of the cable installation, the Contractor shall perform complete copper cable certification tests, including but not limited to: Continuity checks on each cable, checking for opens and shorts. Cable length Correct pair polarity Correct cable labeling at both ends Tests shall be performed with connectors installed. The Contractor shall prepare complete cable test reports in electronic and printed form for all installed cables Category-5e U.T.P. cable installations shall be fully tested and verified in accordance with EIA/TIA-568 specifications. An ETL approved Cable Tester such as the FLUKE DSX-5000 or equivalent shall be utilized for certification testing test. As a minimum, the Category-5e Horizontal Cable Certification report shall include:

TYPICAL VOICE/DATA FLUSH MOUNTED Unless otherwise specified on the drawings, the standard Voice/Data Flush-Mounted outlet plate shall Hubbell IFP OW Series “Office White” one gang flush-mounted for up to three (3) communication jacks and two gang for four (4) or more communications jacks. General Notes: For those locations where cables pass through metal studs or knock outs or areas where damage to the cable is possible, the Contractor shall install a bushings, conduit sleeves and/or grommets to prevent damage to the cables. Contractor must notify CSU IS&T Dept. before removing existing data and telecomm jacks. Note ID number of jacks removed and report this number in writing to the Department of IS&T. New data jack sequence number assignments will be provided by CSU. Contractor must label per labeling specifications. Unused faceplate or openings must be blanked off using appropriate manufacturers part of the same color as faceplate or mounting frame.

All cables shall be installed as single continuous "home-run" pulls from connector block to connector block, or workstation data/telecommunication outlet to patch panel. No "in-line" connectors or splices, etc. will be permitted. All cable should be supported by manufacturer’s recommended method, approved cable tray, “J” hooks or channel hangers. In addition, cables should be run above the bottom of all-metal framing such as floor joists or trusses for the next floor or roof above, when applicable. The maximum spacing of Cable Hangers shall be 48”. Any discrepancies shall be brought to the immediate attention of CSU contact person before proceeding with any installation. All pathways must remain accessible for MOVES, ADDS, & CHANGES without any cutting or unusual construction after the building finishes are complete. Rack Equipment: Rack equipment shall be Hubbell “I Frame” series with Vertical management wireways containing a min of 25 square inch fill capacity between each rack. Appropriate horizontal wire managers must be used above and below copper patch panels. Top and side ladder style cable tray shall be included with each rack. An appropriate wire manager shall be supplied immediately above and below every patch panel and rack crossing point. Matching Rittall rack equipment should be used in existing data closets where Rittall is already installed. Electrical: Each rack must have electrical power receptacles designed for the equipment installed. (Consult with the equipment manufacturer). In addition to these receptacles each rack shall have “Plug mold” style permanently installed receptacles installed on the rear of the side wire management ladder. An electric sub-panel that supplies only these equipment receptacles shall be inside each equipment room and be readily accessible to IS & T department qualified employees or designees. Twenty ampere general purpose receptacles fed from an alternate source must be located every 6ft around the perimeter of the ER, TR or HC rooms. All receptacle circuits shall have dedicated neutrals and a supplemental equipment grounding conductor sized per NEC 70. Raceways alone shall not be considered a sufficient equipment grounding conductor. A equipment bonding system installed per TIA/EIA 607 (ANSI/EIA/TIA/607) shall be included in all telecommunications rooms and areas. Grounding and Lightning protection: All communications cabling, pathways, and equipment shall be installed in accordance with but not limited to articles 770, 800, 820, and 830 of the National Electrical Code (NFPA 70) and referenced articles therein. Other reference standards that may apply include but not limited to NFPA 780, UL 96. UL 96A, UL 467, UL 497, 497A, UL 497B, UL 1449.

An equipment bonding system installed per ANSI/TIA/EIA 607 (JSTD607) shall be included in all telecommunications rooms and areas. This includes bonding of all equipment racks and enclosures. Include properly installed rack grounding strips or equal for grounding of active electronics. Circa wall or rack mount primary and secondary protectors or equal shall be used where the code requires them. Submit product cut sheets per project before proceeding with installation. Cooling: All rooms containing active electronic telecommunications or data equipment shall have a cooling/humidity control system independent of the building HVAC systems per applicable codes and standards. All equipment rooms shall have environmental remote monitoring systems installed to notify directly the department responsible for the service of the HVAC units. Patch Panels Backward compatible/interoperable with all lower-performing category products required. Fiber: Fiber connectivity equipment must be Hubbell. Single mode fiber shall be Corning or OCC indoor/outdoor rated fiber terminated with FC connectors tested to the highest network standards bit rate at the time of installation. All fiber must be fusion spiced with a maximum loss of .3 db loss per connector pair. Multi mode fiber shall be 62.5 micron Corning or OCC indoor/ outdoor rated fiber terminated with ST connectors tested to the highest network standards bit rate at the time of installation. All multimode fiber must be spliced with a maximum loss of .3 db loss per connector pair.
