AUGUST 2015 I n each case, were it not for the most advanced technology available at Saint Peter’s University Hospital, a thorough diagnosis would not have been made quickly. The 72-year-old woman was thought to be in good health until she was hospitalized after suffering severe abdominal pain. A CAT scan and an MRI revealed inflammation of the pancreas, but it wasn’t until gastroenter- ologist Arkady Broder, MD, performed an advanced therapeutic endoscopy procedure called endoscopic ultrasound that the cause of the patient’s pancreatitis was discovered. It was a suspicious mass that Dr. Broder was able to biopsy during the procedure, allowing a pathologist to diagnose an early, and still curable pancreatic cancer. Another patient also suffered from abdominal pain. Blood tests showed that the 36-year-old woman, who had only recently given birth, was suffering from inflammation of the liver. Advanced therapeutic endoscopy made it possible for Dr. Broder, who is specially trained in the procedure, to find the medical problem – a gallstone had escaped the gallbladder and became lodged in a duct, making it impossible for bile to drain from the liver. Dr. Broder was then able to remove the gallstone during the same session and restore the natural flow of bile. Advanced therapeutic endoscopy is a subspecialty of gastroenterology that expands the minimally invasive techniques doctors use to examine the internal organs of the digestive system. “Typically endoscopy or colonoscopy is used for diagnosis, and so, is intended purely to visualize and biopsy a part of the gastrointestinal tract to aid in diagnosis,” Dr. Broder explains. “Advanced thera- peutic endoscopy takes this concept a step further, by allowing a specially trained gastroenterologist to see things not typically seen by a CAT scan or an MRI and offer therapy and early diagnosis for conditions such as bleeding ulcers, colon polyps, strictures in the intestinal tract, gallstone disease, and even pancreatic cancer.” Endoscopic technology Using a long, thin, flexible tube called an endoscope, which features a light and video camera to transmit images to a screen, gastroenterologists capture detailed video of a patient’s digestive tract and identify abnormalities such as tumors, ulcers, polyps, gallstones, and common bile duct stones. Spyglass is a very small camera that can be inserted into the bile ducts, liver ducts and pancreatic ducts in order to very closely investigate and provide treatment for otherwise difficult to treat pancreatic- biliary disease. In the case of the new mom, advanced therapeutic endoscopy made it possible for the stone to be removed from the duct without surgery. Observation, diagnosis and treatment can be performed during one procedure thanks to the technology available and the skill of a highly trained, specialized gastroenterologist such as Dr. Broder, and a team of health- care providers that includes experienced technicians, nurses, pathologists, and surgeons. “It’s the technology that makes it possible for the physician to see not just the intestines, but also through the wall of an organ such as the stomach to the adjoining organs,” says Dr. Broder. “Advanced therapeutic endoscopy allows me to use the most cutting edge endoscopic technologies to provide both difficult diagnoses and therapies in a more precise and minimally invasive fashion.” Endoscopic ultrasound is one of the three procedures made possible by the technology used in advanced therapeutic endoscopy. A trained gastroen- terologist (GI) can also perform: Endoscopic mucosal resection - A technique that allows a specially trained GI to remove large and potentially pre- malignant polyps or growths from within the gastrointestinal tract. This minimally invasive technique allows doctors to resect polyps – considered early tumors – within the gastrointestinal tract and poten- tially avoid a major surgical procedure. Endoscopic Retrograde Cholangio- pancreatogram (ERCP) - Using a specially designed endoscope, a trained GI can provide therapy for a wide range of bile duct and pancreas-related disease. In particular, bile duct and pancreatic duct stones and strictures can be both treated and more definitely diagnosed using this technique.
