The selected applicant will carry out project activities related to the following objectives: Goal 1, Objective 1: (Infrastructure) Contractors will complete and submit all required reports and forms on or before each deadline and participate in a minimum of five Network sponsored community events and trainings throughout the fiscal year. Goal 3, Objective 2: Diffuse lessons learned by new AMII afterschool model programs to agencies in CDE afterschool regions that are un-served or under-served with SNAP-Ed afterschool interventions. Goal 3, Objective 3: Strengthen the role performed by the AMII afterschool model program sites in educating and providing peer-support to other afterschool programs through technical assistance and regional meetings. Goal 3, Objective 4: Strengthen the infrastructure for afterschool leadership; help leverage the power of stakeholder organizations in all spheres of influence; increase the adoption of afterschool model interventions; publicize and increase the resources available to afterschool programs to implement model interventions in nutrition education, physical activity and food security. Goal 3, Objective 5: Build the evidence base for the impact of the afterschool model interventions on children, youth and afterschool staff. The total sub-contact amount for this project is $230,000. Indirect rate may not exceed 25%. Description of Project Activities, Deliverables, and Timeline: Required activities, deliverables and timelines are described at the end of this document.

Request for Application – Sub-Contract Project Overview: In collaboration with the California Department of Public Health (SNAP-Ed) and in sub-contract with the UC Davis School of Education, Center for Community School Partnerships this RFP represents an opportunity to enhance the Afterschool Model Interventions Initiative (AMII) to help achieve the Network for a Healthy California's (Network) goal of increasing fruit and vegetable consumption, physical activity and access to healthy food in families with children who are eligible for SNAP-Ed in at least 1,000 qualifying low income venues. Who Should Apply? One eligible organization will be selected to be the lead sub-contractor on the AMII project. Eligible organizations are, community based organizations, or public agencies that currently involved with the Network for a Healthy California's (Network) goals of increasing fruit and vegetable consumption, physical activity and access to healthy food in families with children who are eligible for SNAP-Ed. Organizations should work closely with their after school providers, school districts and other organizations and public agencies in this effort. Please be aware of the following guidelines:

1. All Network-funded programs must provide income targeting data for the populations served with the USDA NEOP funding. The income targeting data source will verify that your target audience meets the USDA NEOP funding guidelines, which is at or below 185 percent of the FPL. The Network’s Geographic Information System (GIS) mapping tool is located at http://www.cnngis.org/ . This tool will assist you in identifying qualifying census tract data.

2. Please ensure all activities are targeting the Network population and are allowable expenses. Here

3. Awarded funds must be spent within contract year (by September 30, 2012).

4. If travel is required, it should be included in the budget and the reimbursement rates are State DPA rates under USDA funding.
