What is a Continuum of

Care?

Continuum of Care (CoC)?A Continuum of Care (CoC) is a regional or local

planning body that coordinates housing and services funding

for homeless families and individuals.Organizations such as: nonprofit homeless

providers, victim service providers, faith-based organizations, governments, businesses, advocates, public housing agencies, school districts, social service providers, mental health agencies, hospitals, universities, affordable housing developers, law enforcement, organizations that serve homeless and formerly homeless veterans and homeless and formerly homeless persons.

U.S. Dept. of Housing and Urban Development (HUD) Guidance

Continuum of Care Approach

Participation: positive, meaningful, broad, and representative participation.

Transparency: selection process, criteria, and award are clear and available. A clear, written conflict of interest policy will be used to select committee members.

Performance: deliverables will meet criteria and be met on a timely basis.

CoC Responsibilities

Big Picture:

Define performance monitoring and evaluation process

Develop Coordinated Entry System Draft written standards for providing CoC

assistance Designate and operate an HMIS Conduct regular/annual CoC Plan – includes Point-

in-Time count Manage an annual planning process Manage annual CoC funding application

Must be a legal entity Must be an eligible applicant Must be a single HMIS designee

Written agreement between CoC and HMIS Lead is required

regarding local and regional homeless initiatives as well as feedback and recommendations regarding CoC initiatives.

Priority areas identified for 2015 are Coordinated Entry and Assessment, Housing, Prevention, and Mainstream Benefits

Accomplishments 2014 - 15: Convened stakeholders from the seven counties on a

monthly basis Provided input to MDHI Board, Leadership Team and

Community Design Team regarding Coordinated Assessment policies and procedures, future staffing structure, and more

Employment CommitteeGoal and Purpose: Increase and strengthen employment outcomes for

homeless adults, youth and families Identify strong employment opportunities for

homeless job-seekers with significant barriers Develop a platform to learn and implement “best

practices” as employers, providers & public agenciesAccomplishments 2014 - 15: Created an employment network for the CoC Convened 2 employment forums in Boulder and Denver Developed an employment training for all CoC grantees Met twice per month with at least 12 homeless

employment service providers in 3 counties Identified at least 6 employers who want to participateCurrent and Planned Activities for 2015 – 16: Host at least one additional employment training for CoC

Newly created in 2015:Vision Statement: “The agencies serving the homeless population in the CoC

will have a single data resource that allows all who are served to register once and to then be admitted to all services with a basic intake survey.  Records will be available to track an individual’s trajectory through the system and to report on agency utilizations.  Our vision is to establish a concept of data-centric rather than systems-centric operations to meet the goals of the CoC.”

and formerly homeless.

Committee reviews CoC project applications and provides funding recommendation to MDHI Board

of Directors for submission to HUD.
