License Application: The application is available to view

online. Traveler will need to print, fill out, and have

1. Sing the authorization for release of records form (attached to application)

2. Verification of licensure submitted directly to the Division from each jurisdiction where you hold or have

ever held a license or permit to practice occupational therapy, one of which must be current and in good

standing. Electronic verification of licensures will be accepted if they are sent directly from the licensing

jurisdiction.

3. Verification of initial Certification by the National Board for Certification in Occupational Therapy, Inc.

(NBCOT) mailed directly to the State of Alaska from NBCOT.

4. Professional reference form completed and submitted directly to the Division by a qualifying source;

physician, instructor, supervisor, or official of your school of occupational therapy.

5. Verification of one of the following and submitted directly to the Division from the source:

o Performed at least 60 hours of occupational therapy service within the 24 months

immediately preceding the date the application is received by the department

o Passage scores for the NBCOT/NBCOTA exam

o 150 hour internship approved by the Board (contact the Division for further instructions

regarding the internship)

6. Jurisprudence questionnaire (form attached to application)

License Fee: A $150.00 application fee and $240.00 license fee is due for OT’s. A $150.00 application fee and

$175.00 license fee is due for COTA’s. Fees can be paid by check or money order, make payable to the State of

Alaska.

License Time: Per information given from the board, one should plan 4-6 weeks for their license to be processed.

Process could take up to eight weeks depending on if all documents are submitted correctly and in a timely

manner. Board meeting dates are available under the Board Meeting Dates and Agendas tab on their website.

License Duration: All licenses issued by the board expire on June 30 of even-numbered years, regardless of

when initially issued. New licenses issued within 90 days of the current biennium expiration date will be issued

the next biennium.

License Renewal: License renewal can be completed online. Before a renewal license is granted all OT’s and

COTA’s must complete 24 contact hours of continuing education. All applicants must complete at least 12 of

required contact hours in courses or programs offered by an accredited academic institution or professional

organization approved by the board. A $240.00 renewal fee is due for OT’s and $175.00 renewal fee is due for

COTA’s.

online. Traveler will need to print, fill out, and have completed

1. Request NBCOT verification of certification be sent directly from NBCOT to the AZ Board. This can be

completed online through NBCOT’s website.

2. Submit two Professional Recommendation Forms. Fill out the top part of the form and forward to your

two professional references. They must complete the bottom part of the form and mail it back to the

board. Faxed forms will not be accepted.

3. Submit a completed fingerprint card to the board. A card can be obtained from a local law enforcement

office or the board office. Your local police or sheriff department generally has an authorized finger

printer. Take the board’s Fingerprint Instructions to the appointment with you. Your application will not

be reviewed until your fingerprint report is received back from the AZ Department of Public Safety.

Please allow up to three weeks for the card to be received.

4. Request certified letters of good standing be sent to the board from all state licensing jurisdictions in

which you currently hold or have ever held a license. Letters must be sent directly to the board from the

state licensing jurisdiction.

5. Complete the Statement of Citizenship Form and attach to you application. For this form to be considered

in the application process, the board requires both the signed and completed form along with a copy of the

document(s) being used to verify employment eligibility.

License Fee: A $100.00 application fee and $22.00 fingerprint processing fee is due for both OT’s and COTA’s.

The OT license fee is $135.00 and $70.00 for COTA’s. Fees can only be paid by money order or cashier’s check,

make payable to Arizona Board of Occupational Therapy. All fees are non-refundable. Payment for application

must include the fingerprint processing fee.

License Time: Licenses must be approved by the board and can only be done at board meetings. The board meets

on the 2nd

Friday of every month so documentation needs to be submitted in full for them to review your

application at that time. All documentation must be submitted in full for application to be reviewed at the next

board meeting. On average, it takes 2-3 weeks for the application to be processed.

License Duration: Licenses are valid for two years. A license will expire exactly two years from the date it was

granted on.

License Renewal: For license renewal, a renewal application must be submitted to the board. Renewal fee for

OT’s is $135.00 and for COTA’s it’s $70.00. Fees can only be paid by money order or cashier’s check, make

payable to Arizona Board of Occupational Therapy. All fees are non-refundable. Licensees must complete

continuing education before a renewed license will be granted. OT’s must obtain 20 clock-hours of continuing

education for every two year licensing period. COTA’s must obtain 12 clock-hours of continuing education for

Traveler will need to print, fill out, and have completed application

1. Attach a passport style photo to the application

2. Submit a copy of your driver’s license or passport

3. Submit a copy of your current résumé

4. Request NBCOT Verification of Certification be sent directly from NBCOT to the Arkansas Board.

5. Request your NBCOT score report be sent directly from NBCOT to the Arkansas Board.

6. Request an official transcript be sent directly to the board from your college/university. Transcript must

include degree earned and graduation date.

7. Request verification of licensure from every state where you hold or have held a license. Verification

must be sent directly from the state licensing jurisdiction to the Arkansas board. You are responsible for

requesting these documents. All verifications can be faxed or emailed unless specifically requested to be

mailed. Email all documents in PDF format to: [email protected], “Attn: Licensing” in the

subject line.

8. (If applicable) Submit verification of military service. Attach copy of DD form 214, Certificate of Release

or Discharge from Active Duty, if you have served in any branch of the U.S. Armed Forces since

OT/COTA school.

9. Submit verification of Employment (non-therapy)

10. Upon receipt of your complete application and fees by the board, a criminal background check packet will

be mailed to you including instructions on how to complete the background check. You will need to

complete these forms and mail the completed packet directly to the board. It is imperative that the

completed packet be returned in a timely manner as failure to do so will delay licensure. The federal

portion of the background check can take 4-6 weeks or more to process. Payment for the CBC must also

be made by money order.

License Fee: A $75.00 licensure fee is due for OT’s and a $50.00 licensure fee is due for COTA’s. Fees can be

paid by check or money order, make payable to Arkansas State Medical Board.

License Time: Per the board, once all paperwork is received for the application, it takes 2-3 days on average for

them to process and issue the license. Allow 4-8 weeks to complete the entire process and receive a license. If any

paperwork is missing or incomplete, the license process could take longer. Per the board, delays in the process are

usually due to background check results coming back. Board meeting dates and time are posted on the Arkansas

Medical Board’s website under the public links tab, scroll down to the scheduled meetings tab.

License Duration: Licenses are valid for one year. Your license will expire on the last day of your birth month. If

you want to renew, you will have to apply before the last day.

License Renewal: License renewal is available online. You will receive a renewal reminder at least 30 days

before expiration. 10 hours of continuing education units are required per renewal period. The CEU’s must have

been obtained in the year preceding your license expiration date. If audited, certificates of completion are required

to be submitted.

License Application: The application is available to view online. Traveler will need to create an online account with

California Breeze to complete the application process. Paper applications are available but can delay the licensing process by

up to seven months. Additional information required for online application:

1. Verification of all current/expired OT or COTA licenses from all the state boards for which you hold or have held a

license. Traveler must be the one to request a letter of good standing and have the jurisdiction send the verification

directly to the board.

2. Submit an official transcript from the qualifying degree program reflecting fulfillment of the academic and

fieldwork requirements. Diplomas will not be accepted.

3. Submit Verification of Certification or Score Transfer Report from NBCOT. Forms can be requested on NBCOT’s

website.

4. Submit fingerprints via fingerprinting card. To request a fingerprint card, please contact the board via email or

over the phone. Include your name, mailing address, and phone number. If you do not provide the required

information, your request will not be processed. Please allow up to 10 business days to receive the fingerprint card

in the mail. Once the card is received you will need to go to a local location to have your prints taken.

5. (If resident or visiting California) Submit fingerprints via Live Scan. Using Live Scan speeds up the application and

license process. Live Scanning for the California Board can only be done in California. Fingerprints are needed for

the DOJ and FBI. The website for this is: https://www.identogo.com/services/live-scan-fingerprinting

o Before going to the Live Scan location, you must print out three “Request for Live Scan Service” forms

that are available on the California Board’s website. You should also ensure the operator checks both boxes

for the DOJ and the FBI. The form needed to take with to the Live Scan location can be found here:

6. Submit a 2x2 passport style photo, taken within the last six months.

License Fee: A $50.00 application fee is due for OT’s/COTA’s. In addition to the application fee, a $49.00 fingerprint

processing fee is due at the time of scanning. Once the application has been approved, board staff will notify the traveler the

amount of the initial license fee. Fees can be paid through the online system.

License Time: Processing time for all applications is 4-6 weeks.

License Duration: The term of a license or certificate is two years. Licenses and certificates expire at midnight on the last

day of your birth month in odd or even years depending on your birth year.

License Renewal: To renew a license, you must submit a renewal form online with a renewal fee. In order to renew your

license, you must have proof of: certification that you have completed the required number of professional development

units of continuing education and certification that you have or have not been convicted of a crime or disciplined by another

state since your last renewal. Each licensee must submit evidence of completing 24 professional development units during the

preceding renewal period. One hour of participation in professional development activity equals 10 PDU’s. One academic

credit equals 10 PDU’s and one continuing education unit equals 10 PDU’s. The renewal process takes 4-6 weeks on

average. You can apply for renewal up to 90 days prior to your current license expiration date.

License Application: The application is available online. Traveler will need to create an account with the

Colorado Division of Professions and Occupations Online Services to complete the application process. Most

supporting documentation can be found with the online application. Documents needed in addition to application:

1. Verify all licenses and certifications you hold or have held to practice occupational therapy or a related

profession. The board prefers the traveler provide a scanned copy of verification from the other states

jurisdiction website at the time of application. Otherwise, traveler will have to arrange for verification to

be sent to the Office of Licensing at the mailing address provided at the top of this form. The verification

must indicate whether or not you have ever had disciplinary action taken against your

license/certification.

2. Must have successfully completed an educational program accredited by the ACOTE or the WFOT.

Additionally, you must have a minimum of 24 weeks of supervised fieldwork that meet generally

recognized past accreditation standards in effect at the time you graduated.

3. Submit proof of NBCOT certification. Verification of certification must be sent directly to the board from

NBCOT. This can be completed on through their website.

4. Complete Affidavit of Eligibility.

5. Provide social security number.

6. Answer the screening questions asked on the application.

7. Answer the Healthcare Professions Profiling Program questions asked on the application.

License Fee: A $60.00 non-refundable application processing fee is due for OT’s and a $25.00 fee is due for

COTA’s. Fees can be paid with a credit card or electronic check through the online application website.

License Time: On average it takes 5-7 weeks to receive a Colorado license.

License Duration: Licenses are valid for two years. All occupational therapy licenses expire on December 31st of

even-numbered years and must be renewed to continue practicing.

License Renewal: Licenses are available for renewal six weeks prior to the expiration date. Renewals can be

completed online. All OT’s and COTA’s are required to meet the CO Board’s continuing professional

competency requirements. There are three ways to comply with the requirements: participation in the CO Board’s

licensure who are licensed in other states shall arrange for an official NBCOT Certification

Verification Letter be submitted directly to the licensure office.

3. Verify all licenses and certifications you hold or have held to practice occupational therapy or a

related profession. Verification must be sent directly from the state licensing authority to the CT

licensing office.

License Fee: A $200.00 non-refundable application fee is due for OT’s and COTA’s. Fees can be paid by money

order or check, make payable to Treasurer, State of Connecticut. Payment may also be accepted through the

online account created for the application.

License Time: On average it takes 3-6 weeks to receive a license. Board meeting dates and times are

posted on the Department of Public Health website under the meetings and agendas link at the bottom of

the page.

License Duration: Licenses are valid for two years. All OT/COTA licenses expire biennially on July 31 of odd

numbered years.

License Application: The application is available to view online. The

To obtain the license traveler will need to:

1. Submit two identical 2x2 passport size photos on a plain

background. The photos must be original and cannot be

copies

2. Submit official verification from NBCOT confirming you

have passed the examination. Confirmation must be sent

directly to the board from NBCOT.

3. Verify all licenses and certifications you hold or have held to practice occupational therapy or a related

profession. An official letter of verification must be sent directly from the state licensing authority to the

D.C. Board.

4. Submit one clear photocopy of a government issued ID front and back, such as a valid driver’s license or

passport.

5. Obtain fingerprints for criminal background check to be completed. Applicants who live outside of the

D.C. area must use L-1 Enrollment Services Card Scan Processing Program. This program converts hard

card fingerprints into an electronic fingerprint record. You must contact the board and request an FBI

applicant card be sent to you. You must then go to a local law enforcement office to have your

fingerprints taken. After these two steps have been completed follow the directions provided on the DC

DOH Card Scan Processing Procedures Form. This will give you further instructions on how to get the

hard card converted to an electronic copy.

License Fee: An $85.00 application fee and $179.00 license fee are due for OT’s and COTA’s. Fees can be paid

by check or money order at the time of application, make payable to D.C. Treasurer. The application fee is non-

refundable. The license fee is refundable in the event of final denial of a license or a request from an applicant to

withdraw their application. A $50.00 criminal background fee is due for OT’s and COTA’s. Criminal background

3. If you have passed the NBCOT certification exam, you must state on the application whether or not you

have verified that your certification information is available electronically on the NBCOT Verification of

meeting.

License Renewal: License renewal can be completed online. You many renew your license during the renewal

period. The board required all licenses OT’s and COTA’s complete a minimum of 24 contact hours of continuing

education each biennium. At least 14 of the hours must relate to direct hands-on patient care. Up to 12 hours may

be obtained using on-line or home study courses. The board also requires up to 2 hours of ethics continuing

education be completed every two years.

A limited permit is a letter of authority to work under supervision and it is not the same as a license. Applicants

waiting to take the NBCOT Exam may be issued a limited permit upon receipts of their application for licensure,

2. Complete the Maryland Jurisprudence Exam. The exam is 27 questions, open-book. Applicant

must receive 100% to pass. When completed email the form back to the board.

Complete the top part of the form and send it to the state jurisdictions. Once completed, it must

be emailed directly from the state board to the Maryland board.

4. Submit verification of NBCOT certification. This can be completed online through NBCOT’s

5. If you are currently NBCOT certified the continued competency requirement is waived for initial

licensure.

6. Attach a 2x2 passport style photo to the application.

7. Have page two of the application notarized.

License Fee: A $200.00 license fee is due at the time of application. Fees can be paid by check or money order,

License Time: The board meets on the third Friday of every month but does not necessarily need to review

your application at a meeting in order to process it. If all documentation is submitted correctly and in a timely

manner a board member may be able to grant you a license during the interim time between meetings. On

License Duration: Licenses are valid for two years. All licenses expire on June 30. The expiration year

for your license will be determined based on the year you were initially granted a license.

5. Submit two completed Certificate of Good Moral Character forms (attached to the application)

6. If a different name appears on documents submitted as part of the application or if your name has been

changed due to marriage, divorce, or any other legal proceeding. You must submit a copy of the

7. Submit verification from every state in which you hold or have ever held a license. An official letter of

verification must be sent directly to the board from the licensing jurisdiction. The verification form to

send to the state jurisdictions is attached to the application. It must include whether any disciplinary

action was taken against the license or other authorization to practice. Once the form is completed, the

licensing jurisdiction must mail the form directly to the NJ Board.

8. Once your application is submitted, the board will mail you information regarding obtaining a criminal

background check.

License Fee: A $100.00 application fee is due at the time of application. For OT’s the initial license fee is

$160.00 if application is completed during the first year of the biennial period and $80.00 if completed during the

second year of the biennial period. The initial license fee for COTA’s is $100.00 if application is completed

during the first year of the biennial period and $50.00 if completed during the second year of the biennial period.

Fees can be paid by check or money order, make payable to State of N.J. Occupational Therapy Advisory

Council.

5. Submit two letters of reference. The reference form can be accessed from the board’s website under the

“Requirements for Licensure” tab. Print the form and have references complete it. You will then need to upload it

with your online application. This is a required form and your application will not be processed without it. You must

have one OT/COTA complete a reference form and the other can be a personal reference.

6. Take and pass the ND Jurisprudence Exam. This is an exam over the laws and rules governing OT Practice in ND. It

is 20 questions and is open book. An 80% or higher is considered passing. You will need a copy of the Law and

Rules available from the Board office or refer to the Law & Rules page. The exam does not cover the AOTA Code

of Ethics at the bottom of the page. This is not required for those applying for a license by reciprocity from another

state.

License Fee: Initial license and application fees are based upon when your application is received by the board. If

applications are received after 06/30 of an even year and before 07/01 of the next odd year, the fee for OT’s is $200.00 and

$160.00 for COTA’s. If applications are received after 06/30 of an odd numbered year and on or before 12/31 of the same

**Aureus Medical recommends obtaining an Ohio license before

accepting a travel assignment due to the licensing process being so

extensive. Some facilities will wait for you to obtain a license, but this does not happen very often**

License Application: The application is available to complete online. Online applications are required and electronic

submission of all supporting documentation is required. The board can be difficult to get a hold of so please plan

accordingly. Supporting documents required in addition to the application:

1. All individuals applying for licensure must submit fingerprints for a criminal records check. For individuals residing

more than 75 miles from Ohio, you must contact the board and request the appropriate forms be mailed to you. The

board will not mail the forms to you until your initial application has been received. Upon receiving the forms, you

will need to go to your local law enforcement agency to have your fingerprints taken. This process can take 3-4

months to obtain results so it should be done as soon as you receive the forms from the board. You may also elect to

physically come to Ohio to have your fingerprints taken electronically to minimize the time it takes to process your

application. The processing time for electronic fingerprints is 7-10 days.

2. Submit the Certificate of Applicant Form and sign in front of a notary. You are required to submit this with your

online application so you must scan it and upload to your computer.

3. Submit a 2x2 passport style photo of your full face. The photo must have a plain white background, and has been

taken within the six month period immediately preceding the date of your application.

4. Submit an official Verification of Licensure Form for all states in which you currently hold or have ever held a

license, certification, or registration to practice occupational therapy. Fill out the top section of the form and forward

to all licensing jurisdictions you are associated with. They must complete the bottom half of the form and return it

back to you. You will then need to submit it with your online application. If you are licensed in more than one

jurisdiction, convert all forms to be on one PDF attachment.

5. Submit the Certification of Entry Level Education Form. Fill out section one of the form and mail it to the school

where you completed your occupational therapy academic program. The academic form must return the completed

form to you so you can submit the form with your online application.

6. Request verification of NBCOT exam scores. Verification must be sent directly to the board from NBCOT.

7. Take and pass the Ohio Jurisprudence Exam. You are required to register for the exam on the board’s website. You

will also need to download the Ohio Occupational Therapy Practice Act before beginning the exam. You will need

to create a user profile to take the exam. There is no additional fee to take the exam and you must score a 90% or

higher to pass. On the Course Catalog page, select the box in the Enroll column next to the appropriate exam and

click on the "Order now" button. Select the "Ohio OT Jurisprudence Exam For Licensure Applicants" on the

Course Catalog page. Once you complete the exam, you will see your score and will be prompted to send

transcripts. The Board will automatically receive a copy of the transcript that you send to yourself.

**All applicants should apply for a temporary letter in Oklahoma. This is a letter authorizing practice under the supervision

of a licensed OK occupational therapist. This permits legal practice during the interim from the time the application is

complete and the time at which the board grants a license. You will need to

complete Form #5

and have a supervising occupational therapist(s) sign the form in order for the

letter to be issued. The temp licensure board meets every Thursday. On

average, it takes one week to obtain a temporary letter.**

License Application: The application is available to complete online. You

will need to create a profile to begin filling out the application. You can save an incomplete application and return to it at any

time using your login information. Once your online application is complete, you will be required to submit all supporting

documentation to the board. Supporting documents required in addition to the application:

1. To verify education and training requirements submit a notarized copy of your NBCOT card.

2. Submit the Verification of Licensure/Certification Form to all state licensing jurisdictions where you currently hold

or have ever held a license. The licensing jurisdiction must include their seal on the bottom of the form. The

completed form must be mailed directly to the OK Board from the licensing jurisdiction.

3. If you are a U.S. citizen, U.S. national, or legal permanent resident alien, you must submit the Evidence of Status

Form, Part A, with a photocopy of one of the documents listed under the “Acceptable Documents to Establish

Evidence if Citizenship.” This evidence must be submitted with your application.

4. All applicants for licensure must request an Extended Background Check (EBC) by completing the online

authorization form. The link to the form can be accessed here.

5. Submit the Allied Photo and Oath Form. The form must be printed and signed in front a notary.

1. Mail a recent passport style photo to the board. Computer printed photos are not accepted and will delay

2. Request verification of licensure for all states in which you currently hold or have ever held a license. At

least one verification must show current, active licensure. All verification must be sent directly to the

the Exam” for your score to be reported to the board. You must get a 70% or higher to pass. You can take

the exam again at any time if you do not pass the first time.

License Fee: The application/license fee due for OT’s is $140.00 and for COTA’s is $100.00. Fees can be paid

License Fee: A $166.00 application and initial license fee is due for OT’s and COTA’s. You must submit the fee

with the application packet. Fees can be paid by check or money order, make payable to the Department of

Health. Applicants should include the profession for which they are applying for and their credential number on

the check or money order.

License Time: Applications requiring board review may not be considered until the next regularly scheduled

board meeting. The board meets about every six weeks. The board finishes final application reviews for approval

after a credentialing specialist verifies that the application is fully complete and complies with requirement. They

will then complete a background check and make sure you have submitted the required fees. On average, it takes

2-3 months to obtain a Washington license.

License Duration: Your initial license will expire on your birthday unless the license is issued within 90 days of

your next birthday. After the initial license, OT licenses expire very two years on your birthday.

on your application checklist online. Your exam results will be manually posted to your online checklist.

Allow up to 10 business days from the date you finish your exam for this posting to be completed.

License Fee: A $75.00 initial credential fee and $75.00 state law exam fee are due for OT’s and COTA’s at the

time of application. Fees can be paid by check, make payable to Wisconsin Department of Safety and Professional

Services (DSPS).

License Time: The board meets quarterly. Meeting dates and times are available on the board’s website under the

“Meeting Schedule” tab. The board has a detailed explanation of the application process and what it entails on

their website. This information can be found here. On average, it takes 3-6 weeks to obtain a license.

License Duration: Licenses are valid for two years. All OT/COTA licenses expire on May 31 of every odd-

numbered year.

License Renewal: License renewal is available online. A $75.00 license renewal fee is due for OT’s and

COTA’s. OT’s and COTA’s are required to complete at least 24 hours of approved continuing education during

each biennial registration period. A contact hour is considered 50 minutes of actual professional activity.

Continuing education is not required during the biennium when the license was first issued.

License Application: The application is available view online. Traveler

must print, fill out, sign the application, and have it notarized. Once all of this is completed applicant must mail the

completed application to the board. All supporting documentation must come directly from the source and not from the

license applicant. Documents required in addition to the application are:

1. Submit a 2x2 head and shoulder passport style photo. Photo must have been taken within the last year. No

photocopies will be allowed. Sign and print your name in ink on the back of the photo then attach the photo with a

paperclip to the application. Do not staple to application.

2. Request NBCOT send Verification of Certification directly to the board. This can be completed online through

NBCOT’s website. Score reports are acceptable for new graduates –use paper verification form only if not requested

at time of application for exam.

3. Request licensure verification for all states in which you currently hold or have ever held a license. Complete the top

part of the Verification of Licensure Status Form and mail to all applicable boards. They must fill out the bottom

section, affix their seal to the form, and mail it back directly to the WV Board.

4. (COTA’s Only) Print the Supervisory Statement Form and complete with your supervising OT. You can submit the

form with your completed application or upon change of employment or supervision.

License Fee: The initial license fee for an OT is $150.00 and he initial license fee for a COTA is $100.00. The fee must

accompany the application. Fees can be paid by check or money order, make payable to the West Virginia Board of

Occupational Therapy. License fees are non-refundable.

License Time: The board meets quarterly. Meeting dates and times are available under the “Meeting Agendas” tab on the

boards website. On average, it takes 2 weeks to obtain a license.

License Duration: Licenses are valid for two years. Licenses expire on December 31 either every even or odd numbered

year. Expiration dates are dependent upon when you were initially issued a license.

License Renewal: Licenses may be renewed biennially on renewal application forms provided by the board. The biennial

renewal fee for OT’s is $140.00 and for COTA’s is $120.00. All applicants for renewal must complete 24 contact hours of

continuing education and competency activities during the two year period preceding the application for renewal. A contact

hour means 1 hour spent in a continuing education activity. You may carry over up to six excess contact hours from one

consecutive licensure period to another. A licensee who obtains a license for the first time in West Virginia during the first 12

months of any 24 month reporting period must complete 12 contact hours in approved continuing education and competency

activities.

Temporary License Application Available? **YES: Limited Permit Available**

An applicant may obtain a temporary license to perform occupational therapy services for up to 30 consecutive calendar days

if licensed in another state having equivalent requirements or if NBCOT certification requirements are met when coming

from a state without licensure. A letter of eligibility to practice under temporary licensure must be obtained prior to practicing

in West Virginia. To receive this letter, the following information must be submitted to the Board:

1. Completed application and full licensure fee.

2. Copy of current full licensure in another state or NBCOT certification if coming from a state without licensure.

3. Information for facility at which you will be working in WV (name, address, phone number) and agency information

if employed by a 3rd party agency other than where services are provided.

4. Completed Supervisory Statement Form for COTA applicants.

5. Start date of employment in WV.

License Fee: The limited permit fee is $120.00 for an OT and $70.00 for a COTA. The fee is applied toward the permanent

licensure fee.

1. Attach a current head and shoulder photo to the designated spot on the application. Does not have to be a

passport photo.

2. U.S. Code requires proof of legal presence in the United States. All applicants must prove Lawful

Presence by either submitting a copy of your birth certificate or passport. If neither of theses documents is

readily available, you may instead submit a copy of both your driver’s license and social security card.

3. Submit two letters of recommendation provided by two professionals in the occupational therapy field

willing to send a letter of reference on your behalf. Letters must attest to your good moral character,

professional abilities, and ethics in the practice of occupational therapy. Letters must be sent directly to

the board office from the individual and will not be accepted from the applicant. Faxed reference letters

are accepted. All letters must be dated and signed.

4. Submit verification of licensure for all states in which you currently hold or have ever held a license,

active or inactive in any healthcare profession. Verification should include issue and expiration dates as

well as current standing. If this information is available online, the applicant can provide the website to

the board and the board will obtain the information this way. If the jurisdiction does not have online

verification, it is the applicant’s responsibility to request written verification be sent to the board. The

board does not have a verification form available, but the board will accept the information in any format

provided by the jurisdictions.

5. The Wyoming board will obtain NBCOT verification for you. There is no fee required.

License Fee: A $125.00 application fee is due for OT’s and COTA’s. In addition to the application fee OT’s who

submit their application August 01-January 31 must pay a $100.00 license fee. OT’s who submit their application

February 01-July 30 must pay a $50.00 license fee. In addition to the application fee COTA’s who submit their

application August 01-January 31 must pay a $50.00 license fee. COTA’s who submit their application February

01-July 30 must pay a $25.00 license fee. Fees can be paid by cashier’s check or money order, make payable to

Wyoming Board of Occupational Therapy.

License Time: The average amount of time it takes to obtain a license is 3-4 weeks. Be mindful of deadlines

provided to you by the board or your licensing application could be delayed.

License Duration: Licenses are valid for one year. All permanent licenses expire on July 31 annually.

License Renewal: A signed renewal application must be mailed to the board with appropriate fees to be

considered for renewal. A $110.00 renewal fee is due for OT’s and a $60.00 renewal fee is due for COTA’s.

Renewal applications and forms must be postmarked by June 01. Renewal applications postmarked after this date

will be charged an additional $50.00 renewal late fee. For OT’s and COTA’s 16 hours of continuing education

are required before a renewal will be granted. Education hours can be submitted from courses taken during the

previous three years.
