On behalf of the Albert and Mary Lasker Foundation, I invite you to submit a nomination

for the 2004 Albert Lasker Medical Research Awards.

The Awards will be offered in three categories: Basic Medical Research, Clinical Medical

Research, and Special Achievement in Medical Science. This is the 59th year of these

awards. Since the program was first established in 1944, 68 Lasker Laureates have later

won Nobel Prizes. Additional information on previous Lasker Laureates can be found

online at our web site http://www.laskerfoundation.org.

Nominations that have been made in previous years may be updated and resubmitted in

accordance with the instructions on page 2 of this nomination booklet.

Nominations should be received by the Foundation no later than February 2, 2004.

P U R P O S E A N D D E S C R I P T I O N O F T H E AWA R D S

The major purpose of these Awards is to recognize and honor individuals who have made signifi-

cant contributions in basic or clinical research in diseases that are the main cause of death and

disability. The Award winners may reside in any country of the world.

The Awards are intended to recognize contributions that have opened new fields of research, advanc-

ing novel concepts or their applications in a particular biomedical discipline. These advances may

consist of a specific contribution or a long series of contributions that demonstrate the nominee’s

significant leadership towards the development of research concepts or their clinical application.

A list of previous recipients of the Lasker Awards in Basic Medical Research, Clinical Medical

Research and Special Achievement in Medical Science is included at the end of this booklet.

Each Award consists of an honorarium, a citation and an inscribed statuette of the Winged Victory

of Samothrace, symbolizing victory over death and disease.

When the Award is given to more than one individual in a category, the honorarium will be divided

among the winners, and each will receive a statuette and a citation. More than one scientist may be

nominated if they have worked as a team, or if they independently have made comparable contri-

butions in their field. No more than three individuals will be considered for an Award in the basic

or clinical category.

The Jurors’ evaluations of the contributions of the nominees depend heavily on the information

supplied in the nomination forms. Therefore, the nomination should be accurate, detailed, current

and complete. Supporting letters that specifically address the nominee’s scientific accomplishments

will be helpful to the Jury.

Basic Medical Research Award ($50,000)

This Award honors the scientist or scientists who have made fundamental investigations that open

new areas of biomedical science.

Clinical Medical Research Award ($50,000)

This Award honors the scientist or scientists whose contributions, directly or indirectly, have led

to the improvement of the clinical management or treatment of patients and to the alleviation or

elimination of one of the major medical causes of disability or death.

Special Achievement Award in Medical Science ($25,000)

This Award was inaugurated in 1996. It honors a scientist whose contributions to research are of

unique magnitude and immeasurable influence on the course of science, health, or medicine, and

whose professional career has engendered within the biomedical community the deepest feelings of awe

and respect.

1) For all nominations, on page 5 please provide a suggested short citation which highlights the

research for which you wish your nominee(s)’s to be recognized, and provide a short summary of

the nominee(s)’s scientific contributions.

2) Questions 1 and 2 (on pages 6 and 7) apply only to Basic and Clinical Award nominations.

3) Nominators for the Special Achievement Award should utilize only pages 4, 5, and 8 from this

booklet, and should follow other instructions below.

4) Please provide the following additional information:

a) a brief one-page biography including awards received by each nominee;

b) on an additional page, a list of six (6) peer-reviewed journal references that substantiate the state-

ments made in your nomination. DO NOT send reprints.

5) Supporting letters from colleagues familiar with the nominee(s)’s work can be either included with

the nomination package or mailed separately.

6) No nominations or support letters are automatically carried forward from previous years. If you wish

to renominate a previous candidate, please update the earlier nomination and resubmit it by com-

pleting this 2004 nomination form. The 2004 nomination should contain all the pertinent previous

material submitted, including updated letters of support, and any significant developments in the

research contributions which have occurred since the nomination was last submitted. Complete,

updated information should be included to permit review of each nomination without reference to

previous nominations.

7) Submit ONE (1) ORIGINAL and TWO (2) COPIES of the above requested information. Please do not

staple or bind any of the material you are submitting. Instead, using binder clips, please arrange

each set in the following order: the completed nomination forms; the brief one-page biography; the

list of six peer-reviewed journal references; and any enclosed support letters.

8) Submit ONE COPY of a complete curriculum vitae, including publications, of each nominee.

9) The nomination and supporting letters should be received by the Foundation no later than February

2, 2004.
