    The Alberta Health Care Insurance Plan

    provides eligible residents of Alberta and their

    dependants with:

    coverageformedicallyrequired

    servicesprovidedbyphysicians;

    coverageformedicallyrequiredoral

    andmaxillofacialsurgeryand

    medicallyrequireddentalservices;

    coverageformedicallyrequiredstays andservicesinAlbertahospitals;and

    somecoverageforservicesprovided

    byoptometrists,chiropractorsand

    podiatrists(servicesmustbeprovided

    in Alberta).

    An Alberta resident is dened as aperson who is:

    For the purpose of Alberta Health Care

    Insurance Plan coverage, a dependant is

    dened as:

    dependentonhis/herparent(s).Incases

    ofseparationordivorce,thecustodial

    parent should register the child. In cases

    ofjointcustody,thechildisregisteredon

    theaccountagreedtobybothparents,

    generallywiththeparentthechildresides

    withthemost;

    afosterchildand/orwardofthecourt,

    ifanincometaxdeductionwasclaimed;

    asinglechild21yearsofageorolder

    andwhollydependentontheparent(s)

    becauseofaphysicalormentaldisability;and

    asinglechildunder25yearsofageand

    enrolledinthreeormorecoursesatan

    *An adult interdependent partneris a personwho lives with another person in a relationship ofinterdependence:

    AllresidentsofAlbertamustregister

    themselvesandtheireligibledependantswith

    the Alberta Health Care Insurance Plan.

    Newandreturningresidentsarerequiredto

    providedocumentsthatprovetheiridentity,legalentitlementtobeinCanadaandAlberta

    residencywiththeirregistrationforms.

    Registrationformsareavailablefrom

    employers,associations,unions,anAlberta

    HealthandWellnessofce,orfromtheAlberta Health and Wellness website at www.

    health.alberta.ca.

    Registration is required by law,however,

    Albertansmaychoosetoformallyoptoutof the Alberta Health Care Insurance Plan.

    Followingregistration,AlbertaHealthandWellness sends each person their Alberta

    Personal Health Card with their personal

    healthnumber.YourAlbertapersonalhealth

    carenumberandcardareyourpersonal

    theyarealinktoyourhealthinformation.No

    Changes to Coverage

    Toensurethatregistrationinformationstays

    current,pleasenotifyAlbertaHealthand

    Wellnessofanychangesthatmayaffectyour

    coverage.Thisincludesaddingorremovinga

    dependantfromyouraccountandchangesofnameoraddress.

    Adding a dependant (Referbacktopages4and5foralistofwhoqualies

    as a dependant.)If Alberta Health and Wellness receives

    noticationwithinonemonthoftheevent

    (e.g.marriage,adultinterdependentpartner

    relationship),thedependantisaddedtothe

    accountthedateoftheevent.Otherwise,thedependantisaddedtherstdayofthemonth

    followingnotication.

    Deleting a dependant

    If Alberta Health and Wellness receives

    noticationwithinonemonthoftheevent

    (e.g.divorce,childleaveshome),the

    dependantisdeletedfromtheaccount

    thedaybeforetheevent.Otherwise,thedependantisdeletedthelastdayofthe

    monthinwhichnoticationisreceived.

    Newborn baby

    Most babies born in

    Alberta hospitals are

    registeredbythehospital

    and coverage is effective

    the date of birth. If the

    hospitaldidnotregisterthebaby,coverage

    can still be effective the date of birth if

    noticationisreceivedwithinthreemonthsfromthedateofbirth.Otherwise,theeffective

    dateisdeterminedatthetimeofnotication.

    IfthebabysAlbertaPersonalHealthCardis

    notreceivedwithinamonthofregistration,or

    iftheinformationonthecardisnotcorrectorcomplete,pleasecontactanAlbertaHealth

    andWellnessofce.

    Name and address changes

    ItisimportanttonotifyAlbertaHealthand

    Wellness of changes as soon as possible.

    ChangescanbemadebyllingoutaNotice

    of Change formavailablefromemployers,

    associationsorunions,oranAlbertaHealthandWellnessofce.Changescanalsobe

    madeatanyAlbertaRegistryAgentorfrom

    the Alberta Health and Wellness website at

    www.health.alberta.ca.

    Benets

    Albertans who are registered with the Alberta

    Health Care Insurance Plan are entitled to

    anumberofhealthcarebenets.These

    benetsmaybepaiddirectlybytheAlberta

    HealthCareInsurancePlanortheymaybeprovidedbyahealthregionortheAlberta

    CancerBoard.Benetsforhealthservices

    covereddirectlybytheAlbertaHealthCare

    InsurancePlanarepayablewhetherthey

    are obtained within Alberta or elsewhere inCanada,withtheexceptionofprivatefacility

    fees. The Alberta Health Care Insurance

    Planpaysmedicalbenetsaccordingtothe

    ratesestablishedbyotherprovincial/territorial

    health plans.

    Physician services

    The Alberta Health Care

    Insurance Plan provides full

    coverageformedicallyrequired

    servicesprovidedbyphysicians.

    Physiciansareinthebestpositiontoadvise

    which health care services are covered.

    Alternatively,thisinformationisintheScheduleofMedicalBenets,whichare

    available on the Alberta Health and Wellness

    website at www.health.alberta.ca.

    Oral and maxillofacial surgery anddental services

    Somespecicdental/oralandmaxillofacial

    surgeryservicesperformedbyadentistare

    fullycovered.Examplesofinsuredservices

    includecystremoval,jointandjawsurgeryandbonegrafts.Toothrelatedtreatmentsare

    arepayableatspecicratesperserviceto

    amaximumof$200perpersoninabenet

    yeariftheservicesareprovidedinAlberta.

    responsibleforpayingtheseadditionalcosts.

    Podiatry services (foot care)

    Benetsforsomeservices

    providedbyapodiatristare

    payableatspecicratesperservicetoamaximumof$250

    perpersoninabenetyearifthe

    services are provided in Alberta.

    what the Alberta Health Care Insurance Planpays.Thepatientortheirsecondaryinsurer

    isresponsibleforpayingtheseadditional

    Optometric services (vision)

    Benetsarepayableifthe

    services are received in

    Alberta.Benetsarelimited

    toonecompleteexam,onepartialexamandonediagnostic

    procedureperbenetyearforchildren

    under19yearsofageandseniors65

    yearsofageandover.Thereareadditional

    benetsforsomeconditions.Optometrists

    cannot charge their patients for services

    coveredbytheAlbertaHealthCare

    InsurancePlan.Askyouroptometristfor

    moredetails.

    Hospital services

    Alberta Health and Wellness provides

    fundingtohealthregionsformedically

    requiredinpatientandoutpatienthospitalservices. There is no additional charge to

    Albertans for insured hospital services.

    However,ahospitalcanchargefornon-

    insuredservices(e.g.private/semiprivate

    room).

    Insured hospital services include:

    medicallyrequireddiagnostic,clinical

    laboratoryandnursingservices;

    medicationsgiventohospitalinpatients (patientsmayhavetopayfor

    prescriptionmedicationsprovidedin

    hospitaloutpatientdepartments);and

    accommodationatastandardward

    rate(semi-privateorprivate

    accommodationandsomeother

    services are not covered).

    Services covered by the Alberta Health

    Care Insurance Plan and the amounts

    paid for health services are subject to

    change. Discuss coverage with a doctor

    or other health care provider, refer tothe Alberta Health and Wellness website

    about coverage for a treatment before

    proceeding.

    for Seniors

    Alberta Blue Cross Coverage forSeniors

    Alberta Health and Wellness provides

    seniors and the dependants covered on their

    accounts with Alberta Blue Cross coverage atnocost.Thisissupplementarycoveragefor

    prescription drugs and other services. Seniors

    mustprovideproof of their age to receive this

    benet.

    Supplementary Coverage

    (Alberta Blue Cross)

    Alberta Health and Wellness funds the

    following Alberta Blue Cross non-group

    supplementaryhealthcareplans.

    Alberta Blue Cross Non-GroupCoverage

    Coverage is available to residents under

    theageof65yearssubjecttothepayment

    ofpremiums.Thiscoverageprovidesthe

    followingbenetswithsomerestrictionsand/ormaximumlimits:prescriptiondrugs;

    ambulanceservices;clinicalpsychological

    services;homenursingcare;prostheticand

    orthoticbenets;mastectomyprosthesisand

    Premium Subsidy Program

    Non-seniorAlbertanswithlowerincomes

    areencouragedtoapplyforthePremium

    SubsidyProgram,whichmayreduceyour

    non-groupBlueCrossPremiums.Eligibility

    forthisprogramisbasedontheaccountholders(and,ifapplicable,theirspouses

    orpartners)taxableincomeforthetaxation

    yearimmediatelybeforethesubsidyperiod

    requiredbyresidentswhoarediagnosedaspalliative;whosephysiciansornurse

    practitioner have applied for the coverage on

    theirbehalf;andwhoarereceivingtreatment

    athome.Applicationsandmoreinformation

    Please note: Claims for any of the

    government sponsored Alberta Blue

    Absence

    Whentraveling,Albertansshouldalwayscarry

    their Alberta Personal Health Card and show it

    whentheyobtainhealthservices.

    WhentravelingoutsideAlberta,itisimportantto note that the Alberta Health Care Insurance

    Plandoesnotcoversomehealthservicesand

    therearemaximumbenetsonsomeservices

    thatarecovered.Hospitalservicesmustbe

    providedinageneralorauxiliaryhospitaltobeeligible for coverage.

    Tobeeligiblefortreatmentofdrugand/

    oralcoholabuse,eatingdisordersorother

    behaviordisordersoutsideAlberta,prior

    approvalmustbeobtainedfromAlbertaHealth

    and Wellness.

    ResidentswhowillbeabsentfromAlbertaformorethansixmonths,arerequiredto

    contactanAlbertaHealthandWellnessofce

    before leaving to ensure Alberta Health Care

    InsurancePlancoverageismaintained.

    Claimsformedicallyrequiredphysician

    and hospital services received elsewhere

    inCanadaareusuallybilledautomatically

    to the Alberta Health Care Insurance Plan(exceptphysicianservicesinQuebec).Travel

    insuranceisrecommendedtocovercostsfor

    ambulanceservices,privateandsemi-private

    roomaccommodations.Medicalclaimsare

    paid at the established rates of the province

    orterritorywheretheserviceswereobtained.

    Apatientmaybeaskedtopaydirectlyfor

    oral surgical services received outside

    Alberta.AclaimmaybesubmittedfortheseserviceexpensestotheAlbertaHealthCare

    InsurancePlanforreimbursementatAlberta

    rates.

    Travel outside Canada

    Costs for health care services obtained

    outside Canada are high. Alberta Health

    and Wellness recommends that everyone

    purchase travel health care insurance,

    even for short trips. To ensure travelinsuranceisadequate,travelersshould

    considerallpre-existingconditionstheymay

    have.Thiswillgreatlyreducetheriskofany

    unexpectedmedicalcostsforthetraveler.

    Travel health insurance is not available

    through the Alberta Health Care Insurance

    Plan. See private health insurance section

    onpage18forinformation.

    Out-of-countrypractitionerservices

    arepayableatthesamerateanAlbertapractitionerwouldreceiveforthesame

    orasimilarservice,ortheamountpaid,

    whicheverisless.Themaximumamount

    paid forinpatient hospital care provided

    outsideCanadais$100perday(inCanadianfunds),andinpatienthospitalbenetsarenot

    payableforthedayofdischarge.Amaximum

    benetof$50(inCanadianfunds)ispayable

    for one outpatient hospitalvisitperday.

    Hospitalbenetsareinclusiveofx-rays,lab

    tests,roomandboard,etc.

    Temporary absence

    ResidentsmustmaintaintheirAlberta

    HealthCareInsurancePlancoverageifthey

    temporarilyleaveAlbertaforanotherpartof

    Canada and intend to return to Alberta within

    12months,ortemporarilyleaveCanadaand

    intendtoreturntoAlbertawithinsixmonths.

    Iftheyleaveforalongerperiodoftime,but

    intendtoreturntoAlbertatolivepermanently,

    theymayapplyforthefollowingextensionsof

    coverage:

    uptotwoyears(24months)fortravel, personal visits or educational leave (sabbatical);or

    uptofouryears(48months)for absencesduetowork,businessor

    missionaryservice;or thedurationoftimetheresidentis enrolledasafull-timestudentatan

    accredited educational institute.

    Submitting a claim

    Claimsforhealthservicesreceivedoutside
