Integration Description

The SWS solution requires completion of a Health Risk

Assessment (HRA). The HRA requires demographic and

medical data (medications, past medical history, previous

preventive services) that ideally would automatically

populate after integration. In addition, the SWS solution

automates preventive services as well as the appropriate

CPT/HCPCS and ICD9 codes. Ideally, this would integrate

with the Allscripts practice management piece for electronic

billing.

Anticipate integration with Enterprise and Professional Unity

products.

Go to Market Plans

Information on business plan:

• Solution will be brought to market as a value-add to current EHR. No

marketing necessary for patients, they already exist in the practice.

Patients called and scheduled during their regular visits for AWV. Will

separate EHR from competition by demonstrating significant increase in

revenue and improved patient care. Without providing additional

services, the practice can generate $170,000 per 1000 patients. If you

provide additional services approximately $400,000 per 1000 patients.

• The target will be physicians in following practices; Internal Medicine,

Family Practice, and Geriatrics.

• The price point can be either a fixed amount per encounter ($5-$25 per

encounter) or if practice management has percentage of billing

arrangement, the software piece is free and the dramatic increase in

billing will be reflected with practice management profit.

Most EHR companies and value-add applications

hypothetically bring value and/or quality care to a practice.

The Senior Wellness Solution application demonstrates

significant revenue increase from the onset of use. CMS

strongly believes that the AWV will increase quality of care

and decrease overall healthcare costs. The problem is that

the CMS requirements are so complex, the physicians are not

doing the visits. The Senior Wellness Application solves this

problem by automating the visit so that: 1. A non-physician

provider can deliver the visit in 20 minutes or less 2. Meets
