L’élastométrie a été principalement validée dans les
hépatites virales mais nécessitent d’étre validée dans
d’autres etiologies (NAFLD, etc..).
La combinaison de l’élastométrie et des marqueurs
sanguins est la stratégie de choix pour le dépistage de la
fibrose en 1ère intention dans l’hépatite C.
L’élastométrie est actuellement la méthode la plus performante
pour le diagnostic de cirrhose et pourrait avoir une valeur
pronostique au cours de la cirrhose.
L’élastométrie ne peut pas remplacer la fibroscopie pour le
dépistage des VO
Malgré ses limites, l’élastométrie est une technique prometteuse
pour le suivi des maladies du foie mais nécessite d’être mieux
évaluée de façon longitudinale.

Utiliser les méthodes non invasives en association Elastométrie et scores sériques
L’interprétation doit être faite par un clinicien
expérimenté en fonction du contexte clinique et de
façon critique: Faux positifs (Fibrotest)
Critères de qualité pour l’élastométrie IQR < 30% de la valeur médiane
Taux de succès > 60%

Recommandations pour la pratique
Utiliser des “fourchettes” de valeurs plutôt qu’un seuil fixe
pour l’élastométrie.
Ne pas hésiter à répeter l’élastométrie lorsque les résultats
sont discordants avec le contexte clinique.
En cas de discordance inexpliquée, une PBH devrait être
pratiquée: > 15 mm

Les méthodes non invasives constituent un progrès dans la prise en charge des patients atteints d’hépatites virales permettant de diminuer le recours à la PBH sans toutefois la remplacer.
