Annual health checks for people with a learning disability in Bradford District and Craven. Annual health checks are for people aged 14 and over with a learning disability. They help people to stay healthy and well by talking to people about their health and finding any problems early. Annual health checks for people with a learning disability in Bradford District and Craven.

Almost all of the people in Bradford District and Craven who have a learning disability had their annual health check in 2020-2021.

People with a learning disability often have poorer physical and mental health than other people.

This does not need to happen.

Annual health checks are for people aged 14 and over with a learning disability.

They help people to stay healthy and well by talking to people about their health and finding any problems early.

This means they then get the right care.

We call Bradford District and Craven our place.

In our place, we have managed to give nearly everyone who has a learning disability their annual health check.

We have also added 300 more people to the learning disability register.

This is a list of people that have a learning disability.

Doctors use it to make sure people get the right care and support.

Across our place we have a way of working called Act as One.

Our ambition is to keep people ‘happy, healthy at home’.

An ambition is something we want to happen.

Our work on getting more people to have their annual health check and keeping people out of hospital was done in partnership across our place.

By the end of 2020-2021, for the first time, all our GP practices were giving people their annual health check.

Our work has meant that less adults and children with a learning disability have had to stay in hospital.

The work was led by Bradford District Care NHS Foundation Trust.

She is the Frailty/Dementia and Learning Disability Lead for NHS Bradford District and Craven Clinical Commissioning Group (CCG).

The CCG are the people who plan and pay for health and care services in our area.

Dr Sara Humphrey talks about how health and care services worked together as Act as One.

In June 2020 we worked together to make a series of top tips for giving people their annual health check during the coronavirus pandemic.

This included whether people should have their annual health check in person or by phone or video call.

We also arranged zoom calls for GP practices every 2 months so they could share ideas and what had worked well for them.

Our work has been supported by the local medical committee and the Chair of Bradford District and Craven CCG.

The Chair wrote to all GP Practices to remind them of the importance of giving people with a learning disability their annual health check.

We will also be looking at giving people their Covid-19 vaccine when they have their annual health check, especially in areas where not many people have had the vaccine.

We will also support people to stay well by making sure they attend cancer screening appointments and other immunisation clinics.

The community learning disability team, sometimes called Waddiloves, has continued to support people during the Covid-19 pandemic.

Other clinicians have offered face to face appointments for people in crisis so they can avoid going into hospital when they don’t really need to.

Care and support reviews have been done by video call to keep everyone safe.

These have worked well.

People with a learning disability and their families have coped well during the pandemic.

We have not had to our increase our prescribing rates for antipsychotic medication during the last year.

The care and support given by all organisations across the district has helped people to stay healthy and well.

These organisations included NHS, social care, day care providers, self-advocacy organisations and primary care.

Learning from Learning Disabilities Mortality Review (LeDeR)

The LeDeR report reviews the deaths of people with a learning disability.

The Bradford District Care Foundation Trust have developed a respiratory pathway.

This will support people with a learning disability to make sure they get the right care and support.

It will help to make sure less people are dying from respiratory illnesses like pneumonia.
