1. OAFI’S PRESENTATION

1.1 Who we are The Osteoarthritis Foundation International (OAFI) is an independent non-profit foundation, that was created to prevent, treat and improve the quality of life of people suffering from osteoarthritis (OA) by providing them with the necessary tools to help them manage their disease. OAFI has its headquarters in Barcelona (Spain). For this reason, the foundational activity focuses on the following activities:

• Increase social awareness about OA

• Improve the quality of life of patients and the prevention of risk groups (elderly people, sportspersons, postmenopausal women and children)

• Promote research in osteoarthritis and improve treatment For us, patients are the most important.

1.2 Osteoarthritis, a disease that affects all of us OA is a degenerative joint disease that affects the cartilage, bone and soft tissues of the joint. It produces pain and limits us when it comes to doing our daily tasks. It already affects 300 million people in the world according to the Lancet1. Despite its enormous social impact, OA remains an unknown and distant disease for the majority of the population that it is exclusively associated with elderly people. However, it affects very fragmented groups of the population (elderly people, sportspersons, postmenopausal women and children) who have very different needs and who cannot find an answer beyond a general approach.

2. STRATEGIC ACTIONS

2.1 Osteoarthritis International Association OAFI promotes the prevention, treatment, and improvement in the quality of life of OA patients by integrating all the agents involved in this pathology, including patients’ associations, medical societies, research centers, public administrations, and collaborating companies to work together against this chronic disease. At present, OAFI has signed agreements with different entities to collaborate in raising awareness of OA in the field of prevention and treatment of this disease.

OAFI leads this cluster of knowledge participated by interdisciplinary entities that work on the improvement of OA patients’ quality of life. In 2017, there were 23 entities working and collaborating together. The main role of each group:

- Patient associations. Members work together in the dissemination and awareness of OA in the society. Participants: Arthritis Foundation (USA), Pan-American League of Rheumatology Associations - PANLAR (representing the American continent), Fondation Arthrose (Belgium), Spanish League of Rheumatology (Spain), General Patients Association (Spain) and Spanish Association of Osteoporosis and Osteoarthritis (Spain).

- Medical Societies. It integrates relevant Spanish medical societies that have an important role in the care of patients with osteoarthritis, such as the Spanish Society of Rheumatology, Spanish Society of General and Family Doctors, Spanish Society of Primary Care Physicians and the Spanish Society of Traumatology in Sport.

- Research centers. Teófilo Hernando Foundation, Eurecat, Poal Institute of Rheumatology, University of Barcelona, CEMTRO Clinic, DKF Clinic and WSF promote, in collaboration with OAFI, OA research initiatives in the field of medicine, sports medicine and nutraceuticals.

- Environment. FC Barcelona, Obra Social "la Caixa" and Caminoterapia give us support in the promotion of healthy joints and without pain.

- Collaborating companies. These are companies (Ferrer, OPKO Health, Vanir and AS&NL) that are committed to our foundational activities and actively collaborate with the foundation in the scientific field.

2.2 OA Task Force OAFI co-leads, together with the Arthritis Foundation, the OA Task Force initiative which is an alliance between the most important associations of patients with rheumatologic problems in order to work together globally with a focus on osteoarthritis. OA Task Force members are currently working together in the following projects: Quality of Life (QoL) Systematic Literature Review. The purpose of the OA QoL systematic literature review is to provide a “state of the science” review of the international knowledge base on QoL in knee OA patients. During 2017, we reviewed 9,143 international publications. Global Osteoarthritis (OA) Patient Perception Survey. International Survey to develop a multilingual Global OA Patient Perception Survey and implementation strategy that will provide medical professionals, researchers, patients, and policymakers with data on OA patient perceptions at the country, regional, and global levels. This survey, which has already been prepared and validated in the languages of participating countries, will be carried out in Spain, Italy, the US, Mexico, Argentina, Colombia and Venezuela. During the elaboration of the survey, it was key the accuracy of translations in different languages (English, Spanish and Italian) to ensure respondents would receive and understand the same information. In addition, the questionnaire format was addressed for its implementation in each of the countries. The data and privacy management were also handled according to the regulations of each participating country. Finally, the annual meeting was held on October 9, 2017 to follow up the project progression, determine the next steps and design the schedule of activities.

3. SOCIAL ACTIVITIES

3.1 I International Congress of OA Patients

OAFI organized, with the collaboration of Obra Social "la Caixa", it first international congress that took place on the 9th and 10th October 2017 at the CosmoCaixa (Barcelona). The purpose of such annual public event is to provide a forum for the exchange of awareness and social knowledge aimed at patients with osteoarthritis and risk groups (elderly, sportsmen and postmenopausal women) to improve their quality of life. The program included social activities, roundtables and conferences to discuss on OA, its socioeconomic impact and how OA poses a threat for our society and the Health care System. Lecturers presented the prevalence of OA in women, the relationship between doctor-patient, the role of the patient in R&D and in the regulatory issues and how important is doing exercise and go on a healthy diet. 3.2 Social Program: “Talking about Joint Health” This is a collaborative project between the Obra Social "la Caixa" and the Osteoarthritis Foundation International (OAFI). “Talking about OA” program aims to provide elderly people from senior centers of the “La Caixa” with tools to help them manage their joint problems so that they can improve their quality of life by using non-pharmacological measures.

Workshops took place in 20 centers previously selected from “La Caixa”. The first three workshops were done at the end of 2016 and the rest in 2017.

In each center, a visual presentation was made to underpin the message. At the end of each session, questionnaires of assessment and quality of the workshop were distributed. Results obtained are as it follows:

- Attendance: + 1,200 people in 20 sessions. - There were collected 780 questionnaires to get the opinion of attendees. - 65% reported having regular joint pain. - 84% expressed interest in deepening more on joint health issues. - Workshops were highly rated, reporting over 93% as satisfactory or very satisfactory in

the following aspects: o Preparation and knowledge of the topics o Clarity and argumentation o Use of visual tools and others o Workshop methodology

- 89% reported that the session allowed them to acquire new knowledge.

3.3 Workshops A priority of the OAFI is to increase social awareness and run training courses for patients with OA and risk groups in order to stop being OA as an unknown disease which is wrongly associated to ailments that affect mainly people in old age.

3.4 Charitable Activities 3.4.1 Free Access to Advise OA patients are a priority for OAFI. That is why we offer free access to advising patients, mainly with low income. We listen, give advice and help them. In 2017, OAFI visited 23 patients who urged assistance.

3.4.2 Advisory Board of Patients At the end of 2017, OAFI formed its Advisory Board of Patients. The first meeting was participated by 8 volunteers in December. It was agreed to work on the following topics and set an action plan to: - Increase the number of volunteers - Plan and organize social activities for patients - Increase the visibility of the foundation to reach out to OA patients 3.4.3 Nordic Walking training course and a race On October 29, the Osteoarthritis Foundation International (OAFI), the Caminoterapia Foundation and the Walkim company organized a Nordic Walking training course and a race at the Can Dragó Municipal Sports Center in Barcelona, in collaboration with the Eurofitness - sports club. The event was aimed at both osteoarthritis patients and people who wanted to take care of their joint health. Several studies point out the benefits of Nordic Walking for joints and muscles. 3.4.4 Charitable snack event

On December 14, OAFI organized a Charitable event to celebrate Christmas with a snack. The objective was to gather volunteers and friends of the OAFI to share with them a snack and a glass of cava. There were surprises such as live music and the raffle of gifts. People made a donation when having access.

4. SCIENTIFIC RESEARCH

Clinical Trials - Clinical trial for a new treatment of knee osteoarthritis in the general

population…………………. in progress OAFI is co-promoter of the study to provide advice and for the benefit of the patient. OAFI provides the following activities to:

• Support to the STUDY and Protocol design • Supervision and advice in the coordination of the STUDY • Support in the selection of hospitals for the development of the STUDY • Propose the PRINCIPAL RESEARCHER and the rest of the researchers. • Organize meetings with the Ethics Committees (CEICs), medical societies and, if

necessary, with the Spanish Agency of Drugs and Health Products to provide the patients perspective

Regulatory - Appropriate Use of Chondroitin Sulphate (CS), Glucosamine (G), or its combination (CS+G)

for the treatment of OA Patients…………………. in progress There are several treatments (physical therapy, analgesia, drugs, surgery ...) in the management of osteoarthritis. In regard to oral treatments, these ones include Chondroitin Sulphate (CS), Glucosamine (G), or its combination (CS+G). In recent years, the Cochrane has published several articles and systematic literature reviews reporting the efficacy and safety of CS and G compared to other treatments and placebo. However, the treatment of osteoarthritis with CS, G, or CS / G is still controversial. Some guidelines recommend the use of these drugs (CS, G, or CS / G) for some type of patients and others do not do it.

This lack of consensus in the guidelines, together with the controversy among practitioners may lead to the fact that patients with osteoarthritis who could benefit from treatment with CS, G, or CS / G are not receiving it, despite the fact that they could reduce pain and the progression of the disease with fewer adverse effects than other commonly used drugs. Consequently, it is convenient to develop criteria for the appropriate use of CS, G, and CS / G by conducting a DELPHI Study in which experts can discuss, reach consensus and provide recommendations.

Thanks to grants received, this study has been possible to be undertaken. At the end of 2017, OAFI worked on the definition of the foundations of this study and systematic literature review.

Patients - Focus group to adjust the Protocol of a Clinical Trial …………………. Finalized One of the critical aspects during the implementation of a Clinical Trial is the phase of patient recruitment. For this reason, a pharmaceutical company aimed to validate its protocol by collecting and analyzing the views of the participating OA patients. OAFI selected 10 individuals, based on criteria previously defined by the company, who were invited to participate in this focus group study.

5. SCIENTIFIC DISSEMINATION

5.1 Workshop: “Osteoarthritis in the XXI century” On June 12, 2017, OAFI organized the workshop:"Osteoarthritis in the XXI century" at the Spanish Ministry of Health. The event, attended by around 120 people, was participated by representatives of patient associations, medical societies, health authorities and other personalities. This event was relevant in order to give greater visibility to osteoarthritis and stress the need for working out a plan to deal with this chronic disease.

6. COMMUNICATION AND AWARENESS

OAFI has been very active in 2017 and the foundation carried out many dissemination activities:

1. 16 press releases have been sent (9 institutional, 2 Scientific, 5 Scientific and social activities) that have generated more than 200 impacts on media.

2. More than 200 impacts have been obtained in the media: o Television: 6 o Written press: 18 o Online: 175 o Radio: 1

3. Participation in the relevant Spanish congresses (SER, SEMERGEN, SEMG, SETRADE) 4. Presentation of a poster in the PARE section at the EULAR Congress in Madrid 5. More than 50 articles have been published on the website 6. 35 videos have been uploaded to our YouTube channel 7. A community of more than 6,000 digital users has been created through social

networks In addition, OAFI presented the project “Art Norwalking” program, which was finalist in the fifth edition of the INDESCAT Business and Sports Awards in the category of partnership. This collaborative program between the OAFI and the Caminoterapia Foundation to promote the benefits of Nordic walking for OA patients and risk groups. This initiative is addressed to companies concerned about their employees.
