2014 was another year of contrast. Our group of medical and dental providers remained steadfast in their commitment to the Crusader mission of providing high quality health and dental care with respect and dignity.

In contrast, we continued to see unprecedented changes in the healthcare marketplace.

We are committed to face these changes head on and with a healthy spirit of adaptation. Over the past year we’ve become more nimble in helping our patients cope with change. Our patients face many challenges and it is our responsibility to help them with all the social tools and medical know-how we have.

The environments in which many of our patients are born, live, learn, work, play, worship, and age are unique. They affect a wide range of health and quality-of-life issues. Social and economic challenges that our patients face are often difficult for many of us to understand. Yet our providers successfully address these challenges every day.

Crusader can greatly enhance quality of life for our patients. We give them reassurance and hope. Our medical and dental teams engage our patients in a unique manner. The results of our most recent Patient Satisfaction Survey indicate that our patients understand and appreciate the many benefits offered by Crusader.

A highlight of 2014: we welcomed Dawn Mellish, MD as Crusader’s new Chief Medical Officer. Dr. Mellish brings a unique perspective and experience to her new leadership role at Crusader. We are thrilled to have her aboard.

I am personally optimistic about 2015. Yes, many changes lie ahead. And we will tackle each one in the manner that has worked well of us for over 43 years. Plan well for tomorrow. Be present each day. Treat every patient with dignity and respect. Work hard and have fun. It’s The Crusader Way!

What a warm welcome I’ve received from the Crusader family and the Rockford community!As I’ve interacted with patients, I see a genuine appreciation for the care Crusader offers each person. I’ve observed providers and staff truly enjoying their patients and each other. It is obvious every day. That’s a real workplace gift.

And, as we all know, improvements and enhancements can be successfully introduced and implemented. By working to establish policies and practices that positively influence the health and well being of our patients, we can greatly improve outcomes for large numbers of our population in ways that can be sustained over time.

Enhancing the conditions and policies in which we work, learn and interact with patients will create better practices for patients and a more exciting workplace for our providers and staff.

One of my first Crusader experiences was the Winter Conference in early January. What an amazing testament to have 44 individuals honored for having logged 15 years or more at Crusader. It shows the positive character of Crusader in a wonderful way.

I am excited about the future. And I believe Crusader is an excellent place from which to experience it. Thank you again for the gracious welcome!

Thanks to Project First Rate and Alderman Thomas McNamara, Crusader on Broadway installed a Little Free Library ouside their main entrance. It is a take a book, return a book concept. All books are donated. It’s purpose is to promote reading and literacy for children and adults. It is available to all patients and neighbors.

WOW! Look what happened in 2014, day after day, one patient at time!

• Nearly 97% of patients surveyed in 2014 indicated they would refer a friend or family member to Crusader.

• Crusader on Broadway offers Kids Care Now, a walk-in pediatric clinic.

• Crusader had 209,702 patient visits in 2014 - an all time high.

• Our call center answered over 600,000 calls in 2014...about 200 a day!

• The Crusader Foundation October Evening of Caring gala raised over $180,000 (net). A record amount!

• Crusader pharmacies filled more than 91,300 prescriptions.

• Crusader pediatricians gave over 31,150 immunizations and performed over 2900 school physicals.

• Crusader hosted nursing students and professors from Sweden for clinical education.

• Crusader staff generously donated nearly $40,000 to support Foundation clinic improvements and programs.

• Over 10,000 patients were assisted in learning about Medicaid or insurance options.

The Crusader Community Health Foundation granted Crusader Community Health

$367,137 in 2014. In addition, $274,582 was raised

to support the cost of the Crusader School Based Health Center at Auburn High School.

Crusader Community Health initiated the Spirit of Caring Award in 1992 to honor special people in our community who have demonstrated outstanding efforts in caring for others. In 2007, we added caring

organizations to the award categories. For 2014, we added the Spirit-of-the Future award to honor persons 40 years and younger for their caring efforts. Nominated by family, friends and co-workers in recognition of their

care and concern for others selection of just a few is always challenging.

For 2014, Mariah Anast received the Spirit-of-the-Future Award while Charles Smith, MD, Rebecca Epperson, and Allen Pang DDS receives the Spirit of Caring Awards.

Junior League of Rockford received the 2014 Spirit of Caring Organization Award.

Crusader Foundation’s Evening of Caring Shakes Barriers, Rattles Perceptions and Rolls Forward!

1950s theme gala sets new records and honors Spirit of Caring Recipientswhile supporting the mission of Crusader Community Health.

MARIAH ANASTSpirit-of-the-Future AwardVolunteerism was taught to Mariah early in life - even if you can’t give money, you could give time. Mariah has volunteered for many years with the Rockford Park District including Camp Sunshine, Burpee Museum, and the Therapeutic Recreation Team. She has a passion and compassion for people with disabilities helping with Wheelchair Basketball, Buddy Baseball and Art at the ARC. Mariah has demonstrated dedication, understanding, responsibility, volunteerism and genuine caring for others that reaps tremendous rewards for everyone involved.

CHARLES SMITH, MDSpirit of Caring Individual AwardFollowing the suicide death of their daughter, Carrie, Dr. Charles Smith and his wife Bobbi, were moved into action and began a search for the cause of their daughter’s depression as well as possible solutions. Through networking among the mental health community they formed Group Hope, a free support system dedicated to improving the lives of those dealing with mental health issues. Thousands of meetings have been conducted throughout Rockford helping over 20,000 attendees having an impact on both individuals and the community.

REBECCA EPPERSONSpirit of Caring Individual AwardRebecca Epperson deeply cares for the well being of this community and puts forth incredible effort to give back ten-fold. She encapsulates the United Way’s mantra of “Live United” being deeply involved with UW for the past six years including chair of the 2013-2014 campaign raising $5.3 million. Rebecca gives of her time, talent and treasure serving on the boards of the OSF Saint Anthony Foundation, Rockford Symphony Orchestra and being on the Transform Rockford Communication Team.

ALLEN PANG, DDSSpirit of Caring Individual AwardAfter retiring from a successful private dental practice, Dr. Pang joined Crusader Community Health to treat the medically underserved children of Rockford with dental care and compassion for five years. The Winnebago County Dental Society, recognizing Dr. Pang’s “Service above Self,” awarded him the “Dr. Scott Erickson Memorial Caring Award.” He is a member of the Crusader Community Health Foundation Board, Past Potentate of Tebala Shrine Temple and served on the boards of Klehm Arboretum, Wesley Willows and Rosecance Home. His level of commitment is truly valued and worthy of recognition.

JUNIOR LEAGUE OF ROCKFORDSpirit of Caring Organization AwardFounded in 1930, the Junior League of Rockford, Inc. (JLR) is comprised of a diverse group of women who share a commitment to making our community a better place to live and work. Its mission is to promote volunteerism, develop the potential of women, and improve the quality of life in our community by focusing on the education and welfare of women, children and families. The JLR has touched the lives of over 10 million people in the region through projects initiated by its membership.

The Cosmopolitan Club of Rockford is celebrating a 20-year partnership with Crusader Community Health. The Cosmo Club established the Crusader/Cosmopolitan Diabetes Program benefiting thousands of local and area diabetic patients. Known as the club that fights diabetes, Cosmopolitan Club has donated over $450,000 to Crusader to care for its growing diabetic patient population. In 2007, Cosmopolitan International recognized Crusader as one of its five major Cosmopolitan Diabetes Center Program. Warm Hands, Warm HeartKnitting has always been a passion for Amberwood Care Centre resident Yvonne Fridly. In her retirement, Yvonne has donated her knitting talents to Crusader Community Health. She has knitted literally thousands of mittens, hats and blankets for Crusader patients in need. Her beautiful work is so appreciated and is a great example of The Crusader Way in action!

School Based Health Center Opens at Auburn High School for 3000+ StudentsIn September 2014, Crusader opened its fifth location – The School Based Health Center at Auburn High School. The center is a fully staffed, professional health and dental care facility. It is a combined project of Rockford Public Schools, Crusader Community Health, Rosecrance Health Network, Edgebrook Primary Eye Care and Swedish American Health System.

Crusader Community Health Auburn Campus offers medical, dental, vision and behavioral care. Staff members also provide nutritional and wellness education. Professionals at this facility encourage students to improve their overall health and wellness through screenings and disease prevention. Parents must sign a parental consent form before any medical, dental, optical or behavioral services are provided. This is particularly important if services are needed/given without a parent present.

The School-Based Health Center is open to children 18 years old or younger. Young children who are not of school age are welcome. Students can be seen during school hours, even if a parent is unable to attend the appointment due to work commitments or lack of transportation.

Health services provided at this facility are meant to help the 3000+ students from the area around Auburn High School who attend Rockford schools. However, students from any Rockford Public School can come to the Crusader Auburn HS facility.

Healthy students do better in school. Healthy students have higher attendance rates. They also have better behavior in school and higher levels of achievement.

In the six months since the School Based Health Center has been open, the facility has logged more than 600 medical visits and more than 450 dental visits. There have been approximately 258 visits for new eyeglasses or more than 85 behavioral health evaluations.

Remembering John McHugh, MDDr. John McHugh, a Crusader founder, died on December 6, 2014. Crusader lost a dear friend. In the mid-1960s John, Barbara Roman, Joe Terranova, along with others, saw a need for primary care for expectant mothers in low income families. They began a well baby clinic which grew to what we know today as Crusader Community Health. For 43 years, Crusader has continued to focus on John’s dream...to deliver healthcare services with dignity and respect. We will forever be grateful for John’s insight, courage and incredible sense of humor. John will be greatly missed!

Crusader Community Health provided prenatal care to 1180 patients during 2014. Crusader providers delivered 1139 babies (96.5%) for our prenatal patients. That is about 25% of all

babies born in our region. Making sure new moms are educated about proper prenatal care and understand the

importance of good birth weights is a priority at Crusader.

Over the past year, Crusader Community Health saw a significant decrease in babies being born with low birth weights. During 2014, Crusader’s total low birth weight was 6.2% of all births. This is down significantly from the prior seven years. Latest statistics show low birth weight averages for Winnebago County are 9.5%, for Illinois are 8.5% and for the nation are 8.3%.

Low birth weight is defined as 5 pounds 8 ounces or less, regardless of the gestational period. Low birth weight is a leading cause of death before 28 days of age. Babies with low birth weights are more likely to experience physical and development problems during the first year of life. First year medical costs associated with low birth weights can run a staggering ten times that of a normal weight baby.

Crusader Women’s Health department credits early and ongoing education as the major factor in having fewer low birth weight Crusader babies.

“Proper nutrition is key,” according to Vicki Beck, Director of Crusader Women’s Health program. “We emphasize the importance of proper diet and good lifestyle choices from the first visit and throughout each pregnancy. We offer a variety of outreach programs such as Baby Basics where we teach new parents what to expect and how to adjust to a new life. Armed with the proper tools, our patients respond well. And it looks as if we are seeing the fruits of these efforts. We couldn’t be more pleased with this data.”

Crusader Women’s Health department consists of four OB/GYN physicians, eight certified nurse midwives, two nurse practitioners, several family practice physicians and a complete nursing support staff who helps keep Crusader moms well informed and in great health.

44 Crusader Employees Receive Service Awards!

Spirits were high at Crusader’s annual Winter Conference on January 6 when 44 Crusader employees were honored for significant service tenure. Awards were given for employees with service of over 15 consecutive years.

“For an organization our size, this is a staggering number of individuals that have been called to serve the Crusader mission.” remarked Gordon Eggers, Jr. CEO. “This group represents some significant numbers, too. Together they have recorded 879 years of service and over 1.7 million hours spent helping our patients.”

Low Birth Weight babies being addressed at Crusader through education, preventative care and compassion.

Davenport and Thorpe Receive 2014 Advocate AwardsThe annual Crusader Community Advocate Award is given at the January Winter Conference. 2014 awards went to administrative assistant PeggyAnne Davenport (17 years) and security officer Dan Thorpe (10 years). PeggyAnne is always willing to go the extra mile for a patient and co-worker. Dan has the desire to help whoever or whenever it is needed.

This award honors the memory of Annie Dandridge. Annie, who passed away in 2011,was an extraordinary individual who always exhibited unique kindness and concern for others.

Crusader Expansion on Alpine AnnouncedIn October 2014, former Governor Pat Quinn announced an investment to help Crusader Community Health acquire and renovate a building for another community health center in Rockford. The funding will help ensure all area people have access to quality healthcare. Crusader will receive $3 million to acquire property and a building at 1253 North Alpine Road. Watch for more new of this exciting expansion project duing 2015.

Crusader Community Health underwent a rigorous unannounced on-site survey in September 2014.
