South County Hospital’s (SCH) Cancer Program is accredited by the Commission on Cancer (CoC) of the American

College of Surgeons. This accreditation demonstrates our commitment to meet rigorous standards and offer high

quality, patient-centered care. This includes:

• Patient-centered care with a range of state-of-the-art services and equipment.

• A multidisciplinary team approach to coordinate the best cancer treatment options available.

• Access to cancer-related information and education.

• Access to patient-centered services such as psychosocial distress screening and navigation.

• Options for genetic assessment and counseling, and palliative care services.

• Assessment of treatment planning based on evidence-based national treatment guidelines.

• Information about clinical trials and new treatment options.

• Follow-up care at the completion of treatment, including a survivorship care plan.

• A cancer registry that collects data on cancer type, stage, diagnostic and treatment information and results, and

offers lifelong patient follow-up to further research into cancer trends and comparative effectiveness research.

We are a community cancer center that participates in the nationally recognized National Cancer Data Base (NCDB),

a resource sponsored by the American College of Surgeons and the American Cancer Society. Collectively the data

represent approximately 70 percent of newly diagnosed cancer cases nationwide.

In November 2016, South County Health unveiled its renovated and expanded Cancer Center. The year-long renovation of Read 1 resulted in a modern, fully integrated community cancer center, providing a close-to-home alternative to patients in Washington County and surrounding areas.

The South County Health Cancer Center features:

• An integrated services room where patients receive Reiki and massage therapy.

• Nine exam rooms where doctors meet privately with patients.

• A redesigned infusion therapy area that allows a direct line of sight for caregivers. The area features 11 infusion bays with comfortable chairs specific for infusion therapy, and three private rooms.

• Space for patient/family meetings, counseling and support groups, oncology nurse navigators, and a conference room.

A CAMPAIGN for CANCER CARE

The 2016 DBLCL patient study was conducted by James L. Smythe, MD, of South County Oncology/Hematology.

Patients diagnosed between 2013 and 2015 were reviewed for adherence to National Comprehensive Cancer Network (NCCN) guidelines.

Treatment regimens at South County Health aligned 100% with NCCN guidelines.

Continuous complete remission was obtained in 91% of our DLBCL patients.

In terms of performance improvement, two minor areas that could lead to better data were identified, which will be supported by the new oncology module rolled out in December 2016.

PATIENT OUTCOMES

South County Radiation Therapy (SCRT), a joint venture of South County Hospital and 21st Century Oncology, serves as an integral member of the South County Hospital oncology program.

SCRT was established in August 2005 to provide superior radiation therapy services to local patients, providing the latest in radiation technologies.

The center is staffed by two highly qualified Radiation Oncologists, Dr. Angela DeRobertis and Dr. Timothy Shafman.

SCRT uses intensity modulated radiation therapy (IMRT) for optimal sparing of normal tissues and cone beam CT scan localization for the highest quality image-guided radiation therapy (IGRT). Through our 21st Century Oncology of Rhode Island affiliates, local patients also have access to High Dose Rate (HDR) brachytherapy for both breast and gynecologic cancer treatment, as well as Stereotactic Body Radiotherapy for treatment of early stage lung cancer.

Treating hundreds of new patients each year, South County Radiation Therapy has become a fixture in the cancer care community. We network with specialty providers across the state to make sure our patients receive the best possible care.

CANCER PROGRAM DATA

The data reported here reflects “analytic” cases, patients who were diagnosed with or treated for, cancer at South County Health. Patients presenting with recurrence or progression of disease are not considered “analytic” cases.

Based on our 2015 analytic data, our top 10 cancer sites are: breast, lung and bronchus, other GI, bladder, colon and rectum, prostate, non-Hodgkin lymphoma, myeloma, corpus & uterus, and kidney and renal pelvis.

For our 2015 analytic cancer cases, the age group with the most patients was 70-79. Those 60-69 had the second largest number of patients. Patients aged 80-89 outnumbered those 50-59. In our Washington County community, those ages 60-79 are most likely to become cancer patients.

Nearly 4 in 10 cancer cases diagnosed and/or treated at South County Health are early stage cancers (Stages 0 and 1). Stages II and III account for about 22% of cases, and have likely advanced locally, in close proximity to where the cancer started. About 19% are considered metastatic, where cancer has spread to distant areas from the original site.

Quality Measures, Accountability, SurveillanceThe Cancer Program measures that are followed for every Commission on Cancer facility encourage quality improvement. These have demonstrated that “improvements in data quality and patient care are possible when the entire cancer committee supports system level enhancements.”

Radiation is administered within 1 year of diagnosis for women under age 70 receiving breast conserving surgery for breast cancer (BCS/RT)

The Breast Health Program (BHP) offers advanced diagnostic, surgical and treatment options for all breast disease. Our Breast Health Navigator offers guidance at every stage to ensure that all breast patients receive the optimal medical, emotional and educational support while providing a comfortable single point of contact for our patients and their referring physicians.

South County Hospital’s Women’s Imaging Center offers sophisticated imaging technology: diagnostic screening, digital mammograms, breast ultrasound, breast biopsy and bone densitometry. Board-certified radiologists work with a highly trained, compassionate staff in an environment designed with your privacy and comfort in mind.

Medical & Wellness Center Westerly

With the opening of the Medical & Wellness Center in Westerly, women living in the southern communities of Washington County now have the same 3D mammography technology used at South County Hospital. This technology allows radiologists to see breast tissue detail in a way never before possible, making early detection and treatment available.

The South County Health Breast Health Circle of Friends Program, sponsored by the Gloria Gemma Breast Cancer Resource Foundation, provides education and support services to newly diagnosed, in-treatment and post-treatment breast cancer patients and their loved ones. Among the activities, the monthly programs offer art therapy, Reiki, reflexology, massage and nutrition counseling.

Circle of Friends

The South County Health Cancer Center provides a weekly Cancer Conference that brings together the cancer care team of experts to review specific cancer cases. National Cancer Survivor’s Day Location: South County Hospital

Washington County residents attended a Smoking Cessation lecture at the Guild Community Center where techniques & behavior modification strategies were discussed.

Over 70 South County Health staff attended the event to recognize Breast Cancer survivors and represent the Breast Health program at South County Health.

Activities included: Radio interview, team ball presentation and check presentation during halftime.

Oncology medical director, Gerald Colvin, DO, Leah Arsenault, RN and Sherri Zinno presented a discussion on how the Cancer Services at South County Health help cancer patients make their journey as smooth as possible.

Presentation focused on the link between type-2 diabetes and weight.

Promoted South County Health cancer services to thousands of EB employees.

Philip Salko, MD, of Ortho RI represented South County Health at the benefit event for Hope Hospice.

Hematology/Oncology sponsorship for annual American Cancer Society walk to raise cancer awareness.

Annual event recognizes and supports cancer survivors and their families. 50 people attended the event, along with many cancer center staff.

Among the 250-300 attendees were South County Health’s oncology certified specialist in nutrition, community outreach coordinator, cancer program administrator and cancer registry staff.

Sponsored at the $1,500 level and participated in walk to promote cancer awareness in the community.

Over 70 participants and spectators attended the charity event where SCH Cancer Services were promoted.

37 area residents were screened by a physician on the medical staff at South County Health and physician assistant.It was found that:• 24% of all participants had skin cancer of any type, including basal

cell, squamous cell and melanoma. • 3% of participants were found to have reportable melanoma.

Participated through sponsorship to provide promotional and educational assistance to those who are dealing with or caring for someone with cancer. Thousands are drawn to the WaterFire event each year.

Monitoring Adherence to Updated Enteral Support Process for Feeding Tube Patients

Oral complications are common for people being treated for head and neck cancer, often resulting in unintentional weight loss and a reduction in quality of life.

Based on our 2015 study, it was determined that a process for patient education and nutritional management of patients having feeding tube placement was needed.

Corrective actions put into place included the hiring of a Registered Dietitian to provide counseling to patients, identifying a multi-disciplinary process to enhance collaboration focused on patient wellness, creating standardized order forms and using both written and verbal education to promote better patient understanding.

During 2016, we followed our head and neck cancer patients that received feeding tubes to evaluate the updated process.

The results showed that all patients were: • Evaluated by the dietitian prior to or within the first week of initiating treatment• Provided written tube feeding instructions in the form of a new “starter kit”• Delivered tube feeding supplies within an appropriate time frame• Provided homecare nursing teaching within 48 hours of having feeding tube placed

The updated improvement efforts resulted in improved patient understanding and adherence, and reduced weight loss in cancer patients receiving feeding tubes. Cancer patients in need of nutritional support will benefit in the future from this enhanced process.

Oncology nurse navigation (ONN) is a process by which a professional registered nurse, with oncology-specific clinical knowledge, guides patients and families with a diagnosis or suspicion of cancer through and around barriers in the complex cancer care system. The ONN helps to ensure timely diagnosis, treatment, support, education and survivorship care. Barriers to quality care can be:• Financial and economic• Language and cultural• Communication• Education• Health care system• Transportation• Bias based on culture/race/age• FearOncology nurse navigation helps ensure that patients receive culturally competent care that is confidential, respectful, compassionate and mindful of the patient’s safety.

We are fortunate at South County Health Cancer Center to have a Nurse Practitioner leading our Breast Health Program, an ONN to advocate for patients with all cancer types, as well as a nurse navigator educating and following our patients who receive radiation treatment.

Navigation is an important role that makes a quantifiable difference for patients newly diagnosed with cancer. We strive to put a navigator in the hands of every cancer patient to improve quality of care and facilitate communication between the patient, caregiver and their cancer care team.

SUPPORT SERVICES

Oncology Rehab & WellnessIn 2016, our partners in Rehabilitation & Wellness, Oncology Partners LLC, closed its doors. While it will no longer be

known as the STAR program, South County Health has created a new curriculum for training and will continue to offer

all patients Oncology Rehabilation & Wellness Services (also known as a Survivorship Program.)

South County Health is dedicated to this critical component of oncology care and survivorship. We offer a broad range

of rehabilitation services in the out-patient settings. Patients will be scheduled for the appropriate services once an order

is completed. Services include:

• Occupational Health

• Exercise Physiologist

• Massage Therapist

Cardio Rehab Team

Nutrition ServicesIn 2016, the South County Health Infusion Department offered consultations with a Registered Dietitian who is a Certified Specialist in Oncology Nutrition. Patients (with their families, if desired) can meet with our Dietitian during their infusion or clinic visits, making it a convenient support service at our multi-disciplinary clinic.

Optimal nutrition is a key component of cancer treatment and survivorship. Many patients experience side effects of cancer treatment, which often results in unintentional weight loss, fatigue and a decrease in quality of life. Within this past year, our dietitian provided expertise to more than 150 new patients, with over 960 patient encounters.

Our Cancer Support Groups expanded in 2016 to assist people with oral and head & neck cancer. Meetings are held monthly, offering information, support and encouragement to patients, survivors, family members and friends. These meetings, held at the Neighborhood Guild in South Kingstown, continue to grow in attendance.

Understanding that a cancer diagnosis affects those closest to the patient, South County Health offers Caregiver Support Groups. Reliable caregiver support is crucial to the physical and emotional well-being of those with cancer. This monthly support group offers caregiving tips, advice and support.

The Infusion Department began a new tradition in 2016 - “Smoothie Tuesday.” Every Tuesday registered dietitian, Donna Castricone, creates delicious and healthful smoothies for infusion patients, guests and staff to enjoy. Among the most popular flavors are Berry Delicious & Green Goddess smoothies!

The Cancer Care Team at the South County Health Cancer Center is made up of interdisciplinary professionals whose expertise in patient care complement each other to provide an extraordinary patient experience and positive outcomes. From office staff to the surgical team, each member plays a key role in our mission to provide exceptional care. Below are some of the people who make a difference in the lives of others every day.

South County Health Cancer Conference

The Cancer Conference at South County Health is made up of surgeons, radiation oncologists, medical oncologists, radiologists, pathologists, nurses and nurse navigators who meet weekly to discuss a variety of cases being treated at the Cancer Center.

The Cancer Care Team at the South County Health Cancer Center is led by board-certified oncologists/hematologists who are well established and respected among their peers.

Infusion Nurse Team

Carole A. Vincent, DO, general surgeon, joined the South County Medical Group in August 2016. Dr. Vincent is board-certified with 20 years of experience. Her expertise in a variety of cases, including women’s breast surgery, colorectal procedures and biopsies, contribute to the high-quality care and positive patient outcomes of the Cancer Center.From minor procedures to complex surgeries, Dr. Vincent is called upon when surgical options are needed to treat men, women and pediatric patients.Along with Dr. Vincent, the South County Health General Surgery Team includes Umberto Capuano, MD, David Coppe, MD and Matthew Jones, MD.

Prior to construction, PFAC-CC members reviewed design plans and provided input on the Center’s layout and furnishings. Based on their personal cancer treatment experiences, their ideas helped shape the array of services that support patients and help to make cancer treatment less stressful.

During this past year, highlights of the Councils’ meetings and projects include:• Developing a PFAC-CC brochure • Touring the Cancer Center at various stages of construction• Hosting guest speakers, including Anne Schmidt, VP, CNO, Patient Care Services; Dr. James Smythe; and

Dr. J. Russell Corcoran• Gaining information through presentations on music therapy and the South County Health Medical &

Wellness Center Westerly outpatient services• Receiving monthly updates on the Campaign for Cancer Care, given by

Claudia Swain, VP Fund Development• The formation of three subcommittees focusing on elements of care in the Cancer Center:

I. Integrative Therapies: advocated for the inclusion of, and a dedicated space for, Integrative Therapies in the Cancer Center II. Quality of Care: recommended the highest standards of patient-centered care III. Patient Experience: discussed parking and patient amenities

• Providing input on Hospital projects, including naming the new Cancer Center • Members shared their cancer stories at community events and attended the South County Health

Survivorship Reception in June• Obtaining funding to buy insulated Welcome Bags for patients of the Cancer Center

The highpoint of 2016 for PFAC-CC members was the Grand Opening of the Cancer Center on Nov. 17, 2016 where members served as tour guides and greeters for more than 200 guests who attended the ceremony.

The Patient and Family Advisory Council on Cancer Care (PFAC-CC) was formed in 2014 at the recommendation of the South County Health Board of Trustees to provide a forum in which cancer survivors, family members, and SCH staff could provide input and suggestions on plans for the proposed Cancer Center.
