Appendix B – Health Related Services and Intervention Codes (CCI©1)

What is CCI? The Canadian Classification of Health Interventions, referred to as CCI, was developed by the Canadian Institute for Health Information (CIHI). It is a comprehensive list of codes for diagnostic, therapeutic, and support interventions.

Where Can I Get a Complete List of CCI Codes? Visit the CIHI website for more information: http://secure.cihi.ca/cihiweb/dispPage.jsp?cw_page=codingclass_e A downloadable PDF is available on this website that will allow you to learn more about using CCI codes. Providers and software vendors can purchase products and publications related to coding directly from CIHI at www.cihi.ca, including: ICD-10-CA/CCI, 2006 (CD-ROM – Single user)

o This product is similar to CD-ROM encyclopaedias. The infobase automatically indexes every word and users may find information quickly using the software’s search engine.

Code Title Tables 2006 (for use with ICD-10-CA/CCI) o The code title tables include all the valid diagnosis and intervention codes and

their descriptions.

CCI and HCAI HCAI will only recognize valid CCI codes. In many cases it will be important for users of HCAI to learn how to use the HCAI search code utility to locate valid CCI codes. Without valid CCI codes, HCAI users will be unable to submit forms to automobile insurers.

What is a “Healthcare intervention”?

Therapeutic Intervention A service performed for or on behalf of a client whose basic purpose is to improve health, alter the course of a disease or health condition, or to promote wellness. While a therapeutic intervention may contain a diagnostic component, the primary intent of the intervention is to alleviate or treat the underlying disease or health condition.

E-2

Diagnostic Intervention A service performed for or on behalf of a client whose basic purpose is to assess the presence, absence or status of a disease process or health condition.

Other Intervention Any other service that cannot be described as obstetrical (fetal), therapeutic or diagnostic but, nevertheless, contributes directly to improve a client's health, alters the course of a health condition or promotes wellness. One of the key features of the classification is its provider and setting neutrality. The same codes are applicable regardless of whether a doctor, a nurse or a therapist performs the interventions, or whether the intervention is performed in a hospital, a clinic, a doctor’s office or in the home. The list in Appendix B represents a sub-set of CCI. The complete list of intervention codes used in Canada is available from CIHI. CCI is a copyright product of CIHI and may not be changed without their express permission. Providers should contact their associations for guidance with coding. For the purposes of the Automobile Insurance Standard Invoice (AISI), a number of non-CCI codes were developed in consultation with CIHI to complement the existing set of CCI codes. These AISI specific codes are noted with an asterisk (*) and are not part of CCI.

Section (position 1) The Section code represents the broad category or type of intervention. It allows you to quickly differentiate between assessments, treatments (both physical and non-physical) and other healthcare services.

Section Code Section Description 1 Therapeutic interventions (Physical/Physiological) 2 Diagnostic interventions (Assessments, Tests) 3 Diagnostic imaging 6 Therapeutic (Cognitive, Psychosocial, Sensory) 7 Other (Healthcare, Services, Support)

Group (position 2-3) The Group code indicates what was affected. For those interventions related to physical anatomy (Section codes 1, 2 and 3), the group

code represents the body region or body part affected.

Intervention (position 4-5) The Intervention code indicates what was done. A unique set of intervention codes exists for each Section code. These first 5 positions form the base CCI code and are mandatory. For Section 1 (Therapeutic – physical, physiological), the following Intervention codes apply:

Code Description Definition 02 Exercise Assisting and supervising client in performing exercise for

improvement of health or correction of a deformity. Passive, active, resistive. • range of motion exercise • conditioning exercise ( e.g. aerobic) • aquatic exercise, hydrotherapy, pool therapy • postural exercise, balance training, • work hardening, work conditioning

03 Immobilization Rendering a body part incapable of being moved, with or without traction or traction elevation • Application of external immobilization device • Application of splinting device [e.g. cervical collar,

spinal corset] • traction (with or without suspension)

04 Mobilization Moving a joint (or soft tissue) within the physiological range of motion without a high velocity thrust.

05 Manipulation Moving a joint beyond the physiological range of motion with a high velocity thrust.

06 Hypothermy Applying cold to the body or body part to produce a therapeutic effect

12 Therapy The general treatment of condition, disease, or dysfunction to eradicate its cause, manage or reduce the symptoms, or alleviate other associated disabling conditions by using appropriate selected techniques. • Massage • Acupressure, therapeutic touch • Use of vibrating device

For Section 2 (Diagnostic), the following Intervention codes apply:

Code Description Definition 02 Assessment

(examination) Involves evaluating a client's condition, problem, or functional status to establish a diagnosis and/or identify treatment or rehabilitation measures and/or monitor a client's response to an existing treatment/rehabilitation plan. Also includes, where applicable, ordering diagnostic investigations, reviewing results, prescribing medications and counselling patient. • Determination of mental status (2.AZ.02) • General physical examination (2.ZZ.02) • Assessment, language (2.GE.02)

Code Description Definition 07 Facilitation Providing general supportive assistance to a client.

• Assistance with community integration • Mediation of interpersonal relationships

10 Counselling (for a known condition)

Providing health advice to a client that identifies health problems (or risks), encourages a change of client attitude or behaviour in order to initiate a willingness to modify lifestyle or behaviour that will result in an elimination of the health problem (or risk).

30 Therapy The general treatment of a condition, disease or dysfunction to eradicate its cause, manage or reduce the symptoms, or alleviate other associated disabling conditions by using appropriate selected techniques. • Behaviour using desensitization technique • Psychotherapy using active listening technique • Work Hardening

restorative)

For Section 7 (Other – healthcare, services, support), the following Intervention codes apply:

Code Description Definition 01 Assistance The act of helping or aiding a client to perform a task of

daily living. • Bathing a patient [client] • Portering patient • Assistance with feeding

10 Counselling (for a potential condition)

Providing advice to a client that identifies health problems (or risks), encourages a change of client attitude or behaviour in order to initiate a willingness to modify lifestyle

Approach/Technique/Reason (position 6-7) The Technique or Reason code indicates how or why the intervention was done. Generally, this level of detail is not required for automobile insurers. Exceptions include the following:

7.SE.02.AB Assessment of environment (private living space, includes: Home assessment) 7.SE.02.AW Assessment of environment (workplace, includes: Ergonomic and Workplace

assessments) 7.SJ.30.LB Documentation, support activity (for claim form)

Device/Method (position 8-9) The Device or Method Code indicates what was used. This level of detail is not required for automobile insurers.

Attributes In addition to the CCI code, healthcare services can be further specified with Attribute codes. These codes are used to indicate how the service was delivered or the number of views in an X-ray study. The absence of attribute codes means that a service was rendered directly (“in person”), to one individual by an individual provider, and required continuous attendance. Any exceptions to this must include the valid attribute code. Refer to tables below to determine which attribute codes apply to specific interventions. For Section 1 only, the following additional Attribute codes apply:

Code Description IA* Intermittent Attendance

For Section 1, 6, and 7, the following Attribute codes apply:

Code Description IN Indirect (e.g. telephone) SD Self-directed (e.g. using tapes, videos) IM Individual with more than one provider GR Group with one provider GS Group with more than one provider

For Section 3, the following Attribute codes apply:

Code Description A 2 or less views B 3 to 4 views C 5 to 6 views D More than 6 views
