Preface
OverviewThank you for choosing an Applied Biosystems instrument. The Applied Biosystems Instrument Logbook provides a place to store all records for your instrument in one place, for easy reference. The logbook is intended to help you maintain records that meet regulatory requirements. The audience for the logbook includes laboratory staff who use and maintain the instrument, Applied Biosystems personnel who install, upgrade, and service the instrument, and any internal or external auditors.
About the LogbookMany laboratories must comply with regulatory requirements which stipulate that a record of instrument performance be maintained.
The Logbook enables you to maintain a history of your instrument performance. To make sure that the history is complete, please do not remove any pages from this Logbook.
The Logbook also contains a guide to your local Applied Biosystems Customer Support and Service organizations. Applied Biosystems recommends that you keep this Logbook close to the instrument at all times so that it is readily available to all operators of the instrument.
LogbookSections
The Logbook contains the sections listed below. Depending on the instrument you have purchased, initially some of these sections will be empty. They are provided for your convenience so that you can file subsequent performance data, service and test reports, and any other pertinent information.
The following sections are present in the Logbook:
• Section 1: About Your Instrument – This section contains the model, serial numbers, and the installation date for your instrument.
• Section 2: Applied Biosystems Contacts – Use the addresses and telephone numbers of your local Applied Biosystems offices located in this section, if you need assistance.
• Section 3: Installation – File the Installation Service Report in this section.• Section 4: Instruction and Training Log – File copies of all certificates and
signed checklists for training received (including operator or user training, applications training, etc.) in this section.
• Section 5: Work Groups – List all work groups using this instrument and the appropriate contact person for each group in this section.

• Section 6: Verification & Performance Checks – Store all qualification and performance verification documents, including protocols and calibration certificates, if applicable, in this section.
• Section 7: User Maintenance Log – Record all routine maintenance activities performed by your personnel on the pre-printed sheets in this section.
• Section 8: Consumables – Record information on any consumables used during the operation of the instrument.
• Section 9: Planned Maintenance Log – Record all planned maintenance performed on the instrument by an Applied Biosystems Field Service Engineer on the pre-printed sheets in this section. Store completed planned maintenance checklists or protocols in this section.
• Section 10: Service Log and Reports – Record all non-routine service performed on the instrument by an Applied Biosystems Field Service Engineer on the pre-printed sheets in this section.
