INDIANA STATE NURSES ASSISTANCE PROGRAM Chuck Lindquist, Program Director, gave the following report: Samantha Meadows (case manager) has resigned from the program and ISNAP is interviewing for that position. ISNAP is in the process of bidding for the RFP. ISNAP distributed a copy of 4 th quarter 2012 report with 75% full compliance. Ms. McIntosh asked a question about the timeliness of the 4 th quarter 2012 compliance report. Mr. Lindquist indicated it was a month late and ISNAP would work on expediting the information. ISNAP has promised 1st quarter 2013 information six weeks from April 10, 2013, i.e. May 22, 2013. V. EDUCATION A. II. ADOPTION OF THE AGENDA A motion was made and seconded to adopt the agenda as amended. Laux/Ogle, 9-0-0, motion carries

III. ADOPTION OF THE MINUTES FROM THE MARCH 21, 2013 MEETING OF THE BOARD A motion was made and seconded to adopt the March 21, 2013 minutes. Ogle/Laux, 9-0-0, motion carries

IV. INDIANA STATE NURSES ASSISTANCE PROGRAM Chuck Lindquist, Program Director, gave the following report: Samantha Meadows (case manager) has resigned from the program and ISNAP is interviewing for that position. ISNAP is in the process of bidding for the RFP. ISNAP distributed a copy of 4th quarter 2012 report with 75% full compliance. Ms. McIntosh asked a question about the timeliness of the 4th quarter 2012 compliance report. Mr. Lindquist indicated it was a month late and ISNAP would work on expediting the information. ISNAP has promised 1st quarter 2013 information six weeks from April 10, 2013, i.e. May 22, 2013.

V. EDUCATION

A. Vincennes University ASN Program- Request for Curriculum Change (Mistene Halter)

Staff from Vincennes University appeared and requested a curriculum change. Ms. Laux asked whether this was in regards to new general education requirements and staff answered in the affirmative. Staff does not feel as though there was any loss in content in regards to this change. A motion was made and seconded to approve. McIntosh/Laux 9-0-0

B. Kaplan College- Request for Consolidation of Campuses (Jill Buchholz)

Staff appeared from Kaplan College and requested consolidation of campuses. Ms. Narwold wondered if this would require a site visit. They have achieved full accreditation prior to consolidation. The site visited was the northwest site and they will need to go to the south. A site visit will be scheduled to look at space for the program. Staff indicated they plan to consolidate and start the next class at the end of July 2013. A recommendation was made for a site visit and they are accredited by ACCSC. There is no accreditation issue so one person can conduct the site visit. The recommendation is to table for completion of the site visit. Construction will be completed in early July 2013.

C. University of Evansville BSN Program- Request for Curriculum Change (Amy Hall) Staff appeared on behalf of University of Evansville and requested a curriculum change. The Board had no questions or issues on this change. A motion was made and seconded to approve the request. Narwold/Laux 9-0-0, motion carries

D. FYI Only-Indiana University Southeast-Jacquelyn Reid, MSN, EdD, CNE appointed Interim Dean of Nursing

Program replacing Mimi McKay, MSN, EdD

E. Medtech College-Fort Wayne LPN and ASN Programs-Bi-monthly progress report and request to reinstate to

full accreditation (Phyllis Bragg)

Staff appeared on behalf of Medtech College Fort Wayne campus. They have cleaned up the information for the GED issues and consistency in student files. Ms. Cooper asked a question regarding students testing late and intervention processes for late testers. Staff indicated they enroll in ATI prior to graduation. Ms. Laux asked if they are in ATI throughout the program or at the end and staff indicated they are in the ATI throughout the program. Ms. Laux asked if they keep track of results after being given the green light and staff indicated they do. Ms. Laux asked what happens when they do not green light and staff indicated students can meet with campus staff and plan a course of study. Ms. Cooper asked if they have testers not reported since their request was submitted. They have had 4 students pass the NCLEX since then. Staff indicated that students that graduated in 2010 or 2011 they are reaching out to students and have specific plans in place but only some come back to them. Ms. Laux asked what percentage that is and staff could not give that information but indicated they have less than 20 that have certificates of completion out and are not connected with the campus. Ms. Laux asked about TEAS criteria and staff indicated they utilize the TEAS for the RN 52% cumulative and for PN 42% cumulative. Ms. Dolk asked about looking at those not successful as to rigor of exams and their med/surg courses. Staff indicated they have done that. Ms. Cooper asked if ATI is embedded in the curriculum and used with every course and staff indicated they have that. Ms. Cooper asked about attrition rates and their 2012 rates for RN 29% and PN 32%. Staff indicated they have just instituted these rigors in 2011-2012 and are only a year into it. Ms. Laux asked about faculty involvement and staff indicated they are providing evaluations of classroom methodology, looking at exam outcomes and mentoring of faculty. They have provided in services on lesson planning and they have a faculty committee meeting. Ms. Cooper asked about cut scores with TEAS admission asked if this has been changed and she indicated it has not been changed. Ms. Hall indicated that the admission requirements appear to be too low as students are admitted but cannot complete. Ms. Hall asked about NLNAC accreditation or programmatic accreditation and whether they have started that process. Board Action: A motion was made and seconded to continue the conditional accreditation with a site visit. Narwold/Dolk, 9-0-0, motion carries

VI. DISCUSSION

A. Delores Stanley- Request for Modification

Mr. White reviewed Ms. Stanley’s request for modification of the suspension of her license. The Board held a discussion of the matter. A motion was made and seconded to deny the request for modification. McIntosh/Ogle 9-0-0, motion carries B. Bonnie Quick- Request for Modification

C. Legislative Update (Anne Ogle) Ms. Ogle gave an update that the nursing education consultant bill was successfully passed this session. Representative Milo Smith and Senator James Merritt were our legislative sponsors. Governor Pence signed the bill April 15 and it goes into effect on July 1, 2013. VIII. BOARD DIRECTOR REPORT

Ms. Kiefner Crawford reported the following:

• The NCSBN has completed a review of military education vs. traditional education. The Board received a handout of the study of this information;

• The application process for May graduates is going very smoothly and the staff has received positive feedback on the changes;

• Outreach was held at University of St. Francis and Ivy Tech Terre Haute this month. Board members are encouraged to participate in outreach.

• New outreach events have been scheduled for Ivy Tech Richmond/IU East conference and for the Indiana Organization of Nurse Executives fall conference;

• The Board will be traveling to University of St. Francis-Fort Wayne in September 2013 and Valparaiso University in April 2014 for full Board meetings as part of our outreach efforts;

• A progress report was given on review of the RN survey and our collaboration with ISDH and the Indiana Center for Nursing to focus our questions on the national minimum data set.

A. Appearances by Applicants for Licensure

1. Juliann Kendall Lowe LPN Exam CBC Hit/Positive Response (Full Board) Ms. Lowe applied for licensure by examination. There are some issues on her application with a 2005 OWI. She indicates that she had a BAC of .11 on this evening. She had a battery charge in 2007 when bartending she kicked an ex-boyfriend out of the bar. A week later she was arrested and pled guilty to disorderly conduct. She was put on criminal probation as a result of this action. She had two charges of practicing nursing without a license in 2010 and 2011. She indicates she was told she had passed the state board, she was sent a license and she did not pass the state boards. She was convicted of unlicensed practice of nursing and indicates she was on criminal probation for a year. She agrees that she practiced without a license for a year and never took the NCLEX examination. She worked at Zionsville Meadows on a long term care unit as an LPN. Board Action: A motion was made and seconded to deny the ability to take the LPN exam. Ogle/Cooper, 7-0-0, motion carries

2. Tyler Alan Peterson RN Exam Positive Response Mr. Peterson appeared for his personal appearance regarding his application for licensure. In 2012, he received a minor possession charge. He was with some friends sitting around a fire. The police showed up because of the fire. They arrested everyone that was underage. He was 20 at the time. He is allowed to test and license to be issued free and clear.

3. April Nichole Compton RN Exam CBC Hit/Positive Response* Ms. Compton appeared for her personal appearance regarding her application for licensure. She received a DUI 4 years ago. She got off work around midnight and went to a bar with a friend. She had 2 beers and a shot and then drove home. Her blood alcohol level was .12. She completed her criminal probation. She has had no other issues. She is allowed to test and her license is to be issued free and clear.

4. Sierra Mercy Rhodes RN Exam CBC Hit/Positive Response Ms. Rhodes appeared for her personal appearance regarding her application for licensure. She was arrested in 2010. She had been drinking prior to a football game. She went to the game and does not remember anything. The drinks came from a mixed jug that someone had made. She was arrested and received a battery charge. She was underage at the time, she was 20. She has completed her criminal probation and all sanctions from Marion University. In 2009, she had a public intoxication and trespassing charge. She had a couple of shots with friends. She is allowed to test. She needs to have an ISNAP evaluation. If she meets criteria for an RMA, then her license will be issued on probation the length of the RMA. If she does not meet criteria for an RMA, then her license is to be issued free and clear. 5. Tationna Arnold LPN Exam Graduated in 2007 CONTINUED

6. Katie L. Grevenstuk RN Exam CBC Hit/Positive Response Ms. Grevenstuk appeared for her personal appearance regarding her application for licensure. In 2010, she was arrested in Michigan for possession of marijuana. She was driving back with her boyfriend and he was stopped for

speeding. The officer searched the car and found the marijuana. The marijuana was hers. She no longer smokes it. She has not had any other problems. She went through drug classes, drug testing, and NA meetings. She now rarely drinks alcohol. She is allowed to test and license to be issued free and clear. 7. Amandip Kaur LPN Exam Failed 7 Times/Graduated 2007

Ms. Kaur appeared for her personal appearance regarding her application for licensure. She graduated in 2007. She has tested several times in California and did not pass. She will need to take a review course with predictability. Once she sends in documentation showing she has 95% or greater chance of passing then she will be allowed to test and license to be issued free and clear.

8. Michael Schluttenhofer RN Exam CBC Hit/Positive Response Mr. Schluttenhofer appeared for his personal appearance regarding his application for licensure. In 2011, he received a DUI. He was out for dinner and had several drinks. After this he drove home. He does not recall his blood alcohol level but knows it was greater than .08 but less than .11. He reports currently drinking once every other month or so. He has not had any other issues. He is not on any kind of criminal probation. He is allowed to test and license to be issued free and clear.

9. Kimberly Nason RN Exam Positive Response Ms. Nason appeared for her personal appearance regarding her application for licensure. In 2011, she was arrested for PI and criminal mischief. She went out to a local bar. She knew she was too drunk to drive so she and a friend decided to walk. They stopped at a lumber yard and she took some flowers from a flower pot. An officer went by and saw them. Her friend ran and he was caught. They both were arrested. She went through a pretrial diversion program, which she was completed. She is allowed to test and license to be issued free and clear

10. Amanda D. Humphrey LPN Exam CBC Hit/Positive Response Ms. Humphrey appeared for her personal appearance regarding her application for licensure. She reported that 10 years ago she was arrested and took the blame for something her boyfriend at the time had done. She was in a relationship with someone who was addicted to drugs. He took her vehicle out one night and broke into several other cars. She was not aware thishappened until a few days later. Her boyfriend had been in a lot of trouble so he talked her into taking the blame since she had never been in trouble. He told her that she would just get a slap on the wrist. She is no longer in contact with him. In 2009, she was pulled over and had been drinking. This happened 6 months into nursing school. Her school was made aware of the incident. She had gone out with friends. She had a driver and they came back home. The driver got upset and left so she went out looking for her and was pulled over. She rarely drinks anymore and if she does it’s only at home. She is allowed to test and license to be issued free and clear.

B. Appearances by Applicants for Renewal

1. Sharon E. Kreilein RN Renewal Expired since 2009

Ms. Kreilein appeared for her personal appearance regarding her application for renewal. She has worked for 30 years as a nurse with no bad records. She has rheumatoid arthritis which causes her issues when it comes to working. She started working on her masters and is caring for her parents. During this time she let her nursing license lapse. She doesn’t think she could work full time but she has looked into some part time positions. She is allowed to renew her license free and clear. 2. Emily Lois McKnelly LPN Renewal Positive Response

Ms. McKnelly appeared for her personal appearance regarding her application for renewal. I Dnecember 2012, she received a DUI. She had had too much to drink. She was out when she received a call from her babysitter. She needed to leave so she drove home. She was pulled over for accelerating through a yellow light. She is currently on non-reporting criminal probation. The probation will be finished in December 2013. She has had no other issues. She needs to have an INSAP evaluation. Her license is to be renewed on probation. The probation will be the length of her criminal probation or the RMA, if meets criteria, whichever is longer. 3. Mary Ellen Morrow LPN Renewal Previously Denied Renewal* Ms. Morrow appeared for her personal appearance regarding her application for renewal. In 2010, she was going through a divorce and a lot of family issues. She was placed on Xanax. She had never taken Xanax before. She went into Wal-Mart and stole a sock and candle. She does not remember any of this. She went to counseling for this. She renewed her license in 2012, and that renewal was denied because she failed to appear for her personal appearance. She applied for renewal again and she answered “no” to all the questions. . She answered no because she had answered yes before. She is not on any kind of criminal probation. Her license is to be renewed free and clear.
