Spring is here…. Or did we skip spring and go straight to summer? With the record break-ing temperatures in the 80’s here in the Metro East in March, it certainly feels more like early summer. Protecting our skin all year is important, but now that it is warm so early, it needs to be at the top of our minds. April is actually Skin Cancer Awareness Month, so be sure to check out the article from our local American Cancer Society office for tips on skin protection. April is Healthy Vision Month and in conjunction with that, IL Eye Surgeons is the feature article. Keep your eyes healthy by scheduling a yearly exam. IL Eye Surgeons brings state of the art technology and procedures to the Metro East area. National Mental Health and Stress Awareness Month are also in April. National Alliance on Mental Illness gives insight on Post Traumatic Stress Syndrome and how it effects veterans and others who have been through extremely stressful situations. Boy Scouts are taught to be prepared, and five of them got some early preparation recently for a possible career in health care, thanks to Dr. Geoffrey Turner.

Dr. Turner, an OB/GYN on staff at Alton Memorial Hospital, led five Boy Scouts on a tour of the hospital as part of the Medicine Merit Badge program. Dr. Turner is the Merit Badge counselor for Alton Boy Scout Troop 7. “To earn the Medicine Merit Badge, they had to do a fair amount of book work on the history of medicine, famous people in medicine and on the medical systems of different countries,” Dr. Turner said. “They had to look into a variety of healthcare specializations from physicians to medical technologists as well as the requirements for each position.” The Scouts included Stephen Turner, Ryan Haynes, Connor Haynes, Eric Ziino and Aaron Russell. The tour started at Dr. Turner’s primary care office in Alton Memorial’s Medical Office Building A, then involved a surgical skill lab, a tour of Pathology with Dr. Susan Rayne and a visit to Dr. Theodore Vander Velde in radiology. In surgery, Dr. Turner was assisted by nurses Katie Sievers and Laurie Merle, who took over when Dr. Turner was called away to de-

Dr. Turner gave the Scouts a simulation of laparoscopic surgery, with various everyday items hidden inside a “body” while the Scouts operated the camera as well as the surgical tools to manipulate the objects. “Although not a requirement for the badge, I thought that showing the boys how some of their video game skills might translate into the surgical suite might be fun,” Dr. Turner said. “Laparoscopic surgery requires a lot of hand-eye coordination that boys who play a lot of those games will develop.” The Scouts easily discovered the difference between real life and the fantasy world of video games. “They were all impressed with the one-handed surgeon’s knot,” Dr. Turner said. “I asked them, ‘Aren’t all Eagle scouts good with knots?’ One of them responded, ‘The last time I put up a tent it wasn’t bleeding.’”

Post Traumatic Stress (PTS) has always been part of the human experience. Biologically it harkens back to our cave dwelling ancestors who experienced the “flight or fight” choice when con-

fronted with a hungry cave bear. It can even manifest when it is simply the constant belief that said cave bear might always be lurking just around the next tree. What that means is that for traumatic stress to occur an actual event is not always present – just the potential of one happening may be enough! Like being in a combat zone, continually on high alert - but never actually seeing action. The constant exposure is enough to bring on stress. Our bodies are amazing and to help us live through the trau-matic experience of meeting the bear it releases several catechol-amines including adrenaline. We all have experienced that to some degree. On the addition side, adrenaline increases our heart rate and pumps glucose into our muscles which adds to physical strength. Our ability to hear, see, and think is turned up a notch and

we can react faster and with more acuity than before. We are ready for “flight or fight”. Imagine you are an airbag. You are in a car that has suddenly gotten all these crash signals and so instantaneously inflates. Flight or fight is just that quick and does NOT involve a choice on our part. The car is hard wired to deploy that airbag. We are hard wired to respond to stress. What happens next may be the origin of PTS. The body, once the danger or the perception of danger has passed, seeks to return to its pre-stress level by countering with other chemicals including one called cortisol which metabolically reduces the adrenalin and mitigates the formerly useful but now unnecessary effects. Cortisol (and others) helps the body chemically control the stress response. If enough cortisol is not produced then the effects of stress can linger and the neurotrans-mitters that keep our brain working can get re-wired and PTS occur. PTS does not come from cave bears today. The “bears” of today are rape, childhood abuse, spouse beatings, car accidents, fires, and

events like 911. Today, more than ever, PTS is occurring in individuals that have chosen to serve our country in the military. This is a potential epidemic that our country cannot simply wish away. Service members have earned the right for the civilian world to un-derstand their symptoms, to learn how to help, and then to pledge to commit the resources to bring recovery to those that are living with PTS. What are some of the symptoms of PTS? Ones most often re-ported are flashbacks, emotional detachment, and “jumpiness”.• Flashbacks: This is like having a horrifying movie clip running over

and over again and you can’t find the switch to turn it off. There may be a trigger such as an image or a sound that starts the movie run-ning. A flashback can occur while awake or asleep. To the person that is caught up in the movie it is real, very, very real.

• Emotional detachment: This may almost be an invisible symptom as it may not be outwardly apparent except to the person expe-riencing it or to their loved ones. Persons with this symptom may have difficulty forming and maintaining loving relationships with fam-ily. Individuals may simply seek to avoid anything associated with the trauma. Functioning every day may become a challenge. Sadly, this detachment may result in impaired parenting skills so their children may have similar difficulties forming social bonds. Look at this impact on children as an echo of the deafening sound of PTS.

• Anxiety, agitation, or nervousness: Individuals may startle easily or be described as “jumpy” or appear to overreact to what may appear to others as normal stimuli. During the day individuals may have difficulty at work and at night be unable to sleep or sleep for typical intervals.

Any sudden noise might startle anyone, but for someone with PTS that sound might cause them to practically “jump out of their skin” (known as hyperactive startle reflex). There may overreact to small things and have difficulty concentrating, which would affect their job performance. They may appear to be on constant look out as if searching their environment for potential danger (this is hyper vigilance). Trouble falling asleep or stay-ing asleep in this high state of arousal is also a common consequence.

Is there treatment and interventions for this stress disorder? Yes, and it varies from person to person but psychotherapy and medica-tions offer hope. There is promising research that certain medications including ones for depression or blood pressure may be prophylactic if given immediately after the event. Unfortunately there is little to be done in the way of prevention as the causative event may be random or unpredictable.It may come down to finding a way to re-wire the neurotransmitters that make stress a debilitating. We have much to learn and knowledge is one of the best ways to cope with PTS. Want to learn more? Enjoying an Egg a Day can be Part of a Healthy Diet

When it comes to eggs, dietary cholesterol and heart health, what you think you know may be a bit scrambled. Concerns over dietary cholesterol and its impact on heart disease keeps

many people from eating eggs, despite their nutritional benefits. How-ever, more than 40 years of research shows healthy adults can enjoy an egg every day without significantly impacting their risk of heart disease.

year and the root of the tooth the second year, thus requiring at least two years of braces treatment. son were also the first in the area to use ultrasonic phacoemulsification to routinely remove cataracts. That tradition of firsts is still alive today. Doctors from IES were the first to implant all generations of the new Premium Lens Implants. These implants make it so patients are less dependent on glasses after surgery. IES was one of the first 20 sites in the entire country (and still the only in the St. Louis area and downstate IL) to off laser-assisted cataract surgery. They have become national leaders on laser assisted surgery, doing more of these procedures than almost anyone in the entire country. Dr. Bart Jones and Dr. Michael Jones are national consultants on laser cataract surgery and premium lens implants. Because of their expertise, doctors from IES are even involved with FDA clinical trials to study the next generation of lens implant and laser technology.

LASIK/REFRACTIVE SURGERY IES does more than just cataract surgery. They are also the area’s leader in refractive surgery and LASIK. RETINAL SERVICES Dr. Tom Fleming is Illinois Eye Surgeon’s director of Retinal Ser-vices. He has been treating patients of the area for almost 20 years. Patients with retinal problems like macular degeneration or retinal de-tachments often have the sickest eyes, and require special care and intense treatments. Experience is important in these situations and IES and Dr. Fleming perform more retinal surgeries than anyone else in the area. They are also leaders in the treatment of macular degeneration, offering the latest in injectable medicines.

GLAUCOMA Glaucoma is a leading cause of permanent blindness in the U.S. Early detection and prevention is important. IES has the latest technol-ogy to detect glaucoma at its earliest stages. EYE PLASTICS Next to the development of cataracts clouding the vision, the de-velopment of drooping eyelids is the second most common treatable condition the doctors of IES deal with. With a quick and painless opera-tion, the doctors can remove the excess tissue of the eyelids and raise them above the line of vision. Most of the time if the drooping is bad enough it is covered under a patient’s medical insurance as well. Other conditions can affect the eyelids, like lashes growing into the eye, lids turning in or out, or skin cancers on the eyelids. PRIMARY CARE Illinois Eye Surgeons has 9 optometrists that make up its Primary Care Division. This is for patients that need routine eye care, but do not require surgical intervention. IES optometrists can diagnose and treat nearly all common eye problems. They are specialists in dispensing eyeglasses and contact lenses. IES was one of the first practices in the country to offer emPower digital bifocals. These are bifocal glasses that will change magnification with the touch of a button.

RESEARCH AND EDUCATION IES doctors are faculty at St. Louis University Eye Institute, and are involved in the training of medical students and resident physicians. You may see these future doctors accompanying your IES doctor in the clinic or operating room. MISSION CATARACT Dr. Ed Doisy started Mission Cataract as a way to give cataract surgery to uninsured patients 20 years ago. It has grown every year, and in 2011, the 20th anniversary of Mission Cataract, it was the big-gest year ever. Staff, nurses, technician, and surgeons all donate their time to help care for these less fortunate patients. They are often the worst of the worst cataracts in patients who have slipped through the cracks up until now.

To qualify for hospice care, the patient can no longer benefit from curative treatment and life expectancy is about 6 months. Hospice care is available as a benefit of Medicare/Medicaid as well as most private insurance plans. This includes the coverage of medical equipment, supplies and medications pertaining to the hospice diagnosis.

What is Private Duty? Private Duty services provide a customized plan of care that in-cludes; Nursing Visits, Home Health Aides, Companion Care and Homemaker Services. These services are designed to respond to the individual needs of those who require assistance with the activities of daily living - allowing the individual to live independently, often bridging the gap between independent living and institutional care. Private Duty services usually fall outside of the criteria of “skilled, intermittent and short-term home health care” and therefore are not an available benefit of Medicare/Medicaid. The most common methods for covering the cost of private duty home care is through private pay-ment or long term care insurance benefits.

Emergency Preparedness at Home

No matter where you live, your community may experience a natural or man-made disaster; a tornado, flood, winter storm, earthquake, fire, or a terrorist attack. In any type of disaster,

lives can be saved if people are prepared for the emergency, and know what actions to take when it occurs. What we know about disaster preparedness is quite simple; there are not enough shelters and hospitals to house all of us. There are actions that you can take that will help you get ready for, and to cope with, almost any type of disaster. Knowing what to do is your best protection and also your responsibility. In this article, we will focus on the most immediate forms of home preparedness.

Perhaps the most basic thing to remember is to keep calm. This may mean the difference between life and death. In disasters, people often are killed or injured needlessly because they acted thoughtlessly or did nothing to respond to the emergency. In a time of disaster, taking proper action may save your life. Take time to think and then take the appropriate action. Usually this will be an action that you have planned in advance, or the action that you are instructed to take by local authorities. Whenever a major storm or other potential disaster threatens, keep your radio or television tuned in to local stations to hear weather reports and forecasts as well as other information and advice that may be broadcast by local authorities. In our area, it is also advisable to obtain a NOAA Weather radio which can be programmed to set off an audible alert when severe weather threatens your immediate area. Use your telephone only to report important events (such as fires, flash floods, or tornado sightings) to local police. If you tie up the tele-phone lines, you may prevent emergency calls from being completed. Public safety call volumes typically spike during times of disaster and the 911 circuits can become overwhelmed. A knowledge of first aid and basic emergency medical care can save lives and reduce suffering. The Emergency Services personnel that we rely on everyday may be unable to immediately respond to all of the calls for assistance. Both adults and teenagers can acquire these valuable skills by taking general first aid courses and specialty courses, such as CPR, which are offered free in many communities. In a fire or other emergency, you may need to evacuate your house, apartment or mobile home on a moment’s notice. You should be ready to get out quickly. Develop an escape plan by drawing a floor plan of your residence, showing the location of windows, doors, stairways and large furniture. Indicate the location of emergency supplies, fire extinguishers, smoke detectors, collapsible ladders, first aid kits and utility shut off points. Next draw a broken line charting at least two escape routes from each room. Finally, mark a place outside of the home where household members should meet if an evacuation is necessary. Be sure to include important points outside such as garages, patios, stairways, elevators, driveways, and porches. Put together escape plans for each floor of your home. Practice emergency evacuation drills with all household members at least two times each year. Know how and where to shut off gas, water and electricity to your home. Keep necessary tools and wrenches handy. Keep important papers in a safety deposit box. House deed, insurance policies, birth certificates, and lists and photos of your possessions for insurance purposes will be essential after the disaster. This is just the beginning of emergency preparedness at home. To further prepare, think about things that you use every day (food, water, medications…) and build yourself an emergency response cache. But then again why is it so hard for many of us to follow-through? Not enough time? Not enough knowledge? Lack of resources? Em-barrassment? Fear of injury or re-injury? These are all very common reasons that the physical therapists at PRORehab hear when work-ing with patients to institute an exercise regimen. Chris Amick, Clinic Director at the PRORehab in Edwardsville says, “Many of our patients come to us following an injury that in some ways could have been prevented. We work hard to educate our patients on the value of ex-ercise following their formal course of therapy. People must continue to exercise, to move.” So where should people start? “An exercise program should in-clude components of cardiovascular conditioning, strengthening and flexibility exercise. Their program doesn’t have to take hours each day,” states Nate Wilson, PT in Alton. “Getting started with a walking

program is progress. It gets people moving.” Wilson suggests simple things such as parking further from the entrance of work, taking the stairs or walking around the block as a starting point. “Little things can add up to improve overall wellness.” Motion is also crucial following certain surgeries or injuries. “The days of bed rest following a low back strain are over,” states Amick, “the research supports getting people moving early and often.” In cer-tain instances, lack of movement leads to stiffness, joint pain and de-conditioning, all of which are problematic and can lead to further injury. “We see countless instances of stiff joints following surgery or injury. There are very few cases where that is a positive.”

Infections of the gums and the bone that surround the teeth are known as Periodontal Disease. The term periodontal means “around the teeth”. The most common diseases that affect the tissues sur-

rounding the teeth are gingivitis and periodontitis. These are serious infections that not only, cause tooth loss but also lead to other health problems that most people don’t associate with the mouth.

What is Gingivitis? Gingivitis is inflammation of the gum tissues that surround teeth. Nearly 75% of people living in the United States have some form of untreated periodontal disease. Most of these people have gingivitis. The most common signs of gingivitis are red gums, smooth, puffy or swollen gums, and gums that bleed when they are brushed and or flossed. For people with gingivitis, regular cleanings with their dental hygienist, and improved brushing and flossing can cure the disease.

What is Periodontitis? If gingivitis is left untreated, there is a chance that it can change into periodontitis. When plaque and tartar begin to move below the gum tissue, the bone around the teeth can become inflamed. As the bacteria that live in the plaque and tartar move lower and lower in the infected gum tissue, the body has a complex immune response that leads to bone loss around the teeth. As the condition progresses, the teeth can become loose and fall out. Periodontitis is the leading cause for tooth loss.

What Health Problems Can Result from Periodontal Disease? Periodontal disease can be a fast moving, aggressive infection in some patients. Patients that have periodontal disease need to be treated. The bacteria from periodontal disease are shed into the blood stream and cause other problems in other areas of the body. Peri-odontal disease has been linked to:

• High blood pressure• Heart Disease• Stroke• Chronic inflammation• Diabetes• Some lung infections• Low birth weights

Since periodontal disease has been associated with these other dis-eases, it is important that patients be seen regularly by dentists to insure that their gum tissues, the tissues that surround the teeth, and the bone that surrounds the teeth to make sure that they are healthy and stable. There are also some conditions that can cause periodontal disease to worsen in some people. Smoking, diabetes, hormonal fluctuations, and cer-tain medications can lead to periodontal disease progressing in some patients.

How is Periodontal Disease Treated? Gingivitis and periodontal disease are monitored and treated by den-tists, dental hygienists, and periodontitis. If you think that you have peri-odontal disease, it is important to have a dentist evaluate your condition. Dentists work with dental hygienist to measure the amount of inflammation and bone loss that patients have undergone. Then, most patients undergo series of cleanings above and below the gums to remove the inflammation and make sure that the teeth are clean and healthy. In most cases, the disease can be reversed or stopped from progressing further with routine checkups and cleanings as recommended by your dental professional.

Skin cancer is the most common of all cancer types. More than 3.5 million skin cancers are diagnosed each year in the United States. That’s more than all other cancers combined. The num-

ber of skin cancer cases has been going up over the past few decades. The good news is that you can do a lot to protect yourself and your family from skin cancer, or to catch it early enough so that it can be treated effectively. Most skin cancers are caused by too much exposure to ultra-violet (UV) rays. Much of this exposure comes from the sun, but some may come from man-made sources, such as indoor tanning lamps. Finding possible skin cancers doesn’t require any x-rays or blood tests – just your eyes and a mirror. If skin cancer does develop, finding it early is the best way to ensure it can be treated effectively. Ultraviolet (UV) radiation is thought to be the major risk factor for most skin cancers. Sunlight is the main source of UV rays, which can damage the genes in your skin cells. Tanning lamps and beds are also

sources of UV radiation. People with high levels of exposure to light from these sources are at greater risk for skin cancer. The amount of UV exposure depends on the strength of the rays, the length of time the skin is exposed, and whether the skin is pro-tected with clothing or sunscreen. Skin cancers are one result of getting too much sun, but there are other effects as well. The short-term results of unprotected exposure to UV rays are sunburn and tanning, which are signs of skin damage. Long-term exposure can cause prematurely aged skin, wrinkles, loss of skin elasticity, dark patches (lentigos, sometimes called age spots or liver spots), and pre-cancerous skin changes (such as dry, scaly, rough patches called actinic keratoses). It isn’t possible or practical to avoid sunlight completely, and it would be unwise to reduce your level of activity to avoid the outdoors because physical activity is important for good health. But too much sunlight can be harmful. There are some steps you can take to limit your exposure to UV rays. Some people think about sun protection only when they spend a day at the lake, beach, or pool. But sun exposure adds up day after day, and it happens every time you are in the sun. “Slip! Slop! Slap!… and Wrap” is a catch phrase that can help you remember the 4 key steps you can take to protect yourself from UV rays: • Slip on a shirt. • Slop on sunscreen. • Slap on a hat. • Wrap on sunglasses to protect the eyes and sensitive skin around them.

These steps complement each other, and they provide the best protection when used together.
