Abatement Contractors of Montana, LLC. (ACM) is pleased to provide this sampling reportfor a Asbestos Inspection conducted at the referenced site. The inspection was performedon Date by Mr. Mike Foust, a Montana Department of Environmental Quality (DEQ)accredited asbestos inspector. His credentials are attached.
ACM collected a total of Total 19 bulk samples representing Quantity (6) homogeneousareas of suspected asbestos-containing building materials (ACBM). A floor plan drawingwith sample locations is attached.
Property Description:This property is a unique 1 level residential unit originally buill in 1930 and landimprovements in 1980 with detached garage. This property has had some additions to theoriginal structure. These bUildings are comprised of the following components :
House Exterior:1. Foundation -is 1176 square feet of Concrete and cinder block (small
crawlspace for furnace) :2. Exterior Walls-wood sheathing with foam and vinyl siding3. Windows-Aluminum single pane4. Roofing- Asphalt shingle under white metal roofing5. Attic- gable roof with blow-in (recycled paper top layer) and paper insulation on
the base layerHouse Interior:

Samples listed in Table 1 tested POSITIVE for asbestos.
The inspection was performed in accordance with the Occupational Health and SafetyAdministration (OSHA) 29 Code of Federal Regulation (CFR) 1926.11 01(k), DEQ and theNational Emission Standards for Hazardous Air Pollutants (NESHAP) 40 CFR 61, SubpartsA and M, and applicable portions of the currently recognized standard protocol developedfor schools under the Asbestos Hazard Emergency Response Act (AHERA) and aspromulgated in Title 40, Code of Federal Regulations (40 CFR), Part 763 and as amendedin the Federal Register. One (1) copy of the laboratory analytical results are attached.
Required prior to renovation or demolition of building materials, the asbestos inspectionconsisted of a detailed survey of all building materials which will be disturbed by therenovation process. Suspect ACBM were visually identified and touched prior to sampling.Samples were placed into homogeneous area (HA) groups, using a coding method toclassify each material by type , texture, color, and location . Components of the inspectionincluded:
• Identification of homogeneous suspect materials on a room-by-room basis;• Collection and analysis of bulk samples to confirm or deny the presence of
asbestos; and. • Bulk samples were not collected from any homogeneous material made of
fiberglass, wood, foam, glass, or rubber.
A minimum of three (3) samples were collected from random locations of each HA ofsuspect material. Selected materials were limited to HA's affected by the renovationarea. Samples were collected from existing damaged materials , as applicable. Sampleswere placed in pre-labeled plastic conta iners for transport to the laboratory.
Samples were shipped, under chain-of-custody protocol for Standard analysis, to SanAirTechnologies Laboratory in Powhatan, Virginia, for bulk asbestos analysis utilizingPolarized Light Microscopy (PLM) coupled with dispersion staining for asbestosidentification.
The inspection was completed under the National Emissions Standards for Hazardous AirPollutants (NESHAP) 40 CFR 61, which governs asbestos inspections and abatement ofACBM prior to demolition or renovation of all residential, commercial, or public facilities.Building materials confirmed as containing asbestos in concentrations greater than one (1)percent (>1%) are placed into one (1) of three (3) NESHAP categories:
• Category II: non-friable ACBM including transite, shingles, piping and electricalwiring, excluding Category I non-friable ACBM, containing >1% asbestos; or
• Regulated Asbestos-Containing Material (RACM): friable ACBM, Category I nonfriable ACBM that will or may be SUbjected to sanding, grinding, culling , orabrading, and/or Category II non-friable ACBM that have a high probability ofbecoming or have become crumbled, pulverized, or reduced to powder by forcesexpected to act on the material in the course of demolition or renovation operations.
ACM collected Total of 19 bulk samples representing Quantity (6) HA's of suspectedACBM . Materials testing positive for asbestos are summarized in Table 1:

This asbestos inspection summary was prepared based on information gathered duringour site visit and interpretations of laboratory results of bulk samples collected during theinspection.
