14 th September 2014 To : Mr. Mohd Ashraf QHSE Manager From : Severino Valerio Safety Officer Project: Bawabat Al Sharq Phase “III” Villas Baniyas –Abu Dhabi Subject: Safety Violation During site inspection around 3:50 pm at villa 40, it is alarming that Cemix operator climbed the pump hose of the CP13 concrete pump without any fall protection and repairs it on the project site, also two of the mechanics from Cemix came to help in repairing but they used Telehandler as a working platform that is inappropriate to use and this might risk the operator and mechanics fall from height. And repairing of equipment is not allowed in the site as per AECOM Safety Engineer instruction. This kind of unsafe act could lead to fatal outcome.
