Scope of this Product Comparison

This Product Comparison covers surgical aspirators capable of creating a vacuum pressure 500 mm Hg.

Surgical waste disposal systems, which utilize a rover and docker system, and are capable of sterilizing and

disposing of the waste, are not included in this report. Also excluded are centralized suction systems that connect

Purpose

Most surgical procedures require suctioning to remove blood and

irrigating fluids that accumulate in the operative field and obstruct the

surgeon’s view. Hospitals generally use a central high-flow-rate vacuum

pump that provides vacuum through pipelines to standard wall inlets

throughout the hospital. However, portable or mobile vacuum systems

can be used if there is no central vacuum system or if suctioning is

required in areas that do not have vacuum inlets.

Principles of operation

The basic surgical aspirator consists of a line-powered vacuum pump, a

vacuum regulator and gauge, a collection canister, and an optional

 bacterial filter. Plastic tubing connects these components, completing an

open-ended system that continuously draws tissue debris and fluid from the surgical field to the collection

canister. The gauge allows the user to set a safe limit for suctioning, to assess the performance of the vacuum

pump, and to detect leaks or blockages. Units are either portable or mounted on a stand or cart for mobility.

Vacuum pumps

A single- or double-vacuum pump creates suction within the

system. Double-pump systems operate in synchrony, with both

pumps contributing to the suctioning capacity. The three types of

pumps used in surgical aspirators are rotary vane, diaphragm, and

cylinder piston.

In a rotary-vane pump, a rotor is mounted off-center in relation to

the inside diameter of the pump housing (see Figure 1). As the rotor

of the housing. Air that is trapped between a set of vanes

is compressed and expelled through the pressure outlet.

As rotation continues, the space between the vanes

expands, and the pressure of the air trapped between the

vanes drops, creating suction. Because of the friction

 between the vanes and the chamber wall and the need to

achieve airtight contact, several rotary-vane models use

self-lubricating graphite vanes. Some units require oil for

lubrication and sealing and have reservoirs that need

refilling after a specified number of hours of operation.

In a diaphragm pump, a mechanical drive flexes a

rubber diaphragm back and forth in a chamber, alternately

increasing and decreasing the chamber volume (see Figure

2). As the chamber volume decreases, air is compressed

and directed out through the one-way exhaust valve; as it

increases, a vacuum draws air into the one-way intake valve. Diaphragm pumps do not need a constant supply of

oil, and only the motor requires lubrication. Some diaphragm pumps require no lubrication.

Surgical aspirators using pistons to create suction operate similarly to diaphragm pumps; however, the pistons

slide in a reciprocating motion within the pump cylinder to alternately increase and decrease chamber volume

and pressure. One-way valves respond similarly to the pressure differences to draw air from the patient end of

the tubing.

Collection canisters

Aspirate passes through the tubing and into the collection canister. The typical collection canister is plastic

with a removable plastic lid that includes a vacuum port with a filter, a patient port to which a suction tube is

connected, and a shutoff valve to automatically close the vacuum port. Manufacturers supply reusable or

disposable canisters and plastic bag liners for reusable canisters. To save time and prevent cross-contamination

among patients and hospital personnel, ports of filled disposable canisters are capped, and the canister andcontents are discarded together.

When large volumes of tissue debris and fluid are removed, aspirate can quickly accumulate in the canister. As

a result, most surgical aspirators use two collection canisters connected in series. The second canister, called a

“trap,” collects the overflow from the first canister,

allowing the operator to postpone emptying it in response

to the rising level of aspirate.

Most surgical aspirators have an overflow-protection

assembly in the cap of the second canister that prevents

large amounts of fluid from overflowing into the pump

and valves. Some units feature a third canister to collect

overflow. Most surgical aspirators use a float mechanismas overflow protection. As aspirate collects in the second

canister, a ball or other mechanical device floats to the top,

covering the opening leading to the suction source and

cutting off suction, thereby alerting the user to the full

collection canister. This overflow mechanism cannot be

totally relied upon to prevent pump contamination; often,

some fluid passes through the tubing to the pump before

the float blocks the opening. As a result, most aspirators

also include a disposable in-line hydrophobic filter between the trap canister and the pump. This filter prevents

fluid or aerosol contaminants from flowing back into the vacuum pump; it can be reused several times, provided

that no overflow has occurred. Although most units have these overflow-protection features, the user should

check the collection canisters frequently and not allow them to fill above the caution line.

Reported problems

Factors affecting pump operation can reduce aspiration capability. Aspirate that overflows beyond the second

canister and filter can enter the pump and clog the valves; a clogged pump can jam and be difficult to restart. A

pump containing aspirated fluid can also be a source of contamination.

Purchase considerations

ECRI Institute recommendations

Included in the accompanying comparison chart are ECRI Institute’s recommendations for minimum

performance requirements for surgical aspirators.

Surgical aspirators should provide moderate to high vacuum and flow rates and suction of at least 500 mm Hgat maximum settings to remove tenacious materials but should also allow lower vacuum levels to help prevent

tissue damage. Vacuum gauges and vacuum-limiting devices (e.g., regulators) are required to aid clinicians in

applying appropriate and safe suction levels, and the former should be both accurate and easy to read. Collection

canisters—whether disposable or reusable—should have a total capacity of at least 2 liters (2,000 milliliters or

cubic centimeters). The aspirator should prevent overflow of aspirated material, which may clog the aspirator and

should be considered infectious. All controls and connections should be clearly labeled, and the device should be

quiet so that it does not interfere with communication among clinicians.

Other considerations

The aspirator should be easy and intuitive to turn on, adjust, and operate. It should provide an appropriate

vacuum limit and free airflow capability. Surgical aspirators typically have vacuum limits of 500 mm Hg andfree airflow capabilities of 25 to 40 L/min. (Free airflow has been defined as the maximum flow of air into the

aspirator with no restriction at the intake.)

If reusable canisters are used, emptying and exchanging the canisters should be uncomplicated and without

significant risk of instrument or operator contamination. Reusable containers are less common because of the risk

of cross-contamination; reusable containers sometimes include disposable liners that minimize this risk.

Discarding disposable canisters and their contents should be easy, with minimal risk of spills or contamination.

The cost of disposables should be weighed against the cost, time, and infection risks associated with the

disposal of infectious waste and disinfection of reusables. Most units with disposable containers can accept

alternate manufacturers’ containers. These standard containers may be more convenient to order and offer the

user a choice of suppliers. However, installed filters supplied with the canisters will differ; some may severely

limit airflow rates.

Cost containment

Because surgical aspirators entail ongoing operational costs, the initial acquisition price does not accurately

reflect the total cost of ownership. Therefore, a purchase decision should be based on issues such as life-cycle cost

(LCC), discount rates and non-price-related benefits offered by the supplier, and standardization with existing

equipment in the department or hospital (i.e., purchasing all aspirators and/or collection canisters from one

supplier).

An LCC analysis can be used to compare high-cost alternatives and/or to determine the positive or negative

economic value of a single alternative. Because it examines the cash-flow impact of initial acquisition costs and

operating costs over a period of time, LCC analysis is most useful for comparing alternatives with different cash

flows and for revealing the total costs of equipment ownership. One LCC technique—present value (PV)

analysis—is especially useful because it accounts for inflation and for the time value of money (i.e., money

received today is worth more than money received at a later date). Conducting a PV/LCC analysis often

demonstrates that the cost of ownership includes more than just the initial acquisition cost and that a small

increase in initial acquisition cost may produce significant savings in long-term operating costs. The PV is

calculated using the annual cash outflow, the dollar discount factor (the cost of capital), and the lifetime of the

equipment (in years) in a mathematical equation.

The following represents a sample seven-year PV/LCC analysis for a surgical aspirator.

Other costs not included in the above analysis that should be considered for budgetary planning include those

As illustrated by the above sample PV/LCC analysis, the initial acquisition cost is only a fraction of the total

cost of operation over seven years. Therefore, rather than making a purchase decision based solely on the

acquisition cost of a surgical aspirator, buyers should consider operating costs over the lifetime of the equipment.

Prices for disposable tubing and catheters vary greatly depending on a number of factors, including type, size,material, manufacturer, and bulk-rate discounts. Many of these disposables can be used with surgical aspirators

from a number of manufacturers; therefore, disposables are usually more relevant to an LCC analysis for an

aspirator than the device itself.

Stage of development

Surgical aspiration is a mature and well-accepted technology, although some new innovations have been

made. Surgical waste disposal systems have been developed that do not utilize canisters at all, making waste

collection easier, safer for healthcare workers, and more cost-effective than previous systems. These systems

consist of a rover (a mobile collection tank) and a docking station (a wall unit). After a surgical procedure, the

rover is transported and connected to the docker, which is capable of sterilizing and disposing of the waste(Williamson 1999).

The Healthcare Product Comparison System (HPCS) is published by ECRI Institute, a nonprofit organization.
