HISTORY OF THE COLLEGE OF HEALTH & SOCIETYHawai‘i PacificUniversity began in 1965 asHawai‘i Pacific College, an independent, non-sectarian liberal artscollege.In1973theuniversityreceiveditsfirstWASCaccreditation.Thecollegecontinuedtogrowandby1990hadapproximately30associateandbachelorsprograms/fieldsofstudyandtwomastersprogramswhenitwasrenamedHawai‘iPacificUniversity.Inthesummerof1992Hawai‘iPacificUniversitymergedwithanotherindependentliberalartscollegeonOahu,Hawai‘iLoaCollege(HLC).Themergeraddedanadditionalcontingentofliberalartsstudents,amajorinmarinescience,pre-professionalprograms,andaBachelorofScience innursingprogramthat includedanRNtoBSNcompletionprogram,anLPNtoBSNcompletionprogram,andafour-yearBSNprogram.TheDivisionofNursingalsoexpandedtooff-campusclasses,offeringsatelliteprogramsatTheQueensMedicalCenterandTriplerArmyMedicalCenter.InMarchof1992,theNationalLeagueforNursing(NLN)accreditedtheprogramforfiveyears.AsecondvisitoftheNLNinNovemberof1996resultedinaneight-yearre-accreditation.ThenextvisitinFebruary,2005resultedinanothereight-yearre-accreditation.Dr.Hessel Flitterwas the firstdirectorof theDivisionofNursingandwas succeededbyDr.AdeleMitchell in1988.Dr.CarolWinters-Moorheadbecamethedirectorinthefallof1991andbecameDeanofNursingin1992,servinguntil2006.Duringthistimethedivisionofnursing’sdesignationchangedtoSchoolofNursingin2002.Dr. Patricia Lange-Otsuka served as Interim Dean from 2006-2008 when Dr. Randy Caine became Dean ofNursing.Dr.Caineservedasdeanfrom2008–2011duringwhichtimetheSchoolofNursingwasrenamedtheCollegeofNursingandHealthSciencesin2009. Dr.DaleAllisonwasthenappointedInterimDeanandserveduntil2014.DuringthistimethebachelorsprogramofnursingwasaccreditedbytheCommissiononCollegiateNursing Education (CCNE) in 2011. Dr. Lynette Landry then served as dean from 2014 – 2017 and HealthSciences changed to Public Health. In fall 2014, the School of SocialWork joined the College of Nursing andHealthSciences. In2015,DeanLandry led facultyandstaff throughastrategicplanningprocess that includedthe renaming of the college to the College of Health & Society (CHS), reflecting the three inter-professionaldisciplinesofnursing,publichealth,andsocialwork.AMasterofScienceinNursing(MSN)programwasinitiatedinthefallof1998withDr.PatriciaLange-Otsukaasthe program’s chair. Accreditation by NLNAC for theMSN programwas achieved in October 2000. The nextcombinedBSN/MSNsitevisitbytheNLNACwasheldinFebruary2005andresultedinarecommendationforre-accreditationforanothereightyears,until2013.CCNEaccreditationwasalsoattainedfortheMSNprogramsin2011.Dr.DaleAllisonservedasgraduatechairpersonfrom2006–2009andDr.PatriciaBurrellwastheChairofthe Department of Graduate and Post-Baccalaureate Nursing Departments until April 2012. Dr. Diane Knightthen served as Chair of the Department of Graduate and Post-Baccalaureate Nursing Departments untilDecember 2016. Dr. Michelle Johnson was appointed as Chair, Graduate and Post-Baccalaureate Nursingprogramsin2017.InJune2017,theHawai’iPacificUniversity’sNursingDepartments–bachelor,master,andpost-masterreceivedreaccreditation through 2027 from the Commission on Collegiate Nursing Education (CCNE). The CCNEreaccreditationcameontheheelsoftheexcitingnewsthatHPU’s firstdoctoralprogram,aDoctorofNursingPractice(DNP),hadbeenaccreditedbytheWesternAssociationofSchoolsandColleges(WASC)SeniorCollegeandUniversityCommission.Infall2017,theHPUDNPprogramadmittedtheirinauguralcohortofdiverseDNPstudents.TheHPUDNPandAGACNPprogramsareuniqueinthattheyweretheonlineprogramsinthestate.In July 2017, the nursing department leadership structurewas redesigned. Dr.Michelle Johnsonwas namedChief Nurse Administrator overseeing the Graduate and Post-Baccalaureate Nursing Departments with Dr.KathleenBurgerservingasinterimAssociateChiefNurseAdministrator.Dr.JayneSmittenwasappointedasCHS

AssociateDean in July, 2017, and subsequently appointed toCHSActingDeanuntil 2018whenDr.Halaevalu“Valu”Vakalahi,PhD,MSW,MEdwasnamedthenewDeanoftheCollegeofHealth&Society.InJuly,2019,theNursing Department leadership was restructured with Dr. Pamela Smith as the Chief Nurse Administratoroverseeing the Nursing Department encompassing the Undergraduate, Graduate, and Post-BaccalaureatePrograms.

CLINICAL COURSE POLICIES & PROCEDURES

ClinicalFacilitiesTheNursingDepartmentutilizes a varietyof clinical health care agencieson the islandofOahu for students’learning experiences. Facility use is a cooperative arrangement with the various clinical agencies, as well asother nursing, medical, and allied health programs requiring clinical placements. An HPU nursing facultymember serves as thedepartment’s Clinical FacilitiesDirector and acts as liaison to thehealth care facilities,negotiating for the clinical placements forHPUnursing students each semester. Clinical placements are thenprovidedtoHPUbyhealthcarefacilitiesbasedonanumberoffactorsincludingthenumberofrequestsfromallnursing,medical,andalliedhealthprograms,internalagencystaffingandavailability,courserequirements,andinstructoravailability.Therefore,clinicalplacementsathealthcareagenciesarealwayssubjecttochangeandnostudentcanbeguaranteedtheirpersonalpreferencesforday,times,orfacilitywhenregisteringforclinicalnursingcourses.Clinical facilities choose to include students as part of their commitment to nursing education. There is noreimbursementfortheeducatorswhoperformstudentorientationsorcomputertrainingorforthenurseswhoworkwithstudentsontheunits.Bothstudentsand facultyshouldconsider themselvesguests in the facilitiesandabidebyanyrequestsorrequirements.Unprofessionalbehaviorbystudentsorfacultycanjeopardizethecollaborative relationship that HPU and the Nursing Department have with the clinical agencies. This canpotentially lead to the loss of the facility for educating students. Unprofessional behaviors in clinical nursingcoursesareconsideredseriousandunacceptablepracticeswhichmayresultindismissalfromthenursingmajor.

ClinicalPreceptorships Someclinicalexperiencesutilizeaclinicalpreceptor, inadditiontoafacultyinstructor,tofacilitateastudent’slearningexperience inaparticularagency. Preceptorsare registerednurses,preferablywithaminimumofabaccalaureatedegreeinnursing.Preceptorsareselectedbyafacultymemberwhoisthepreceptorshipclinicalcoordinatoronthebasisofahighlevelofclinicalexpertiseinaparticularareaandtheabilitytoprovideapositivelearningexperienceforthestudent.Studentscanapplyforaselectnumberofclinicalpreceptorshipplacementopportunitiesinthesemesterpriortotheirfinalsemesterinthenursingprogram.Ifaccepted,thestudentworkswiththeirRNpreceptortofulfilltheclinicalhoursassociatedwiththefinalComprehensiveNursingCarecourseratherthanattendingtraditionalgroupclinicalhoursofthecourse.

Community&HomeVisitationsInsomeclinicalcourses,studentswillprovidepatientteachingand/ornursingcareincommunityand/orhomesettings. All clinical course policies and procedures apply to these learning experiences and students areexpected to strictly adhere to confidentiality guidelines. Student attiremust be professional and follow anyguidelinesprovidedbythecourseinstructorand/orstudentnurseassociationfacultyadvisor.Nametagsmustbeworn. StudentsareremindedthattheyrepresentHPU,theCHS,andtheNursingDepartmentwhenintheclinical, community, and/or home setting. Impeccable professionalism is therefore expected. BackgroundCheck

Allnursingstudentsarerequiredtocompleteabackgroundcheckthroughadesignatedcompanypriortothestartoftheirfirstclinicalexperienceandatothertimesduringthenursingprogramas indicated.Studentsarealso required to report alleged violations of federal, state and local laws that have occurred since latestsubmission of the background check. If a student has a violation reported on their background check, anaffiliatedclinical sitemaynotallowtheirpresenceat their facility.Studentswhocannotparticipate inclinicalexperiencesduetoadversebackgroundsareunabletofulfilltherequirementsofthenursingprogramandmayneedtowithdraw.Students are also reminded that licensing boards for nursingmay deny, suspend, or revoke a license ormaydenyanindividualtheopportunitytositforanexaminationifanapplicanthasacriminalhistoryorisconvictedorpleadsguiltyornolocontendertoafelonyorotherseriouscrime.StudentsshouldconsulttheStateBoardofNursinginthestatetheyareseekinglicensureformoreinformationonadditionaldocumentationrequirementsinthesecases.TransportationtoClinicalFacilitiesTransportationtoandfromclinicalfacilitiesandparkingfeesarethestudent’sresponsibility.Someagenciesofferdiscountedparkingfeestostudentsorhavedesignatedparkingareas.Studentsareencouragedtocarpoolandatsomefacilitiescarpoolingmayberequired.Duetoverylimitedparkingatsomefacilities,studentsmayneedtoparkonthestreetinnearbyneighborhoods.Studentsareexpectedtofollowtheguidelinesforparkingprovidedtothembytheirassignedclinicalfacilityandtoutilizerespectfulandresponsibleparkingbehaviors.

ClinicalOrientation EACH CLINICAL FACILITY HAS SPECIFIC ORIENTATION REQUIREMENTS FOR STUDENTS AND FACULTY. ClinicalRegistration

Allstudentswhomaintainstandardprogressioninthenursingprogram(i.e.arenotrepeatingcoursesandareintheir original cohort) are guaranteed a clinical site placement for their clinical courses. However, as notedabove, personal preferences for clinical agency assignments cannot be accommodated due to the manyvariables impacting clinical placement availabilities. ClinicalAttendance Students are expected to be on time and are required to attend all assigned clinical days and complete allrequiredclinicalhours.ThisistomaintaintheclinicalhourcurriculumrequirementsoftheHawai’iStateBoardofNursing.

• Studentswhoareacutely ill,potentiallycontagious,havean injury that limits their strengthand/orfullrangeofmotion/mobilityorwhohavebeensoadvisedbytheirhealthcareprovidershouldnotattendclinical.

• Priortoreturningtoclinicaldutiesaclearanceletterfromthestudent’shealthcareprovider(HCP)mustbesubmittedtothecoursecoordinatorandChiefNurseAdministrator.ThismustbeonHCPletterhead,withHCPoriginalsignature,andstatethatthestudent“isclearedforallclinicalnursingdutieswithoutlimitation”.

• If needing to be absent from, or late to, a scheduled clinical session, the studentmust contact theirclinicalinstructordirectlybyphone,text,oremailasdesignatedbythefacultymember.Thismustbedonenolaterthan2hourspriortothestartoftheclinical.Itisnotacceptabletosendamessagetotheclinicalinstructorindirectly(i.e.viamessagefromaclassmate).Failuretonotifytheclinicalinstructorofanabsenceorlatenessaminimumof2hourspriortothestartofclinicalwillresultinanunacceptablepracticeviolation.

• Allmissed clinical hoursmust be ‘made-up’ with alternativemethods for achieving required clinicalhours. The clinical instructor and/or clinical coordinator will advise the student on the method formakeuphours.

• Excessiveabsence(morethanoneday)fromclinicalcoursehoursatanassignedclinicalagencypreventsadequateopportunityforlearningandevaluationofstudentperformance.Thiswillrequirethestudenttowithdrawfromtheclinicalcourseandcorrespondingtheorycourse

• Studentsarenotpermittedtoprovidepatientcareintheclinicalareaunlessunderthesupervisionofafacultymemberortheyareassignedspecificallybyafacultymembertoaspecialtyunitorcommunitysettingwithspecificperformanceguidelines.

• Attendance by students and/or faculty on days not included in the HPU calendar for the designatedclinicalcourseand/orduringuniversityholidays/breaksisstrictlyprohibitedduetoliabilityissues.Thisincludespreceptorship,community,andhomevisitationhours.

IllnessorInjuryintheClinicalSettingEachclinicalagencyhasapolicyforstaffmemberswhobecomeillorinjuredintheworksetting.Anillorinjuredstudentshouldimmediatelynotifythefacultymemberandthenfollowtheagencyprotocolforstaffmembers.Thestudentmusthavethefacultymember’spermissiontoleavetheagency.AninjurymustbereportedtotheClinicalFacilitiesCoordinatorwithin24hoursoftheinjuryintheformofanIncidentReport.Priortoreturningtoclinicaldutiesaclearanceletterfromthestudent’shealthcareprovider(HCP)mustbesubmittedtothecoursecoordinatoraswellastheChiefNurseAdministrator.ThismustbeonHCPletterhead,with HCP original signature, and state that the student “is cleared for all clinical nursing duties withoutlimitation”.NOTE:StudentsarerequiredtohavecurrentmedicalinsurancethroughouttheNursingDepartmentandshouldhavepolicyinformationavailableifneededintheclinicalsetting.Costsforanynecessarymedicaltreatmentareto be coveredby students’ personalmedical insurance plans. Students are responsible for costs that are notcoveredbytheirownmedicalinsuranceplan.

ClinicalHealthRecord&DocumentationRequirements Undergraduatenursingmajorsacquireandmaintainthroughouttheircourseofstudyanumberofhealthrecord and documentation requirements. Requirement standards are set according to Center for DiseaseControlregulations,andguidelinesfromtheStateofHawai'i.Specificrequirementsarealsodeterminedbyeachfacility,andsomefacilitiesmayrequiremoretestingorpaperworkthanothers.Requirementsarealsosubjecttochange.Studentswho are not in compliancewith clinical health record and documentation requirementswill not bepermittedonsiteatthedesignatedclinicalfacility.Non-compliancebythedesignatedsubmissiondeadlineswillresult in the student being dropped from their clinical course(s) and corresponding theory courses, with noopportunitytore-registerfortheminthesamesemester.Clinicalcoursesareamandatorypartofthenursingcurriculum,sostudentsmustcomplywithhealthandclinicaldocumentationrequirementsinordertomeettheBSNdegreerequirements.

CastleBranch&GettingCleared Inordertomaintain,track,andsubmityourhealthrecords,youmustcreateaprofilewithCastleBranch.Yourprofilewillserveasyour individualizeddocumentmanagementsystemduringyourtimeasanursingstudent.Through your account, you will be able to upload and update your health and other clinical documentationrequirements, maintain your records in one secure location, track upcoming due dates, and view anyoutstandingrequirements.To get cleared to attend a clinical course, HPU’s nursing department reviews your records and preparespaperworkforeachclinicalfacilityviayourCastleBranchsubmissions.Ifyourrecordsarenotonfileordonotmeet the requirements, the nursing department will not be able to submit information to the appropriateclinical facility.Youmustmaintainyourfiles inCastleBranchsothatthedepartmentcanaccessyourrecords.Emailandhard-copyversionsofdocumentationarenotacceptable.

ONE TIME SUBMISSIONS 1) MMR & Varicella

Proof of a positive titer or proof of immunization is required.

5) Physical Exam Complete the Physical Exam Form. The form must be stamped by your healthcare provider.

RECURRING SUBMISSIONS

1) TB Skin Test: Negative Result A negative, 2-step PPD test is required upon entry into the Nursing Department. Thereafter, a 1-step renewal is required each year, placed annually, no more than 365 days from your last test.

1) TB Skin Test: Positive Result If you have ever tested positive for TB, you must submit:

• A record of the date of your positive result, along with the amount listed in mm (one-time submission)

• A clear chest x-ray from the year you started Level 1 of the Nursing Department (one-time submission)

• A clear TB Monitoring Form, submitted annually

2) AHA BLS Card An American Heart Association (AHA) BLS Healthcare Provider card is required.

2) Flu Vaccination All students are required to submit an Influenza Vaccine Form, and proof of vaccination for the current flu season.

3) HIPAA & BBP Certification Submit proof that you have watched and completed the HIPAA & BBP training modules and post-tests, via the instructions listed online.

3) Confidentiality Statement Complete and sign the Confidentiality Statement.

4) Declaration of Fitness & Background Check Complete and sign the Declaration of Fitness & Background Check Form.

4) Background Check Renewal Your background check must be issued within 12 months of the semester's start date to be considered current.

Licensure: RN & LPN/LVN to BSN Students Only

StreetClothes

ACADEMIC POLICIES & PROCEDURES

GradingPolicy AllnursingstudentsmustachieveaminimumfinalcoursegradeofC(73%)ineachnursingcourse.FailuretoachieveagradeofCorbetterinanursingcourseconstitutesafailureofthecourseandwillpreventthestudentfromcontinuingtothenextsequentialnursingcourse.FailedClinicalorFailedLecture:Anursingstudentwhofailstoachieveagradeof“Pass”intheclinicalcoursewillreceiveagradeof“F”forthecourseandmustrepeatboththeclinicalandcorrespondinglecturecourses.Ifastudentreceivesa“C-”,“D(+)or“F”inthelecturecourse,thenthecorrespondingclinicalcoursegradeisanautomatic “F.” The studentmust repeat both courses. A nursing studentwho fails to successfully repeat therequired nursing courses will be dismissed from the nursing major even if a 2.75 GPA is maintained.Correspondinglectureandclinicalcoursescountasonenursingcoursefailure.Twocoursefailureswill leadtodismissal from the Nursing Department. NURSINGPROGRAMPROBATIONPROCEDURE:

1) NursingstudentswillbenotifiedwhenplacedonNursingProgramProbation.Aletterwilldetailrecommendationsandstepstobefollowedbythestudentwhoisonprobation.

2) Nursingstudentsareresponsibleforschedulinganappointmentwiththeiracademicadvisorand facultymentorandmeetingwith themno later than the 3rdweek of the semester todiscussstrategiesforimprovementandtodevelopanactionplan.

3) Students are placedon a Learning Contract by their facultymentor at the timeof their firstface-to-face meeting. This document details the student’s action plan and is signed by thefacultymentorandstudent.

4) ThestudentandfacultymentorwillmeetregularlyduringthesemestertoreviewtheLearningContractandtofollow-uponstudentprogress.Meetings(virtualorface-to-face)shouldoccurevery2-3weeksuntiltheendofthestudent’sprobation.

5) Studentswho fail to complywith the above procedures are at risk for failure in the nursingprogram.

6) Attheendofthesemester,bothstudentandfacultymentormustonceagainsigntheLearningContractandacopyofthecompletedLearningContractwillbeplacedinthestudentfile.

ClinicalCourseGradingClinical coursesprovidenursing studentswithanopportunity toapplynursingprinciples inapractice setting.Student clinical performance is evaluated by the clinical instructor and graded as P/F. Clinical evaluation isbased on a student’s ability to make accurate clinical decisions for safe, quality, patient care. A clinicalevaluation tool outlining the expected student competency in Patient-centered care, Teamwork andCollaboration, Evidence-basedPractice,Quality improvement,Healthcare Informatics, Safety, Professionalism,andLeadershipisutilizedasanumericalgradingrubric(seebelow).Studentsenrolledinsemesterlongclinical

ClinicalSkillsPolicyNursingstudentsareresponsibleforachievingcompetency inallnursingskillstaughtthroughouttheprogramandtomaintaintheirskilllevelthroughouttheprogram.Studentsareexpectedtoengageincontinuouscriticalself-evaluation to determine if they are underperforming or unsure of performing any clinical skill, and thentaking action toward improvement if necessary. This should include consultationwith the clinical instructorand/or nursing facultymentor, aswell as independent actions such as reviewingof skills videos andphysicalpracticeintheExperientialLearningCenterandanyotherremediationrecommendedbytheirfacultymember.Studentswhodonotattainormaintain clinical skill competencieswill receiveanacademicwarningandmayultimatelyreceiveafailinggradeinclinicallabcourses.

MedicationCalculationCompetencyPolicyBACKGROUND:Inthatstudentsarerequiredtodemonstratesafepracticeintheirclinicalcareofclients,theNursingDepartmentprovidesMedicationCalculationCompetencyAssessment(MCCA)everysemestertoensurethestudents’abilitytocalculatemedicationdosagesaccurately.ThesuccessfulcompletionoftheMCCAisapre-requisitetoproceedinginallclinicalcourses.APowerPointTutorialonDosageCalculationandpracticequestionsisavailabletostudentsontheHPU-NursingUndergraduateNursingCommunitysite.Studentsareresponsibleforreviewingthetutorialanditsaccompaniedinstructionsregardingsemester-by-semestercompetencyexpectations,completingpracticequestions,andseekingwhateveradditionalsupporttheyneedfromHPUandnursingdepartmentresourcesbeforeMCCAtesting.BothlectureandclinicalfacultywillbeavailablebyappointmenttoassiststudentstobesuccessfulonMCCAs.TherearetwopathwaysforMedicationCalculationCompetencyAssessment:

UnacceptablePracticeNursingDepartment facultymembersareacademically committed topreparingbaccalaureategraduateswhoarecompetentintheskillsofnursingpractice:technical,diagnosticandinterpersonal.Facultymembershaveanethicalandlegalresponsibilityandaccountabilityfortheprotectionofotherswithinthehealthcareandlargercommunity from unsafe, unethical, or illegal nursing practice on the part of HPU students and graduates.Therefore, students may be disciplined or dismissed from the nursing major for practice or behavior whichthreatens, or has thepotential to threaten, the safety of a client, familymember, authorized representative,studentpeer,facultymember,healthcareprovider,and/orself,orisunethicalorillegal.Unacceptablepracticemaybeaone-timeeventoraseriesofevents.An Unacceptable Practice Citation advises students of an imminent potential for failure in a course and/ordismissal from theNursingDepartment.Withinnursing courses, counselingandadvisingprocesses andotherinstructional forums,studentswillbeprovidedtheopportunity todiscuss thepolicyonunacceptablepracticeand its implications. All students are expected to be familiar with the principles of safe, ethical, and legalpracticeandareexpectedtoperformaccordingly.DEFINITION:Unacceptablepracticeisdefinedas:

6) An act(s) or behavior(s) that is unethical, dishonest or illegal. 7) Unauthorized entries into the computer or misuse of clinical records. 8) Violation of laws or rules relating to patient record confidentiality. 9) A pattern of behavior often characterized by being unprepared or inadequately prepared for clinical. 10) Continued inability to perform safely, ethically, or legally without immediate/close instructor

PROCEDURE: When an incident or pattern of behaviors occurs that a faculty member believes to beunacceptablenursingpractice,orwhen suchan incident isdiscovered, the facultymemberwill complete theUnacceptablePracticeCitation.Thestudentwillbenotifiedimmediately(whenfeasible)andmayberequiredtoleave the clinical or classroom setting (if appropriate). Failure to leave the clinical or classroom settingwheninstructed to do so is grounds for dismissal from the Nursing Department. If a student is removed from theclinicalsettingforunacceptablepractice,thestudentwillnotreturntothatclinicalsettingunlessclearedbytheCourseCoordinatororChiefNurseAdministrator.TheunacceptablepracticeincidentorpatternofbehaviorswillbereportedtotheCourseCoordinatorandChiefNurse Administrator. The incident will be investigated and the Course Coordinator and Chief NurseAdministratorwillthendeterminetheconsequencesoftheunacceptableaction,inaccordancewithHPUpolicy.Thestudenthastherighttoappealbyfollowingtheuniversity’sgrievancepolicy.Consequencesmayinclude:

CourseWithdrawal/CourseDropNursing studentsmaintain their rights towithdraw from a nursing course or courses according to universitypolicies. However,repeatedcoursewithdrawalstoavoidachievingafailinggradearehighlydiscouragedasan inappropriate remedial approach to academic difficulties. ProgramEvaluation&QualityImprovementEvaluation is a very important part of any academicprogramandone that involvesboth faculty and studentinput.Thefacultyhasacommitmenttoprovidinghighqualitynursingeducationaswellasanobligationtothenursing profession, the health care community, and clients to ensure that HPU graduate nurses are safepractitioners. Ongoing evaluation of every aspect of the Nursing Department helps to accomplish thiscommitment and obligation. Student participation in this process of programevaluation is integral to qualityimprovement.Eachsemester,studentswillhavetheopportunitytocollaboratewithfacultyregardingprogramcurriculumandpolicies at various meetings, and to provide evaluation of program components such as course and facultyevaluations.Inthelastsemester,studentinputwillbesoughtwithanendofprogramsurveyandapproximately4-6months after graduation studentswill be contacted to complete another program evaluation survey. Inadditionthenursingdepartmentconductsclinicalfacilityevaluationsandemployersurveys.End-of-Semester evaluation surveys are to be completed by students. These are emailed directly to eachstudent’s HPU email account. Course evaluations are available 24 hours a day/7 days a week during theevaluationperiod.Studentsmaycompletetheircourseevaluationwithanymobiledevicewithinternetaccess,including smart phones, tablets, or laptop computers. Courses with 4 or fewer students enrolled are notprovidedauniversitycourseevaluationtoprotecttheanonymityofthestudents.

Information on ordering theHPUpinwillbemadeavailablebytheStudentNurseAssociationandPinningSubcommitteeinthelastsemesterofthenursingmajor.TheSNAPinningsub-committeeplanstheceremonywithassistancefromafacultypinningadvisor.Thepinningcommitteeconsistsofseniorlevelstudentsandothers.Asyourgraduationtimeapproaches,getinvolvedwiththepinningcommitteesothattheceremonyismeaningfultoyou.UniversityGraduationThe Bachelor of Science in Nursing (BSN) degree is conferred upon studentswho satisfactorily complete theGeneralEducationrequirementsandtheprescribedcurriculum.Toearnthisdegree,astudentmustcompleteaminimumof120credithoursandmeetalltherequirementsofthenursingmajor(60credithours)withatleasta2.75cumulativenursinggradepointaverageandanoverallHPUcumulativegradepointaverageof2.75orhigher.RefertotheHPUAcademicCatalogforadditionalgraduationrequirements.StudentwhoarecompletingtheirdegreerequirementsbytheendofagiventermmustcompleteaPetitiontoGraduate(PTG)formatthebeginningofthatterm.ThePTGmustbeapprovedandsignedbythestudent’sHPUacademicadvisor.AdditionalinformationcanbefoundontheHPUAcademicAdvisingwebpage. HawaiiStateBoardofNursingRegulationsforLicensureandCertificationsIn a recent announcement from the Hawaii State Board of Nursing (2017), changes to the regulations forlicensurewereclarifiedinregardstoRNstudenteligibilityforPNlicensure.Pleasenotethatstudentsenrolledin, or who have completed, a registered nurse program are NOT ELIGIBLE to take the NCLEX-PN exam andcannotbeissuedalicensetopracticeasapracticalnurseunlesstheyhavecompletedalicensedpracticalnurseprogramapprovedbytheboard,andtheyholdadiplomaorcertificatetherefrom.HPUdoesnotoffer,andhasnocurrentplantooffer,alicensedpracticalnurseprogram.Also, studentsenrolled in,orwhohave completed, a registerednurseprogramareNOTELIBIGLE to take theNurseAideCertificationexamforstate-licensedandstate-certifiedhealthcaresettings.TobeaCertifiedNurseAide(CNA)anapplicantmustsuccessfullycompleteaCNAtrainingprogramapprovedbythedirectorofhumanservices.HPUdoesnotoffer,andhasnocurrentplantooffer,aCNAtrainingprogram.

NCLEX-RNExam

Graduates of the HPU BSN program ARE ELIGIBLE to take the National Council of State Boards of NursingExaminationforRegisteredNurses(NCLEX-RN)whichisacomprehensiveexaminationdesignedbytheNationalCouncilofStateBoardsofNursing(NCSBN)totesttheabilityofagraduateofanAssociateDegree,Diploma,ofBSN-grantingNursingDepartmenttopracticeentry-levelregisterednursinginasafeandeffectivemanner.Thislicensing exam evaluates the graduate’s ability to apply beginning-level knowledge and interventions to

The test is administered in a single, six hour setting in a“computerizedadaptivetesting”(CAT)format.Thisisanationwideexamwithanationallysetpassingscore.Thismeansthatifyoupasstheexaminonestate,youcanseekreciprocityintheDistrictofColumbiaandanyotherstateintheUnitedStates.YoumaytaketheexaminanystateortheDistrictofColumbiaandcertaincountriesoverseas.YoumustapplytotheappropriatestateboardofnursingforpermissiontotaketheNCLEX-RNandpayaseriesoffees.Inordertotaketheexam,youmustbeagraduateofanationallyaccreditedNursingDepartmentbythedateoftheexaminationandanofficialtranscriptmustbesentbytheHPURegistrartotheappropriatestateboardtodocumentcompliancewithrequirements. AppendixB:HIPAAPOLICYPOLICYSTATEMENTTheHIPAAPrivacyRule establishes national standards to protect individuals’medical records andother personal healthinformationandappliestohealthplans,healthcareclearinghouses,andthosehealthcareprovidersthatconductcertainhealthcaretransactionselectronically.TheRulerequiresappropriatesafeguardstoprotecttheprivacyofpersonalhealthinformation, and sets limits and conditions on the uses and disclosures thatmay bemade of such informationwithoutpatient authorization. All College of Health & Society (CHS) nursing students are required to complete HIPAA training.Studentsmeet the requirements of HIPAA through this mandatory training. Reminders of these requirements occur ateverycourselevelandaStatementonthePrivacyofPatientInformationisincludedineachcoursesyllabus.Students are expected to know and abide by HIPAA privacy requirements.
