ALL AGES WELCOMEInova Loudoun Hospital prides itself on caring for people at all stages of life. As proof of this, just peruse the pages of this InHealth issue. Our cover story on sports medicine features two star high school athletes. These teenagers reflect the younger population that comes through our doors. Read on and you’ll notice that the patients who use our services run the age gamut — from chil-dren to mothers to mature joint replacement candidates.

Just as you grow up with us, our hospital has grown up with you. This year is our 100th anni-versary. (Be sure to see the back page for details on our free birthday celebration!). We have changed a lot since 1912 when we started as a six-room hospi-tal on Market Street. As one example, consider the advance-ments in our joint replacement surgery department. We are now offering MAKOplasty®, a minimally invasive knee replace-ment surgery. In the area of obstetrics, we have opened the Antenatal Testing Center, which provides testing for high-risk moms. We also have an expanded Lactation Center. In a different area of medicine, a new concussion program opened earlier this year.

So, in short, we are here for you. Don’t hesitate to give us a call to set up an appointment. Inova's new 24-hour, bilingual phy-sician-referral service helps you find a doctor close to your home or work. Our goal, besides making medicine more advanced, is to make it more accessible.

GET SCREENEDAmong the events to be held at the Women’s Community Living Fit Conference will be FREE health screenings provided by Mobile Health. The following screenings will be available to attendees: ■ Asthma screening (provided by board-certified allergy/asthma specialist) ■ Blood pressure ■ Body mass index ■ Waist circumference■ Glucose (first 50 participants are free, thereafter $10)

Additional screenings are available for a fee:■ Cholesterol: $15■ Facial sun damage: $10

Breastfeeding Basics ExpANdEd LACTATiON CENTEr, bOuTiQuE iN ONE CENTrALizEd LOCATiON Moms-to-be and breastfeeding mothers can now learn in a new expanded location. The hospital’s Lactation Center and Boutique has moved to a space that is twice the size of its former location. There is now a centralized place for educa-tion, support/consultations and supplies. Some of the features of the new location include:■ Four private consultation rooms ■ A dedicated educational meeting area ■ Breastfeeding classes ■ Even more breastfeeding supply offerings (birthing gowns, more

brands of nursing bras, abdominal binders, to name a few)

GOOD THINkINGConcussion Management Program begins Head injuries should not be taken lightly, no matter what the patient’s age, which is why Inova Loudoun Hospital started the Head2Head Concussion Manage-ment Program. ”We now know that concussions can result not only from serious head trauma but also from multiple minor head injuries or even injuries people did not know occurred,” says Ron Waldrop, MD, Medical Director of the program. The full-service clinic, which serves patients up to age 30, evaluates the patient to determine a diagnosis and whether therapy is needed. The program is for those who have been diagnosed with a concussion only and not for those who have had head injuries with fractures or bleeding.

A Head StartThe Head2Head Concussion Management Program is located at the hospital’s Lansdowne campus, 44035 Riverside Parkway, Suite 500A, Leesburg. To find out more about the program, call 703-858-6667.

When a person suffers a concussion or other brain injury, axons in the brain are

damaged. Axons are specialized extensions of neurons, the cells in your brain that com-

municate with distant nerve cells. Severe sudden trauma to the brain can stretch, twist and damage these delicate axonal

fibers, leading to permanent brain injury.

NEW hOspiTAL TEsTiNg CENTEr OpEN FOr high-risK MOMs

Women in and around Loudoun County who have high-risk pregnancies now can receive testing at Inova Loudoun Hospital. The Antenatal Testing Center, which is located in the physician office building attached to the hospital, opened in March.

“It’s an opportunity for us to increase the services that we’re giving to our moms, particularly our high-risk pregnancy moms-to-be, and give them an opportunity to have a higher level of care,” according to Cindy Andreja-sich, Patient Care Director for Labor and Delivery and Perinatal Services.

The staff at the Center includes perinatologists (special-ists in high-risk obstetrics) and OB sonographers. The Center features three nonstress testing stations, three ultrasound/diagnostic rooms and consultation areas. While the Center has services to accommodate high-risk moms, all moms-to-be can be cared for in the Center.

“This Center represents a positive development locally for women in the antenatal (before and up through birth) stage,” comments Alfred M. Khoury, MD, who special-izes in high-risk pregnancies. “Those moms who need high-risk monitoring now have easy access to it.”

Conveniently located across from the Center is The Ladies Board Birthing Inn, so, if necessary, patients can go from one building to the other.

Among the testing services available at the

Antenatal Testing Center is 4-D ultrasound imaging

equipment, which gives doctors the best view of

developing babies.

THIS IS JUST A TESTDuring pregnancy, women receive antenatal (or prenatal) testing. These tests detect or monitor health problems in the fetus and establish fetal charac-teristics, including age, gender and weight. The Antenatal Testing Center at Inova Loud-oun Hospital offers the latest technology for evaluating and monitoring high-risk pregnan-cies. Among the services:■ Obstetric consultations■ Level II obstetric ultrasounds■ 3D/4D ultrasounds for medi-

cal indications■ Genetic/lung maturity amnio-

centesis■ First-trimester genetic

screening■ Prenatal diagnostic proce-

dures■ Biophysical profiles■ Nonstress testing■ Fetal transfusions

It’s PrimaryPregnant women who are high-risk and would like to come to the Center for testing should talk to their primary care physician about a referral. For more information, visit inova.org/ilh-atc or call 703-858-Test (8378).

pArTiAL KNEE rEpLACEMENT prOCEdurE OFFErs FAsTEr rECOvEry, shOrTEr hOspiTAL sTAyInova Loudoun Hospital is now offering MAKOplasty®, a partial knee resurfacing procedure for those suffering with painful early- to mid-stage osteoarthritis.

A minimally invasive surgery, it involves resurfacing only the diseased portion of the knee, saving as much of the original knee as possible. Surgeons perform the surgery with the RIO® robotic arm system.

Here, doctors answer some commonly asked questions about the procedure:

Q WhAT ArE ThE bENEFiTs OF ThE MAKO-pLAsTy prOCEdurE?

A The advantages of partial joint replacement are numerous. With MAKOplasty, we can add the precision of the robotic technique to limit the bone removed, which allows for precise and better ligament tensioning. In addition, we don’t have to use bulky larger guides to remove the bone and cartilage, allowing it to be done through a limited capsular incision. This makes recovery faster and less painful. Randolph B. Cook, MD, FAAOS

Q hOW dOEs yOur LiFEsTyLE ChANgE AFTEr MAKOpLAsTy?

A Patients will have much less pain with daily activities after MAKOplasty. This allows people to increase their level of activity and return to things they enjoy doing. Matthew Gavin, MD

Q hOW LONg dOEs ThE iMpLANT LAsT?A The first MAKOplasty knee resurfacing

was done in June of 2006. Therefore, we do not have data beyond five years on the survival rates of MAKOplasty. The short-term data on MAKOplasty at five

years looks very promising. Because of the improved precision of implant positioning with MAKOplasty, it is expected to have at least similar long-term outcomes as reported in specialized centers for partial knee arthroplasty. Current 10-year survival rates in these specialized centers are 90 percent to 95 percent at 10 years and 85 percent at 20 years. David C. Johnson, MD

Q dO pATiENTs hAvE ANy rEsiduAL KNEE pAiN Or disCOMFOrT AFTEr ThE prOCEdurE?

A Patients can have pain after surgery, but it depends on several factors such as how hard they work post-surgery and how quickly they rehab. Patients are usually very satisfied with the outcome. Michael Kavanagh, MD

Q WhO is A TypiCAL CANdidATE FOr MAKOpLAsTy? is ThErE AN AgE LiMiT?

A Ideal candidates for the MAKOplasty ■ have arthritic disease isolated to only one

compartment of the knee.■ weigh less than 220 pounds (100

kilograms).■ do not have significant deformity

(bowlegged or knock-knee deformity of the knee).

■ are still able to extend the knee to within 10 degrees of full extension

■ are able to bend the knee more than 90 degrees.

■ have an average activity level and do not have the goal of returning to high impact sports or heavy labor. Timothy Johnson, MD

Q hOW is MAKOpLAsTy diFFErENT FrOM OThEr KNEE rEpLACEMENT ApprOAChEs?

A A total knee replacement often removes arthritic areas of the joint as well as some healthy portions. This state-of-the-art technology allows for a minimally invasive strategy that leaves the ACL and healthy bone in place.

The MAKOplasty robot features three-dimensional presurgical planning. During surgery, the MAKOplasty system provides the surgeon with real-time visual, tactile and auditory feedback to facilitate minimally invasive joint resurfacing and implant positioning. It is this optimal placement that can result in more natural knee motion following surgery and a quicker recovery. Nauman Akhtar, MD, MBA

MAkO materialThere is no need to suffer with pain. We can help. Learn more at one of our FREE community lectures on MAKOplasty at inova.org/ilh-mako.

MAKOplasty Partial Knee Resurfacing preserves more of the joint than a traditional knee replacement procedure. The treatment targets the diseased portion of the knee and spares the knee’s healthy bone and tissue. An implant is then secured in the joint.

GAMEBACkin the

Two athletes overcome injuries to return to peak performance Stone Bridge High School’s junior pro-style quarter-back Ryan Burns, of Ashburn, was running down the sidelines late in the fourth quarter when his football season dramatically changed. “As I was going out of bounds, a guy pushed me,” says Burns, referring to the illegal hit in the third game of his sophomore year. “I went air-bound and felt a pop.” After coming out of the game, Burns tried to take off his shoulder pads. “When I tried to take my pads off, my whole arm came out of the socket with my pads,” he says.

Luckily, the team physician, Tim Johnson, MD, of the National Sports Medicine Institute, affiliated with Inova Loudoun Hospital, was there watching from the sidelines. “It happened right in front of me,” says Dr. Johnson, a specialist in sports medicine who also serves as one of the team orthopedic surgeons for the Baltimore Orioles. “Ryan did what most people do when they dislocate their shoulder: he started moving his arm up and down like a chicken wing trying to move it back into place.”

X-rays revealed a fractured and dislocated left shoulder.

SIdeSwIPedLike Burns, running close to the gridiron sidelines was a dangerous spot for Connor Jessop, a senior at Broad Run High School in Ashburn, which earned a Virginia AA Division 4 title in 2009 during his sophomore season. It was the first play during the second game of his junior year, when Jessop was hit on the sideline. “I landed funky on my elbow and my shoulder felt weird,” Jessop says. “At first, I just thought it was a stinger. I tried to move my shoulder around and I heard a snap.”

Jessop soon learned he had broken his collarbone, known in medical terms as a displaced clavicle fracture, in his right throwing shoulder.

After their diagnosis, both athletes were faced with a choice: delay surgery and try to continue playing — with uncertain consequences — or have surgery immediately. “I chose the smart option,” says Burns, about the season-ending injury.

That meant undergoing outpatient arthroscopic surgery at the Inova Surgery Center at Inova Loudoun

David Johnson, MD, works with Connor Jessop (below), while Tim Johnson,

MD, performs an examination on Ryan Burns (right). Following football injuries, both high schoolers opted for surgery with the expert physicians at

Inova Loudoun Hospital. They are both grateful they did. Both had stellar fall

seasons and are planning to play college football when they graduate.

Hospital, which is equipped with the latest technology, in September 2010. Dr. Johnson removed a fractured piece of Burns’ dislocated and fractured shoulder, inserted two screws and repaired soft tissues that ruptured.

After sitting out the rest of the season, he was a starter the next season and completed a stellar junior year where he passed for 1,801 yards. Now, the 6-foot-5 Burns is a top college football prospect. Division I programs courting him include Boston College, University of Virginia, Stanford, Rutgers, Boston, West Virginia University and Penn State.

SPeedy RecoveRyJessop, too, opted for surgery — this with both Dr. Johnson and his younger brother, Dr. David Johnson, who practice together at the National Sports Medicine Institute. Within two months of the outpatient surgery at the Inova Surgery Center, Jessop made a return for Broad Run High School’s first playoff game, finishing his junior season with 1,000 yards and 12 touchdowns, despite playing in just four games.

“Dr. (David) Johnson got me back as soon as I could,” Jessop says. “He told me not to push too hard.” Jessop started rehabbing first with arm bands, then shoulder exercises. He then progressed to throwing a tennis ball like a dart, then a baseball, a football and finally a medicine ball. “I think I’m better after the surgery than

DON’T OVERDO ITOvertraining is common in today’s competitive sports world. There are plenty of ways to monitor yourself or your child to prevent overuse injuries.

“kids shouldn’t be playing with pain,” says Tim Johnson, MD. “If an athlete is complaining of pain, you have to respect that. As a responsible parent, you don’t have to rush out to the doctor, but you need to monitor it.” For example, if a pitcher is complaining of elbow pain, start counting his pitches. Ask yourself if he is playing for more than one team or playing multiple games in one day. “It may seem like common sense,” Dr. Johnson says, “But common sense isn’t that common and there’s a lot of pressure for young athletes to perform.”

Here are some additional tips: ■ Sports have off-seasons for a reason. It’s important

to take a break from repetitive activity. Don’t play your sport more than nine months a year, recom-mends Dr. Johnson.

■ During the off-season, find another athletic activity that doesn’t use the same muscle group.

■ If you do sport-specific training in the off-season, make sure you’re only training, not competing.

■ Increase training gradually. Typically that means not more than a 10 percent increase in weight, mileage or pace in a week.

beforehand,” says the 6-foot-4 Jessop, who also plays shortstop on his high school baseball team. “My throwing shoulder actually feels better than before the surgery and my fastball went up 4–5 miles an hour.”

He recently signed with Virginia Tech as a preferred walk-on after leading Broad Run to Virginia’s region semifinals this past year, passing for 1,888 yards with 16 touchdowns and five interceptions. “That’s what sports medicine is all about,” Dr. David Johnson says. “To help prevent injuries and help people return to their level of play as quickly and safely as we can.”

“ I think I’m better after the surgery than beforehand. My throwing shoulder actually feels better than before the surgery and my fastball went up 4–5 miles an hour.”
