THERAPY TOPICS: THE SCIENCE OF PD

SESSIONS OVERVIEW AND OBJECTIVES

PERITONEAL DIALYSIS: WHAT IT IS & HOW IT WORKSIn PD, the innate peritoneal membrane serves as a dialyzing membrane that enables solutes to diffuse from the capillary blood compartment to the dialysate-filled peritoneal cavity. This presentation explores basic peritoneal membrane anatomy, the peritoneal transport process, PD solutions and membrane transport variability. Learn the fundamentals of what PD is and how it works.

LEARNING OBJECTIVES: – Develop a foundational understanding of the components of the PD “System” – Comprehend the principles of solute clearance and ultrafiltration which forms the basis of PD as an intracorporeal therapy – Apply knowledge related to the PET procedure in determining the patient’s transport membrane type

ESSENTIAL PROCEDURES OF PD: A HANDS-ON APPROACH This is a skills-based workshop designed to help you understand important concepts and essential techniques for preparing and completing a PD exchange. Engage in hands-on activities to perform basic PD exchanges for both CAPD and APD, as well as other key essential procedures.

LEARNING OBJECTIVES: – Learn the fundamentals of CAPD and APD – Acquire specialized skills that meet the needs of essential PD procedures

PRACTICE TOPICS: MANAGING PD PATIENTS

THE PD NURSE’S GUIDE TO STARTING A NEW PATIENT: PART 1 – PATIENT TRAINING & PD ACCESSBased on the International Society of Peritoneal Dialysis (ISPD) syllabus to help PD nurses train patients and caregivers, this robust course offers day-by-day descriptions of topics to be covered during patient training and suggests methods of teaching and learning for a home PD program. It is designed to guide the PD nurse to organize topics according to the learner’s needs utilizing current theories and principles of adult education. An emphasis is presented on PD access that includes catheter features, placement, post-implantation care, chronic exit-site care and troubleshooting.

LEARNING OBJECTIVES: – Outline a simple method for planning and developing training for a new PD patient – Identify skills for delivering training that sticks, guided by adult learning principles – Review the basics of PD access and management – Discuss resources and activities aimed at increasing patient engagement, understanding and confidence in their own care

THE PD NURSE’S GUIDE TO STARTING A NEW PATIENT: PART 2 – SUCCESS AT HOMEIn Part 2 of this series, we delve into helping the PD nurse prepare patients to become independent with PD and perform it safely at home. The course provides a foundation for ongoing learning and problem solving as well as an overview of available resources and services to patients in a variety of settings. Learn how to help your patients achieve success in performing PD at home.

LEARNING OBJECTIVES: – Prepare the dialysis patient to integrate home life with PD – Review the impact of PD on work, travel, safety, retraining and how to support the patient at home – Describe the services available to Baxter customers and patients when ordering PD solutions and supplies

FLUID MANAGEMENT IN PDThe goal of volume management in PD is the normalization of extracellular volume. In this course we will review fundamental concepts and approaches to achieving target weight in PD. You may help your patients achieve euvolemia by learning to apply a structured diagnostic approach to the evaluation of fluid balance problems.

LEARNING OBJECTIVES: – Review the fundamental concepts and goals of volume management – Describe how to achieve target weight in PD – Identify key resources to help optimize your understanding of fluid management

PRESCRIPTION MANAGEMENT & ASSESSMENT OF ADEQUACY IN PERITONEAL DIALYSISEach patient’s peritoneal membrane is unique. Objective evaluation of peritoneal membrane permeability is important when determining the most appropriate treatment modality in managing the dialysis prescription. We start with an overview of important concepts regarding PD adequacy and prescription management; then move on to the application of these principles in a workshop-style format.

LEARNING OBJECTIVES: – Discuss the difference between prevalent and incident PD patients and key areas to focus on – Review how to analyze the PET for a prescribed dwell time – Evaluate a crystalloid versus colloid osmotic agent and when to consider for prescription management – Determine therapy volumes when calculating a patient’s Kt/V – List strategies to increase dialytic clearance and achieve adequacy as recommended by KDOQI guidelines

INFECTIOUS COMPLICATIONS IN PDPeritoneal dialysis-related infections continue to be a serious complication for PD patients. This workshop-style presentation will discuss the basics of peritonitis prevention and review the latest ISPD recommendations on prevention and current treatment strategies for infectious complications.

LEARNING OBJECTIVES: – Identify recommendations for preventing PD-related infectious – Describe possible treatment interventions for infectious complications – Discuss opportunities and indications for retraining

NON-INFECTIOUS COMPLICATIONS IN PDThis workshop-style presentation will review a variety of non-infectious complications that can present during PD therapy. Knowledge and recognition of these complications, along with key management strategies can reduce unnecessary patient morbidity and enhance long-term technique success.

LEARNING OBJECTIVES: – Recognize common non-infectious complications in PD – Describe assessment and clinical interventions for common complications – Identify ways to educate patients to help prevent recurrence of non-infectious complications

REMOTE PATIENT MANAGEMENT: LEVERAGING TECHNOLOGY TO WORK SMARTER NOT HARDERA remote patient management platform (RPM) used with and Automated Peritoneal Dialysis (APD) cycler helps guide patients through home peritoneal dialysis (PD) therapy, while keeping them remotely connected with their healthcare providers. This hands-on workshop will review the system’s functionality as well as provide opportunities to detect a variety of APD related issues.

LEARNING OBJECTIVES: – Identify benefits of the RPM clinical portal to patients, nurses and physicians – Consider special situations where remote management of PD therapy may be of particular importance – Outline the skills needed to assist your patients in performing their home therapy with confidence while allowing you to monitor their therapy remotely

APD CYCLER TRAININGThis session will provide hands-on cycler training where you will gain a better understanding of the fundamental logic required to program and manage patients using an APD cycler.

PD PATIENT RETENTIONThe risk of transfer to Hemodialysis (HD) is the highest in the first 90 days of PD initiation. PD patients may switch to HD for modality-related, system-related or patient-related reasons. This presentation reviews causes of PD dropout as well as important concepts to help patients stay on PD therapy.

LEARNING OBJECTIVES: – Identify technique failure – Practice utilizing the SBAR method for effective communication – List reasons for PD dropout and identify strategies to prevent PD dropout
