State Plan Changes - 2012• Certification Requirement for CTs and MRIs in Emergency Room Setting Removed

• Additional certification requirements for transplant evaluations with Active Health in addition to regular transplant pre-certification

• HealthQuotientSM Requirement for Medicare Participants – if Medicare Primary then no Health Quotient required; if on Medicare but it is not primary then a Health Quotient is still required.

• Participating Pharmacies to Administer Certain Vaccines under Wellness Benefits like flu shots

• Weight Management Program through Active Health

• Premium Changes for Retirees only – no changes for active employees
