Epinephrine
Indications: Anaphylaxis, severe allergic reactions,
Contraindications: Patients with angle-closure glaucoma, shock (other thanAnaphylactic shock), organic brain damage, cardiac
dilation, coronary insufficiency, cerebral arteriosclerosis orlabor and delivery. Do not use to treat overdose of
adrenergic blocking agents.Adverse Effects: Nervousness, tremor, headache, agitation, dizziness,
weakness, cerebral hemorrhage, palpitations,hypertension, tachycardia, anginal pain, nausea and
vomiting, dyspnea.
Dosage: Anaphylaxis: 0.3 mg(1:1000) SC Peds Anaphylaxis: (1:1000) 0.01 mg/kg (0.01 ml/kg) SCor IM up to 0.3 ml is patient was exposed to commonly
recognized allergen and has respiratory distress orhypotension.
Precautions: Raising blood pressure and increasing heartrate may cause myocardial ischemia, angina and
increased myocardial oxygen demand.. Do not useconcurrently with Brevibloc. The effects of the drug may
be potentiated by tricyclic antidepressants.
ANAPHYLAXIS
--Begin oxygen therapy per general guidelines--Assist respirations with BVM as needed
--Consider placing venous tourniquet proximal to sting or injection site--Consider placing ice pack at sting or injection site
--If patient was exposed to commonly recognized allergen, and has respiratorydistress or SBP <90, administer one adult EPI PEN (0.3 mg 1:1000) S.C.
--Begin transport immediately unless ALS responding--IV access per guidelines
CONTACT MEDICAL CONTROLMAY RECEIVE ORDERS FOR ADDITIONAL EPI PEN
MAY RECEIVE ORDERS FOR PCT AND/OR VOLUME LOADING FORHYPOTENSION
