PCB 24B Failures

8/14/2004 – Hurricane Charley exiting service areaTransmission Line Insulator FailedPCB 24B cycled open as designed, but fault current was erroneously still detected which resulted in B 230KV bus lockout; LOOP to U1.PCB 24 B was repaired and returned to service.

Page 17: BNP - USNRC Region II Update

2nd Failure; Reason for Fault

October 2004 - PCB 24B failed again.Based on Visual Inspection and Vendor Input - B Phase Bus Side Bushing Experienced Internal FaultBushing Sent to ABB for AnalysisDetermined that standard pressure test had not been performed & would have identified assembly errorNewly Installed & Recent Shipment (3) Rebuilt Bushings were pressure tested within spec.

Extended Power Uprate Margin Improvements

Plant Change Margin Impact

SLC Boron Increase (ATWS)Improves SLC MarginRequires only 1 Pump Versus 2 for ATWS

Power Range Instrumentation

Improves Operating Margin, Operator Interface and reduces required Maintenance/TestingAddresses Obsolescence

Upgrade Condensate & Feedwater

Provides Standby Pumps and Increased Capacities Accommodates Feedpump Trips for all Power Levels

Power System Stabilizer Improves Grid Stability

Steam Dryer

Prompted by Quad Cities failuresGE recommended design changes

BNP has a less susceptible design Based on very conservative load calculations

Design changes Increases weld size to strengthen vulnerable areasAdds Gussets for additional supportSignificantly improves design margin
