For the record, Chairman Reilly and member Miles Beatty, III, Steve Orlando, Pam McDonough and Jack Carney were present at the Chicago meeting site. Board member Glyn Ramage was present at the Collinsville meeting site. Ms. McDonough moved and Mr. Beatty seconded a motion for the adoption of the agenda. Chairman Reilly called for a vote, and the motion was approved unanimously. Mr. Orlando moved and Ms. McDonough seconded a motion to approve the minutes of the November 13, 2018 meeting. Chairman Reilly called for a vote, and the motion was approved unanimously. All attendees and guests in Springfield and Chicago introduced themselves to the Board. There were no guests in Collinsville. Chairman Reilly called for a vote, and the motion was approved unanimously. Ms. Noll, Deputy General Counsel, proposed changes to the Administrative Rules to the Board. Ms. Noll updated changes to Proposed Administrative Rules Implementing the Quincy Veteran’s Home Rehabilitation and Rebuilding Act. Ms. McDonough moved and Mr. Jack Carney seconded a motion to approve the proposed changes to Administrative Rules as submitted. Chairman Reilly called for a vote, and the motion was approved unanimously. Mr. Lance noted the following Architect/Engineer selected by the Executive Director as an informational item:

Mr. Wright gave an update on the Emergency Project Change Order Report to the Board. Mr. Wright gave an update on the Construction PSR Report. Ms. McDonough made a request to provide a report that lists all of the Architect/Engineer Selections made in the last five years for Community Colleges and Board of Higher Education projects, as well as any minority, female, or veteran consultants utilized. Ms. McDonough moved and Mr. Beatty seconded a motion to move to Executive Session for the purpose of a pending litigation update. Chairman Reilly called for a vote, and the motion was approved unanimously. Executive Session convened at 12:24 p.m. Mr. Carney moved and Ms. McDonough seconded a motion to return to open session. Chairman Reilly called for a vote, and the motion was approved unanimously. Regular session was reconvened at 12:26 p.m. Ms. McDonough moved and Mr. Orlando seconded a motion to adjourn. Chairman Reilly called for a vote, and the motion was approved unanimously. The meeting adjourned at 12:28 p.m.

Capital Development Board Request for Board Approval of Change Order

Project History: This project consists of the construction of a 200 bed, 5 story precast concrete veterans home, located at the southwest corner of West Forest Preserve Road and North Oak Park Avenue. The facility will include private bedrooms and private bathrooms, 16 residential kitchens, a commercial kitchen, administrative, recreational, and support spaces, outdoor space and parking for 181 vehicles. The completed building will be LEED Silver Certified. In March 2017, the AE identified to CDB that their design was not in compliance with CDB protocol and needed to be redesigned to comply with structural seismic requirements identified in the CDB Design and Construction Manual(DCM). This change is for material/labor escalation due to project delay of approximately 18 months caused by the structural redesign of the project to meet seismic and gravitational load requirements. The cost is based on a revised schedule that has been agreed to by all Prime Contractors with a Substantial Completion Date of December 2, 2019.

These change order requests are for the Mechanical/Electrical/Plumbing (MEP) Contractors. MEP Trades - Plumbing

Contractor–CR Leonard, Inc, Heating Contractor–Great Lakes Plumbing and Heating, Inc, Ventilation Contractor–Amber

Mechanical, Inc., Electrical Contractor–Elite Electric, Inc, and Sprinkler Contractor -Automatic Fire Protection to ensure

that these firms meet the revised completion date.

Description of RFP Change: These change orders are for additional costs to all MEP Prime Contractors. They include

escalated costs for time, material and labor due to the structural redesign schedule delay. They are based on a revised

Substantial Completion date of December 2, 2019. Approving these RFPCO’s will maintain the projected schedule.

Requested Action: We are requesting Board Approval of (5) five change orders. RFPCO P-4 change order in the

amount of $469,839.25, RFPCO H-5 change order in the amount of $399,493,28, RFPCO V-4 change order in the amount

of $382,922.54, RFPCO E-8 change order in the amount of $563,427.54, RFPCO S-4 Change Order in the amount

$167,155.52. These RFPCO’s will be classified as Architect/Engineer Error.

Capital Development Board Request for Board Approval of Proceed Order

Project History: This project consists of the construction of a 200 bed, 5 story precast concrete veterans home, located at the southwest corner of West Forest Preserve Road and North Oak Park Avenue. This change is for additional material and labor to relocate electrical conduit from floor topping slab to overhead in all areas of the building. The base bid topping slab was not a structural component, the redesign changed the topping slab to a structural component of the building, therefore the conduit can not be placed in the slab and needs to be installed overhead.

This proceed order request is for the Electrical Contractor–Elite Electric, Inc.

Description of RFP Change: This Proceed Order E-11 is for labor and material to install conduit overhead opposed to

in the topping slab areas of the building, Approving this Proceed Order will maintain the projected completion schedule.

Requested Action: We are requesting Board Approval of Proceed Order E-11 in the amount of $656,990.00,

This Proceed Order will be classified as Architect Engineer Error.

Project History: The modernization of the infrastructure systems at the University of Illinois at Chicago College of

Dentistry Building addresses multiple operational and code deficiencies within the 40+ year old facility and is intended to

extend the useable life of the facility infrastructure for a minimum of 30 years.

The scope of work includes replacing all of the buildings’ existing air handling units with new, reliable, energy efficient

fan array air handling units (AHUs); replacing the existing dual duct terminal air boxes with new energy variable air volume

(VAVs) air boxes that modulate to meet the needs of occupied rooms and reduce air flow in unoccupied rooms at a

considerable energy savings to the State; a new digital building automation system to optimize the energy efficiency of

the new AHUs and VAVs; new perimeter heating system throughout the building, including additional energy efficient

controls; new ceilings and LED lighting; a full fire sprinkler system; new 12,000 volt electrical service transformers; new

main electrical distribution system, including new service entrance switchgear, bus risers, and distribution system for the

new AHUs; and new vacuum and compressed air systems for the dental labs.

During the course of investigation, it was discovered that there were four roof exhaust fans which were not specified in

the project documents. During the bidding of this project it was not clear that the University was undergoing construction

and the four roof exhaust fans were installed under a separate project. It was later discovered that these fans were

feeding off of the existing motor control center that was slated to be demolished. As a result, a total of 16 fans along with

twelve starters will need to be connected so that the building can exhaust properly.

Description of RFP Change:

• Twelve (12) existing fans: Provide 12 starters, including labor and material, for the 12 existing fans shown on the

contract documents. Note that the fan feeders and overcurrent devices are in the original contract documents.

Work to be provided by the electrical contractor.

• Temperature Control contractor to provide labor and material to include stop/start capabilities for eight (8) fans

total. The other fans shall run continuously and do not require Temperature Control work.

• Four (4) existing fans: Provide 4 starters and 4 electrical feeders, including labor and material, for four existing

fans. Connect to existing spare circuit breakers in the new penthouse. Work to be provided by the electrical

contractor.

Requested Action: We are requesting approval of Proceed Orders E-49, TC-49 and V-49, in the not to exceed amount

of $108,355.90. Without this approval, the demolition of the existing motor control center in the penthouse cannot take

place. This will delay the start up of new air handling systems S1/R1 and S2/R2 resulting in a delay to the overall

construction schedule. This Proceed Order work will be for time and materials; thus all labor and material expenses will

be verified prior to payment.

Subject: Proceed Orders: V-49 $0.00, TC-49 $31,213.11 & E-49 $77,142.86 The design team received an RFI from the electrical and controls contractors regarding to remain exhaust fans located on the roof of the building. The issued for construction documents indicated the 12 existing roof top exhaust fans. It was discovered that there were 4 roof exhaust fans that were not specified in the project documents. These fans were being supplied from an existing motor control center in the penthouses which is scheduled to be demolished as part of the project.

During the bidding 4 roof exhaust fans were installed under a separate University project. As a result of this finding, I am requesting approval for a total of 16 fans that will need to be connected so that the building can exhaust properly.

The scope of this work will provide: • Twelve (12) existing fans: Provide 12 starters, including labor and material for

Project History: The design and construction of a new 105,000 square foot, three story building with mechanical

penthouse. The new Transportation Distribution and Logistics Center will be located on the existing campus of Olive

Harvey College, located at 10001 South Woodlawn Avenue, Chicago, Illinois.

The basic building telecommunication bid package was prepared for City Colleges of Chicago (CCC). However, due to

scheduling challenges between the designer, CCC, and the Capital Development Board, CCC will not be able to procure

these services directly. The Using Agency has requested that the scope of work for the audio-visual systems (AV),

information technology systems (IT), and security systems (CCTV and access control) be completed by the Capital

Development Board (CDB) and the Phase three electrical contractor as part of this project. Conduits and raceways for

these low-voltage systems, as shown on the contract documents, is part of the base bid scope of work. Phase three

electrical contractor will provide the work for the AV, IT, and Security Systems.

Description of RFP Change: Contractor shall provide the structured cabling system, including the fiber optic connection between the existing building and new building on campus. This cabling system is required in order to provide telephone lines for the elevators, fire alarm system, and data connections for the building automation system (BAS). This work is to be completed in accord with the bid documents. This work is necessary for building occupancy. The elevator will require a phone line to pass inspection and the fire alarm system will require data connections. This will be classified as a user request for additional scope of work and will be user funded.

Requested Action: We are requesting Board Approval of Proceed Order E-3039 in the amount of $2,500,000.

The project may also include improvements and connection to existing campus infrastructure. LEED Silver certification will be required and additional energy efficiencies will be considered. Major design features will include the following elements: Design parameters: • Demolition of existing buildings may be required to accommodate new construction. This work may require phasing and/or preparation of a separate RFP and will likely include asbestos abatement. • Pavement, utility infrastructure and site development will be a part of the scope. • Phased construction as recommended by the report dated January 26, 2017. Program analysis and schematic design will include the following major elements: • Identify and document requirements for compliance with applicable codes, regulations, standards and agencies having oversight of design and construction. • Schematic design for programmed functions. The bridging documents will include, but are not limited to: • Project Intent • Preliminary Site Plan • Program Elements • Conceptual Designs • Required Infrastructure Expansion or Modification • Preliminary Specifications The A/E will develop a comprehensive bridging document, including all of the above elements, in addition to all other miscellaneous criteria as required to provide clearly defined design guidance as a precursor for the follow-on design-build contract. The firm selected to design the bridging documents must have prior experience in developing these types of documents, and the type of heavy mechanical facilities and boiler systems and should have experience as part of a design-build team. The firm will also be required to provide on-site observation and quality assurance services during construction. Selection of a design firm will be based on prior work which 049
