CALL TO ORDER CHC Board Chair called the meeting to order at 12:08pm. A quorum was present. Due notice had been mailed.
APPROVAL OF MEETING MINUTES The minutes of the April 2, 2013 CHC Board Meeting were approved as presented.
EXECUTIVE COMMITTEE The by-laws are being amended to reflect a range of members between nine and 13 instead of a specific number. The by-laws currently require 11 members, so changing this to a range may prevent non-compliance concerns. This was a recommendation for compliance training attended by Executive Director and Quality Improvement & Risk Management Officer.
QA COMMITTEE No April meeting.
FINANCE COMMITTEE Robert reported CHC is 4.38% under budget at this time. The Finance Office attended the BKD Boot Camp for training on finance and grants management. The three positions that created shortages effecting financial summary have been filled.

The Finance Office is preparing to finalize documents for audit. The last year helped CHC boost reserves.

CHC EXECUTIVE DIRECTOR'S REPORT •:• The Executive Director introduced Brandon Durkes who was identified as
someone with financial background interested in serving on the CHC Board. An informational letter was sent to a prospective consumer member as well.
•:• The Executive Director explained the by-law revisions in section 4B that the Executive Committee approved at April meeting. Instead of reading "The CHC Board shall be comprised of eleven (11) members," it reads, "The CHC Board shall be comprised of between nine (9) and thirteen (13) members." The revision was recommended in Corporate Compliance to ensure compliance even if a member resigns.
•:• A memo regarding proposed rev1s1ons to the fee schedule was included in packet. The three changes came about from working through fee schedule, new staff learning processes, and the need to clarify information for documentation purposes. The Accountant II explained the revisions. One proposal will be to charge patient $19.50 for Depo-Provera shot for time period of January 1 through April30.
Wanetta Bean made a motion to approve charging patient $19.50 for Depo-Provera shot for the time period of January 1 through April 30. Martha Prim seconded the motion. Motion carried.
Starting May 1, the Depo-Provera shot will be $24.

The last revision is regarding in house labs. The proposal will be to charge a flat fee of $20 for in house labs.

There was discussion regarding if $20 was the minimum for labs. The Executive Director explained the average cost overall was used to determine the $20 minimum.
The motion was reiterated. Motion approved. Motion carried.
•:• Applied for a Kansas Health Foundation Grant in the amount of $20,000. Qualifying factor was participating in an organizational assessment through

KAMU. With this grant funding, focus would be put on three common measures including tobacco cessation, obesity, and immunizations. Immunizations fall under FQHC measure. FQHCs are expected to focus on three additional measures including immunizations, diabetes control, and cervical cancer screening. The funding will help with educational activities for those we serve and training for staff. The University of Kansas school of Preventative Medicine is developing training modules for staff available at no cost.
•!• The Board of County Commissioners (BCC) selected Schwerdt Design as the architecture firm to assist with identifying new location for Main Office.
•!• Sheldon Weisgrau with KAMU is now the speaker for annual Board Retreat. He is very knowledgeable about the Affordable Care Act.
•!• Patient Attendance Policy was handed out to members. The staff working on New Patient Orientation has also been working with Managed Care Organizations (MCOs) to address the No Show Policy. The MCOs do not allow the $15 to be charged to their members. After the third missed appointment, a patient may have services discontinued. The Assistant County Counselor recommended moving "If patient is served by the KanCare Medicaid program, a call or fax may be placed to Provider Relations of the designated MCO," in the Third Missed Appointment section to the Process section.
Wanetta Bean made a motion to approve Patient Attendance Policy with revisions. Sylvia Herrmann seconded the motion. Discussion took place.
Martha inquired about patients calling four hours instead of 24 hours ahead of appointment time to cancel. The Division Manager explained it may help prevent no shows if patients are not required to call 24 hours in advance to cancel appointment. The policy will be introduced to each new patient and reintroduced to established patients.
The motion was reiterated. Motion approved. Motion carried.
•!• Work continues with MCOs to address ongoing payment, credentialing, and website information issues. Payments are being received but the amounts are not always correct.
•!• The Safety Net Task Force met at the end of April. There may be funding available through Sunflower Foundation for consultant work. No action at this time.
•!• Requested BCC to create and fill physician and LPN position. Additional information was requested and item was deferred until May 16.
•!• Notified that LabCorp interface is built. Next step will be staff training on how to order labs electronically.
•!• Did not get deemed under Federal Tort Claims Act (FTCA) this cycle. Visited with representatives from HRSA regarding it. They provided documentation to the Executive Director showing where improvements need to be made. Several

items revolved around policy refinement. An application will be submitted again sometime in August.
•!• The Executive Director requested technical assistance for clinical operations during quarterly conference call with project officer. Technical assistance team tentatively scheduled to visit clinic September 10-12. A team will come and focus on workflows, scheduling, and policies and procedures.
MEDICAL DIRECTOR'S REPORT •:• The Medical Director has been working with The Quality Improvement & Risk
Management Officer to restructure the Quality Improvement (QI) Committee. Trying to get more Provider involvement in Ql Committee. Two committees have been developed. One will be focused on Ql Committee measures and grant compliance and one will be focused on Patient Centered Medical Home (PCMH). Timely refills Plan Due Study Act (PDSA) has been finalized.
•:• ICD-10 implementation set for October 2014.
•:• Referral process identified as issue for FTCA deeming. The Medical Director is working to improve process.
HEAL THACCESS REPORT No new business.
NEW/MISCELLANEOUS BUSINESS The Executive Director's Evaluation was handed out to members. Robert advised members to fill the form out today and return to him or place in a sealed envelope and return to Main Office.
The Needs Assessment and Scope of Project is a document that is part of grant application that outlines the services available through the CHC. It is made up of three forms.

Board Member McKenzie asked for a change in wording in the March minutes to reflect that the County Fire Chiefs are in the process of hiring another Medical Director.

Mr. Keller indicated that AMR would like to see an increase in the subsidy from the present $1,057,798. AMR would like to fund the purchase of five ambulances and ten heart monitors that are badly in need of replacement. Mr. Keller also indicated that a combination of rate increase and subsidy increase would allow them to purchase the equipment. Mr. Keller provided a document indicating the funding resources needed for the next seven years as well as the SL Depreciation Worksheet he had provided which detailed the payments for the next seven years.
Mr. Keller explained and discussed the information in the· Revenue Analysis and the Transport Analysis documents he had provided at the March meeting.

Mr.- Keller indicated the subsidy increase could also assist in removing the 24 hour shift crews from the streets and replacing them with 12 hour shift crews.
Mr. Keller reported that he had promised the Board of County Commissioners in past years that he would commit at least 932 hours a week for his crews to be on the streets.
Chairperson Sterbenz indicated that the new Audit-Finance Director has not laid out the process for the budget yet; however it will be laid out in the very near future. Chairperson Sterbenz and the Board members showed a consensus that getting the proposal and recommendation to the Board of County Commissioners as soon as possible is the intent of the Board.
Mr Keller followed up on an idea whereby the contract could be rewritten or amended to include that certain monies be earmarked for tangible items that are provided by the County. This had been discussed in previous meetings as a manner by which AMR can show dedication to the community.
Chairperson Sterbenz asked the Board and Mr. Keller if there was interest in the county using their purchasing power to purchase the ambulances and leasing them to AMR to use. Board Members indicated this was a good idea and Chairperson Sterbenz would need to examine the financing options.
Mr. Keller discussed the depreciation of ambulances and heart monitors and how the monies from a subsidy increase would actually allow there to be a savings and cut costs.
Mr. Keller discussed the transparency of AMR and their cost cutting measures that have been in place.
Chairperson Sterbenz discussed the need for a public relations apparatus for AMR to ensure the community becomes better acquainted with AMR.
In answer to a question from Board Members, Mr. Keller explained the process for scrapping older units that are taken out of service.
Board Members and Mr. Keller discussed the advantages of a rate and subsidy increase, as well as the consequences of there not being an increase in the subsidy.
Mr. Keller emphasized that their profit margin for the Shawnee County operation was 3% and that AMR would not want to operate in Shawnee County at a loss.
Mr. Keller provided a spreadsheet from the Board of EMS that detailed the financial statistics of other EMS in the State of Kansas.
Board Member members agreed that AMR runs an efficient operation in Shawnee County, however there was concern expressed that the higher up levels of the company are not coming to the aid of the local operation.
In answer to questions from Board Members, Compliance Officer Casteel read Sections IV, V and VII of Shawnee County Resolution 2012-59 pertaining to the duties and expectations of the Board.

Board Members indicated their appreciation for the performance of Mr. Keller in providing the needed information during the meeting.
There was consensus among the Board Members that new units and heart monitors are needed.
Board Members asked Chairperson Sterbenz to contact the County Commissioners and Audit Finance Department to ascertain if the County would want to own the equipment and lease it to AMR or not. It was agreed that this would cut back on taxes and other expenses that private companies are subjected to.
Chairperson Sterbenz was also asked to get some guidance as to the type of funding the Commissioners are looking towards for this year in relation to AMR.
Discussion and planning approach for upcoming BOCC budget process
New Business
Chairperson Sterbenz and Compliance Officer Casteel indicated that the process is still being lined out. The Compliance Office will forward the process to AMR and the Board when the process is ironed out.
Chairperson Sterbenz indicated again that this will be a difficult budget year and that there could be cuts still made because the legislature has not finished its work.
Letters of thanks to Kansas Board of EMS Executive Director Steve Sutton, Kansas Board of EMS Chairperson JR Behan and AMR Medical Director Dr. David Kingfisher AMR Medical Director.

Compliance Officer Casteel reported that 2nct quarter of 2012 was completed with AMR being in compliance with response times. Compliance Officer Casteel indicated that he hopes to have the 3rct quarter done before the end of May 2013.

Please be advised that our office has approved the claim of Outside Plant Damage Recovery (OPDR) in the amount of $311.13, for damages to a Cox mainline cable located in the vicinity of 5700 SW Burlingame by the Shawnee County Public Works Department on or around January 28, 2013. OPDR is an agent for Cox Communications. The Shawnee County Public Works Department investigated the claim and recommended that it be paid. Our office concurred with the recommendation and a check was issued to OPDR on May 31,2013.

COMES NOW, Plaintiff, Wells Fargo Financial Kansas, Inc., and for its cause of action

1. Plaintiff is a corporation duly organized and existing by virtue of law.
2. Venue is proper in this District by way of the subject matter, i.e., the foreclosure of a
mortgage on certain real estate located in the County of Shawnee, State of Kansas.
3. On or about November 28, 2005, Sandra Gosser AKA Sandra L. Gosser and Alan L. Gosser
[hereinafter "Borrower(s)"] borrowed money from, executed and delivered to Wells Fargo
Financial Kansas, Inc., a Note, payable in monthly installments, in the original principal sum of
· $81,179.50, together with interest as stated. A true and correct copy of the Note is attached
and incorporated by reference as if fully set forth as Exhibit 1.
4. As a part of the loan transaction and in order to secure payment of the Note, Borrower(s)
executed and delivered to Wells Fargo Financial Kansas, Inc., a Mortgage of the same date
upon the following described real estate:
LOT 30, BLOCK A, THE VILLAGE SUBDIVISION "A" IN THE CITY OF TOPEKA,
SHAWNEE COUNTY, KANSAS.
known and numbered as 1814 SW Kent Place, Topeka, KS 66604. The Mortgage was duly
recorded on December 12, 2005, in the Office of the Register of Deeds of Shawnee County,
Kansas, in Book 4301 Page 040 . The Mortgage tax was paid. A true and correct copy of the
Mortgage is attached and incorporated by reference as if fully set forth as Exhibit 2.
5. Wells Fargo Financial Kansas, Inc. is the current legal holder of the debt, and possesses all
rights, including the right of enforcement of the Note and Mortgage.
6. Borrower(s) have failed and refused to make the payments as provided for in the Note and
Mortgage, and said contracts are delinquent and the last payment received was the payment

CONSENT AGENDA - Pulmonary and Sleep Associates- Contract Renewal
Action Requested: Approval to renew the contract with Pulmonary and Sleep Associates for contractual physician and consultation services for Shawnee County Health Agency (SCHA).
SCHA will pay William Leeds, MD of Pulmonary and Sleep Associates $150.00/hr plus $250.00 per month for providing professional medical and health care consultation services for SCHA Communicable Disease programs. Services provided are on an as-needed basis and average $3,800 per year. This contract is paid using county tax support.
These services are required to provide our nursing staff with medical supervision and orders that allow for the treatment of clients with tuberculosis infection or disease. In Shawnee County, in any given year, the SCHA evaluates ·and/or treats approximately 145 cases of tuberculosis infection and 3-6 cases of tuberculosis disease. Treatment for tuberculosis infection and disease is complex and long term; most treatments last a minimum of six months.
The services provided by Dr. Leeds benefit the clients we serve as individuals as well as the community. Through appropriate treatment, transmission of tuberculosis can be stopped thereby minimizing risk to others.

TillS CONTRACT, entered into this day of 2013, by and between the Board of County Commissioners of the County of Shawnee, Kansas, hereinafter referred to as ''the County," and Pulmonary and Sleep Associates, 515 SW Horne, Suite 200, Topeka, Kansas 66606, hereinafter referred to as "the Provider."
WHEREAS, the County is desirous of contracting with the Provider for the purpose of providing professional medical and health care services for patients of the Shawnee County Health Agency.
WHEREAS, the Provider is desirous of providing said professional medical and health care services for patients of the Shawnee County Health Agency.
NOW THEREFORE, in consideration of the mutual covenants contained herein, the parties agree as follows:
The Provider agrees to provide professional medical and health care services for the County. Professional medical and health care services shall be defined as: services which are usually and customarily provided by a licensed physician under the laws of the State of Kansas which are delivered to persons who have been identified by the Shawnee County Health Agency as patients of the Health Agency.
The time and location of the delivery of the professional medical and health care services shall be by mutual agreement between the Provider and the County and shall be scheduled, in writing, at least forty-five (45) days prior to the anticipated date of delivery. The County may authorize the Provider to provide additional professional medical and health care services, in addition to the services which have been scheduled, when in the opinion of the County, such services are needed to maintain the efficient and effective operation of the Shawnee County Health Agency.
The Provider shall, for the duration of this contractual agreement, maintain a license to practice as a physician in the State of Kansas. The Provider shall furnish verification to the County of Kansas licensure.
The Provider shall, for the duration of this contractual agreement, maintain professional liability and malpractice insurance as required by the State of Kansas. The Provider shall fhrnish to the County, verification of insurance coverage.
The Provider shall, while performing services under the terms and conditions of this agreement, comply with all laws, public health regulations, grant agreements and Shawnee County rules and regulations, which govern the operation of the Shawnee County Health Agency. The Provider specifically agrees not to discriminate against persons served or employed by the County on the basis of race, creed, gender, national or ethnic origin, or disability.
The County shall pay the Provider the sum of $150.00 per hour plus $250.00 per month for providing professional medical and health care consultation services for the Communicable Disease program under the terms of this agreement. The County shall not be liable for payment for any services provided which were not authorized by the County, or which were not performed for persons who have been previously identified by the County as patients of the Shawnee County Health Agency.
In no event should this engagement be intended or determined to constitute an employee/employer relationship. Provider has sole right to control and direct the means, manner, and method by which the services required by this agreement will be performed. Provider waives any and all claims to benefits otherwise provided to employees of the County, including, but not limited to: medical, dental, other personal insurance, retirement benefits, unemployment benefits, and liability or worker's compensation insurance. Provider shall be responsible for its own tax liability and related issues and legal obligations. Provider has the right to perform services for others during the term of this agreement.
This agreement shall become effective on the date approved by the Board of Shawnee County Commissioners of the County of Shawnee, Kansas, through 2014. This agreement may, by mutual consent of the parties and by approval of the Board of Shawnee County Commissioners, be renewed on an annual basis. However, it is specifically agreed by the parties hereto that the County and/or the Provider retain the right to terminate this agreement unilaterally with thirty (30) days written notice to the other party. This agreement may be immediately suspended by the Shawnee County Health Officer or the Shawnee County Health Agency Director, without prior notice, if in the opinion of the Shawnee County Health Officer or the Shawnee County Health Agency Director, the Provider's conduct or action violates accepted standards of clinical practice or jeopardizes the operation of the Shawnee County Health Agency.

CONTRACTUAL PROVISIONS ATTACHMENT The undersigned parties agree that the following provisions are hereby incorporated into the contract to which it is attached and made a part thereof, said contract being dated the ___ day of _____ _ 20_.
I. TERMS HEREIN CONTROLLING PROVISIONS. It is expressly agreed that the terms of each and every provision in this attachment shall prevail and control over the terms of any other conflicting provision in any other document relating to and a part of the contract in which this attachment is incorporated.
2. AGREEMENT WITH KANSAS LAW. It is agreed by and between the undersigned that all disputes and matters whatsoever arising under, in connection with or incident to this contract shall be litigated, if at all, in and before a Court located in the State of Kansas, U.S.A, to the exclusion of the Courts of any other states or country. All contractual agreements shall be subject to, governed by, and construed according to t.~e Jaws of the State of Kansas.
3. TERMINATION DUE TO LACK OF FUNDING APPROPRIATION. Shawnee County is subject to the Kansas Cash Basis Law, K.S.A. 10-1101 et seq. If, in the judgment of the Financial Administrator, Audit-Finance Office, sufficient funds are not appropriated to continue the function performed in this agreement and for the payment of the charges hereunder, County may terminate this agreement at the end of its current fiscal year. County agrees to give written notice of termination to contractor at least thirty (30) days prior to the end of its current fiscal year. Contractor shall have the right, at the end of such fiscal year, to take possession of any of contractor's equipment, leased or otherwise, provided to County under the contract. County will pay to the contractor all regular contractual payments incurred through the end of such fiscal year, plus contractual charges incidental to the return of any such equipment. Upon termination of the agreement by County, title to and possession of any equipment purchased by the County under the contract, but not fully paid for, shall revert to contractor at the end of County's current fiscal year. The termination of the contract pursuant to this paragraph shall not cause any penalty to be charged to the County or the contractor.
4. DISCLAIMER OF LIABILITY. Neither the County of Shawnee nor any department thereof shall hold harmless or indemnify any contractor for any liability whatsoever.
5. ANTI-DISCRIMINATION CLAUSE. The contractor agrees: (a) to comply with the Kansas Act Against Discrimination (K.S.A. 44-1001 et seq.) and the Kansas Age Discrimination in Employment Act, (K.S.A. 44-1111 et seq.) and the applicable provisions of the Americans With Disabilities Act (42 U.S.C. 12101 et seq.) [ADA] and to not discriminate against any person because of race, religion, color, sex, disability, national origin or ancestry, or age in the admission of access to or treatment or employment in, its programs or activities; (b) to include in all solicitations or advertisements for employees, the phrase "equal opportunity employer"; (c) to comply with the reporting requirements set out in K.S.A. 44-1031 and K.S.A. 44-1116; (d) to include those provisions in every subcontract or purchase order so that they are binding upon such subcontractor or vendor; (e) that a failure to comply with the reporting requirements of (c) above or if the contractor is found guilty of any violation of such acts by the Kansas Human Rights Commission, such violation shall constitute a breach of contract; (t) if the contracting agency determines that the contractor has violated applicable provisions of ADA, that violation shall constitute a breach of contract; (g) if (e) or (t) occurs, the contract may be cancelled, terminated or suspended in whole or in part by the County. Parties to this contract understand that subsections (b) through (e) of this paragraph number 5 are not applicable to a contractor who employs fewer than four employees or whose contract with the County totals $5,000 or less during this fiscal year.
6. ACCEPTANCE OF CONTRACT. This contract shall not be considered accepted, approved or otherwise effective until the required approvals and certifications have been given and this is signed by the Board of County Commissioners of the County of Shawnee, Kansas.
7. ARBITRATION, DAMAGES, WARRANTIES. Notwithstanding any language to the contrary, no interpretation shall be allowed to find the County has agreed to binding arbitration, or the payment of damages or penalties upon the occurrence of a contingency. Further, the County shall not agree to pay attorney fees and late payment charges; and no provisions will be given effect which attempts to exclude, modify, disclaim or otherwise attempt to limit implied warranties of merchantability and fitness for a particular purpose.
8. REPRESENTATIVE'S AUTHORITY TO CONTRACT. By signing this document, the representative of the contractor thereby represents that such person is duly authorized by the contractor to execute this document on behalf of the contractor and that the contractor agrees to be bound by the provisions thereof.
9. RESPONSmiLITY FOR TAXES. The County shall not be responsible for, nor indemnify a contractor for, any federal, state or local taxes which may be imposed or levied upon the subject matter of this contract.
I 0. INSURANCE. The County shall not be required to purchase, any insurance against loss or damage to any personal property to which this contract relates, nor shall this contract require the County to establish a "self-insurance" fund to protect against any such loss or damage. Subject to the provisions of the Kansas Tort Claims Act (K.S.A. 75-610 I et seq.), the vendor or lessor shall bear the risk of any Joss or damage to any personal pro to which vendor or lessor holds title.

CONSENT AGENDA- Safe Routes to School Grant Application
Action Requested: Acknowledge Safe Routes to School Grant Application.
On May 30, 2013 the Board of County Commissioners approved a Resolution allowing the submission of the Safe Routes to School Grant Application by the Shawnee County Health Agency (SCHA) to the Kansas Department of Transportation. The complete grant application is attached to this request.
Through this funding opportunity, the SCHA and the public will gain a good understanding of issues preventing students from walking and riding bikes to school, including but not limited to lack of sidewalks, poor condition of current sidewalks, and high traffic intersections. The information gathered will lead to a Master Plan that addresses the issues identified.
The total funds to be requested through the Kansas Department of Transportation are $12,000.00. This money will fund 20% of the Health Promotions Coordinator positions already in place at the Health Agency as well as supplies and materials that will be needed to conduct the work for the grant.
There is a 20% match for the grant; a total of $2,400.00 that will be met through the current tax allocation already in the Health Agency's budget to help fund the Health Promotion Coordinator position. There is no additional financial burden to the County.

Safe Routes to School Grant Application

Quincy Elementary
Current Conditions
Quincy Elementary school is located north of the Kansas River in Topeka, encompassing a variety of area
including residential, commercial, and farm land. During the 2012-2013 school year approximately 250
students grades K-5 attended Quincy.
Working with Topeka Public Schools (TPS) USD 501, Quincy was selected as the ideal school for this
project for a number of reasons including, neighborhood development, high traffic areas, make-up of
the boundary areas, BMI rates of students and overall academic performance ofthe school.
There are many streets in the area, that have no sidewalks, sidewalks in poor condition or brick
sidewalks that are overgrown with vegetation, which make them unsafe to ride bikes on or walk. There
is a high traffic intersection located within two blocks of the school.
We know children who are more physically active tend to perform better in school so to help promote
walking and biking to and from school, could go hand in hand with the current changes taking place at
the school. While this funding opportunity will focus on completing a master plan for Quincy in regards
to environmental change, it is important to note some of the other challenges this particular school
faces. Due to poor academic performance, Quincy Elementary is currently a target Priority School for
TPS for academic attention. This program requires the change of leadership and SO% of the teachers
must also change. With this change, comes the perfect time to help engage parents, students and
teachers alike.
In the 2012-2013 School Profile for Quincy it was identified that Quincy is a high need school in both
reading and math. Almost 94% of all students at Quincy were on lunch support.
We also know 40% ofthe students at Quincy are either at risk {85th-94th percentile) or overweight {95th
percentile or greater) on their BMI screenings. By increasing access for students to walk and bike to
school, we will have the potential to impact the overall health of the students.
Proposed Activities
For first year funding, we would be looking for the opportunity to assess the two-mile area around the
school, by conducing walking assessments and collecting traffic counts at high traffic intersections. We
also would be looking at conducting surveys with the parents as well as students and staff about how
they get to school and why they choose that mode of transportation. We would also like to learn from
parents about what things would make them feel more open to allowing their child to walk.
The Shawnee County Health Agency {SCHA) would be the coordinating agency for the work to be done
through the Safe Routes grant. As the lead agency, SCHA would be responsible for ensuring the timeline
is adhered to, bringing community partners to the table, including members of a community coalition,
Heartland Healthy Neighborhoods {HHN), and working to help address barriers that may arise during the
process of working through the grant cycle.
Topeka Public Schools will serve as a major partner in this process, with representation at the table
throughout. The main role is to keep open communication with the specific school officials as well as
district officials. The school district will assist in completing school surveys with teachers as well as
parents and students.
The Topeka Bikeways Project will be important as we start to look at possible engineering aspects, we
will rely on the Bikeways Master Plan and members of the Bikeways committee to give guidance on
possible ways to connect into the Bikeways Plan, specifically Route 23.
Medical Reserve Corps, is a volunteer program through SCHA. This program will serve as an important
resource as we look at walking assessments. We will be able to utilize volunteers, who can go through a
training on conducting walking assessments, and then the volunteers can go out and conduct the
assessment. The biggest benefit to this, would be the money saved by having staff conducting the
assessment and having to pay them, which could possibly result in overtime pay.
Shawnee County has a very active community coalition, Heartland Healthy Neighborhoods (HHN), which
is made up of a variety of businesses and organizations who strive to make change in Shawnee County
related to health and wellbeing. Some projects they have been a part of include Complete Streets and
Clean Indoor Air Act for Shawnee County. HHN would serve as a great resource for support in moving
aspects of the plan forward. There are a variety of representatives on the coalition including elected
officials.
The North Topeka Art District (NOTO) would also have a stake in this process. NOTO is located within the
two mile radius of Quincy Elementary. Students at Quincy partake in art classes through NOTO and it
would benefit both the shop owners and the students to have easy pedestrian access to each other.
NOTO is located on the south side of the high traffic intersection mentioned in the beginning.
Shawnee County Parks and Rec has a community center with park just to the north of Quincy
Elementary. There is currently a partial trail that stops just east of the school. The county Parks and Rec
department is working on some other trail systems along with the Topeka Bikeways Project. It would be
wise to engage them in regards to future plans for this particular trail.

Engineering
Infrastructure changes will be decided on after further evaluation. Both physical and operational
improvements to the infrastructure will be looked at after thorough evaluation of sidewalks, crosswalks,
and safety signage throughout the vicinity of Quincy Elementary School.
Within blocks of Quincy Elementary, what is perceived as a high traffic area is an intersection ofthe
Kansas Ave. Bridge and Morse Ave. While there is a crosswalk, this is a high traffic intersection with
multiple lanes of traffic. We would like to conduct a traffic count at this intersection as part of our
planning process to determine additional needsto assist children needing to cross this intersection.
In addition, we would like to study the ability to connect improvements alongside the work being done
through the Topeka Bikeways Master Plan, specifically Route 23, North Topeka Bikeway.
Education
Classes and programs to teach students basic pedestrian and bicycle skills will be an important aspect to
the Safe Routes to School (SRTS) program. A program based to teach health, fitness, and the
environmental consequences of various modes of transportation will help students to make healthier
choices in how they get to and from school on a daily basis. A driver safety campaign within the vicinity
of the school will attract the attention of parents who drive their children to and from school and raise
awareness of the SRTS program.
If it is founded through our initial evaluation in phase I, that there are safer alternate routes for students
to take when commuting to and from school, those would be part of the education campaign. In
addition, working directly with parents at the school and the PTO about bike and pedestrian safety
would be important.
Enforcement
Partnering with local law enforcement to ensure school zone speeds are enforced in the area, as well as
students are using designated crosswalks would be important to help ensure students and drivers are
helping to create a safe environment for the students to walk and bike to school. In addition, helping
parents feel more at ease with their child(ren) walking and biking to school.
Encouragement
Holding special events in conjunction with the school would be an important piece to help bring the
neighborhood and community together to support walking and biking activities. Topeka Public Schools
host a number of events within their schools as well as a district, such as Family Fitness Nights and
school carnivals that would serve as an important outlet to engage both students and their families
about the benefits of students biking and walking to school.
In addition, working with school personnel and Parent/Teacher Organization (PTO) will serve as an
avenue to address bike and pedestrian safety and work to promote special events and programs that
encourage students to commute to school via these forms of travel.
Evaluation
The Safe Routes to School plan will be monitored at different times throughout the process. Working
with the City of Topeka we hope to conduct traffic audits to monitor motor vehicle traffic before and
after school within the immediate school vicinity as part of the initial process in phase I, as well as at the
end of phase I.
In addition, student/teacher surveys that will monitor how students are getting to and from school will
be conducted at the start of phase I, different points throughout and at the end. It would be important
to collect this information during several points throughout the school year to see what things may
impact the number of kids walking or biking to school (i.e. weather).
A baseline of data will be important before the master plan can really be developed. A clear
understanding of the number of students walking or biking, reasons they are not and a picture of the
neighborhood conditions will be important to understand the barriers.

The Metropolitan Topeka Planning Organization (MTPO) is eager to work in partnership with the Shawnee County Health Agency (SCHA) and USD 501 to initiate a Safe Routes to School program. This study supports and is consistent with the 2040 . Metropolitan Transportation Plan (MTP). We hope to encourage more of our students to walk and to bike to school with this program. Safe Routes has the potential to benefit our entire community and improve children's health. The MTPO has reviewed and supports this SRTS proposal.
We support the SCHA's application to request SRTS funds for phase one funds. On behalf of the MTPO, please consider this a commitment that we will assist SCHA and USD 501 in this process.

Board of Commissioners acknowledgment is requested of the attached report submitted by Amanda Bridges, Adaptive Recreation/Inclusion Services Supervisor. The Board of Commissioners approved the out of state travel for Amanda Bridges to attend the National ADA Symposium held in San Antonio, Texas, May 12 through 15, 2013.
Amanda Bridges serves as the Supervisor of the Adaptive Recreation/Inclusive Services subdivision and also serves as the Shawnee County ADA Coordinator. The Board of Commissioners approved the travel at the December 27, 2012 meeting. Recently, the Board of Commissioners additionally advised that reports following the travel would be important.
Attached is the report submitted by Amanda Bridges that outlines her travel and attendance at the National ADA Symposium.

Thank you for allowing me to attend the 2013 National ADA Symposium on May 12-15 in San Antonio. I enjoyed learning pertinent information that will help the county better serve the disability community. I also enjoyed meeting people such as Anthony Fadale, the ADA Coordinator for the State of Kansas and Jamie Simpson, the ADA Coordinator at KU, who will be valuable resources in our future efforts at ADA Compliance.
I attended sessions on the following topics: ADA Basics, Role of an ADA Coordinator, 2010 Standards, 2011 Reasonable Accommodations, Emergency Preparedness, Self-Evaluation and Transition Planning, and The ADA and Recreation.
There were many more advanced topics presented that I hope to learn about at future symposiums.
Based on information I have learned at this and other trainings, I recommend that the County consider the following: '
Perform an access audit of all our physical structures, policies, and programs which we will use to create a current transition plan.
Create procedures that ensure that this new transition plan be utilized. Create budgets for both physical structure improvements and inclusion services (ADA
accommodations.) Create procedures that ensure that any contracted work specifically outline how the contractors
will maintain ADA compliance to avoid noncompliant new construction. Creation of an advisory board that includes county constituents with disabilities who can advise
us on which areas of accessibility are most important.

Approval of Agreement for HV AC Controls Maintenance (Johnson Controls, fu~ .
I am requesting approval of the enclosed service agreement with Johnson Controls, Inc. (JCI) for the Shawnee County Department of Corrections HV AC systems controls. The current agreement with JCI was for a three-year period ending December 31,2012.
JCI currently provides the computer support and maintenance to the HV AC systems in the Adult Detention Center addition, Juvenile Detention Center, and the Corrections Annex. The cost of the new agreement continues with a 3% increase the frrstyear, then no increases for 2014 or 2015. The agreement sets the maintenance costs at $11,999.00 for each of the three years of the agreement.
JCI is a sole source vendor for our HV AC controls in these locations. The County Counselor has approved the agreement as to form and legality. Funds are available in the FY2013 detention budget (1 ODFOOO 50230) for this agreement.

This warranty does NOT cover: -Fabrics, COMs, finishes, filling materials and normal wear items. - Failure due to accident, misuse, negligence, assembly or installation. ModuForm, Inc., at its discretion and after inspection,
\!\ill repair or replace furniture returned under the terms of this warranty coverage or will supply the customer with replacement parts.
Installation quotations are based on standard site conditions and utilize non union labor unless otherwise specified. Projects requiring prevailing wages, union labor or similar requirements must be clearly stated in the body of the installation quote. Standard site conditions assume the availability of loading docks, elevators for multi floor buildings and easy access and parking far 50 foot trailers.
Freight estimate is based on full truckloads to be packed and shipped the most economical way. Solid Waste does not have the computer or the manpower at this time to diagnose this type of problem. We sent it to Hoyt's Truck Center to connect it to their diagnostic computer. They found that the DEF filter for the exhaust had breached. The truck has to have this filter to operate and stay within the EPA regulations.
2. Sole source only - Explain why the recommended vendor is the only one qualified to provide the requested services at the exclusion of all others, i.e., what makes this vendor uniquely qualified? We have started using Hoyt's Truck Center to spread out some of the repairs on the fleet. They also have a quick turn-around time. This allows us to get the truck back on the route as quickly as possible.
