BNF 1 Gastro-Intestinal System

Anti-platelet drugs-

Doxazosin (standard preparations) –Not to be used 1st or 2nd line for hypertension

Laxido is better value, however when using with thickeners for dysphagic patients Movicol is preferred

Where a PPI is indicated the LOWEST EFFECTIVE DOSE should be prescribed. Currently it is estimated

that 25-50% of all PPI prescriptions are inappropriate

Mesalazine (prescribe by brand. If 400mg MR formulation required, use Mesren MR ® brand)

Antacids and alginates- Alginate containing products have low acid suppressant activity and should be

reserved for patients with reflux symptoms - Mucogel® (low sodium)

Spironolactone may be used for specific indications such as heart failure, ascites, hypertension,

nephrotic syndrome, primary hyperaldosteronism. Eplerenone may be used in the management of

chronic heart failure (NYHA class II).Careful monitoring for hyperkalaemia and hypovolaemia is

required especially for people taking other diuretics and/or ACE inhibitors

ARBs have poorer outcome evidence than ACEi and should be reserved only for those patients who

suffer from unacceptable side effects using ACEi. ARB is not recommended if patient has suffered angio-

odema on an ACEi

Note : neither Candesartan nor Valsartan are licensed to treat diabetic neuropathy

Candesartan ARB of choice in heart failure. Titrate to 32 mg or max tolerated dose. Valsartan licensed

for LV dysfunction post MI

Losartan 1st choice for hypertension or diabetic nephropathy due to cost differential. Also in older

patients due to lower incidence of postural drop

Angiotensin II receptor antagonists (ARB) –

Clopidogrel - For prevention of all other occlusive vascular events (with aspirin for 12 months post

ACS), in multivascular disease, or post MI if aspirin not tolerated

Aspirin - Use post MI or post stroke if clopidogrel not tolerated. EC use is not recommended

(1.5) Chronic bowel disorders- aminosalicylates and corticosteroids

This formulary is to be adhered to be all staff and contractors of Bromley Healthcare.

It should be taken fully into account when prescribers are exercising their clinical

judgement. However, this does not override the responsibility of prescribers to

make decisions based on the individual patient appropriate to their

circumstances.

It is intended to cover :

Statins-

Simvastatin 40mg nocte

OR

Pravastatin 40mg nocte

Simvastatin 20mg can be used, where interacting

drugs are in use* or in primary prevention where

40mg is not tolerated

Atorvastatin- First line statin in patients with ACS. Atorvastatin 80mg recommended in patients with

confirmed Acute Coronary Syndrome until 1 year after last event in line with Clinical Threshold Policy.

SEL cardiac network guidelines available from:

Simvastatin- First line statin in ALL patients except ACS. Must be given in evening or at night due to

short half life.


Pravastatin- guidelines available from:

Fenofibrate - Similar efficacy to bezafibrate in the reduction of triglycerides and is more potent than in

terms of LDL reduction and HDL elevation.

Omacor® capsules 1g - For hypertriglyceridaemia only

There is no outcome data to support the use of this drug (Ezetimibe).

* Patient has familial hypercholesterolemia and is on maximum tolerated statin

Compliance with therapy must be achieved before escalating therapy**

If PPI cover is needed for patients on clopidogrel, lansoprazole is preferred. Other gastrointestinal

therapy such as H2 blockers (except cimetidine) or antacids may be suitable in some patients

BNF 3- Respiratory System

Beclometasone+ formoterol (Fostair®▼) (licensed for adults > 18years) Fostair® has extra fine

particles and is more potent than traditional beclometasone CFC free inhalers.

Asthma

Fostair is indicated in the regular where use of a combination product (inhaled corticosteroid and long-

acting beta2-agnoist) is appropriate:

-patients not adequately controlled with inhaled corticosteroids and 'as needed' inhaled rapid-acting

beta2-agonist or

-patients already adequately controlled on both inhaled corticosteroids and long-acting beta2-agonists.

COPD

Symptomatix treatment of patients with severe COPD (FEV1 <50% predicted normal) and a history of

repeated exacerbations, who have significant symptoms despite regular therapy with long-acting

bronchodilators.

Inhaled corticosteroids – eg Beclometasone, prescribe by brand e.g. Clenil® or Qvar®The MHRA (2006) advised that beclometasone cfc-free inhalers should be prescribed by brand name

e.g. Clenil Modulite® and Qvar® are not interchangeable. Qvar® has extra fine particles and is

approximately twice as potent as Clenil Modulite® 100mcg Qvar should be prescribed for 100mcg

fluticasone, 200-250mcg beclometasone dipropionate (or Clenil Modulite®) or budesonide.

MHRA (2007) advised vigilance for the development of pneumonia and other LRTI (i.e. bronchitis) in

patients with COPD. If treated with drugs that contain steroids as clinical features of such, infections

and exacerbations can overlap. Any patient with severe COPD who has had pneumonia during

treatment with inhaled steroids should have their treatment reviewed.

Long-acting beta agonists (LABA).- Formoterol, Salmeterol

They should not be used without concomitant steroid dosing and should not be initiated in patients

with rapidly deteriorating asthma.

Antidepressants:

Relvar Ellipta: combination inhaler for asthma and COPD

Relvar Ellipta (vilanterol/fluticasone furoate) is a new fixed-dose combination inhaler for asthma and

COPD.

In asthma patients fluticasone furoate 100 microgram once daily is approximately equivalent to

fluticasone propionate 250 microgram twice daily.

The dry powder inhaler is indicated for regular treatment of asthma where a long acting β2-agonist

(LABA) and an inhaled corticosteroid is appropriate. It is also indicated for the symptomatic treatment

of COPD in patients with a post-bronchodilator FEV1 < 70% predicted and a history of exacerbations

despite regular bronchodilator therapy.

Elira Genuair: new long-acting anticholinergic for COPD

Eklira Genuair (aclidinium bromide) is the second inhaled long-acting anticholinergic to be licensed for

the maintenance treatment of COPD.

Aclidinium bromide is a competitive muscarinic receptor antagonist. The metered dose of aclidinium

bromide from the Genuair inhaler is 400 micrograms, which corresponds to a delivered dose of 322

micrograms of aclidinium per inhalation.

Onbrez Breezhaler (indacaterol): New LABA inhaler for COPD

It is a dry-powder inhaler containing a novel long-acting beta agonist for COPD.

Fluticasone (Flixotide®) is not included in the formulary for the following reasons:

• British Thoracic Society (BTS) guidelines suggest that there is little to choose between the efficacies

of the available steroid preparations.

• Relatively large numbers of adverse reactions (including fatalities) have been reported with

fluticasone. The CSM has issued two specific safety warnings regarding fluticasone safety.

• NICE TA 138 (Asthma (in adults) – corticosteroids) recommends use of the least costly product that is

suitable for an individual, within its marketing authorisation. NICE TA 131 (Asthma (in children) –

corticosteroids) recommends use of the least expensive product that is suitable for the individual child.

Theophylline - Prescribe by brand name and only as a last resort.

NB AVOID repeat issue, consider first prescription maximum of 7 days only

Zopiclone 1st line , Temazepam 2nd line

Neuropathic pain (Please note NICE guidance currently under review) Pregabalin should only be

prescribed if other first choice options and subsequent gabapentin are contra-indicated, intolerant or

ineffective. Regular review should assess on-going effectiveness

Selective serotonin reuptake inhibitors - For depression, mixed anxiety disorder:

Citalopram is licensed for treatment and maintenance therapy for depression and for panic disorder.

Citalopram is associated with dose-dependent QT interval prolongation and the maximum daily doses

should be restricted to 40 mg for adults; 20 mg for patients older than 65 years; and 20 mg for those

with hepatic impairment

Fluoxetine is licensed for depression, OCD and bulimia, has a long half life and has a higher propensity

for interactions. 60mg capsules are more expensive than 3x20mg

Discontinuation reactions are highest with Paroxetine and lowest with fluoxetine

Only fluoxetine has been shown in trials as effective in treating depressive illness in adolescents

however the risk of self harm and suicidal thoughts must be monitored.

Venlafaxine is licensed in treatment and prevention of relapse/recurrence of depression, and in XL

form for treatment of GAD and social anxiety disorder. Discontinuation syndrome is common and BP is

increased at higher doses. It should be avoided in patients with a high risk of cardiac arrhythmia. Use

cheapest form which may be branded generic

Analgesics:

Avoid effervescent versions of Paracetamol and co-analgesics -particularly in patients with

cardiovascular disease, renal disease or hypertension, as a standard dose of effervescent analgesic

contains about 8gms of salt. Paracetamol capsules are nearly twice as expensive as tablets

Analgesia in palliative care: Oxycodone 2nd line (where Morphine is not tolerated). Oxycodone efficacy

and side effect profile similar to morphine. Used primarily for control of pain in palliative care. May be

of use for acute pain management in the renally impaired where accumulation is a risk.

Fentanyl patches useful if oral route impaired, renal impairment or intractable constipation due to

other oral strong opioids. Not suitable for management of acute pain / unstable pain

Prescriptions for strong opioid medicines should include the brand of the product. Also please see

NPSA alert on: Reducing dosing errors with opioid medicines – NPSA 2008

Tricyclic and related antidepressant drugs -Not recommended first line for depression (use SSRI

instead) but remains an option for neuropathic pain and migraine

Antiepileptics: NICE guidance recommends that the anti-epileptic drugs (AEDs) should be prescribed

by brand name where appropriate for treatment of Epilepsy

NICE CG35 Parkinsons Disease

Brand prescribing may be considered where it is felt differences in appearance of a medication may

cause a patient anxiety or where familiarity with a particular product in a particular device is important

e.g. with apomorphine

Drugs used in parkinsonism and related disorders: All drugs must be initiated by a specialist.

The NICE guidance on Parkinson's Disease (PD) 2006 states it is not possible to identify a universal first-

choice drug therapy for people with PD. The choice of drug first prescribed should take into account:

• clinical and lifestyle characteristics

• patient preference, after the patient has been informed of the short- and long-term benefits and

drawbacks of the drug classes.

The combination of NRT with varenicline or bupropion is not recommended

ADHD:

Melatonin- This is only used in cases of children with neurodisability

BNF 5 – INFECTIONS

Initiation is by specialists in childhood or/and adolescent behaviour disorder with transfer to primary

care after at least 3 months if a patient responds to therapy and is stable, in line with an approved SEL

shared care guideline.

It should be used in line with the NICE CG72 for ADHD, positioned as an alternative to atomoxetine or

dexamfetamine.

Current interim advice from the Home Office and the Royal Pharmaceutical Society is that it should be

treated as a Schedule 2 Controlled Drug, the same as dexamfetamine and methylphenidate.

Atomoxetine is not treated as a Controlled Drug

Clonidine -this is what NICE guidelines quote:

Drugs for Dementia:

– states that varenicline is recommended as an option for smokers who have expressed a desire to

quit smoking. Varenicline should normally be prescribed only as part of a programme of behavioural

support.

TA217 Alzheimer's disease - donepezil ... - NICE Guidance -

donepezil, galantamine, rivastigmine and memantine. Following specialist initiation only

Lisdexamfetamine is recommended for use in South East London for the treatment of ADHD in children

aged 6 and over as an alternative to either atomoxetine or dexamfetamine in patients who are

unresponsive to a maximum tolerated dose of methylphenidate.

The use of nicotine replacement products in an individual who is already accustomed to nicotine

introduces few new risks. It is widely accepted that there are no circumstances in which it is safer to

smoke than to use NRT. Some patients benefit from having more than one type of NRT prescribed

Patients should be advised to discontinue treatment and seek prompt medical advice if they develop

agitation, depressed mood or suicidal thoughts. Patients with a history of psychiatric illness should be

monitored closely while taking varenicline.

Varenicline for smoking cessation Guidance TA123 - NICE Guidance

Drugs used in substance dependence: Cigarette Smoking Therapy is chosen according to the smoker’s

likely compliance, availability of counselling and support, previous experience of smoking cessation

aids, contra-indications and adverse effects of the products and smokers preference.

In children and young people whose ADHD is unresponsive to methylphenidate, atomoxetine and

dexamfetamine, further treatment should only follow after referral to tertiary services. Further

treatment may include the use of medication unlicensed for the treatment of ADHD (such as

bupropion, clonidine, modafinil and imipramine)

BNF 6 – Endocrine System

Quinolones should be used in caution in patients with a history of epilepsy or conditions that

predispose to seizures.

CSM Advice: Tendon damage (including rupture) has been reported in patients receiving quinolones.

Tendon rupture may occur within 48 hours of starting treatment; cases have also been reported

several months after stopping a quinolone

Following the CSM warning Co-amoxiclav is only recommended as first line treatment in specific

indications. The risk of liver toxicity is 6 times greater with co-amoxiclav than with amoxicillin.

Cholestatic jaundice is more common in patients over the age of 65 and in males. The duration of

treatment should not usually exceed 14 days

Clarithromycin is an acceptable alternative in those who are unable to tolerate erythromycin because

of side effects. An alternative to erythromycin in the treatment of community acquired pneumonia

and in the treatment of many skin and soft tissue infections is doxycycline.

The HPA recommends clarithromycin as it has less side-effects than erythromycin, greater compliance

is used twice rather than four times daily & generic tablets are similar cost.

The antimicrobial guidelines say that broad spectrum antibiotics including Cephalosporins and

Quinolones have been linked to Clostridium Difficile Associated Diarrhoea (CDAD) and should be

avoided where possible. They should be reserved for use in the very following restricted circumstances

outlined in the guidelines.

• treatment of cystitis in pregnancy - 3rd line choice, Cephalexin or Cefadroxil (caution in those with a

penicillin allergy)

• Gonorrhoea - Cefixime 400mg stat (Also applies to pregnancy)

• Gonorrhoea : Ciprofloxacin 400mg single dose .

• Human/Animal Bites (adults and children) – severe infection in Penicillin allergic patients :

Clindamycin four times a day plus Ciprofloxacin (adults) twice a day.

• Acute Prostatitis : Ciprofloxacin 500mg twice a day for 4 weeks

• Pyelonephritis: in Penicillin allergic patients, consider Ciprofloxacin (adults only) 500mg twice a day

for 14 days if mild symptoms

• Acute Otitis Externa (AOE) : Swimmer’s ear may require Ciprofloxacin (adults only)

Azithromycin 250mg capsules to be used normally, 500mg tablets only used for treatment of

Urinary tract infections - Nitrofurantoin should not be used in patients with renal dysfunction because

of the risk of peripheral neuropathy

Key points:

Metformin Currently the only oral therapy with patient

outcome data:

► Standard tablets should be considered the first

line therapy in all patients with type 2 diabetes unless

contraindicated.

► Sukkarto SR (please prescribe this brand only):

Reserve for patients with proven intolerance to

standard metformin or concordance issues.

Reminder: titrating slowly reduces risk of GI side

effects

Sulphonylureas Should still be considered as the initial 2nd line

therapy unless patient at significant risk of

hypoglycaemia or obesity related illness.

► Gliclazide MR 30mg: Reserve for patients with

concordance issues.

► Gliclazide 40mg tablets: More cost-effective to

prescribe gliclazide 80mg tablets which are score and

can be halved

PioglitazoneAvailable generically, potential cost savings can be

made if patients are suitable for this 3rd line agent

► Review current patients on pioglitazone for

heart failure and osteoporosis risk

► Bladder cancer risk

( see MHRA Aug 2011 for full article)

► Stop pioglitazone in patients who do not respond

adequately to treatment (e.g. reduction in HbA1c).

► Use in elderly patients should be considered

carefully before and during treatment as risk of

bladder cancer increases with age. Elderly should be

started on lowest possible dose and regularly

monitored because of risk of bladder cancer and

heart failure associated with pioglitazone.

DPP4 (dipeptidyl peptidase IV) inhibitors ► Sitagliptin: is the preferred first line DDP4

inhibitor in Croydon

► Saxagliptin and linagliptin should be reserved as

alternatives in patients with moderate or severe

renal or hepatic impairment.

► Vildagliptin: not recommended for use in

Croydon (CPC).

► Algoglitin▼ – not yet considered by CPC / new

drugs. Primary care advised not to prescribe until a

formal recommendation on its use is made.

SGLT2 inhibitors (sodium-glucose co-

transporter 2)

Inhibits glucose re-absorption in the proximal renal

tubules, leading to increased urinary glucose

excretion

► It is reported to produce weight loss when used in

combination with metformin

► Common adverse events include vulvovaginitis,

balanitis and related genital infections, UTIs, back

pain, dysuria, polyuria, dyslipidaemia and increased

haematocrit.

► Dapagliflozin (Forxiga®▼)recommended

following NICE TA

► Canagliflozin (Invokana®▼) – not yet considered

by CPC / new drugs. Primary care advised not to

prescribe until a formal recommendation on its use is

made.

Combination oral products Combination oral products are generally not

recommended. If non-adherence suspected, consider

referral to community pharmacist for Medicine Use

Review (MUR).

• Rapid acting insulin analogues have a more rapid onset of action compared to soluble insulin, and

should be injected with or just after food.

• All available pen devices are equally easy to use and reliable, costs are similar

Intermediate Acting Insulin (Isophane) - These insulins are first choice in type 2 diabetes when insulin

is commenced in addition to oral medication

Long acting insulin analogues should be administered at the same time each day and usually requires

once daily dosing. Some patients may require BD dosing on Specialist advice.

GLP-1 receptor agents(glucagon-like peptide-1) - Exenatide and Liraglutide should only be considered

as third-line therapy (in accordance with NICE CG87/TA203/TA248) when control of blood glucose

remains or becomes inadequate (HbA1c ≥ 7.5%, or other higher level agreed with the individual), and

the person has:

These intermediate acting insulins are usually administered twice daily

Short acting Insulins – Soluble acting insulins should be administered 20 minutes before food as they

take 30 to 60 minutes to act. Their effects last for 6 to 8 hours giving greater potential for

hypoglycaemia

Rapid acting Insulin analogues - Novorapid® has evidence to support use in pregnancy so may be an

option in these patients

• People with glycaemic control problems should be properly assessed for underlying causes before

newer, more expensive insulins are considered (including education, understanding of the disease and

its management)

• Type 2 diabetes and a BMI >35 kg/m2

• At least 2 oral hypoglycaemic agents at maximum dose.• Those patients in whom other oral therapies/therapeutic approaches have been considered but

rejected for specific reasons.

People with glycaemic control problems should be properly assessed for underlyingcauses before

these newer, more expensive insulins are considered (including education, understanding of the

disease and its management)

Combined Oral Contraceptive

Oral progesterone only contraceptives (POP)

Drugs for genito-urinary disorders

BNF 7- Obstetrics, Gynaecology, and Urinary Tract Disorders

Prescribe by brand name & refer to BNF comparison chart for content/cost comparison. Cilest® ,Loestrin-20® (low dose) Marvelon® (3rd generation note increased risk of VTE) ,Mercilon®

(low dose and 3rd generation -note increased risk of VTE). When offering combined oral

contraceptives (COCs) first-line options are monophasic preparations containing 30-35mcg of

oestrogen and either, norethisterone or levonorgestrel. However all COCs can be considered based on

patient acceptance.

First choice products should be considered for new patients. Patients on existing treatments do not

need to be switched.

Other versions of COCs are available and are appropriate to be prescribed including the Branded

generic/equivalent versions which may be more cost effective. Check availability from local

pharmacies. Also see COCs content and cost comparison table in BNF. All to be prescribed by brand

name and not generic name to avoid confusion and enable patients to receive the same product each

time

Bisphosphonates and other drugs affecting bone metabolism - Alendronic acid Tablet 70mg,

Risedronate 35mg tablets, Strontium Ranelate 2g sachets. Weekly dose with an appropriate calcium

and vitamin D supplement. The dose of the calcium supplement should be omitted on the morning the

weekly bisphosphonate dose is taken.

Any other copper IUD with 375 -380 of copper can be used after clinical assessment, as long as the

practitioner has been trained to fit the device. IUDs with 300 mm^2 should be reserved for emergency

use where the IUD is to be removed at the next period. IUD should be offered to all women requesting

emergency contraception

Ulipristal (EllaOne®) recommended where unprotected intercourse has taken place between 72 and

120 hrs ago

Micronor® should be taken at the same time each day, if taken outside a “3hr window”, Levonelle

1500® (within 12hrs of unprotected sexual intercourse, and no later than 72hrs) should be prescribed.

Transdermal patch – (Evra® patches) Restricted for use in women who are likely to comply poorly with

COCs, or have conditions causing malabsorption

Coils: can be inserted up to 120hrs after unprotected sexual intercourse

Nova-T® 380

T-Safe Cu 380 only requires replacement every 10 years alternatives eg Nova-T® 380 require

replacement after 5years.

Cerelle should be restricted for use in patients who cannot tolerate oestrogen containing

contraceptives or in whom these preparations are contra-indicated. If compliance may be an issue

Cerelle can be used first line over other POPs

Oxybutynin immediate release tablets are firstline choice for urinary frequency, enuresis and

incontinence, but are not well tolerated in many patients. If they experience any side effects consider

the modified release preparation which may be better tolerated due to sustained release of the drug.

Trospium may be a useful alternative choice in the elderly, as it doesn’t cross the blood brain barrier so

is less likely to cause drowsiness and dizziness

Treatment Advice

Etoricoxib - monitor BP before treatment and 2 weeks after initiation and also periodically during

treatment

All NSAIDs antagonise the cardioprotective effects of low dose aspirin. Using a coxib while on aspirin

cancels any GI protection of coxib. Use conventional NSAID + PPI if patient is on aspirin.

Otitis Externa (Anti - inflammatory) - Prolonged use of topical corticosteroids should be avoided, Use of

>5ml of betamethasone per week may need additional systemic monitoring

Aim for 100-200mg elemental iron per day for treatment of iron deficiency. If not tolerated, try other

salts. Avoid modified release preparations.

Enteral Nutrition (Sip feeds)- Advise acute prescribing only, not repeat.Do not prescribe unless a MUST assessment has been carried out. Ensure a fortified diet is used and

has been used for at least 1 month prior to trial of Sip feed. Patients must have an ACBS indication and

a goal for treatment. Prescribe starter/sample pack which includes mixed flavours, then agree with

patient preferred flavour and prescribe these (do not prescribe more than 1 starter/sample pack per

patient).

Second choice is fluticasone propionate nasal spray. 150 dose pack specifying branded generic Nasofan

is most cost effective.

• Prescribe or recommend an analgesic for symptomatic relief

- paracetamol or ibuprofen is usually sufficient, codeine can provide additional analgesia for severe

pain

• Prescribe a topical ear preparation for 7 days

Otitis Externa (anti-infective) - First choice is Locorten-Vioform® ear drops. The anti-infective agent

clioquinol in this product is considered a reasonable first line choice as it is not used systemically and

resistance is growing to topical aminoglycoside preparations

Nasal Allergy - First choice is Beclometasone Nasal Spray 50mcg/metered dose. Prescribe 200 dose

packs as more cost effective.

(Acute attack)- First choice is Colchicine 500mcg tablets

Dose= 500mcg 2 to 4 times a day until symptoms relieved. Max 6mg (12 tabs) per course. Course not

to be repeated within 3 days

Use colchicine as an alternative where NSAIDs are contra-indicated

(Long term control of gout) – Allopurinol 100mg or 300mg tablets. Starting dose is 100mg daily. Do

not start during an acute attack. Caution in renal impairment – initial dose may need to be 100mg

alternate days

• Remove or treat any precipitating or aggravating factors

Non-steroidal anti-inflammatory drugs (NSAIDs) - When using oral NSAIDs consider gastro protection

in those at high risk e.g aged 65yrs and above. Consider Lansoprazole 15mg capsules or omeprazole

20mg capsules .

Emulsifying ointment or 50% Liquid Paraffin and 50% White Soft Paraffin Ointment in contact with
