• Employees who work 30 hours or more per week on a regularly scheduled basis
• Lawfully married spouse or domestic partner, as documented
• Dependent children, married or unmarried, up to age 26
• Dependent children over the age of 26, if mentally or physically disabled before the age of 26
• Coverage is effective the day of employment
Your Elections are Effective January 1, 2022 through December 31, 2022 unless you have a qualifying family status change

YOU MUST MAKE CHANGES TO YOUR ELECTIONS WITHIN 30 DAYS OF A FAMILY STATUS CHANGE
Benefits terminate on the last day of the month in which you are an active employee for medical, dental and vision
• You get married• You get divorced, legal separation or
annulment• You change from full time to part time
and lose your benefit eligibility• Your spouse or child loses employment• Your child is no longer qualified
dependent• You have a death of a qualified
dependent• You have a baby, adopt a child or are
granted legal guardianship of a child• You become eligible or lose eligibility for

Remember, if you enroll in the HDHP, BWC Terminals will match your contributions to your health
savings account up to:
1. Do you prefer to pay higher premiums out of your paycheck, but less when you need care? (PPO) Or, do you prefer to pay less out of your paycheck, but more when you need care? (HDHP)
2. What planned medical services or prescriptions do you expect next year?3. Are you able to budget for your deductible by setting aside pre-tax dollars from your
paycheck in an HSA or FSA?

• If you elect the High Deductible Health Plan (HDHP) you can contribute to a Health Savings Account (HSA)
• You can NOT be enrolled in ANY other coverage except BWC’s High Deductible Health Plan (including being entitled to or enrolled in Medicare)
• Eligible expenses include medical, dental, RX, and vision deductible or cost share• Account balances rollover from year to year
• Funds are deposited pre-tax, grow pre-tax and can be taken out pre-tax • You or your dependents must be eligible at the time the expense is incurred. If
funds are taken out for ineligible expenses, you will be taxed on the amount and be subject to a penalty
• You can change your HSA elections throughout the year
• You can invest funds once your balance reaches a $1,000
• This is YOUR account. If you leave BWC Terminals, you take it with you! • You will receive a new Debit card if you are new to the plan. Otherwise, your
current debit care will continue to work.

Keep your receipts. You may be asked by the IRS to verify that your HSA funds were used for eligible expenses if you are audited

• If you elect the PPO Enhanced or PPO Basic plan you can contribute to a Flexible Spending Account (FSA)
• You may contribute up to $2,750 annually for expenses incurred in 2022
– “Use it or lose it” rule applies– You may not change your election
during the year– You may use funds for eligible Medical,
Dental and Vision expenses• You may have to submit receipts for FSA
Claims. If you do not submit receipts when requested, your card may be suspended, you may not be reimbursed, or you may be taxed. Check your account regularly for notices
Healthcare Flexible Spending Accounts will be administered by Discovery Benefits in 2022
All claims for 2022 expenses must be submitted to Discovery no later than March 31, 2023 to be eligible for reimbursement

• Dependent Care Flexible Spending Accounts (DCFSA) are administered by Discovery Benefits
• DCFSA is for eligible childcare expenses such as preschool, summer day camp, before or after school programs, and child or adult daycare
• DCFSA cannot be used for dependent healthcare expenses• Your spouse must be working, actively looking for work or a full-time student• You can only take out the funds you have deposited pre-tax• The maximum election and withdrawal per married couple is $5,000 per calendar year • If your spouse contributes to their DCFSA, you are both limited to $2,500
o Different expenses must be submitted to each plan
All claims for 2022 expenses must be submitted to Discovery no later than March 31, 2023, to be eligible for reimbursement under the plan.
NOTE: Your money must be in your account before a reimbursement can be made
