Josef’s is located at 627 NP AVE in downtown Fargo, and is owned by Josef’s School of Hair Design, Inc. This school is located at the heart of historic downtown Fargo. It has been an educational hub for over 50 years. Founded by Joseph Olivieri in 1949, the school is the center for learning about the ever changing world of hair design, fashion and spa services. Josef’s is comprised of two main fl oors totaling 13,000 square feet. Josef’s uses several visual aid tools to assist the students and has a complete and updated library. The campus is situated close to many apartments, restaurants and retail shops in Fargo’s revitalized downtown.

Facilities include classrooms, clinic and reception area. Josef’s uses many visual aids to assist the students and has a complete and updated library. It is conveniently located near the West Acres Mall. The Fargo-Moorhead metro area has a population of 184,857. The Red River runs through the metro area, and it is just 40 minutes from the Lakes area. Fargo-Moorhead is fortunate enough to have North Dakota State University, Minnesota State University Moorhead and Concordia College. The universities sponsor many community events, including sports, musicals, symphonies, theatre and educational classes. Other metro area events include: The Red River Valley Fair, Winter Festival and many concerts, shows and sporting events at the Fargodome.

Josef’s uses many visual aids to assist the students and has a complete and updated library. Grand Forks has a population of 52,000. Grand Forks-East Grand Forks have much to offer – spacious parks, great fi shing, cross-country skiing, canoeing, shopping at Columbia Mall, and many arts and theater events. We are also home to the University of North Dakota, Alerus Center, Ralph Englestad Arena and the North Dakota Museum of Art.

8:45-9 AM School is about to begin. Students are ready for the day – hair, make-up and uniform looking awesome and professional.

9 AM Clock in for Theory Class. Make sure to be on time because State Law requires Theory classroom doors close at 9:05!

9:45 AM Theory is completed. Beginning students report to the Basics classroom for the rest of their day; all other students go directly to their stations on the clinic floor to take clients work on floor sheets, or attend advance classes.

Lunch Lunch (30 minutes) and short breaks are scheduled depending on students’ clinic floor appointments and Basics schedule.

4-4:30 PM The day is almost over! Students work together on assigned duties to sanitize and close up the classrooms and clinic floor.

Cosmetologist/Nail Technician Employment OpportunitiesThe cosmetology career path varies based on the fi eld of employment (nail tech, makeup artist, skincare specialist, hairstylist, etc.). Depending on what fi eld of cosmetology you choose, typically the next step would be working your way to a management position, branching out to have your own salon, or teaching cosmetology classes to beginners (if the educational route is of interest).

Compensation The compensation plan differs for each type of license and level of experience. Usually however, the compensation plan for the cosmetologist will be guaranteed salary for a certain period of time or a commission, whichever is greater. To increase the compensation, the individual may have to complete more services, work more hours or increase responsibilities.

Usually there are partial or comprehensive benefi t packages, such as vacation pay, sick benefi ts or insurance benefi ts. For salary ranges and job growth statistics go to the U.S. Department of Labor’s website at www.onetonline.org.

Often the stylists who become most successful are those who continue learning and building upon the basic techniques they have learned while in school. They do this by attending beauty education shows and/or classes to learn more advanced techniques and trends in fashion.

Physical Demands of Practicing ProfessionAn individual should be in stable mental health with the ability to cope with stress, pressure, praise and criticism that may occasionally occur while working with people and doing more than one activity at a time. The individual must put the desires of the client ahead of his or her personal desires.

Working Conditions of Cosmetology IndustryCosmetologists are employed in some of the most beautiful and pleasant environments. The décor of most salons consists of mirrors, music and pleasing colors.

Safety Requirements of Cosmetology IndustryThe most dangerous aspect of a career in Cosmetology is working with the chemicals used in many of the products in the industry. If used incorrectly, the chemicals could harm the cosmetologist and/or client. Instruction of proper procedures is provided in school and on the product packaging to prevent any occurrence of harm. Some individuals experience allergic reactions or irritations from product chemicals used in the industry. Precautions can be taken in many instances to prevent or lessen the severity of these reactions/irritations.

The Massage Therapy course is a total of 750 clock hours (full-time/35 clock hours per week). Josef’s will teach you how to revitalize your clients through the completion of the Massage Therapy Course. Our program includes learning not only technique, but also how to create a healthy environment, appropriate client/therapist relationships, as well as the essentials of proper body mechanics.

Josef’s will prepare you with a combination of theory and hands-on-training focusing on the use of skin care products and highly specialized equipment together with personal one-on-one guidance. The Skin Esthetics course is 600 clock hours (full-time/35 hours per week) in length. The program encompasses the study of principles and practice of skin care and hair removal, the application of cosmetics, and skin disorders.Program length: 18 weeks

Nail Technology is one of the fastest growing and most creative, rewarding and high-paying professions in our industry today. As a nail technician, you will use the latest technology to apply artificial nails and use your artistic ability to create original design on nails. You will also perform manicures and pedicures using treatments from Creative Nails.

Course Curriculum - 350 HoursStudents enrolled in the Nail Technology course must complete 350 hours (full time/35 hours per week) of instruction in all aspects of manicuring. Payment – Tuition, Fees, Kit & BooksTuition and fees are subject to change for any available program; please call for current investment. Financial Assistance/Aid is available to those who qualify; to apply, go to www.fafsa.ed.gov for your free application. Please refer to the Student Services section of this catalog for more FAFSA instructions. Alternative tuition/fee payments may be made by cash, money order, credit card or through non-federal agencies. Examples of payment methods include payment of the application and enrollment fees with the balance paid in full prior to the start date; a down payment followed by monthly installments for the balance to be paid in full prior to the expected graduation date; or a combination of any of the above. Students are responsible for paying the total tuition and fees and for repaying applicable loans plus interest. A $20 cash fee is collected on the first day of attendance ($15 for the North Dakota State Board of Cosmetology registration for the Cosmetology, Skin Esthetics & Nail Technology programs and a $5 fee for locker and station keys). The locker and station key fee will be refunded upon the return of these items.

Kit & BooksYour kit of equipment and textbooks will be provided during orientation. Students must replace lost or broken articles in order to conduct their work properly. Additional Hours For each additional hour a student is here past their contract date, an overtime fee ($5) is assessed for each remaining hour needed to complete the state licensure required hours. The additional fee may be waived by the school director with special circumstances. Attendance must be at 90% and hours must be completed prior to the 133% maximum timeframe of the program.

Make-Up Work Upon returning to school after being absent, it is the student’s responsibility to contact the instructional staff to determine what assignments, subject areas, notes and/or tests were given during the student’s period of absence. All efforts will be made by the instructional staff to help the student attain the current status and progress level expected.

Satisfactory Academic ProgressJosef’s School of Hair Design, Inc. and Josef’s West Academy encourages students to maintain high standards for attendance and academics to complete the program and achieve a career in the industry. Students are encouraged to strive for 100% in attendance and academic performance. However, Josef’s recognizes students need a school-life balance by setting minimum levels of attendance and academic performance in compliance with the Department of Education (DOE) and the National Accrediting Commission of Career Arts & Sciences (NACCAS). This Satisfactory Academic Policy is discussed with students prior to enrollment, on orientation day and applied consistently amongst all students throughout their program, whether they receive financial aid or not.

Attendance Progress EvaluationAll students must maintain at least a 75% cumulative attendance average of scheduled hours in order to be considered making Satisfactory Academic Progress. The attendance percentage is determined by dividing the total accrued clocked hours by the total number of scheduled hours.

Academic Progress EvaluationAll students must maintain at least a 75% GPA in theory and practical/clinic work. Grades will be determined by averaging all the weekly theory grades, all the practical/clinic grades for the period; these two averages will then be averaged together to determine your overall GPA. Theory and practical/clinic work will be graded according to the following scale: 91% - 100% Excellent 81% - 90% Very Good 75% - 80% Passing Below 74% Failing/Unsatisfactory Evaluation Periods:For the purposes of meeting federal and accreditation standards, students must have performance evaluations at the following checkpoints based upon actual hours attended: Cosmetology 450 hours and 13 weeks 900 hours and 26 weeks 1350 hours and 39 weeks Massage Therapy 375 hours and 11 weeks Skin Esthetics 300 hours and 9 weeks Nail Technology 175 hours and 5 weeks Cosmetology Instructor Program 900 hours and 26 weeks

Determination of ProgressFor the purposes of meeting federal and accreditation standards, each student’s attendance and academic performance is evaluated at the required checkpoints related to their program (see above). Upon completion of the evaluation, students will be given a copy of their evaluation, and the original will be stored in the student’s file. At that time, the students will acknowledge receipt of a copy of the Satisfactory Academic Progress Report which indicates their performance evaluation results. The students will be notified of any evaluation that impacts their eligibility for financial aid.

The Satisfactory Academic Progress evaluations apply to all students. It does not depend upon whether they receive federal financial aid. For students who do receive financial aid however, failure to meet the minimum standards of the Satisfactory Academic Progress Policy may delay or terminate eligibility Title IV, HEA programs funds (if applicable) unless the student is on Warning or has prevailed upon appeal of the termination that has resulted in the status of Probation.

Warning Period: Upon failing to meet the minimum of 75% in attendance and/or academic performance, a student will be placed on “Warning” until the next SAP evaluation checkpoint. A student on Warning is eligible to receive Title IV funds. The student will receive an academic plan providing corrective actions to achieve the minimum of 75% in attendance and/or academic performance by the end of the Warning period

Probationary Period: If a student has failed to meet 75% in attendance and/or academic performance by the end of the Warning period, and it is determined that the student is capable of meeting the satisfactory academic progress standards by the next evaluation

period, the student may submit an appeal in an effort to remain enrolled as a student at Josef’s. The written appeal must be received within five (5) school days of receiving the report and clearly describes the circumstances that affected the student’s academic and attendance performance since the last evaluation period. (See the Appeal Procedures of SAP Determination of Progress Policy below).

If the student prevails upon appeal, the student may remain in school on Probation and be eligible to receive Title IV funds. The results of the appeal will be printed and stored in the student’s file. Additionally, the student will receive an academic plan providing corrective actions to achieve the minimum of 75% in attendance and/or academic performance by their next SAP evaluation period, or the maximum time frame established for the individual student within the academic plan. If at the end of Probation, the student has still not met both the attendance and academic requirements by not following through with the academic plan, the student will be determined not making Satisfactory Academic Progress (SAP). This status requires the student’s financial aid to be terminated and the student may be administratively withdrawn from Josef’s. To re-enter the program, please refer to the Re-Entry Policy described below.

If no appeal was received or the appeal did not prevail, the student will not be eligible to receive Title IV funds, HEA program funds (if applicable) and the student will be administratively withdrawn.

Students may only file one appeal under the Satisfactory Academic Progress Policy.

If at any SAP evaluation checkpoint, it is determined that the student will be unable to complete the program within the maximum time frame of 133%, the student may be administratively withdrawn from Josef’s and his/her eligibility for Title IV funds will be terminated. To re-enter the program, please refer to the Re-entry Policy below.

Josef’s School of Hair Design retains the discretion to administratively withdraw an enrolled student.

Appeal Procedure A student may appeal by stating, in writing, his/her explanation and/or concern regarding their attendance/academic performance and submitting it to the school director within five (5) days of the report. Reasons for an appeal may include a death in the family, a minor injury or illness of the student, or any other allowable special or mitigating circumstance. The student must submit with the appeal any documentation related to the circumstance, for example; a doctor’s note, hospital record, obituary, police report etc. The appeal should also include what has changed about the student’s situation that will allow them to achieve Satisfactory Academic Progress by the next evaluation point.

The school director will review the appeal and meet with the student. The school director will rule on the appeal after the meeting and notify the student and instructional staff regarding this ruling within five (5) days. All appeal information (written appeal, meeting notes, appeal ruling, etc.) will be documented and kept in the student’s academic file. All appeal determinations are final.

During the appeal procedure, students are not allowed to clock in while writing the appeal or during the determination of the appeal, not to exceed ten (10) days. This specific absence will not affect the student’s next performance review.

Re-establishment of Status: Students not maintaining Satisfactory Academic Progress may re-establish Satisfactory Academic Progress and/or financial aid eligibility during their next evaluation period by achieving the minimum attendance and academic standards required for Satisfactory Academic Progress. Academic plans are provided to students which outline the steps necessary to achieve Satisfactory Academic Progress by their next evaluation period.

Re-entry PolicyWhen a student discontinues or withdraws from Josef’s School of Hair Design, he/she may not re-enter for a minimum of six (6) months but no more than five (5) years to receive credit for his/her completed hours. If the student’s attendance issues have been resolved to the satisfaction of the School Owner and Director, the student may be approved to re-enter sooner. A student will re-enroll with the same SAP status they had at the time they previously discontinued or withdrew. If a student is administratively withdrawn due to non-compliance with the Satisfactory Academic Progress Policy, re-entry to the school is up to the discretion of the School Director.

Josef’s School of Hair Design requires all prospective re-enrolling students to submit a letter of intent stating their purpose and goals for re-enrolling. Pending approval, the student must complete a re-enrollment application and pay a re-enrollment fee. Josef’s charges tuition for the remaining hours needed to reach the ND licensure requirements. The re-enrolling student will be responsible for any additional items such as books or equipment needed to fulfill the requirements of the course. Josef’s will determine a suitable date for the student to re-enter the program and approve the requested campus.

Transfer StudentsUpon approval by the ND State Board of Cosmetology, a student who transfers to Josef’s School of Hair Design or Josef’s West Academy from a cosmetology school located in another state will be given 100% credit. A student transferring from a cosmetology school within ND will be given credit rounded down to the nearest completed hour upon verification by the ND State Board of Cosmetology.

To transfer to Josef’s School of Hair Design, the approved transfer hours are counted as attempted and completed upon enrollment to determine when the allowable maximum time frame has been exhausted. When a student transfers to this school, the evaluation periods are based on the actual contracted hours and academic achievements at Josef’s School of Hair Design.

Josef’s School of Hair Design requires prospective transfer students to provide copies of official transcripts and SAP records from any and all prior institutions where related coursework was being pursued. A letter of intent stating the student’s purpose and goals for transferring to Josef’s must also be submitted. Pending approval, the student must pay a tuition down payment to enroll. In addition, Josef’s charges tuition for the remaining hours needed to reach the ND licensure requirements. The transferring student will also be responsible for any additional items such as books or equipment needed to fulfill the requirements of the course. Josef’s will determine a suitable timeframe for the student to enroll and approve the requested campus.

Reinstatement of Financial AidTitle IV financial aid will be reinstated to students who have 1) prevailed upon an appeal, 2) re-established Satisfactory Academic Progress status, or 3) returned to school

after an interruption of training that commenced at the time the student was not making Satisfactory Academic Progress and has now re-established Satisfactory Academic Progress requirements. The Satisfactory Academic Progress report will reflect a student’s progress status on scheduled financial aid disbursement dates to establish if and when re-established satisfactory status occurred. When Satisfactory Academic Progress (SAP) status has been re-established, the Financial Aid Director will proceed with the disbursement of scheduled financial aid and/or advise the student of financial aid eligibility and if applicable, the financial aid application procedures necessary to obtain financial aid.

Graduation RequirementsEach student must pass 80% of all weekly theory exams (passing with at least a 75%) and achieved a 75% GPA on the final program examination both in written and practical work. Each student must have also completed 1800 hours in the cosmetology program (or 600 hours in the skin esthetics program, 750 hours in massage therapy program, 350 hours in the nail technology program, or 960 hours in the cosmetology instructor program).

The maximum time frame for a student to complete their program of study is 133% of the published length of the educational program.

Leave of Absence PolicyJosef’s School of Hair Design & Josef’s West Academy may grant a leave of absence to an enrolled student in satisfactory standing with medical circumstances. Only cosmetology, massage therapy or skin esthetics students are eligible to apply. An approved leave of absence may be a minimum of two weeks (70 hours) and no more than two months (280 hours). The leave of absence may not be taken intermittently. If more than one leave of absence (LOA) is necessary, the number of days in the approved LOA, when added to the number of days in all other approved LOA’s, does not exceed:

Cosmetology: 120 Days (40 days/280 hours maximum per LOA) Massage Therapy, Skin Esthetics & Cosmetology Instructor Program: 40 Days (20 days/140 hours maximum per LOA)

Procedure1. Enrolled students are provided a copy of the LOA Policy in their orientation packet. 2. The student must speak directly with the school director to discuss a leave of absence as

soon as possible, unless unforeseen circumstances prevent the student from doing so. 3. The student must submit in writing a signed request for a leave of absence to the school

director, stating the reasons for the request, the effective date, and the return to school date.

4. The request must be accompanied by a letter from a certifying professional (physician, physician’s assistant or nurse practitioner). The letter must be on official letterhead and must include the diagnosis, the limitations it imposes on the student and a reasonable estimation of the recovery time. Josef’s School of Hair Design reserves the right to verify qualification of the certifying professional.

5. If unforeseen circumstances prevent a student from providing a written request, Josef’s may grant the student’s request for a leave of absence and collect the written request within ten (10) days from the last day of attendance. In the meantime, Josef’s will document that the LOA is approved for a full 280 (or 140) hours and calculate the return date. The student’s contract with an amended completion date will be placed in the student’s file for signatures upon their return.

6. The Josef’s School of Hair Design Leave of Absence Review Committee (consisting of the

school director and other appointed Josef’s officials) will notify the student in writing of its decision.

7. If all of the above documentation is not received within ten (10) days from the student’s last day of attendance, Josef’s will administratively withdraw the student.

8. The student must return to school on the determined date or will be administratively withdrawn. The student may return earlier than the expected return date but no later. It is the student’s responsibility to keep track of their return date.

9. A student placed on a leave of absence is not eligible to receive student services.10. The student must also speak directly with the Financial Aid Director unless unforeseen

circumstances prevent the student from doing so. The student’s financial aid will be put on hold until they return to school. A leave of absence will not affect the 133% maximum completion time for Title IV financial aid funding. An approved LOA does not involve additional charges by Josef’s.

11. If the student chooses to not continue the course, the student will be required to withdraw by following Josef’s Withdrawal Policy. Once withdrawal notification is given, the Title IV refund calculation will be processed according to the last day of attendance. Failure to return may affect their loan repayment terms, including the exhaustion of some or all of the student’s grace period.

12. Student’s contract period and maximum time frame is extended by the same number of days taken in the leave of absence.

13. Students returning from a leave of absence or other official interruption of training must return to school with the Satisfactory Academic Progress as they had as before.

Noncredit and Remedial CoursesThe above courses are not offered. Therefore, these do not have any effect upon Josef’s School of Hair Design, Inc. or Josef’s West Academy’s Satisfactory Academic Progress Policy.

Dress CodeStudents are required to furnish their own uniforms. A copy of Josef’s School of Hair Design, Inc.’s Dress Code Policy is provided to each student upon enrollment and reviewed during the first day of class.

Rules of the SchoolRules are enforced by our school director. Our belief is that a well-disciplined student will be a well-disciplined, successful professional. Grades and attendance are monitored on a regular basis.

GraduationIn order to graduate from Josef’s School of Hair Design, Inc. & Josef’s West Academy, students must complete the required number of hours of instruction per program, pass the final practical exam, pass the final theory exam and pass 80% of theory tests with a grade of at least 75%. A diploma will be awarded upon graduation. Massage Therapy graduates will also obtain a certificate or card signifying the successful completion of a reputable first aid CPR course that was instructed by a certified instructor by a nationally recognized organization which provides first aid and CPR instruction.

LicensingA Cosmetology, Skin Esthetics, Cosmetology Instructor Program & Nail Technology graduate who has received a diploma from a licensed ND cosmetology/massage therapy school, must provide evidence of a high school or GED completion and must pass state licensing exams in order to practice services within North Dakota. The exams consist of a practical test, a state law written test and a national written exam. The graduate must pass all aspects of the state exam with at least a 75%. The graduate will be considered a professional when a license has been granted by the State of North Dakota. The license must be renewed once a year. The first

issued licenses are called the Operators License, which enables an individual to practice all aspects of Cosmetology or specialize in one area of Cosmetology and the Massage Therapist License which enables an individual to practice all aspects of massage therapy or specialize in one area of massage therapy. Rights to Privacy and Student Record AccessIt is the school’s policy to guarantee the right of students or parent/guardian of a dependant minor to gain access to their own files. Students can review their files at any time with a written request to the Financial Aid Director. The school also allows a parent/guardian of a dependant minor to give written permission for the release of student-file information to a third party. The school does not need written consent to release student-file information requested as party of a legal request or for accreditation purposes. In all other instances the school will not release information about individual students without written permission from the student.

School ClosuresJosef’s School of Hair Design, Inc. is closed for the following recognized national holidays and special days: New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and the day after, Christmas Day and the day after. Special time off must be approved by the School Director. Additional and unexpected school closings will be announced to staff and the student body.

Grounds for Termination Josef’s School of Hair Design, Inc. and Josef’s West Academy reserve the right to terminate a student for any reason, including but not limited to: Alcohol or drug abuse in the school, failure to abide by the school rules and regulations, failure to comply with Satisfactory Progress Policy, and unapproved absenteeism for ten consecutive school days.

Refund Policy - Cosmetology, Skin Esthetics, Nail Technology & Cosmetology Instructor ProgramThe following policy will apply to all terminations for any reason, by either party, including student decisions, course or program cancellation, or school closure. Any monies due the applicant or students shall be refunded within 45 days of official cancellation or withdrawal. Official cancellation or withdrawal shall occur on the earlier dates that:

1. If a student (or in case of a student under legal age, his parent or guardian) cancels his/her enrollment and demands his/her money back, in writing within three (3) business days of signing the enrollment agreement or contract. All monies collected by the school shall be refunded with the exception of the application fee. The cancellation date will be determined by the postmark on written notification, or the date said information is delivered to the school administrator/owner in person (applies whether or not the student has started training).

2. If a student cancels his/her enrollment after the three (3) business days after signing the contract but prior to entering classes, he/she shall be entitled to a refund of all monies paid to the school less the application fee. If the school is permanently closed and no longer is offering instruction after a student has enrolled, the student shall be entitled to a pro-rata tuition refund. If a course is canceled subsequent to a student’s enrollment, the school shall at its option: (1) provide a full refund of all monies paid; or (2) arrange for the completion of the course.

3. In case of personal illness or disabling accident, death in the immediate family, or other circumstances beyond the control of the student, the school will determine and present a settlement offer that is reasonable and fair to both parties.

4. The following schedule for tuition adjustments is used for all students who enroll and begin classes:

4a. The Return of Title IV Funds Policy shall be used to calculate funds returned by Josef’s School of Hair Design, Inc. or Josef’s West Academy to the student for all first time students receiving Title IV funds who discontinue (see Josef’s School of Hair Design, Inc.’s and Josef’s West Academy’s Financial Aid Director for more information).

5. If a student is rejected by the school for enrollment, the applicant shall be entitled to a refund of all monies paid with the exception of the non-refundable application fee.

6. Enrollment time is defined as the time elapsed between the actual starting date and the date of the student’s last day of physical attendance in the school. Formal termination by the school shall occur no more than 30 days from the last day of physical attendance. Formal termination may be determined by the school upon failure to abide by school regulations, alcohol or drug abuse in the school or failure to comply with the Satisfactory Academic Progress Policy.

7. If a student on an approved leave of absence must notify the school that he/she will not be returning. The date of withdrawal shall be the earlier than the date of expiration of the leave of absence or the date the student notifies the institution that the student will not be returning.

8. This refund policy will also apply if a student is administratively withdrawn (expelled) by Josef’s School of Hair Design, Inc. or Josef’s West Academy.

Refund Policy – Massage Program OnlyThe following policies and requirements will be used to determine refunds:1. When notice is received prior to, or within seven days after completion of the first day

of instruction, all tuition and other charges must be refunded to the student with the exception of the non-refundable application fee.

2. When notice is received prior to, or within thirty days after completion of the first day of instruction, or prior to the completion of one-fourth of the educational services, all tuition except twenty-five percent thereof must be refunded to the student, with the exception of the non-refundable application fee.

3. When notice is received upon or after completion of one-fourth of the educational services, but prior to the completion of one-half of the educational services, all tuition and other charges except fifty percent thereof must be refunded to the student, with the exception of the non-refundable application fee.

4. When notice is received upon or after the completion of fifty percent of the educational services, no tuition or other charges may be refunded to the student.

5. If the school is permanently closed and no longer offering instruction, after a student has enrolled, the student shall be entitled to a refund of tuition.

6. If a course is cancelled subsequent to a student’s enrollment, the school shall leave in, provide a full refund of all monies paid, or arrange for the completion of the course.

7. In a case of a student suffering a personal illness or disabling accident, a death in the immediate family, or other circumstances beyond the control of the student, the school will determine and present a settlement offer that is reasonable and fair to both parties.

8. A student on a leave of absence must notify the school in writing that he/she is not returning. The date of withdrawal shall be the earlier of the scheduled date of return from the leave of absence or the date the student notifies the institution that the student will not be returning.

This refund policy will also apply in the case a student is administratively withdrawn (expelled) by Josef’s School of Hair Design, Inc., for activity such as failure to abide by school regulations, alcohol or drug abuse in the school or failure to comply with the Satisfactory Academic Progress Policy.

Collections PolicyWithin 30 days of receiving the school’s discontinuation notice with an amount owed to the school, the student must contact the Financial Aid Director to make payment arrangements. If a student fails to do so, the account will be turned over for collection.

The annual report describes our policy regarding crime and accident prevention, public safety, criminal and accident reporting, and emergency evacuation procedures.

Drug & Alcohol Free Standards Policy & Drug & Alcohol Abuse Prevention ProgramIn keeping with the local state and federal laws, Josef’s School of Hair Design, Inc.’s students and employees shall not be permitted to possess, use or sell alcohol, illegal or controlled substances while on school premises. The policy and program information is distributed during orientation.

Copyright Infringement PolicyStudents who illegally download or distribute copyrighted materials without permission will face civil and criminal penalties in addition to school termination. For more information, go to www.copyright.gov.

Social Media PolicySocial media plays a large part in today’s communications, both personal and professional. Josef’s “Social Media Policy”, distributed during orientation, defines acceptable use for Josef’s students and employees.

Complaints, Investigations & Disciplinary Action Josef’s is committed to maintaining a healthy working and learning environment. All complaints and concerns are taken seriously to resolve the situation professionally and appropriately. Josef’s has an internal complaint policy, which is distributed to enrolled students during orientation and can be requested from the school director. External grievances may be directed to NACCAS; for massage therapy, contact the ND Department of Career & Technical Education (all contact information on page 1) however, students are required to exhaust all attempts for resolution with Josef’s prior to filing a complaint.

Attendance Guidelines • Each student is to be ready for class at that time. Students are responsible for their own

books, pens, pencils, etc. These items are to be brought to class each day. If not prepared, they will be asked to leave the class.

• Students not in school by 9:05 a.m. will not be allowed to enter Theory class. This is STATE LAW.

• If a student is going to be late or absent, it is the students’ responsibility to call the school prior to the start of the school day. Students must speak with an instructor if they are unable to attend at by the previous time given.

• Students who are continually tardy or truant could be faced with termination.

• A written doctor’s excuse is required for absences due to illness. • It is the student’s responsibility to log in and out at any time. • Students should not leave the school except at meal breaks. • If a student is absent the day of an exam, he/she must make it up within one week upon

return. After this, a grade of zero will be automatically given.• Special and emergency time off must be approved by the school director.• No food or beverages are allowed in the classroom or on the clinic floor. Talking and

sleeping during class will not be tolerated. This behavior may result in the student’s removal from the class.

WithdrawalTo officially withdraw, the student can initiate the withdrawal process either verbally, or in writing with the School Director. The tuition refund policy will apply to withdrawn students. The school will terminate (unofficially withdraw) a student upon ten consecutive school days of unapproved absenteeism.

Student Lounge and Scheduled Breaks• The student lounge is for students only. Students are to be in the lounge only during

designated lunches and scheduled breaks.• Lunch is prescheduled for each student. The student will take his/her 30 minute lunch at

the scheduled time only. Breaks are 15 minutes long in the morning immediately after theory class and in the afternoon between 2 and 2:30 p.m. Breaks taken outside of the time must be cleared with an instructor.

• It is the responsibility of all students to keep the lounge area clean.

Shop Management Duties• All students must participate in daily cleanup of the school at the end of the day.• It is each student’s responsibility to keep their own work station and surrounding area

clean and sanitized.• Stations will be inspected daily to ensure proper sanitation.

Clinic Floor Guidelines• No gum, food or beverages are allowed on the clinic floor at any time.• Every service that is performed on a client must be checked by an instructor prior to the

client leaving.• It is the student’s responsibility to walk the client to the front desk after the service is

completed. • If you need assistance with a client, you must see an instructor. Students-teaching-students

is not allowed by state law.• Lock your station to protect valuables and equipment. Your station and equipment must

be clean and sanitized at all times.

Front Desk and Appointment Guidelines• Appointments can be moved only with an Instructor’s approval. • The office phone is for business. No personal phone calls are allowed. Messages will be

taken.• Students will remain with their client until the service is completed and the client has

paid. • Never leave the front desk unattended. • Students cannot refuse a client. Refusal to take a client will result in the student being

asked to leave school. • Student working the front desk will need approval and training

Other School Policies• A full kit of equipment must be maintained at all times. Students are responsible for any

lost or broken equipment.• Josef’s is not responsible for lost, stolen or damaged items. • Credit for work will be granted only when work is performed and completed by the

student.• Any student found guilty of theft or cheating will be terminated.• It is the student’s responsibility to report their current address and phone number to the

school director. • Tuition is paid on the first Tuesday of each month.• Curriculum cards and daily ledgers must be kept up to date.• Credit for time will be rounded to the nearest quarter hour.• Any display of unprofessional conduct will result in termination (examples – foul language,

drugs, alcohol, theft, poor attitude, disrespect, etc.).

Josef's Annual Fashion Awards, held on January 31, 2014 was a huge success and proudly claims its name as the Biggest and Best Runway Show in the Midwest! Over 250 models walked the runway and Josef’s once again set new standards in the industry awarding four trips to the TIGI Academy in New York City to top placing students and alumni.
