• CDC will generate ECE performance measure data annually

Use this to populate Annual Progress Report (APR)• 2015: CDC will populate ECE measures for grantees opting in to the
LMCC/CDC data

(If applicable) Use this to target offers of assistance to ECEs that have not yet taken the quiz, without indicating that you have access to LMCC data.

2. If applicable, populate and return list of target ECEs (including funding level) by 2/24
• Helps CDC distinguish between Basic plus- and Enhanced-funded ECEs for APR reporting purposes

A director of multiple facilities should provide a unique email for each facility
