Selecting a Medical Plan:
The Process
CCPS 2017 Retiree Benefits
Prescription Benefits Prescription benefits will be included in your chosen Medical plan. Some plans have a separate prescription deductible. Co-pays for medications will vary.
Subsidy and Health Reimbursement Accounts (HRA) IRS Guidelines state members cannot have an HRA and receive the
government subsidy for lower premiums. If an individual chooses to use government subsidy, Board
contributions will go towards cost of member’s dental and vision benefit.
Proof of Application for Coverage Must be submitted no later than 4:00 p.m. on December 2, 2016 to the
Benefits Office to be enrolled in an HRA. We do not need a copy of your invoice or completed HRA forms at this
time. Documentation must show that you will receive the government
subsidy, if applicable.

CCPS 2017 Retiree Benefits
Making Changes Mid-Year Changes to medical can only be made if you experience a qualifying life event. Documentation will need to be obtained from the current insurance carrier and
submitted to the new insurance carrier for the change to take effect.
Becoming Medicare El igible Members who become Medicare eligible will be able to enroll in the Aetna
Medicare Advantage plan on their Medicare effective date. You will need to notify the Public Exchange that you will be canceling your
current policy due to a life event. Contributions to your HRA will stop at the end of the month prior to your
Medicare effective date.
Spouse Enrol lments As long as your spouse was on the plan prior to your enrollment on the
Exchange, they will be eligible to come back when Medicare eligible.
Spouse is Medicare El igible but I ’m Not You will each have your own individual plan. Deductions for the Medicare supplemental plan will come from your monthly
pension check, in addition to dental and vision deductions.

Key Information
Plan Year: January 1, 2017 – December 31, 2017
Eligibility: Retirees who had at least 14 years of service with CCPS at the time of retirement.
If you qualify for and choose to use the Government subsidy, you will not be eligible to receive your Board HRA contribution.
Deposits into Your HRA: Deposits will occur on a monthly basis into each eligible member’s account.
The amount deposited will be based on your years of service with CCPS when you retired.

Follow These Easy Steps
1. Gather your first month’s medical premium invoice provided by your chosen insurance carrier.
2. Complete the appropriate Claim Form.
3. Submit your form and documentation to the Benefits Office, Attn: Liz Miller or Deidre Hairston. Completed forms and invoices can be dropped off in person, mailed, faxed, or emailed.
4. Ongoing reimbursements will occur automatically, and reimbursements are processed on the first Monday of each month.

Remember to file your claims with a completed claim form and to respond to all requests for card substantiation. Please allow 2-3 business days for the processing of your submitted claims once they have been received.

How does it work? Members first provide Glasses.com with their basic
information to access their benefits. As they view great

No need to complete an Enrollment Form unless you wish to: Cancel your Dental and/or Vision coverage, and/or
