Through this six-hour testing program, students will be reevaluated in clinical settings with hands-on skills. May be repeated once. Graded as pass/fail. All students successfully completing the course must take and pass the “hands on” skills and written, state approved competency evaluation. With successful passing, the students name is placed on the Nurse Aide State Registry. Illinois statute requires all new nurse aides to have a criminal background check before their names can be added to the Nurse Aide Registry. This check is completed prior to course enrollment. REQUIREMENTS Prospective students wanting to enroll in NUR 108 (0.5 credit hours) must meet the following criteria: • Students must have undergone a 24 consecutive month lapse in CNA employment. Must be on the Illinois Department of Public Health’s registry. • Must submit validation of immunizations: Two separate mantoux skin tests are to be performed and read. The second skin test must be given 7 days (but not more than 14 days) after initial test. The result of each test is to be read and documented. Documentation must accompany this form if yearly updates have been given since the 2-step was received. Provide proof of immunity to measles, mumps, and rubella by immunization or laboratory screening. The MMR vaccinations must be at least 1 month apart. A DPH fingerprint background check issued from the Illinois State Police (must be less than one year old). • Approval from the BNA Coordinator. *SEE “PROCEDURE” BELOW TO BEGIN RECERTIFICATION PROCESS. Tuition and Fees The approximate total of tuition and fees for the Certified Nursing Assistant Recertification Program is:$271.50* (In-district); $367.50* (Out-of-district)Out-of-state participants, please contact the Business Office for tuition and fee costs. *These costs are subject to change. Testing Sites Manual performance skills will be scheduled on an individual basis and conducted by an IDPH approved CNA Instructor/Evaluator at: DeKalb County Rehabilitation and Nursing Center 2600 N. Annie Glidden Road, DeKalb, IL 60115The Illinois Nurse Assistant/Aide Training Competency Evaluation Exam will be administered through Southern Illinois University-Carbondale either at Kishwaukee College (held monthly) or at other participating college’s. Testing will be scheduled after successful completion of the Program.Performance Skills Validation Students will have three (3) attempts to pass each performance

skill. After validation of performance skills, students are eligible to retake the written Competency Evaluation Examination.1. Wash hands2. Perform oral hygiene3. Shave a resident4. CERTIFICATE OF COMPLETION REQUIREMENTS1. Complete specific course and program graduation requirements for the Certificate of Completion as outlined in the Career/Occupational

programs section of the catalog. Each curriculum identifies the specific course requirements needed to complete a Certificate of Completion.2. Meet the College’s academic residency requirement.3. Achieve an overall 2.000 GPA in all Kishwaukee College required and elective course work applicable to the Certificate program

requirements.4. Resolve any incomplete grades in Kishwaukee College course work applicable to the certificate.5. Apply for graduation in the Student Services office.
