February 2016 Changes to RBC Urinalysis Reporting Effective February 16 th , 2016, and to comply with the 2012 American Urological Guidelines for diagnosis, evaluation and follow-up of asymptomatic microhematuria (AMH). Red blood cells reported when a microscopic examination of urinary sediment is performed will follow the format below: None Seen, 0-2, 3-4, 5-6, 7-10, 11-25, 26-50, 51-100, 101-200, Packed AMH is defined as three or greater red blood cells per high powered field on a properly collected urinary specimen in the absence of an obvious benign cause. Urinalysis at Providence Regional Laboratories is performed using either a dipstick with a dipstick reader or an automated instrument similar to the one used for cell blood counts. When the dipstick is used, the presence of RBC’s will trigger a manual review by a medical technologist. A positive dipstick does not define AMH, and evaluation should be based on findings from microscopic examination not on a dipstick reading. If the automated instrument is used, only abnormal flags will trigger a manual microscopic review. Epic Orderable: LAB2479 (Urinalysis Reflex Microscopic) Patient Preparation First morning clean catch specimen preferred Collect Clean Catch Urine in a sterile leak-proof urine cup (container) without preservative Specimen Preparation Transport entire collection or a 10.0 mL aliquot from a well-mixed urine collection (Min: 4.0 mL) Storage/Transport Temperature Refrigerated (2-8°C) Stability (from collection to initiation) Ambient: 2 hours, Refrigerated: 24 hours, Frozen: unacceptable Unacceptable Conditions Frozen specimens; timed collections; Urine Contaminated by feces or unknown contents which invalidate examination; Other liquids submitted for Urinalysis, which are not urine. Remarks A Microscopic will be performed if any of following conditions are met: Appearance - Slightly turbid or turbid, Nitrite - positive, Leukocyte esterase- trace or greater, Protein - 1+ or greater, Blood - trace or greater.
