CHAPTER - 1

INTRODUCTION

Central Government Health Scheme (CGHS) under the

Union Ministry of Health & family welfare was started in 1954.

CGHS provides comprehensive medical facilities to the Central

Government employees and pensioners and their dependents

residing in CGHS covered cities. Apart from this sitting and

retired Members of Parliament, Ex Governors and

Lt.Governors, Freedom fighters, Ex Prime Ministers, Vice

Presidents, Sitting and Ex-Judges of the Supreme court and

High courts, Employees and pensioners of certain

autonomous organizations in Delhi, Delhi Police Personnel in

Delhi only, Railway Board employees etc are at present getting

medical facilities under CGHS.

CHAPTER – 2

PREAMBLE

CGHS scheme at present is functioning in the following 25 cities:

At present, CGHS provides medical facilities

under Allopathy, Ayurveda, Unani, Yoga, Siddha and

Homoeopathy apart from basic diagnostic facilities and

specialized consultation services through the laboratories

and polyclinics. The details of the various units functioning

as on date are follows:

The total number of cards and beneficiaries

availing medical facilities under CGHS across the country

as on 31st March 2014 are as follows:

Emergency service at some wellness center.

Domiciliary Visits.

Preventive Health Check up at earmarked Wellness

Centers.

The other components of the scheme include

Purchase,

Storage, distribution and supply of medicines, Health

Education.

Family welfare and vaccination for general public also.

The role and responsibilities of medical and

paramedical staff working in CGHS have changed drastically

with the introduction of computers and various other systems

in the day to day handling of the Wellness centers and other

units of CGHS.

Group C and erstwhile Group D employees as above under

various categories are working in this organization. Due to large

number of vacant posts, there is extra burden of work on the

existing staff.

All the Group ‘C’ employees and erstwhile Group ‘D’

employees are solely represented by All India CGHS

Employees Association (AICGHSEA).

AICGHSEA is the only Association recognized by the

government of India to represent Group C and erstwhile Group

D employees all over India. Being the only recognized

association of the staff we take this opportunity in submitting

this memorandum and will also render oral evidence before the

VIIth Central Pay Commission.

AICGHSEA request the Hon’ble Chairman and

members of the Seventh Pay Commission to consider our

submission sympathetically and invite the delegation of the

Association for oral evidence. We will readily furnish any other

information as may be called for by the Commission.

CHAPTER – 3

1. UNIFORM TO ALL CGHS EMPLOYEES:

The CGHS establishment is a peculiar type of

organization comprising of medical, paramedical,

scientific and all other employees who have to remain

always alert during the time of their duty and they have

to handle different type of patients and their relatives.

Wearing of uniform easily identifies the staff and avoids

the spread of infection.

Presently the uniform are purchased from the

Government owned companies such as Lidkar, NTC, ETC

etc, their rates are not realistic and these companies

have also stopping the supply of these items. The rates

of uniform are not revised from many years.

So this association suggests in lieu of uniform,

uniform allowance should be given annually to all

the staff in lump sum at par with the Staff Nurses.

2. PATIENT CARE ALLOWANCE

The Ministry of Health and Family Welfare

constituted a committee of the Medical Superintendents

of various Hospitals, which recommended that Risk

Allowance and Night Weightage Allowance being paid to

the Health staff at that time may be merged. Not only

this, the Committee had further recommended that

health staff need additional support to take care of

preventive and nutritional needs as they are exposed to a

highly infectious environment loaded with highly virulent

and drug resistant bacteria.

The recommendations of the committee were

accepted by the Ministry of Health & Family Welfare and

forwarded with their approval. After approval of the

cabinet, HPCA/PCA was sanctioned wef 1.4.1987.

Presently HPCA/PCA is being paid as follows:

This associations suggests HPCA/PCA to be paid at

par with Nursing Allowance.

3. WASHING ALLOWANCE

At present the CGHS employees are getting meager

amount of Rs.180/- as washing allowance which is very

insufficient. In view of the prevailing high prices of

detergents, soaps and other washing materials including

ironing of uniforms, the government has not providing

hospital services to the patients its employees have to keep

their uniforms properly neat and clean to on avoid the danger

of gross infection and as such they have to spend heavily on

washing of their uniforms. In view of above this association

suggests that washing allowance should be paid at par

with the washing allowance paid to Staff Nurses.

4. NURSING/WASHING/UNIFORM ALLOWANCE TO NURSING

STAFF.

Nursing allowance, washing allowance and uniform

allowance are being paid to the Nursing staff because Nursing

is an independent profession and has its own identity.

In view of this it is demanded that the

provision of Nursing allowance/ washing

allowance/uniform allowance to all the Nursing staff

irrespective of their status and nature of job, should be

enhanced to double from the present rate.

1. Dental Hygienist work as a Clinician.

3. The pharmacist will write the name of the medicines with details of the does on the mixtures/syrup bottle and container envelope and also explain the doses verbally.

4. He will ensure that no defective medicine is issued to any of the patient/employee in respect of following: -

i) Physical appearance ii) Batch No., date of manufacturing / expiry iii) Proper stamping of CGHS not for Sale etc.

5. The Pharmacist will maintain proper account of items issued to him, dispensed by him and the balance in stock with him. At the end of each day he will stock out the balance with him and record

it. 6. The Pharmacist will perform such other duties as may be assigned

to him by the MO/LMO of dispensary/Sr. LMO, CGHS/Administration from time to time. 7. The Pharmacists are also responsible for cold chain and drug

storage as per drug and cosmetic Act 1940

3. Pharmacist has been removed from the list of paramedical there

should be a separate independent All India and state level council for

Pharmacist i.e Pharmacy council of India.

Note: In all pay Commission Pharmacist (Ayush) were placed at par

with Pharmacist (Allopathy). Therefore there should not be any

disparity of pay scale of all the common categories of Pharmacists.

JUSTIFICATION

1. The drivers working on field duty drive upto 200 kms a day. Two

important factors for drivers, one is skill requirement and the other is

level of responsibility. If a small error is there then it will lead to a major

accident. Hence the driver job is very tough.

2. Drivers in our organization are granted grade pay of 1900 at entry

level, where in the Secretariat it is 2400 grade pay, so that our

associations demands pay scale at par with Secretariat.

JUSTIFICATION

1. Presently the qualification of Staff Nurse is BSC(Nursing)

with experience, therefore it is difficult to fill up the post. In such circumstances pay scale of the post should be upgraded

and needs more promotion avenues.

Note-In CGHS organization, the duties and the

educational qualification for the post of NMW/Staff Nurse

are similar, but after the 6th Pay commission, the pay of

staff nurse was upgraded to Gr.Pay 4600, however the Gr

Pay of NMW was kept 4200 only. The disparity should be

removed. The pay of NMW to be kept at par with Staff

Nurse.

1. Radiographer, MRT, ECG Technician and OT Technician are isolated posts in CGHS. Their duties are risky, hazardous and highly responsible. They should be granted at least pay at par with the posts in central

government hospitals.

2. In CGHS due to lack of promotion and non sanction of

promotional posts, LDCs are performing duties of UDC

Accountant, Office Supdt etc such as Maintenance of cash books,

Maintenance of ACRs, GPF ledgers, Hospital bills, preparation of

Budget, etc.

JUSTIFICATION

1. The education qualification of LDC has been raised from 10th

Std passed to 10+2. Moreover LDC earlier were using Typewriter,

however today LDCs are using computers and skill requirement is

more for the uses of computers. Due to the shortage of staff,

many LDCs are actually performing duties of UDCs in CGHS.

The Data entry operators are having grade pay of Rs. 2400, hence

LDCs are eligible for better pay scales higher than the Data Entry

operators

2. Wherever the posts of Office Supdt/Accountant are not

available or not filled, their duties are being performed by the

UDC/LDC.

3. In CGHS in most of the cities Administrative officer/Accounts

Officers posts are not filled or not sanctioned, hence their duties

are being performed either by Office Supdt or Accountant, in

some cities these duties are performed by UDCs/LDCs

& Hindi Translator should be placed at par with

those in the Secretariat.

JUSTIFICATION

1. Dressers work independently in CGHS dispensaries with a separate

room. All the materials like lotions, ointments, cotton, linen bandages

and gauze required in the dressing are stored properly and not exposed to dust.

2. In discharging their duties they carry out lot of responsibility. In Railways Dresser is given a different scale. Based on the qualification and the scale available with other departments, Dressers in CGHS

should be given the entry grade pay of Rs.2400/

3. Dresser post will be the only promotional channel for the MTS (Technical Posts)

PANCH KARMA ASSISTANT: Panch karma Assistant is an

isolated posts in CGHS. Their existing pay should be

promotional post to the post of Laboratory Technician. The

qualification of Lab. Asstt is 10+2 with DMLT in CGHS. Their

duties are risky, hazardous and highly responsible. Their

existing pay should be upgraded to Rs.2400/- Grade Pay

CHAPTER – 5

PROMOTIONS.

Like other departments CGHS has no promotional

channel for the staff, LDC, Staff Nurse, Pharmacists, Lab.

Technician, Dressers, etc join in these posts and after

rendering more than 30 years of service, retire on the same

posts. It is very unfortunate and disgraceful; the employees are

deprived off the right for promotion due to the government’s

old and saturated policies. Now it is high time all these policies

should be made more attractive. The problem of promotion

should be taken up seriously in the 7th Pay Commission.

Whenever DA is announced, it is uniformly applicable to all the

employees, then why promotion to Central government

employees should not be uniform?

In CGHS itself there is a disparity between the staff

and the doctors.

The Officers and staff of CGHS are classified into four

categories as follows:

Group A - This category consists of all the Medical Officers. Group B - The Administrative Officers, Accounts Officers,

Asst Accounts Officers & Asst Depot Managers

Group C - Pharmacists, Staff Nurse, Laboratory staff and

Other paramedical staff.

At present only the Group A category ie Medical

Officers are getting time bound promotions as follows:

After 16 years : Sr Chief Medical Officer (S.A.G)

The Group B posts such as, 3 posts of Asstt Depot

Managers are sanctioned in Delhi, Mumbai and Hyderabad. 1

post of A.N.S is sanctioned in Delhi which is to be filled by

eligible Group C staffs. At present 99% of the Group B posts

are lying vacant.

Like the time bound promotions of Doctors,

AICGHSEA appeal that all the Group C and erstwhile

Group D staff should get 5 time bound promotions.

CHAPTER – 6

SOME JUSTIFIED DEMANDS OF CGHS

EMPLOYEES.

1.NON PRACTICING ALLOWANCE

Doctors in CGHS are getting NPA at the rate of 25% of Basic +

Grade pay. Considering the nature of medical duties of Pharmacists,

Staff Nurse, Laboratory Technicians, Dental Technicians, Dental

Hygienists, Operation Theatre Technicians etc in CGHS. These

categories may be permitted to practice privately or else they may

allowed NPA like the doctors.

2.ACADEMIC ALLOWANCE

As the technical posts, to provide best possible health care

services and to keep the latest knowledge of the subject up-to-date,

government should encourage these categories with academic allowance

for purchase of new books, acquire training of the latest order so that

best services be provided to the public health. This association suggests

that 10% of Basic pay + grade pay to be paid to all the technical posts.

3.INCENTIVE FOR ADDITIONAL HIGHER QUALIFICATION All the categories technical and non technical with higher

qualification then their basic qualification should be give an incentive

for additional higher qualification. Officials with additional qualification

will perform better and the output will be far more better.

4.CGHS CONTRIBUTION TO BE WAIVED.

Railways annually provide 6 privilege passes to each of its officers and

four passes to its workers, allowing them to travel anywhere in India.

Like Railways many other departments give benefits, privileges to his

employees.

On the same pattern we demand that CGHS contribution from

CGHS employees should be waived.

SUGGESTIONS FOR BETTERMENT OF

CGHS SCHEME.

EXPANSION OF CGHS IN OTHER CITIES:

At present CGHS is functioning only in major cities.

Beneficiaries especially the pensioners of other cities who have taken

CGHS cards are facing lot of inconvenience to avail the facilities. They

have to travel from one place to another in their old age to get the

medicines. Due to non availability of CGHS in their cities, most of the

pensioners are deprived off the medical facilities. IT IS STRONGLY

SUGGESTTED THAT CGHS WELLNESS CENTRES SHOULD BE

OPENED IN EVERY CITY OF THE COUNTRY.

CGHS OWN SUPER SPECIALITY HOSPITAL

CGHS is functioning in 25 cities, but none of CGHS cities has

its own hospital. Government is incurring crores of rupees on the

treatment of its beneficiaries for Indoor treatment at Private hospitals.

The expenditure can be curtailed to a great extend if CGHS own hospital

is opened in all CGHS cities. Secondly the current procedure for getting

an approval letter from the government hospital even after the

recognized hospital under CGHS has recommended it before the

procedure is also a major hurdle. In most of the cities government

hospitals are very few, patients find it extremely difficult to get the letter

from government hospitals. Further The Parliamentary Standing

Committee have also strongly recommended for CGHS own hospital.

IN VIEW OF ABOVE IT IS STRONGLY RECOMMENDED THAT

A SUPER SPECIALTY HOSPITAL ON THE LINE OF RAILWAYS,

DEFENCE ETC SHOULD BE OPENED IN ALL THE CITIES SO THAT

THE BENEFICIARIES CAN AVAIL THE TREATMENT WITHOUT ANY

PROBLEMS.

CGHS OWN DIAGNOSTIC CENTRES:

In most of the CGHS cities there are no diagnostic centres of

CGHS. Patients are referred to private hospitals. The diagnostic centre,

Laboratories and other attached facilities should be made available

under one umbrella so that patients need not have to run from pillar to

post to avail of the treatment.

IN SERVICE TRAINING PROGRAMME:

The knowledge and skills of paramedical staff need to be

improved along with new researches being carried out in the medical

field. For this purpose periodical in-service training programmes from

time to time is very essential for the paramedical staff and Doctors in

CGHS. This would enhance the quality of the service being rendered to

the CGHS patients.

MEDICAL SERVICE NOT TO BE OUTSOURCED:

Medical service should not be out sourced. With outsourcing

on one side the government may save some amount but on the hand it

would only result in more corruption. Therefore it is strongly suggested

that Regular Doctors and regular staff members should be appointed for

better services to the patients and to avoid corruption at all level.

VACANT POSTS TO BE FILLED IMMEDIATELY:

Almost 30% sanctioned posts in CGHS is lying vacant. Due to

this the functioning of the dispensary is very badly affected. Vacancy of

posts is putting lot of pressure on the existing staff. One can witness

long queues at the dispensing and other counters in all the dispensaries.

We suggest that all sanctioned posts promotional or direct should be

filled up immediately for the smooth functioning of the wellness centres

of CGHS throughout the country.

PROCUREMENT OF MEDICINES IN CGHS.

In CGHS medicines are procured through Government Medical

Store, Bulk purchase through Pilot project, Local Chemists etc.

Although in the existing system medicines are available at the counter

but the government is paying more. CGHS can purchase these

medicines in bulk from reputed Pharmaceutical Companies after quality

check in the generic form and then repack it according to the

organization set up. This would reduce the purchase of medicines

through local purchase and would also control the cost of medicines.

CONCLUSION.

In conclusion we fervently believe and also are

confident that the Seventh Pay Commission will analyze

and evaluate the duties, skill and responsibilities of the

entire paramedical and clerical cadre in CGHS and

prescribe suitable pay scales beside allowances and

concessions.
