We believe in our staff’s ability to coach/mentor any child to become an all-star cheerleader! Therefore, most children will have the opportunity to be part of one of our teams! Below is information with how our tryouts will work this season:

Ages 6 – 18 are eligible to be assessed Assessments will take place on Tuesday, April 17th & Thursday, April 19th from 6:00 p.m. – 8:00 p.m. Participant will meet one-on-one with coach to be assessed based upon the following skills: Jump Technique;

Standing Tumbling; Running Tumbling; and Overall Impression. We evaluate each child’s current skills to determine the team placement.

Participant should wear comfortable t-shirt or sports bra, shorts or sweats, tennis shoes and hair pulled away from face. They are more than welcome to wear a hair bow and any colors of the gym.

Pre-registered individual assessment is $35.00; non-registered individual assessment is $45.00. These fees must be paid at time of registration.

In order to register/pay, please follow the account registration process below and select “2018 – 2019 Tryouts” to enroll. If you already have an account, please enroll your child(ren) in the 2018 – 2019 Tryouts event.

For the success of each individual and team(s), it is important to properly place each cheerleader on the appropriate skill leveled team. In order for us to have the highest level of technical skills on each team, certain elements are to be performed by all members of each team. Some of the elements that come into play when forming a team are a member’s maturity, abilities at stunting, tumbling, pyramids, tosses, jumps, dance, motions and also age. Having a balance and variety of each of these factors are important for a team to be successful. A skill will only be counted for a member if they are able to demonstrate the proper technique while performing that skill. Although tumbling is important, it is not the only thing considered when forming a team. Strong skills in all areas of cheerleading are necessary in order to do well and all areas will be taken into account when placing members on teams. Our teams are designed to give each team an equal chance for success within their division. Having a balance and variety of each of these factors are important for a team to be successful. Please take note that no member has an absolute guarantee of a roster spot on a particular team. Due to this factor the coaching staff reserves the right, at any time and without advanced notice, to change the rosters of the teams by adding, rearranging, or removing members to or from teams for a performance. Additionally, no member has a right or guarantee to any particular role or location in a routine. Finally, members may be removed from a certain team or from our program at any time for reasons which may include but are not limited to: attitude problems, absences, tardiness, skill loss, lack of improvement, financial issues, fitness level of an member, and personality conflicts between members, coaches, parents, or anyone involved in our program. There will be no refunds of tuition given after removal from a team or voluntary leave. Crossovers Crossing over to another team is when a member competes and practices with a second team in addition to their main team. The only additional cost to cross over to another team is the competition fees. Those who would like to crossover must indicate this on their registration form. Mandatory Classes All Prep and Elite team members must attend one tumbling class per week. All members who have been chosen as flyers must attend one flyer class per week. A discount will be applied to the first tumbling class registered due to team membership. No other classes will be discounted. All-Star Cheer Levels

Time Commitment Punctual attendance at all team practices and tumbling, competitions and exhibitions, choreography & awards ceremonies is mandatory for all Cheer Legend Champions LLC All-Star team members. Please take into consideration competitions, performances, team practices and tumbling when scheduling family-related activities. The implementation and enforcement of all attendance-related issues is at the discretion of the Gym Owners. In order to help members and families keep track of team practices and classes, competitions, choreography, important events and gym holidays, a calendar is maintained at www.clchampions.com/monthlycalendar . Practices Practices will begin for the All-Star Elite teams on Tuesday, May 1, 2018. A schedule will be provided once tentative teams have been announced. Practices will begin for the All-Star Prep teams at the beginning of September 2018. Practices will begin for the All-Star Recreation teams at the beginning of each session. Team Practices Team practices will be held weekly for each team. Additional mandatory practices may be added as needed, especially in the weeks prior to competition. Practices will run according to two schedules for the Elite teams throughout the year:

1) Summer Attendance: May 1st – August 26th (4 excused practices available for vacation) 2) Regular Attendance: August 27th – April 30th (standard attendance policy applied)

Attendance Policy Full commitment by the cheerleader and family is important and is part of preparing our teams for success; therefore, all practices required by Coaches must be attended. Excused absences only: contagious illness with a doctor’s note, and school event that earns a grade and school cheerleading (documentation is required). If cheerleader is unable to practice due to other illness, they must come to practice and watch in order to be prepared for the next practice. Excused and/or unexcused absences that occur one week prior to any performance and/or competition may result in a member being removed from performing in the routine. The following holidays will be observed throughout the 2018 – 2019 program unless a competition is leading up to or on any of these dates: May 13th – Mother’s Day

May 28th – Memorial Day June 4th thru June 8th – Summer Break June 17th – Father’s Day July 4th – 4th of July Holiday Break September 1st thru September 3rd – Labor Day Weekend November 21st thru November 25th – Thanksgiving December 23rd thru December 26th – Christmas/Hanukah December 31st thru January 1st – New Year’s March 10th thru March 16th – Spring Break

Absences must be reported to the gym owner(s) via text or email prior to the beginning of practice.

Summer Attendance - Summer attendance is VERY important to the development of the team. Please note that we expect team members to be at all summer practices. Practices are still a high priority during the summer, low attendance will effect team placement. ONLY TWO WEEKS VACATION PERIOD IS ALLOWED PER CHEERLEADER (no more than four practices) during the months of May – August and not counted against cheerleader. Monthly tuition remains the same no matter the vacation time taken.

All practice sessions are mandatory. Cheerleaders must be on time to practice. If they will be late, please contact via text to gym owner(s) prior to the scheduled practice time. If consistently late three times in a row for any practices/competitions, cheerleader will be moved to alternate status. All events are mandatory.

If cheerleader is ill on the day of competition, they will be expected to show up to the competition and do their best to compete.

School sport schedules that conflict with practices must be addressed immediately upon receiving their school sport schedule.

IMPORTANT: During each competition season, your child is allowed to miss up to 6 unexcused practices. If you miss more than 6 unexcused practices, you will be removed from the squad for the entire season. No refunds for any costs/payments received will be refunded if child is removed from squad or for any vacation time taken. ATTENDANCE IS CRUCIAL TO A TEAM’S SUCCESS. MEMBERS NOT ATTENDING PRACTICE CAUSE A HUGE INCONVENIENCE AND UNFAIRNESS TO THE OTHERS ON THE TEAM. Choreography Camp Choreography Camp is mandatory for all All-Star ELITE team members. During choreography camp, the team works together to learn their routine for the season. If all members are not in attendance, this will not be as productive. Choreography Camp will tentatively be held in July. Competitions Competitions are typically held during weekends and holidays. Therefore, school absences are limited. Each team (Tiny/Mini/Elite) usually attends approx. six (6) local or national competitions (dates will be given out as soon as they are available). Competition season is normally from November – April. All members will be expected to be present and show support for all Cheer Legend Champions LLC teams. IMPORTANT: During competition readiness, we will work on competition material only and practice dates/times will be adjusted as needed. Practice times may increase in preparation for competition. Competition Bids Several competitions that we attend can provide bids, wild card bids, at-large bid and/or paid bids. Cheer Legend Champions LLC will require parents to vote on attending these competitions. Any competition fees and all travel expenses would be the responsibility of each participant/family. Once your child is a member of Cheer Legend Champions LLC, you are responsible for any and all fees associated with being on one of our teams. It is assumed that you have made the appropriate arrangements for your child to participate. Loss of job, injury, or lack of desire to continue, does not constitute any refund or waive any upcoming payments for your child. We do not want to be put into the position of being “bad guys” when payments are not received per this agreement and paid on time. Payment is expected to be on time in order for your child to participate in any activities. Please understand that failure to any fees that are owed to Cheer Legend Champions LLC, will result in your child being unable to participate in practice. Multiple Member Discount For families with multiple children in the program, the oldest child will be charged for full tuition, the 2nd child (in order of level, highest to lowest) will receive a 5% discount, and the 3rd + children will receive a 10% discount. These discounts will be on team tuition only. Deposit and Monthly Payments The first payment is due on May 1, 2018 upon executing the Cheer Legend Champions, LLC All-Star Competitive Team Member packet. All remaining monthly payments are due on the 1st of every month. Roll will be maintained and if the monthly fee is not paid, the cheerleader will not be able to participate. If the monthly fee is paid after the 1st of the month, a $25.00 late fee will be applied. No refunds will be given at any time. Monthly/annual commitment DOES NOT include the cost of any other expenses outside of the items noted above in each program. If a member quits, no refunds will be given for any tuition or fees paid to program and the member is not eligible to receive any merchandise ordered and/or received after they quit. Tuition will not be pro-rated for gym closings, holidays and weeks without practice(s). Any and all payments, fees, tuition, etc. received are NON-REFUNDABLE. Appearance The team members will always be dressed in appropriate attire for all practices and performance events. Unless specifically told otherwise for a special event, team members may not wear parts of the uniform with any other clothing items. Tops and skirts must be buttoned and zipped in public. Behavior Everyone associated with Cheer Legend Champions, LLC (i.e., owners, coaches, members, family members, etc.) is expected to uphold a high level of sportsmanship and character. Positive attitudes, words, and actions are expected at all times. Team members must fully understand their roles as being part of Cheer Legend Champions, LLC. They are highly visible symbols of the Cheer Legend Champions, LLC program; therefore, must conduct themselves appropriately at all times. They should do nothing which will bring embarrassment to Cheer Legend Champions, LLC, to themselves and their families.

Cheerleader Rules NO jewelry is to be worn at practice, events, competitions, or when in practice wear or uniform. NO gum chewing, arguing with team members, giving attitude, being disrespectful to others. Cell phones are to not be heard, viewed/used during practice time. All choreography including cheers, dances, stunts, transitions, tumbling, music selections, editing, etc. should be highly

protected and shall not be shown or discussed with others. NO VIDEOS of routines/choreography should be uploaded to any social media online sites such as YOUTUBE or FACEBOOK, etc.!

Proper attire (shorts, gym shoes, t-shirts) are to be worn to practice. Child must be in practice wear. Consequences for not following any of this policy is 20 pushups. If items are lost, must purchase new attire.

Late arrival: If a member is going to be late, they must text, email or call as soon as possible. If a member is not 100%

ready to go for any practice/event, they are considered late. If late three times in a row, a $25 late fee will be applied. Uniforms are to be used for team approved functions only. When uniforms are worn, they are to be complete. The complete uniform is hair bow, cheer shoes, socks, skirt, brief,

top and hair is to be in a tight ponytail, no nail polish except clear. Make-up if used is to be done in good taste, coach specific. Competitions: Members must be 100% ready to go at the scheduled meet time. This includes hair, makeup, shoes, and

no warm-ups. Transportation to/from practices/competitions/events is the sole responsibility of the cheerleader and parent(s). Notification of a missed practice must be email/text to coaches prior to practice date/time in order for it not to count as

unexcused. No smoking during practices, while in uniform, attending events, or traveling to and from events. No alcoholic beverages will be consumed at any event, competition, or practice. Any violation of this clause will result in

immediate dismissal from the program. (Cheer Legend Champions, LLC reserve the right to involve a student's parents if alcohol is involved in any situation.)

Foul or abusive language by members or parents during competitions or practice will not be tolerated. Sportsmanship and team unity are very important to our organization. Please be courteous and considerate to all teams and members. Do not speak negatively about any person, decision, or result. This includes social media (Facebook, Twitter, Instagram, Snapchat, etc.) and message boards! We strive to teach our members respect and sportsmanship. We have a zero tolerance anti-bullying program. If your member is found or suspected of negative behavior or bullying, we have the right to immediately remove them from the program with no refund of fees.

Please treat the gym with respect and follow the gym’s guidelines. No food or drink (aside from water) is allowed in the overall Fieldhouse facility. Participants are responsible for picking up after themselves. Cheer Legend Champions, LLC is not responsible for lost or stolen items. Parents are responsible for their team member during any "free time" at away events. If a parent cannot attend the event he or she must make arrangements for another team parent to supervise their child at any event. All parents & visitors are not allowed in the gym, but are welcome in the waiting area during practices/classes. NO GOSSIP ALLOWED. Members whose parents are excessively negative, rude, or persist in gossiping about team members, coaches, or the gym will be banned from the viewing area and may be asked to leave the program. We want a positive, pleasant learning environment for our members and a positive teaching environment for our coaches. Safety While we strive to provide the safest possible environment for our members, injuries and/or illnesses may occur. Cheer Legend Champions, LLC is not liable for any expenses rising from injuries or illnesses resulting from participation in our program. Every member must have their own insurance coverage to defray medical or other expenses. Team members may not attempt tumbling, stunts or partner stunts without proper instruction, adequate facilities and equipment, sufficient supervision and spotters. Team members will not use equipment without coach supervision. Team members who arrive early for practice or class or stay late to wait for a ride may not be on any equipment! Representing Our Name The name “Cheer Legend Champions” and “Cheer Legend Champions, LLC” as well as the Cheer Legend Champions logos are the property of Cheer Legend Champions, LLC. Permission is required before anyone may create any product that uses or displays our name or logo. All sales involving Cheer Legend Champions merchandise must go through Cheer Legend Champions. No one may produce ANYTHING with the words “Cheer Legend Champions”, “Cheer Legend Champions LLC”, “CLC” or the logo without expressed, written consent. Injuries If a student is injured at home or during other extra-curricular activities, it is the parent’s responsibility to notify us as soon as possible. This will give us the opportunity to make any necessary changes of the routine prior to practice time. A doctor’s note will be required noting return to activities, time frame involved, any restrictions, etc. in order for the cheerleader to be evaluated and re-considered to perform. Cheerleader is expected to attend all practices/events even when injured. All injured cheerleaders must continue to pay their monthly payments during down time. Member Code of Conduct Cheerleader must at all times be a strong representative of Cheer Legend Champions LLC and a positive reflection of their teammates. Abusive behavior, lying, cussing and or any other form of negative behavior are grounds for removal from this team. Teammates are expected to treat one another with mutual respect. They should treat and talk to one another in a manner in which they want to be spoken to. We do not tolerate pettiness, gossiping or cliques; which attempt to exclude or alienate certain girls. Back talk, rolling of eyes and any other disrespect for instructors or teammates is unacceptable. After a warning, the team member will sit out of practice until the problem is resolved. We will handle any disciplinary problems privately and professionally. CLC has a demerit system which is in force for the entire season. Parental Obligation Rules 1) Any negative behavior towards a staff member(s), coach(es), etc. or to another parent will result in dismissal of your daughter

from the team and no refunds for any costs/payments received will be refunded. 2) No parents are allowed in CLC gym except to speak with coaches (appointment only), drop child off or purchase pro shop

items. Parents are able to watch while inside Fieldhouse facility. If the viewing area for parents becomes a negative environment at any time in the year, the parents of the squad(s) involved will be asked to follow a drop off policy. This includes parents talking negatively about the team, coaches, other parents or other team members. There will not be any tolerance for negativity at our gym.

3) To make sure your daughter is on time and attends all practices and events. Parents are to inform the coach if cheerleader is to be late or absent from practice.

4) Parents are to fulfill any financial obligations on time. If not paid on time, dismissal of daughter from practice(s), event(s) or competition(s) could take place and no refunds for any costs/payments received will be refunded. Whoever signs the release form is financially responsible.

5) Parents are to encourage and support Cheer Legend Champions to be the best they can be. 6) Do not question the coach’s decision about the routine or practices, removal from performances and/or squad. 7) Parents need to maintain manners and civility at all functions. 8) Participant, parents, legal guardians, visitors, family members (“others”) must adhere to all facility rules. Any destruction of

Terms and Conditions

Acknowledgement and Understanding Please complete this form in its entirety no later than 5/1/2018 to confirm a spot for your son or daughter on a 2018 – 2019 Cheer Legend Champions All-Star team. Please confirm your understanding of the following by placing an “x” at each box and sign below:

I have read this entire agreement, acknowledge, agree and assume the indicated responsibilities; and I am aware of the consequences for failing to carry out the indicated responsibilities.

I have read the “Financial Obligations/Agreement” and understand the costs involved in being a Cheer Legend Champion All-Star team member. I understand that all annual/monthly fees are due, even if my child leaves the team prior to the season end.

I understand that this is a commitment to a team from May 2018 through April 2019 and that all monies paid for fees, tuition and expenses are non-refundable and non-transferrable.

I have turned in or will turn in a copy of my child’s birth certificate by May 15, 2018.

received. I also understand a $200 Quitters Fee will be charged to relieve me from my contract for the remainder of the season.

____I understand that all fees are NON-REFUNDABLE. These fees will be drafted in payments as per selected option noted on the acknowledgement above.

____I understand that if I have an outstanding balance after 15 days, my child will be asked to sit out and a replacement will learn my child’s position. I also understand that if I have an outstanding balance after 30 days, my child will be asked to leave the program.

____I understand that my child will not receive any practice wear/uniform items or participate in any competitions unless full payment for these items have been received.

____I understand that no travel expenses for an event will be considered for reimbursement. ____Members are allowed 6 unexcused absences for other commitments outside of contagious illness and/or school event provide

a grade, and 4 excused absences during the months of May – August for vacation. Each absence must be excused at least 1 week ahead of time so that alterations to schedule may be made if necessary.

____I understand that if my member is part of a team that wins a bid, all practices in the month of March and April are mandatory, no exceptions.

____I understand that Competitive Cheerleading is a team sport and that a practice cannot be taken away from the member as a consequence for bad behavior at home or at school. Please find a punishment that does not also punish the coaches/teammates or this action is grounds for dismissal from the team.

____I understand that if a member loses the skills they were assessed for team placement, it is the coaches’ discretion to appoint the member as an alternate until the skills are performed at the level necessary.

____I understand if a member is injured, a Doctor’s note is required for return participation; however, until all required skills can be performed, it is at the coaches’ discretion at return the member to the routine.

____I understand that if my child is injured with a Doctor’s note showing, he/she must attend all practices/events no matter the

circumstances. They must be ready to perform once released from doctor; therefore must be prepared and know all changes to routines, etc.

____I understand that when representing Cheer Legend Champions at any event, all members will be in complete uniform. Members will reflect a positive image at all times when representing Cheer Legend Champions.

____I understand that good sportsmanship will be practiced at all times while representing Cheer Legend Champions, in person and/or social media. Every member/parent is expected to be a good ambassador of our program.

____I understand that we have a zero tolerance anti-bullying program. If your member is found to be bullying, we have the right to immediately remove them from the program with no refund of fees.

____I understand that if any negative behavior towards a staff member(s), coach(es), etc. or to another parent or member will result in dismissal of your daughter from the team and no refunds for any costs/payments received will be refunded.

____I understand that if the viewing area for parents becomes a negative environment, at any time in the year, the parents of the squad(s) involved will be asked to follow a drop off policy. This includes parents talking negatively about the team, coaches, other parents or other team members. There will not be any tolerance for negativity at our gym.

Cheer Legend Champions (“CLC”) retains the right to: 1. Place its cheerleaders on the team(s) it feels will best suit them and the program. 2. Move, replace, add, suspend, or even dismiss a cheerleader for a period of time or indefinitely from a team or the entire

program based on criteria including but not limited to: a. Attendance b. Conduct c. Skills d. Finances e. Parent conflicts

3. Decide if a cheerleader may participate on more than one team. 4. Decide the roles and/or positions a cheerleader will have/play on their team. (E.g. base, flyer, back spot, tumbler, dancer,

alternate, etc.). 5. Request that a cheerleader or team take additional classes or camps to improve their skills. 6. Request that a cheerleader or team practice longer than their regularly scheduled time or add additional practices, camps, or

competitions if deemed necessary. ATTENDANCE/INJURIES All members must: 1. Make CLC priority over any other extracurricular activities. 2. Attend and be prepared to participate in all CLC activities including those unexpectedly added throughout the season

regardless of illness or injury unless otherwise recommended by a doctor through valid written documentation. 3. Notify CLC coaches immediately when an injury occurs so changes to routine choreography can be made prior to practice. 4. Arrive at least 30 minutes early to all CLC activities unless otherwise stated by coaches. 5. Schedule all vacations so as not to interfere with any CLC activities. 6. Notify CLC coaches in writing immediately of all expected tardiness or absences. 7. Notify CLC coaches by calling or text message immediately of any unexpected tardiness or absences.

COMPETITION DRESS CODE At the time of competition: 1. Cheerleaders must be dressed as instructed by their coach. 2. All rollers must be taken out and hair and make-up must be complete as per coach’s request prior to arriving at competition. 3. All jewelry and colored nail polish must be removed. 4. All non-uniform items such as sunglasses, cell phones, CD players, and iPods must be put away. 5. Any braces or tape needed to perform must be supplied by the member and put on before going to the warm ups. During awards ceremonies, cheerleaders must be in full competition uniform and may not wear backpacks, warm ups or any other items. SPORTSMANSHIP/CONDUCT All cheerleaders AND parents must always: 1. Set a positive example for others to follow by being respectful and courteous to everyone. 2. Speak with a coach or other staff member to discuss any issues that may arise. 3. Refrain from gossiping or any form of verbal or physical confrontation. 4. Refrain from celebrating the misfortune or defeat of another person, team, or program. 5. Accept team placements and awards with dignity and class. HEALTH All cheerleaders must: 1. Provide CLC with current health insurance and emergency contact information as well as a list of any medications that they

are currently taking. 2. Inform CLC of all medical conditions that may limit or prevent their ability to participate in any CLC activities. 3. Notify CLC of any injuries sustained as a result of their participation in any sanctioned CLC activities. 4. Provide valid written documentation from a doctor explaining the reason(s) and the duration for which they may be limited

or unable to participate in any CLC activities. FINANCIAL OBLIGATIONS All cheerleaders and parents understand that: 1. Assume full responsibility for all costs incurred as a member including but not limited to: gym registration, monthly tuition,

practice outfits, uniforms, competition and travel expenses, or any other item(s), reservations, or services purchased or rendered to me and the payment in full of those items regardless of any circumstances that may arise such as dismissal from the team, or disbandment of the team.

2. Monthly tuition must be paid no later than the 1st of the month for the upcoming month. 3. Monthly payment for tuition rate is due for entire program season. 4. Tuition does not fluctuate based on the number or duration of practices in any month. Tuition pays for training. It does not

pay for the right to perform. 5. Vacations or time off will not be prorated. 6. A $25.00 late fee will be assessed in the event that a payment is past due. 7. A member’s account must be current and in good standing to participate in practices/competitions/special events. 8. CLC reserves the right to remove a member from their team at any time for failure to make financial obligations. 9. CLC reserves the right to cancel any and all travel arrangements made that are not paid in full by the due dates. 10. Any monies received from a member/parent will be applied first to any overdue registration/fees. 11. All registration/fees must be current before a member may collect any clothing/uniform or other retail items. 12. If a member chooses to leave or if asked to leave CLC for any reason before the season is over, any and all funds are

completely non-refundable. 13. Any individual (member, parent, legal guardian, visitor, family) who destroys any facility item, the parent of the member will
