SURGERY CLERKSHIP DESCRIPTION The 3rd Year Surgery Clerkship is an 8 week rotation that is conducted at 13 affiliated hospitals: Advocate Condell, Advocate Illinois Masonic (AIMMC), Advocate Lutheran General (ALGH), John H. Stroger Jr. at Cook County (CCH), Vista Health System, Cancer Treatment Centers of America (CTCA), Presence St. Mary of Nazareth, Little Company of Mary, Advocate Good Shepherd Hospital (AGSH), Edwards Hospital, Elmhurst Memorial Hospital and Ambulatory Surgical Center, Centegra Health System, and The Billings Clinic. The rotation is primarily inpatient. The curriculum covers core topics in surgery, as presented below. Learning activities at each site, which are organized by the respective site directors, include small group discussions, lectures, and hospital-based conferences. Basic science concepts are integrated into clinical discussions as well as the core didactic lectures. Depending on the site, students are either incorporated into surgical teams, which include residents, or they are directly precepted by attending surgeons. Students document their activities by maintaining a log of the patients they see in the tracking program, One45. Feedback is given by the site director and by supervising house staff and attendings. Clinical evaluations are collated by site directors and final clinical grades are assigned by the site director, utilizing the Clinical Evaluation Standards for all clerkship rotations. An NBME shelf exam for surgery is given at the end of the rotation. SURGERY CLERKSHIP OBJECTIVES LINKED TO CMS COMPETENCIES AND OBJECTIVES

1. Upon completion of the clerkship, students will be able to: 2. Describe the normal and abnormal structure and function of the body, distinguish the causes and underlying mechanisms of disease,

including epidemiology of common illnesses, and describe the principles and methods of diagnostic and therapeutic decision-making in each of the surgical cases listed in the required patient encounters. This includes distinguishing the preoperative factors which influence the advisability and timing of operative interventions and recognizing common postoperative complications. (1.1, 1.2, 1.3, 1.4, 1.5)

3. Apply the medical knowledge described in course objective #1 through performance of an accurate History, Physical Exam, routine technical procedures, as well as the accurate ordering and interpretation of diagnostic and therapeutic needs of the surgical patient. (2.1, 2.2, 2.3, 2.4, 2.5)

4. Demonstrate professionalism through ethical decision making, respectful care and treatment of surgical patients and families, and through active participation on the surgical care team. (3.1, 3.2, 3.3, 3.4)

5. Demonstrate analytical and critical thinking to solve clinical issues in the context of surgical patient care by using evidence-based medicine, current technology, and reflective practice to foster improvement of self and patient outcomes. (4.1, 4.2, 4.3)

6. Recognize one’s own role on the health care team, the resources available and how to utilize them to work collaborative to improve patient care, patient outcomes, and team performance along with reducing health care disparities and prevention of medical errors. (5.1, 5.2, 5.4, 5.5)

7. Perform effectively on the surgical team through clear communication, respectful and response actions, collaboration with the surgical team as well as the patient and family, and document in a clear and timely manner. (6.1, 6.2, 6.3, 6.4)

Reference D2L for SURGERY CLERKSHIP COMPETENCY MAP CLERKSHIP-SPECIFIC REQUIREMENTS *Any late assignment (or attendance, where applicable) will account for a 0.25 point decrease in the student’s Professionalism grade* OSCE: At the end of the 8-week rotation students will participate in a 30-minute OSCE to demonstrate their ability to evaluate and plan care for a patient presenting with post-surgical complications. Each student will have 10 minutes to evaluate their patient and 10 minutes to write a note. Basic information about the patient and a grading rubric will be provided. This will be worth 20% of the Medical Knowledge/Patient Care portion of the student’s grade. Assignments: Descriptions of, and instructions for, all assignments are posted on D2L. The following assignments have solid due dates. 6 CLER Reflection posts (see CLER Instructions.pdf on D2L for multiple due dates) 7 Reflection Questions (on D2L) about Interprofessional Team Retreat (due within 2 weeks of participation) 1 Mid-Clerkship Critical Self-Evaluation (due at the end of the 4th week) 1 Mid-Clerkship Preceptor Form (due at the end of the 6th week) 1 H&P Observation Sheet (due by the end of the 4th week) 20 Patient Encounters (16 due by end of the 3rd week; rest due by beginning of the 7th week) 15 Operative Procedures (due by the NBME Exam date) 7 Wise-MD computer cases (one due each week, weeks 1-7) 7 mandatory didactic sessions every Thursday (weeks 1-7) at Advocate Lutheran General Hospital Attendance is MANDATORY for both Interprofessional Team Training at RFUMS (on your assigned date) and the didactic days at Lutheran General Hospital (every Thursday). As per the CMS grading policy, unexcused absence from any clerkship required activity, for this and all clerkships, will decrease the Professionalism portion of your grade an ENITRE LEVEL. It is YOUR RESPONSIBILITY to ensure you give yourself adequate travel time to these sessions. For the Interprofessional Team Training at RFUMD, students arriving any more than 10 minutes late, or leaving more than 10 minutes early will not get credit for attending, and will have to make the session up or they will receive an INCOMPLETE for the clerkship. Additionally, once the session has been made up, a deduction of 0.25 points will be taken from your Professionalism grade as this assignment will be considered late.

Didactic Days: Students are required to attend EVERY didactic day at Advocate Lutheran General Hospital, located at 1775 Dempster Street, Park Ridge, IL 60068. Students are expected to be present for lectures at the assigned times. Students arriving after the assigned time without a valid excuse approved by the Education Director, will have a deduction of 0.25 points, per occurrence, from their originally received clinical Professionalism grade (where 6=Honors, 4=High Pass, 2=Pass). Topics may include fluids and electrolytes, shock, colon and inflammatory bowel disease, pediatric surgery, vascular disease, breast disease, neurosurgery, urology, trauma, orthopedics, thyroid/parathyroid, adrenal/surgical hypertension, biliary tract/duodenum /pancreas, stomach/small bowel and acute abdomen. Confirmation of topic, time, and room location will be emailed to students, weekly by the Clerkship Coordinator. Students must park in the open surface lot on the north side of Dempster Street, at the Luther Lane traffic light. Public Safety will remotely open the gate to the North Garage when the surface lot is full. If you are not able to find parking and the North Garage is not open, you should be able to find parking near the water tower on the west end of the surface lot and/or the gravel lot on the far east end. If a student has not had the opportunity to achieve two (2) endocrine clinical encounters by the mid-clerkship evaluation, then the attending will arrange and supervise a patient encounter, or student will complete one of the following WiseMD cases: Thyroid Nodule, Adrenal Adenoma.

Hernias (2) I,O FP, PP, O Student will participate in the evaluation of the patient. Student will obtain history, perform physical, and participate in developing clinical assessment and diagnostic and therapeutic plan, along with or under the guidance of an attending physician or resident.

If a student has not had the opportunity to achieve two (2) hernia clinical encounters by the mid-clerkship evaluation, then the attending will arrange and supervise additional patient encounters or student will complete one of the following WiseMD cases: Hernia, Hernia & Hydrocele.

Oncologic (2) I,O FP, PP, O Student will participate in the evaluation of the patient. Student will obtain history, perform physical, and participate in developing clinical assessment and diagnostic and therapeutic plan, along with or under the guidance of an attending physician.

If a student has not had the opportunity to achieve two (2) Oncologic clinical encounters by the mid-clerkship evaluation, then the attending will arrange and supervise patient encounter, or student will complete one of the following WiseMD cases: Adrenal Adenoma, Breast Cancer Surgery, Colon Cancer, Lung Cancer, Skin Cancer.

Trauma/Shock (2) I,O FP, PP, O Student will participate in the evaluation of the patient. Student will obtain history, perform physical, and participate in developing clinical assessment and diagnostic and therapeutic plan, along with or under the guidance of an attending physician.

If a student has not had the opportunity to achieve two (2) Trauma/Shock clinical encounters by the mid-clerkship evaluation, then the attending will arrange and supervise a patient encounter, or the student will complete the D2L mini trauma exam or the Trauma Resuscitation case module on WiseMD.

Evaluation of Abdominal Pain (1)

Participation in Operative Procedures (15)

I,O FP, PP, O Student will scrub in procedures as directed by their services or attending’s and assist in the procedures as instructed.

If a student has not had the opportunity to participate in fifteen (15) Operative Procedures by the mid-clerkship evaluation, then the attending will arrange and supervise the procedure.

REQUIRED OPERATIVE PROCEDURES (THIS IS IN ADDITION TO THE PATIENT ENCOUNTER MINIMUMS) Students must scrub in on a minimum of 15 operative procedures. The type of operative procedures are to meet the minimum requirements of the categories listed within the chart in the following section. *PLEASE NOTE THAT IF THE SAME PATIENT WAS EVALUATED AND ALSO HAD AN OPERATIVE PROCEDURE IN WHICH YOU PARTICIPATED, YOU SHOULD ENTER THE PATIENT INTO BOTH THE PATIENT LOG AND PROCEDURE LOG (one patient can be counted twice in such cases). NBME Online Shelf Exam The NBME Online Shelf Exam is typically held at the RFUMS Campus on the last Friday of the rotation from 9:00 a.m. to 12:00 p.m. However, please refer to D2L for the most recent updates.

The NBME is responsible for grading exams. No challenges are accepted for NBME exams. Scores will be available approximately 1-2 weeks after the exam has been completed, but will not be published until clinical evaluations have been received.

ALTERNATIVE ASSIGNMENTS Every attempt should be made to obtain the minimum number of operative procedures (20) and patient encounters (15). In extenuating circumstances, when these numbers cannot be met, the student can do one of the following assignments where each can replace 1 operative procedure or 1 patient encounter, due by the end of Week 7:

- A WISE-MD module for any given category not met. - Write a 1-page/300-word paper on a patient category found in the Minimum Patient Requirements (chart in following section) until the

minimum of 35 is met. This will be written directly on your case notes in the patient log. You must log the remediation in the patient logs to get credit.

In general, Clerkship Directors will determine how the student will make up patient encounters, clinical activities or other clerkship requirements. Clerkship Directors will determine if and when the student will be required to repeat the clerkship. Clerkship Directors will use case-by- case evaluation and coordinate with the Office of Student Affairs & Education.
