Vaccine information statements (VIS)It’s federal law!All vaccine providers, public or private, are required by the National Childhood Vaccine Injury Act to give the appropriate VIS to the patient (or parent or legal representative) before every dose of specific vaccines.1

You must record the following information in your patients’ medical records:

• VIS edition date

• Office address, name and title of the person who administered the vaccine

• Date the VIS is provided (a VIS must be provided before a vaccine is administered)

• Date the vaccine is administered

• Vaccine manufacturer and lot number
