RFQ Sections Purpose of RFQ: The City of Haslet is seeking a qualified engineering consulting firm to provide site planning, surveying, engineering design, and construction services for an Elevated Storage Tank to service the City’s utility system. The services to be provided include: field surveys, design and analysis, construction plans, bid documents, and construction administration. The firm shall demonstrate previous experience in the design and construction administration of similar elevated storage tank design projects. Questions Pertaining to the Selection Process: Any questions related to the project that are not covered in the RFQ must be in writing and directed to: Travis N. Attanasio, P.E., CFM City of Haslet (817) 439-5931 ext. 112 [email protected] Changes in the RFQ The City of Haslet shall not be held responsible for any oral instructions related to the project. Any changes to this RFQ will be in the form of a written addendum, which will be published on the City website at www.haslet.org. Withdrawal of RFQ The City reserves the right to withdraw the RFQ at any point. At any time prior to the specified solicitation due time and date, a consultant (or designated representative) may withdraw their Statement of Qualifications (SOQ) by submitting a written request stating the reason for withdrawal. General Information:
1) The City of Haslet reserves the right to reject any or all SOQs, to waive any informality or irregularity in any SOQ received, and to be the sole judge of the merits of the respective SOQ’s received.
2) Project Description: An elevated storage tank will be necessary to sustain the growth on the west side of the City of Haslet. Qualified engineering consulting firms are being sought to provide site planning, design, and construction administration services for this project.
3) The Engineer will be selected on the basis of demonstrated competence and qualification for the type of services required without regard to fee, and thereafter the City of Haslet will negotiate a contract for the site planning, design, and construction administration services at a fair and reasonable fee with the best qualified firm.
4) The Engineer will be an integral member of the Project Team, consisting of the Engineer, Contractor, and City staff personnel.

Scope of Services The Engineer should expect to provide the following services on the project:
1) Design and construction of a 500,000 gallon Elevated Storage Tank 2) Field surveys 3) Subsurface utility engineering (SUE) as necessary 4) Geotechnical investigations and recommendations for tank support 5) BNSF Railroad Crossing Permits 6) 16” Water line design in accordance with City of Haslet Specifications 7) Preliminary plan submittals for review (30, 60, 90 percent completion) 8) City involvement which includes meetings with city staff and presentations to City Council 9) Erosion Control Plans 10) Recommendations on equipment additions such as pumps, valves, lines, and flowmeters 11) Recommendations on upgrades to existing utility operations such as the installation of
pressure reducing valves, hydro tank removals, plant upgrades etc. 12) Existing SCADA system integration 13) Land and easement acquisition support as required 14) An Engineer’s Opinion of Probable Construction Cost for each phase of the design 15) Conduct the bidding process including the preparation of a detailed bid tabulation of all
bidders 16) Construction phase services including but not limited to: contract administration (RFIs,
Change Orders, etc.), and construction inspections 17) Site planning and analysis with public interaction 18) Maintain compliance with regulatory agencies including but not limited to, FAA, TCEQ, etc. 19) Public Outreach including attending public information meetings and construction
notification 20) Original and digital Submittal of record drawings to the City of Haslet including plans, shape
files, and CAD drawings RFQ Schedule The dates and times listed below are estimated and are subject to change without notice.
1. December 19, 2018 – Issue Request for Qualifications to provide engineering consulting services on the City of Haslet - Elevated Storage Tank Project.
2. January 4, 2019 – Deadline for written questions 5:00 PM (CST). 3. January 8, 2019 – Questions and Responses will be posted to the City of Haslet website
(http://www.haslet.org/) by the close of business day. 4. January 15, 2019 – Four (4) paper copies and One (1) electronic copy of the Statement of
Qualifications (SOQ) due no later than 4 PM. 5. January 16-18, 2019 – The selection committee reviews each SOQ. 6. January 21, 2019 - Consultant will be notified of contract award selection and will have ten
(10) business days to prepare a Professional Services Agreement for review by the City Attorney.
7. February 19, 2019 – Regular City Council Meeting to Approve Consultant Selection and Professional Services agreement which will include the Scope of Services.
8. February 20, 2019 – Consultant will be notified of contract award selection and will have ten (10) business days to execute the Professional Services Agreement.

Qualifications The project will require the firm to have the following qualifications:
• Five (5) or more years of related elevated storage tank design experience • Experience working with municipal and regulatory agencies, including military installations • Familiarity with the City of Haslet dynamics (i.e. staff, council, community involvement) • Experience with public outreach and consensus building including effective communication
with private property owners, special interest groups, and other stakeholders Instructions to Proposers Response Format: The response shall be organized and formatted as follows:
• All pages shall be 8 ½ x 11 (Fold-out pages shall be allowed for the schedule only) • All pages shall be white 20 lb. “copier-style” paper. Cover stock, plastic pages or laminations
shall not be allowed • All pages shall be bound together only at the upper left-hand corner by a single staple.
Plastic bindings, wire bindings, screw-type bindings or notebooks shall not be allowed • All pages shall be single-sided • Page 1 shall be a Cover Letter • Page 2 shall be an organizational chart identifying primary consultant and each proposed
sub consultant with the following information included: name of firm, office address, name of project manager, telephone number, and email address.
• Pages 3-9 shall be one-page resumes for each of the following: Principal, Project Manager, Project Engineer, Surveyor, Geotechnical Engineer, Structural Engineer, Quality Manager
• Pages 10-12 shall be a summary of at least 3 projects within the last five years that are similar in nature identifying the project owner, location, include contact name, title, current phone number, and e-mail address for each of the project
• Page 13 shall be a Gantt chart showing the proposed schedule for the project phases and disciplines
• Pages 14-15 shall be a written document that describes the project approach and any recommendations
• Page 16 shall be a written document that describes quality assurance and quality control procedures
• Responses that are longer than 16 pages will not be considered Evaluation Criteria All proposals received will be reviewed, evaluated, and ranked according to a numerical scoring system based on the responses to the criteria listed below. The Selection Committee may select firms to be interviewed, based wholly on qualifications.

Special Conditions
1. The Consultant/Engineer must demonstrate adequate Insurance coverage, including but not limited to Errors and Omissions. The City of Haslet shall be added to the policy.
2. A City of Haslet selection committee will review the submittals and recommend the most highly qualified submitting firms.
3. The Consultant/Engineer’s project manager shall be a Professional Engineer licensed in the State of Texas.
4. The Consultant/Engineer shall have an office within reasonable proximity to Haslet City Hall (NCTCOG Area). The Consultant’s project manager shall be based in that office.
Contract Negotiation Process The City reserves the right to reject any or all Statement of Qualifications. The highest ranked Respondent(s) may be invited to enter into Contract negotiations with the City of Haslet. If an agreement cannot be reached with the highest ranked Respondent(s), the City shall notify the Respondent and terminate negotiations. The second highest Respondent may be contacted for negotiations. The process may continue until successful negotiations are achieved. The City reserves the right to terminate negotiations with any and all Respondents should it be in the City’s best interest.
