P R E F A C E

Vital Statistics are useful for Socio economic Development and Health Planning, Population Control Programme and Policies etc. In Gujarat, the Vital Statistics are generated through Civil Registration System (Registration of Births and Deaths), Sample Registration System, Decennial Population Census, Rural Survey of Cause of Deaths, Medical Certification of Causes of Deaths from Hospitals and Health Centers and Ad-hoc surveys conducted by National and International Research Organization.

The registration of Births and Deaths Act was placed on the statute book in 1969 with the aim to collect and compile vital Statistics, which is necessary for Planning and Administration. Births and Deaths are the vital event in any Population knowledge about their events is essential for the understanding of Demography, Health and Civil needs. The Act has given statutory authority to the Chief Registrar, Gujarat to co-ordinate the work of Civil Registration throughout the State. In Rural Areas, the local registrars are from Panchayats and in Urban Areas, Health Officers of the Municipalities or Corporations or the executives officers are the Registrars.

Every registrar has to register Births and Deaths occurring within his/her administrative area. The information regarding occurrence is to be supplied within 21 days in both events of Births and Deaths. Events not registered within prescribed time are registered as delayed events, on payment of prescribed fee if it is done within one month. In case of delay more than 30 days but within a year, an affidavit is required from a Notary Public or an officer for registration besides late fee. For registration of events beyond one year an order from Class-I magistrate is necessary.

Every registrar is required to send periodical returns to the chief Registrar who send their reports to the Registrar General, India. On the basis of the annual vital statistics returns received from the districts, the Chief Registrar Gujarat brings out a comprehensive Annual Report entitled "Civil Registration System in Gujarat,"

In views of above, the Chief Registrar (Births and Deaths) is pleased to bring out the Annual Statistical Report for the Year 2009. This report provides information on valuable indicators like Birth Rates and Death Rate by districts and also by Rural-Urban classification and the type of Medical Attention received at the time of Births.

We appreciate the efforts made by officer and staff of vital Statistics division for preparing this report

It is hoped that the results given in this report will be useful to all the stakeholders.

Birth Registration Gujarat state had achieved 100.0 percent level of birth registration since

2008. Percentage level of births registration under CRS against estimates of SRS

is increased from 88.2 percent to 103.2 percent over 2005 to 2009, which shows significant improvement in level of Birth Registration.

Births rate under CRS has increased from 20.9 in 2005 to 23.0 in 2009. There is no significant difference in estimates of SRS birth rate and CRS birth rate

Birth rate under CRS, in urban areas (35.5) is more than double to that of rural areas (15.8), which is mainly, due to the lower estimates of vital events than the actual and/or due to the fact that SRS estimates of vital events are based on the usual residence while events are registered at the place of occurrence irrespective of the place of residence.

The districts having higher CBR than state average are Dangs (32.7), Panchmahals (31.7), Dohad (29.5), Banaskantha (29.3), Patan (29.2), Kachcha (27.6), Narmada (27.5), Bhavnagar (27.4), Amreli (26.7), Anand (26.0), Sabar kantha (25.9), Kheda (24.1), Mehsana (23.7), Rajkot and Surendranagar (23.2)

Districts reported less than 90 percent birth registration against SRS estimates are Surat (74.2 percent), Navsari (77.0 percent), and Vadodara (87.4 percent)

Sex Ratio at Birth The Sex Ratio at birth has shown improvement from 845 to 905 over 2005

to 2009. Highest Sex ratio reported by Dangs (999) district and lowest reported by

Surat (873) district. In census 2001, four District i.e. Surat (801), Ahmedabad (813), Anand

(836) and Gandhinagar (849) had reported sex ratio at below 850 in 0-6 age group. These four districts have shown significant improvement in sex ratio at birth i.e. Surat (873), Ahmedbad (883), Anand (901) and Gandhinagar (885)

Institutional Delivery Institutional Delivery has increased by 4.7 point over previous year. As per

CRS-2009, the institutional Delivery rate is 72.4%. Out of total institutional delivery, 31% institutional Delivery in Public Institution and 69 % is in Private Institution. Private Institutions include 40 percent delivery attended by skilled personnel i.e. Doctor, Nurse and Trained Midwife.

Death Registration Percentage level of deaths registration under CRS against estimates of SRS

is increased from 63.4 percent to 79.7 percent over 2005 to 2009. Death rate under CRS has increased from 4.5 in 2005 to 5.5 in 2009. Death rate under CRS-2009 is 6.9 in urban areas and 4.7 in rural areas.

Death rate under CRS in urban areas is higher than SRS estimates, which is mainly, because SRS estimates of vital events are based on the usual residence while events are registered at the place of occurrence irrespective of the place of residence.

The districts having higher CDR than state average are Anand (7.4), Navsari (7.2), Bharuch and Dangs (7.0), Ahmedabad and Narmada (6.6), Kheda (6.4),Vadodara(6.3), Mehsana (6.1), Rajkot (6.0) and Valsad (5.7)

Districts reported less than 75 percent death registration against SRS estimates are Dahod (41.8 percent), Banaskantha (52.6 percent), Kuchcha (63.2 percent), Surendranagar (64.2 percent), Panchamahals (67.6 percent), Surat (69.1 percent), Jamnagar (72.8 percent), Sabarkantha (74.2 percent) and Gandhinagar (74.6 percent)

Only 14.7 percent Infant deaths were registered in 2009 under CRS against SRS estimates

Registration of Infant death improved in 2009 (9578) compared to 8301

infant death registered in 2008. Registration of Maternal Death has also improved in 2009 (321) compared

to 144 maternal death registered in 2008. The registration of stillbirths has improved in 2009 as compared to

previous years (2549 in 2005, 7791 in 2009) Medically Certified Cause of Death (MCCD) MCCD project initiated in the year 2002. Six teaching hospitals and three municipal corporations included in first

phase up to 2006.In 2007, 7 district hospitals were included. In 2008 all district hospital and entire municipal corporation were included.

During the year 2009, all public and private hospital of Municipal corporation areas and all district hospitals are covered under the MCCD

58,940 deaths covered under MCCD, which shows 15% rise over 2008. Total no. of death reported under CRS-2009 is 317985. Out of them 96,310

(30.29%) deaths were institutional. Out of which 58,940 deaths were covered under MCCD i.e. 61.2 percent of Institutional Deaths and 18.5 percent to total death registered

Out of total deaths 21.9 percent deaths reported over 70+ age and 1.9 percent deaths reported in the age group of 1-4 Age

In the age group of 25-65 year, male deaths reported more than female deaths.

170 deaths reported due to pregnancy, childbirth and puerperium, direct obstetric related causes.

Out of 6001 Infant death reported under MCCD, 3820 deaths were peri natal period i.e. 63.7 percent to infant death

In the Infant deaths of perinatal period, Deaths due to hypoxia, birth asphyxia and other respiratory conditions come to about 35% during 2009.

As per ICD 10 Category, the proportion of deaths reported shows that , 21% due to circulatory system, 24% due to abnormal, signs and symptoms, 14% due to Infectious and parasitic diseases, 10% due to Respiratory system, 6% due to Perinatal period, and 6% due to external causes during the year 2009.

1. CIVIL REGISTRATION SYSTEM – AN OVERVIEW

The necessity for a complete, up to date, and reliable data on vital events hardly needs

emphasis. These data are a pre-requisite for socio-economic planning and to evaluate the

effective implementation of various programmes. In particular, they are the cornerstone of

public health systems today. The chief sources for data on vital events in India are Civil

Registration System (CRS), Sample Registration System (SRS) and Population Census. The

Population census remains the main source of information on population and a host of

population characteristics. However, the census, being a decennial exercise, does not provide

the measure of population change from year to year. The measures of fertility and mortality

derived from the Census are centered on the mid point of the decade and as such do not

provide information on annual change. Of the other two sources of vital statistics for getting

the vital rates on a regular basis, the SRS has been designed to provide reliable estimates at

National and State Level. The possibility of getting robust estimates of vital rates at district

and Sub-district level, on annual basis, through sample survey is ruled out on account of

prohibitively large sample size and variety of resultant factors such as controlling of non-

sampling errors etc. CRS is thus the only source for providing vital rates at district and sub-

district level, rather a complete CRS system can provide these rates at all administrative

levels.

The Civil Registration System may be defined as a continuous, permanent and

compulsory recording of the occurrence and characteristics of vital events as defined in and

as provided through a decree or regulation in accordance with the legal requirements of a

country. In India, the Civil Registration System covers events of births and deaths only.

The registration records are primarily useful for their value as legal documents and

also as a source of vital statistics. Civil registration as a source of vital statistics is relatively

inexpensive, as the statistics generated from the registration records are the by-product of an

essential administrative process. The registration records can also generate current and

continuous statistics, which can be more accurate than any other method of data collection.

However, the reliability of the statistics obtained through the registration system depends

much on the completeness and promptness with which events are registered and the accuracy

of the information in the registration records. The improvement in the system of registration

of births and deaths is critically dependent on how much importance is attached to the data on

births and deaths. Besides, the level of awareness among the public about the utility of

birth/death certificate also plays a crucial role.

Vital statistics generated through registration records enable estimations of the size,

structure and geographical distribution of the population for the current year except for

migration. Vital statistics also enables the projections for future years based on probable

trends of fertility and mortality as derived from a study of these factors and their interactions

with social, economic and other demographic factors. The vital statistics data generated

through an effective Civil Registration System is of immense use for formulation of various

development plans and welfare programmes/schemes. Civil Registration data can act as an

objective source for allocation of financial resources at State and District level. The Civil

Registration data acquires paramount importance particularly for planning public health

services and elementary education at State and District level. The vital statistics generated

through Civil Registration is quite useful for medical research and in the study of sex ratio,

mortality rates and in the study of causes of deaths.

Registration of births and deaths in Gujarat

Provision for registration of births and deaths and the mechanism there of had been in

existence for more than a century. The work of registration is carried out under various laws

and by-laws existing at that time in the state. The need for improvement in the vital statistics

system was felt for long time and, therefore on recommendations of various committee and

conferences, the Registration of Births and Deaths Act, 1969 providing for compulsory

registration of births and deaths was enacted in 1969. Since then, this act has been enforced

uniformly in all States and Union territories. The Act provides for registration of births and

deaths and publication of statistical reports annually based on the civil registration data.

In Gujarat, the registration of births and deaths is carried out under provisions of the

Registration of Births and Deaths Act, 1969 (RBD Act, 1969. The registration is done by the

local Registrar appointed by the State Government, under whose jurisdiction the event has

taken place.

The registration work in Gujarat is performed by involving officers of different

Departments. The existing organisational set-up involved in the work of civil registration at

various levels can be seen from following organisational chart.

In order to simplify and speed up the process of registration as also to bring in

uniformity in birth and death registration formats, the registration system was revamped in

the year 2004. Under the revamped system, the procedure for registration of births and deaths

was also modified to some extent and the forms redesigned.

In Gujarat, 18,119 rural registration units and 174 urban registration units were

required to send monthly returns of births and deaths for the years 2009

In case of rural units, reports are sent through Taluka Registrar to the District

Registrar of births and deaths. While reports from urban units are sent directly to Chief

Registrar by Health Officer in case of Municipal Corporations and to District Registrar by

Chief Officer in case of municipalities/ by registrar concerned in case of notified areas.

Thus, from 18119 rural registration units 217428 returns and 2088 returns from 174

urban registration unit were expected for the years 2009 respectively.

Since year 2005 the receipts of returns from rural as well as urban areas is 100

percent.

2. Brief Analysis of Data on Registered Births, Deaths and Infant Deaths

In this chapter, an attempt has been made to summarize the Civil Registration data in

order to highlight and analyze the effectiveness of the Civil Registration System (CRS) in

Gujarat. Births and Deaths are the two most important vital events. The basic statistics on live

births and deaths registered have been used to generate vital rates and some other related

important indicators and the results are presented here. The comparison of this data with

Sample Registration System (SRS) data brings out the coverage of Civil Registration System.

However, while interpreting the comparison it should be borne in mind that the registration of

births and deaths is carried out at the place of occurrence of the event, whereas, the vital rates

based on SRS are generated based on place of usual residence of mothers/deceased (as the

case may be) irrespective of the place of occurrence of events. It is also important to mention

that the registration data, at present, is deficient not only in terms of non-registration of

events but also with regard to non-reporting by the local registration units to the Districts

Registrar’s office.

Trends in Registered Live Births and Deaths

The number of live births and deaths registered in Gujarat is from the year 2000 to

2009 are presented are presented in the following table.

An overall increasing trend has been observed in number of registered live births and

deaths over the years. The number of live births registered has shown a gradual increase from

11.15 lakh to 13.46 lakh. In 2001, 2002, 2003 and 2005 decline observed in the registration

of births was mainly due to non-availability/partial availability of information on registration

of vital events from some of the reporting units. The number of registered deaths also

displayed an increasing pattern with some minor fluctuations in some of the intervening years

during the corresponding period. The number of registered deaths gradually increased from

2.26 lakh in 2000 to 3.18 lakh in 2009. As in the case of births, the fluctuations in number of

registered deaths can also be attributed to partial availability/non-availability of information

from some of the reporting units for certain years. Chart 1 and 2 highlights the trend of

number of registered live births and deaths from 2000 to 2009.

Live Birth and Live Birth Rate

The number of live births registered in the rural and urban areas of the State during

It indicates that in Gujarat during the year 2009, 13.46 lakh live births registered as

against 13.16 lakh during 2008, which is showing an increase in registration by 2.2 percent.

The registered birth rate has increased from 22.6 in 2006 to 23.0 in 2009. The rural-urban

break up indicates that registered live birth rate in urban areas (31.1 to 35.5) is more than one

and half times to that of rural areas (17.1 to 15.8) during last four year. This may be because

the registration is more in urban areas as compared to rural areas due to:

Higher level of awareness regarding birth registration

Higher level of institutional deliveries registered in urban areas.

Women from rural areas going to urban areas for institutional delivery

Level of Registration of Births

The level of registration, defined as the percentage of registered births/deaths to the

estimated births/deaths, determines the performance level of a district so far the efficiency of

civil registration system is concerned. For the purpose, the estimated total number of events

(births/deaths) as revealed from SRS rates is used in the absence of reliable districtwise

estimates from any other source. The level of registration so calculated is very high for some

district, mainly due to the lower estimates of vital events than the actual and/or due to the fact

that SRS estimates of vital events are based on the usual residence while events are registered

at the place of occurrence irrespective of the place of residence.

The percentage of CRS rates compared with the rates of Sample Registration System

The registered birth rate obtained through CRS is varying from 22.1 (2007) to 23.1

(2008), whereas in 2009 it is 23.0. The lower values of birth rate under CRS indicates under

recording during 2007, while the birth rate recorded for 2008 and 2009, indicate considerable

In Gujarat the sex ratio at birth has shown significant improvement. Sex ratio as per

the birth registered in 2008 and 2009 under Civil Registration System and estimated at birth for 2007 & 2008 (latest available) under SRS is also presented in the following table

Live Births by Type of Attention at Delivery

As per the Registration of birth during the year 2009, percentage of institutional

deliveries were 72.4 percent in total. Out of institutional deliveries 30.74 percent were in

government institution, 69.26 percent were in private medical institutions which includes

39.66 percent deliveries by qualified personnel i.e. Doctors, Nurses and Trained Midwife.

The rural-urban breakup shows that 42.4 and 94.2 percentage of institutional deliveries were

observed respectively in rural and urban areas.

The total registered death rates obtained through CRS were 4.7 and 6.5 for rural and

urban area respectively for the year 2007 while the same were 4.7 and 6.9 for rural and urban

areas respectively in 2009. The over all level of death registration, for 2009 is around 80

percent compared to SRS estimate, which indicates that around 20 percent deaths were not

registered under CRS.

Following Chart provides the CBR as per SRS and CRS for the year 1995 and 2001

to 2005. No significant improvements observed in death registration in spite of aggressive

efforts were made by the state government for people’s awareness through various IEC

activities. On the analysis of age group wise death registration under CRS and age group

wise estimates of death rate under SRS, it is observed that the death registration in the age

group of under five year is very poor in the state, which is main reason of low performance .

It indicates that in Gujarat, the overall registered death rate has fluctuated between 4.5

to 5.5 during the year 2005 to 2009. The Rural-Urban break up indicates that registered death

rate in urban area is fluctuating between 5.7 to 6.9, while in rural it is fluctuating between 3.8

to 4.7 during 2005-2009.

In urban areas, proportion of registration of deaths is nearly complete whereas in

rural areas it is poor which is mainly due to:

Lack of awareness in the people regarding importance of death certificate SRS estimates of vital events are based on the usual residence while events are

registered at the place of occurrence irrespective of the place of residence. Health facilities are available in urban areas. Registration of deaths occurred in

health care facilities are complete. Most of the urban areas have created the facility for registration at cremation and

burial place and after entry in the register; the families allowed to perform the religious rituals.

District i.e. 8.1 and minimum death rate observed in Dohad district i.e. 2.9.

Following chart provides the district wise death rate as per CRS, 2009. It also indicate

the state level death rate as per CRS-2009 and SRS-2008( latest available). Death rate

calculated under CRS for Anand, Navsari, Bharuch, Dangs, Ahmedabad, Narmada and

Kheda districts are nearer to State level SRS estimates. As per CRS, death rate of Surat,

Panchmahals, Kachchh, Banaskanta and Dahod is very low which is mainly due to low level

of death registration.

The registered death rate in Municipal Corporation areas during the year 2009 varied

between 5.0 (Surat) to 15.6 (Junagadh). It appears that percentage of death registration in

Municipal Corporation area is quite high as compared to rural and other urban areas in the

State, mainly due to institutional deaths and awareness of people for death registration.

Population Growth

Natural increase in population can be measured in terms of excess of birth over

deaths. The natural increase for the year 2005 to 2009 is shown in the following Table Table-11 : Natural increase in Population - 2005 to 2009

The natural increase from registered live births and deaths works out to 1.8 percent

only, which is near to SRS estimates.

It indicates that about 84.0 percent of the deaths fall under "Other Disease group"

while about 6.74 percent of deaths are due to Heart Diseases followed by 2.34 percent deaths

due to bronchitis and asthma, 2.42 percent due to Cancer, 1.93 percent due to Transport

accident and 1.36 percent due to Tuberculosis, in the year 2009.

Percent distribution of deaths by broad age group

Following table 12 shows the age group wise percentage of deaths registered under CRS-

2009 and SRS estimates for the year 2008. If we look at the table given below majority of the

Maternal Deaths

Number of maternal deaths registered under CRS during year 2007 to 2009 is shown

in the following Table.

The number of maternal deaths registered during the year 2009 is 321, which accounts

for 122.9 percent higher than the maternal death registered in 2008. Now the State has put up

Mother and Child tracking system in place which will more improve the level of registration

of maternal deaths.

Infant Mortality Rate (IMR)

The Infant Mortality Rate for any region is the number of infant deaths (deaths below the age

of one year) per thousand live births in that region during the year. The SRS rates can be used

to arrive at the expected number of infant deaths in the state. However, the number of infant

deaths at district or Sub- District levels can be obtained from a complete Civil Registration

System as SRS is not designed to give reliable estimates below state level. The only source to

get data on infant deaths at all level is CRS. Following table shows that the number of

registered infant deaths has increased from 8301 in 2008 to 9578 in 2009.

Above table indicates that there is lot of under registration particularly of infant

deaths. Generally, in case of the children who die within one month of their births, their

deaths are not registered which results into under registration of infant deaths. Such low

registration of infant deaths is a matter of concern. Full coverage of infant deaths by civil

registration machinery only can help in identifying areas, which is an essential requirement

for any health planning.

lack of awareness of the community and infant death certificate is hardly of any use for other

District wise IMR and IMR based on CRS as percentage of from CRS is shpwn in the

statement -5. Following Table provides the percentage distribution of infant deaths by age

group of children for the last 3 years.

During the year 2009, 54.2 % of infant deaths to total infant deaths took place in first 7 days

and 64.5 percent of total infant deaths were neo-natal deaths i.e. infant deaths within 28 days,

while remaining were post-neo-natal deaths. The pie chart given below shows the infant

death distribution by age group.

CRS, Gujarat, 2009 - Highlights

During the year 2009, the receipt of returns were 100.0 percent

The registered live birth rate based on CRS is worked out to 23.0, which is higher than

the estimated to SRS rate for the same year.

As per CRS registration of death rate per thousand of population is 5.5 during 2009

whereas SRS estimated is 6.9, which is around 80% to the estimate.

Birth Registration

The total number of births registered during 2009 was 13.46 lakhs as against 13.16

lakhs during 2008.In 2009, average births registered per registration unit of rural area

is 31 and in urban areas it is 4485. Average number of births registered per day in

Gujarat works out to 3687 in 2009 as against 3605 in 2008.

Of the total births registered, 7.06 lakhs (52.5%) were male and 6.39 lakhs (47.5%)

female.

Out of the total births, 3.71 lakhs (27.6%) were domiciliary in nature.

42.0 % of births were registered in rural area and 58.0% in urban area in Gujarat.

Among the total births registered 44.2% were of first order, 31.0% second order, 12.2

% third order and 12.2 % were of subsequent order during 2009.

Age-group of the mother at the time of delivery was analyzed and it was observed that

in 46.5 % cases it was 20-24 years and the age group in 34.2 % cases was 25-29 years.

15758 (1.2 %) births were registered with the age below or equal to 19 years of the

mother age.

An inverse relationship between the level of education attainment of mother and birth

order was observed. Higher the birth order lesser is the level of educational

attainment. In case of third birth order, the education attainment of mother in 10.7% of

the cases was illiterate or below matriculation while in case of fourth birth order, this

proportion was 4.1%.

Death Registration

In Gujarat, 317985 death were registered during 2009 as compared to 311990 deaths

registered during 2008. Out of the total deaths registered 196282 (61.7%) were males

and 121703 (38.3%) female.

The average number of deaths registered per day in Gujarat worked out to 871 in 2009

as against 855 during 2008.

During 2009, 30.3 % of the total deaths were reported by the medical institutions.

Out of total 317891 deaths registered during the year 2009, 9577 were infant deaths.

Deaths by age group, revealed that, 38.1 % deaths occurred in 71 and above age group

followed by 15.6 % in 55-64 age group and 11.5 % in 45-54 age group

The level of death registration is quite high in Municipal Corporation areas, and

comparatively higher in towns with population 30,000 and above.

Only 14.7 percent Infant death were registered in 2009 under CRS against SRS

estimates

Sex Ratio at Birth

Sex ratio at birth is increased by 60 point (905 in 2009) as compared to 2005 (845).

Highest sex ratio reported by Dangs District (999) and lowest sex ratio reported in

Surat District ( 873).

In census 2001, four districts i.e. Surat (801), Ahmedabad (813), Anand (836) and

Gandhinagar (849) had reported sex ratioat below 850 in 0-6 age group. These four

districts have shown significant improvement in sex ratio at birth i.e. Surat (873),

Ahmedabad (883), Anand (901) and Gandhinagar (885).
