Approval of the April 19, 2012 meeting with changes On a motion from Commissioner Dicus, seconded by Commissioner Terry, the minutes from the April 19, 2012 Civil Service Meeting were approved. Cancellation of the David Vialpando Hearing Information item only. Due to the resignation of Commissioner O’Grady, the David Vialpando Hearing scheduled for May 16, 2012 was cancelled. It will be rescheduled when a new Commissioner is appointed. Approval of Firefighter Inspector Selection/Promotional Process On a motion from Commissioner Dicus, seconded by Commissioner Terry, the process was approved.

Fire Inspector Process Proposed Selection Process

May 2012

I. Purpose: This memorandum proposes the selection process and schedule of events to establish an eligibility roster for Fire Inspector.

The Fire Department seeks to establish an eligibility register for this position for a period of two years.

II. Eligibility:

Journey-level Firefighters All applicants must have completed their apprenticeship program in order to be eligible to compete in this process.

III. Application: Interested and eligible firefighters must submit a letter of intent to compete for Fire Inspector beginning Friday, May 18, 2012. The deadline for application will be 1600 hours on Friday, June 15, 2012.

The letter of intent will serve as the application to compete in the process. There is no prescribed application form. Letters can be submitted to Kim Matthews in the Fire Prevention Bureau during regular business hours. No letters will be accepted following the May 15th deadline. To verify your submittal, ask to sign a roster maintained by the Fire Prevention Bureau.

IV. Accommodations: Candidates may request a reasonable disability-based accommodation for any part of the

examination process. Requests accompanied by medical documentation must be submitted to the Fire Department HR Representative at least ten (10) business days prior to the examination date.

V. Process:

A. Written Examination 50% The written examination will be based on the Fire Inspection Guide (available in the

Fire Prevention Bureau.) B. Oral Interview 45% The oral interview will be designed to measure additional skills required to be

successful as an Inspector. The Interview Panel will rate the following dimensions:

Oral Communication Problem Analysis/Problem Solving Motivation Planning & Organizing Interpersonal Skills The Interview Panel will consist of at least three individuals.

C. Seniority 5% In accordance with established practices, seniority will be calculated at .25 for every

year of service up to a maximum of 5.0 points and will be factored into the total score upon completion of the entire process.

VI. Schedule of Events:

VII. General Information This position is a 40 hour work week, but will require performing inspections during scheduled

evening and week-end hours. With approval from the Fire Marshal, you may elect to work 4 day 10 hour shifts. Within 180 days of promotion to Fire Inspector, candidate must successfully complete Inspector I training and obtain Inspector I certification.

SALT LAKE CITY POLICE DEPARTMENT Crime Lab Technician I Process

Continual Hire

I. SUMMARY: This classification is reserved for incumbents whose position is covered by the contract of a recognized collective bargaining unit. Under the general supervision of a Crime Lab Supervisor, provides crime scene processing, and evidence collection for the Police Department; particularly in the areas of fingerprints, photographs, and other types of physical evidence. This is a working level position that requires problem-solving abilities, critical thinking, organizational skills and computer skills.

II. ELIGIBILITY: Qualified applicants must meet the following minimum eligibility requirements:

1. Two year Associate Degree in Forensic Science (Criminalistics), Criminal Justice, or a related field. OR equivalent formal training/full-time work experience in crime lab techniques from a recognized forensic laboratory. Experience and education may be substituted for each other on a year for year basis.

2. Ability to communicate effectively both verbally and in writing. 3. Ability to physically reach and effectively perform job duties at crime scenes.

4. Ability to establish and maintain positive and effective working relationships with

Department Management, Supervisors, police officers, other employees, professional peers, court representatives, other public safety agencies, and the general public.

5. Must successfully pass all appropriate examinations as required by the Salt Lake City Civil Commission Rules and Regulations.

6. Must be able and willing to develop skills and certify in a specific Forensic Discipline, such as latent fingerprint examiner.

7. Must have above average computer skills and must be able to learn and use advanced

computer programs such as SICAR, for tire and shoeprints, AFIS, and MIDEO, for latent fingerprint comparison.

8. Offers of employment are contingent upon successful completion of a drug screen.

9. Must have valid Utah Driver License or the ability to obtain one within 30 days of employment.

PREFERRED QUALIFICATION:

1. Four year Degree in Forensic Science (Criminalistics), Criminal Justice, or a related field. Other four year degrees may be considered on a case by case basis.

III. APPLICATION PROCESS: To apply for this position, interested candidates must complete and submit an online application on the City’s website at www.slcgov.com/jobs. If a candidate does not have computer access, he/she may go to SLC Human Resources located in the City and County Building at 451 South State Street, Suite 115, Salt Lake City, for assistance. Applications turned in after the designated application deadline will not be accepted. IV. ACCOMODATIONS:

COMPONENTS AND WEIGHTS: The Crime Lab Technician I selection process will consist of the following components and weights: Civil Service Examination 35% Behavioral Based Interview 65% VI. SELECTION PROCESS: If the candidate pool is too large a supplement questionnaire may be mailed to all applicants that meet the minimum qualifications of the job. This supplemental questionnaire will ask for additional information regarding specific skills, training, and experience. Candidates will be provided an opportunity to complete and return the questionnaire by a deadline. This additional information will be reviewed and evaluated to narrow the pool of applicants to more manageable number. We will consider adverse impacts and departmental need in determining the natural break. Top-ranked candidates will be contacted and given an opportunity to compete in all proceeding stages of the selection process. ORIENTATION Candidates who are asked to advance in the process may be required to complete a mandatory orientation session. The purpose of the orientation is to familiarize the candidates with the duties and expectations of the position.

CIVIL SERVICE EXAMINATION The written examination will be a written “hurdle” examination and will measure each candidate’s knowledge of basic forensic skills. NOTE: Picture identification is required for admittance to the test. The cutoff or “pass rate” is determined by the written test scores, and will consider the natural break closest to 15 candidates, adverse impacts and departmental need. Those candidates successfully achieving the “pass rate” will be invited to proceed to the next phase of the selection process. The scores of those individuals who will proceed to the next phase of the selection process will be based solely on their civil service examination score. BEHAVIORAL BASED INTERVIEW The interview will be used to measure specific job-related dimensions. The interview will be a panel interview comprised of standardized, behaviorally based questions. VII. PERFORMANCE DIMENSIONS: The interview will measure multiple behavior dimensions that may include, but are not limited to, the following:

Leadership Planning & Organizational Skills Problem Solving & Decision Making Delegation & Control Commitment to Organizational Values Communication Skills Interpersonal Skills Initiative Flexibility & Persuasiveness Work History/Experience Computer & Data Entry Skills

VIII. RATERS: The interview board will consist of at least three trained raters from within and/or outside the Police Department. IX. METHOD OF SCORING AND RANKING: Each behavior dimension will be rated separately on a 1-to-5 scale, with 1 being the lowest score and 5 being the highest score. Ratings may also include a checklist of total points possible. Scores will be totaled from the "raw" ratings. The scores will be statistically analyzed, weighted appropriately, and converted to a percentage. Candidates will be ranked according to their final score and placed on the Crime Lab Technician I eligibility register. X. PROPOSED SCHEDULE OF EVENTS:

Continual Hire

I. SUMMARY: Under general supervision of a divisional manager, section supervisor or office manager, performs special project work and a variety of skilled technical duties in providing support services to professional and managerial staff. This is very responsible and specialized clerical work which requires independent judgment, the ability to establish and manage records and tracking systems, and initiative in reviewing for accuracy and completeness the systems established. II. ELIGIBILITY REQUIREMENTS:

Qualified applicants must meet the following minimum eligibility requirements: 1. Graduation from high school or equivalency and four years’ work experience as a secretary

or in a related clerical position. Experience and training must include records management, research, data entry and retrieval from computer terminals, and the use of personal computers to do word processing or spreadsheets. Education and experience may be substituted one for the other on a year-for-year basis.

2. Ability to type 60 wpm net and/or proficiency in the use of computers for filing system, word processing and/or spreadsheets, depending on departmental requirements. Depending on department’s needs, position may require demonstrated ability to take dictation, speed write and accurately record minutes of meetings in an efficient and satisfactory manner.

3. Ability to operate various standard office equipment and other specified technical

equipment. 4. Must be well organized, meticulous and able to prioritize tasks. Must be able to develop and

manage automated and complex filing systems. Knowledge of office procedures, Business English, proper correspondence formats, and letter and memoranda composition.

5. Considerable knowledge of divisional operations, departmental procedures and systems and

related law and regulations. 6. Must possess good research and mathematical skills and be able to use calculators. 7. Ability and means to travel between office and other worksites or locations when necessary

Request to Approve the Reinstatement of Police Officer, Jeremy Gaskins.

Police Officer, Jeremy Gaskins, resigned from the Police Department on 7/8/2011. Jeremy has asked Chief Burbank to be reinstated to his former position. Chief Burbank would like to recommend the Commission’s approval to reinstate Jeremy to his former position. As per rule 2-6-3, an employee who resigns may request reinstatement within twelve (12) months of resignation.

Approval of Engineer Promotional Process

The current eligibility register is exhausted. The Fire Department is seeking to create an eligibility register for a period of two years.

SALT LAKE CITY FIRE DEPARTMENT Engineer Promotional Process

2012 I. Purpose:

This memorandum proposes the application and examination process to establish a two-year eligibility register for the position of Engineer.

II. Eligibility:

All Journeyman-level firefighters who have successfully completed the Apparatus Driver Operator (ADO) Pumper Course are eligible to compete in the selection process. Non-probationary apprentice firefighters must petition the JATC before competing.

III. Application: Interested and eligible firefighters must submit a letter of intent to compete in the process beginning June 22 and ending not later than 1700 hours on July 20, 2012.

The letter of intent will serve as the application to compete in the Engineer selection process. There is no prescribed application form. All applications must be submitted in person at the Training Division to the Office Facilitator, Training Division Chief or Captain. Upon submitting application, applicants will receive a list of suggested study materials and/or references to assist in preparing for the written entrance exam which will be held July 26, 2012 at the Salt Lake City Fire Department Training Division at 0900 hours.

No applications will be accepted following the July 20, 2012 deadline. IV. Accommodations:

Candidates may request a reasonable disability-based accommodation for any part of the examination process. Requests accompanied by medical documentation must be submitted to the Fire Department HR Representative at least ten (10) business days prior to the examination date. V. Process:

A. Written entrance exam ranking 15% The written entrance exam will serve as a “cut-off” instrument in the event there are more candidates than can be accommodated for the class. If it is necessary to use the written entrance exam as a “cut-off” instrument, the cut-off or “pass-rate” will be determined by the written exam score, and will consider natural breaks, adverse impacts, and department need. Those successfully achieving the “pass-rate” will advance to the next phase of the process. (Reference: Civil Service Rules and Regulations, Section 4-2-7)

The written entrance exam ranking will be derived from the following. • Salt Lake City Fire Department Policies and Procedures, Chapter 4 • Executive orders

• Salt Lake City Fire Department Policies and Procedures, 05-01, 05-02, 05-04 05-04A, 05-21, 05-21A, 05-22, 05-23.

• The revised Pump Panel Companion and department hydraulic standards. • IFSTA Aerial apparatus driver/operator first edition. • IFSTA Pumping apparatus operator 2nd edition.

B. Engineer School 60% Engineer School will be an intensive three-week program therefore a good, basic working knowledge of Engineer duties will be required. The school will be deployed internally under the direction of the Salt Lake City Fire Department Training Division. The following weights will be assigned to the listed elements of the school to make up the total 60%:

1. Self study written exams and quizzes 20%

The tests and quizzes will be cumulative in nature. A minimum score of 80% will be maintained at all times. Failure of any test or quiz will result in a retest with a score of 90% or better. Failure of two tests (or one retest failure) will be grounds for removal from the process.

2. Final practical evaluation Pass/Fail

The practical examination will consist of a pumping operation, an aerial operation, and an apparatus maintenance inspection. Each practical evaluation must be completed with a minimum score of 80%. Failure of any practical evaluation will be grounds for removal from the process. 3. Final written examination 40% The final written examination will be cumulative in nature. A minimum score of 80% will be required. Failure of the final examination will be grounds for removal from the process.

C. Comprehensive Oral board 20%

The Oral Board will be a comprehensive review by Engineers and Officers selected specifically for their expertise and competency. It is designed to measure the candidate’s subject matter expertise as well as additional dimensions involved in the Engineer position. The Comprehensive Oral Board will examine candidates on the following areas of competency and attributes:

Safe emergency vehicle operations Apparatus construction and design Apparatus maintenance

• Pumper • Aerial

Pump operation Hydraulic theory Fire ground hydraulic applications

Aerial apparatus construction and design Emergency aerial apparatus operation Department Aerial device load ranges Apparatus placement Apparatus Strategy and tactics Oral Communication Independent Thinking Problem Analysis/Problem Solving Judgment and Decision Making Resistance to Stress Motivation

D. Seniority 5% Seniority will be calculated at .25 for every year of service as a sworn firefighter to a maximum of 5 points and will be factored into the total score upon completion of entire process.

VI. Schedule:
