DEAC Clinic 2135 New Walkertown Road Winston-Salem, NC 27101 (336) 723-7904 Delivering Equal Access to Care (DEAC) Clinic of Wake Forest University School of Medicine (WFUSM) has been a student-led initiative since its conception in 2006. At its core is an entirely student-run free health clinic which seeks to serve the long-term primary care health needs of local underserved community in the Winston-Salem area. The DEAC Clinic is proud to partner with the Community Care Center and is located within the same facility at 2135 New Walkertown Road. The Community Care Center handles patient financial screening for DEAC and patients can receive treatment and follow-up care at either clinic once screened. Crisis Control Ministry Pharmacy 200 E. 10th Street Winston-Salem, NC 27101 (336) 724-7875 - Residents of Stokes County can also use this service Crisis Control Ministry is a Christian-based ministry whose mission is to assist people in crisis to meet essential life needs and to become self-sufficient. Crisis Control Ministry’s Pharmacy is the largest free pharmacy in Forsyth County and was the first licensed free pharmacy in North Carolina when it was established in 1987. In our last fiscal year, we dispensed more than 28,000 prescriptions, valued at over $1.9 million. We want to be sure that individuals in Forsyth and Stokes Counties do not have to make the choice between food and life sustaining medications. Residents of any age of both Forsyth and Stokes counties are eligible to receive services from the Crisis Control Ministry Pharmacy. There are no set limits on the number of prescriptions filled or the value of medications given to one client. There are also no set financial guidelines for eligibility, but the client must demonstrate a true financial need. In addition, Operation Breakthrough's WAP also runs the Durham County area’s Heating & Air Replacement Program (HARRP), which provides funds to repair or replace heating and air units.

• Urgent Repair Program

• The Family Empowerment Action to Self-sufficiency Program (FEATS)

This assistance program was created to assist enrolled program participants in removing obstacles and solving longer term problems that stand in their path to becoming self-sufficient over the long term. The program really puts the focus on assisting families in raising their income level above the low-income or Poverty Guidelines set by the United States Department of Health and Human Services. The program provides numerous services to clients, including human resource readiness and development training, case management, employment search and referrals, job training, special financial support services, and emergency cash grants for limited expenses such as rent help.
